or email a copy of this form to Michael so that we are sure we have your correct details in order to send
your copies of Exakta Times to you. If you opt for the electronic copy of the magazine please make sure
that you supply your CORRECT preferred email address, otherwise your ET will not reach you. If paying by
PayPal please tick the Family & Friend’s option to ensure the Club does not incur additional costs.
