RBMonline 2009 Fertil Steril (in Press) 119 Effet du stockage à long terme 122 123 CONCLUSIONS : Vitrification of oocytes in case: o of unexpected semen production or inability to produce the sample according to a normal schedule post hCG is a valuable strategy. o of severe oligozoospermia is also an approach that permits to rescue oocytes. Of course such strategy implies the vitrification of zygotes (or embryos) post fertilization of the warmed oocytes. Three cycles of vitrification (oocyte – zygote – blastocyst) seems to have no adverse effect (but only one case) Kryo-accumulation of oocytes in case of poor responder may be a good option. However a disparity in our results in term of embryo development post warming is noticed, reflecting probably the intrinsic problem linked with the oocyte quality. Conclusions générales: En système ouvert et fermé la vitrification d´ovocytes et embryons permet d´obtenir des taux de grossesses supérieures à la congélation lente. Une moyenne de 6 á 8 ovocytes est suffisante pour obtenir une grossesse. Pas de différence en terme de malformation après vitrification (ovocytes et embryons) Le stockage sur une période de 6 ans n´a pas d éffet négatif en terme de survie, naissance et santé des enfants nés. Les concentrations intracellulaires en CP sont de loin inférieures à la concentration de la solution vitrifiante. à la concentration intracellulaire après congélation lente. Conclusions générales: Dans le but d´augmenter le taux de grossesses cumulatives: La vitrification d´un embryon par support est indispensable (fct qualité) Eviter les revitrifications Vitrification de blastocystes de qualités moyennes donne des résultats acceptables. La culture jusqu´au jour 6 (7) des embryons avec un retard de développement permet de vitrifier avec succès des embryons qui normalement n´auraient pas été cryopréserver à J5 Future de la vitrification • On peut s´attendre que dans un future proche les cycles de vitrifications vont augmenter au dépend des transferts à frais afin de replacer les embryons dans un environnement utérin plus physiologique. • Le transfert frais d´embryons de seconde qualité et la vitrification des meilleures est une stratégie à ne pas négliger.
