SAOUDI (Monaco) Récidive d’événement ischémique après stenting : une double étiologie Il s’agit d’un jeune patient de 50 ans pris en charge à la phase aigue (H 4) d’un infarctus du myocarde en juin 2013 en rapport avec une thrombose de l’IVA début du secteur II. Cet événement a été traité par une désobstruction au guide, puis par une thrombo-aspiration (figure 1), et enfin par la mise en place d’un stent actif avec un bon résultat angiographique. L’IRM pratiquée avant la sortie avait retrouvé une séquelle de nécrose antéro-septo-apicale non transmurale sur un ventricule gauche de fonction systolique conservée (FEVG à 69%, volumes indexés à 81 et 26 ml/m2). Le patient a ensuite été suivi régulièrement par son cardiologue avec notamment une bonne observance du traitement médicamenteux. Figure 1. En février 2014, alors que le patient était toujours sous bithérapie, survient un syndrome coronarien aigue sans sus-décalage du segment ST, mais avec augmentation de la troponine pour lequel le patient est de nouveau hospitalisé. Une coronarographie de contrôle (figure 2) est pratiquée objectivant l’absence de néo lésion, le stent apparaissant perméable avec persistance d’une sténose intermédiaire à la sortie du stent. Devant cette augmentation de troponine non expliquée, une nouvelle IRM (figure 3) a été organisée. Cet examen a permis de mettre en évidence séquelle de nécrose récente non transmurale septale moyenne. A la lumière de ces nouvelles données, le film de coronarographie a été revu, et l’attention a été attirée par de discrètes images d’addition au niveau du stent. L’indication d’une étude OCT du stent a été retenue. Cette dernière a permis de démasquer des plages de mal apposition au niveau des parties proximales et moyennes du stent dans des zones probablement de remodelage positif de l’artère. Ces mal-appositions ont été traitées dans la foulée par une redilatation au ballon avec au contrôle OCT une meilleure apposition du stent (figure 4). Parallèlement, une étude de l’anti-agrégation plaquettaire a été réalisée démontrant une non efficacité de l’Efient chez ce patient. La thérapeutique a donc été modifiée avec changement de l’Efient pour le Brilique. L’actualité médicale du Centre Cardio-Thoracique de Monaco Figure 2. 1. Bilan d’une communication intra-atriale à l’irm Il s’agit d’un jeune patient de 40 ans dont la sœur a été opérée d’un myxome de l’oreillette gauche et chez lequel une communication inter-atriale a été suspectée devant des cavités cardiaques droites dilatées. Figure 1. 4 Chambres L’IRM cardiaque pratiquée a permis de confirmer le diagnostic et de faire une étude précise de cette malformation tant sur le plan anatomique que sur le plan fonctionnel. La communication inter-atriale était de type ostium secundum, calibrée aux alentours de 16x14mm.Les rebords ont pu être correctement mesurés dans l’optique d’une fermeture per-cutanée (10 mm pour le rebord postérieur, 13 mm pour le rebord inférieur, 17 mm pour le rebord supérieur, 6 mm pour le rebord antéro-supérieur et 13 mm pour le rebord antéroinférieur, le septum inter atrial étant calibré à 34 mm en antéro-postérieur). Il n’a pas été mis en évidence d’anomalie du retour veineux pulmonaire. Cette communication est responsable d’un shunt gauche-droite qui a pu être quantifié avec un Qp/Qs estimé à 2.2, avec un flux dont la vitesse est de l’ordre de 1.5 m/s au niveau de la communication. Le retentissement hémodynamique a également été appréhendé, le ventricule droit étant dilaté de fonction systolique conservée (FE estimée à 64 %, EDVI et ESVI à 146 et 53 ml/m²). Une accélération non significative a été enregistrée au niveau de la valve pulmonaire correspondant à l’hyperdébit engendré. L’IRM a donc rendu possible une étude précise anatomique et fonctionnelle de la cardiopathie de ce patient. Une indication de fermeture per-cutanée a été retenue. Septum interauriculaire RECURRENT AORTIC VALVE INFECTIVE ENDOCARDITIS: A NEW SURGICAL OPTION A 42 year old male was admitted in the presence of an important aortic insufficiency due to a para prosthetic leak, and some false aneurysms and fistulas both above and below the aortic valve. His native aortic valve was bicuspid. In 2007 he underwent a bioprosthetic aortic valve replacement for aortic insufficiency and vegetations for Staphylococcus Aureus endocarditis. No entry site was found (dental panoramic, coloscopy, PET: all were negative). In May 2013, following a febrile episode, the patient was reoperated for prosthetic detachment and presence of peri annular abscesses. He underwent an “extra anatomical” Bentall procedure : the aortic valve was implanted just below the infected anatomical annulus. * * * Periprosthetic hematoma Patient was controlled with serial CT scans during the months after aortic valve replacement. * * The first control scanner showed the appearance of two small false aneurysms in the arterial ventricular junction, while the last one ( August Figure 1. * False aneurysms 2013), showed an increase in numbers of fistulas and also in the diameters of these ones ( Fig. 1). They were growing from the left ventricular outflow tract, their diameters were : 3,5x 2,5 cm - 3,5x 2 cm and 1,5x 1 cm ( Fig. 2-3). In September 2013, the patient was referral to us for an important aortic hematoma valve insufficiency , associated symptoms included general malaise, anorexia and a loss of body weight. The TEE performed in our centre, showed an important aortic peri prosthetic leak which caused a severe aortic regurgitation and the detachment of prosthetic implant for about one third of its circumference. The trans aortic gradient was 19 mmHg. There was also a false aneurysm all around the tube, communicating with left ventricular cavity; there was no communication with the right cavities. Ejection Fraction was 60%, diastolic LV volume was 75 and systolic LV volume was 26. SURGICAL TECHNIQUES Surgical approach was through standard redo median sternotomy. Cannulation took place through the femoral artery and femoral vein. The flow was achieved and cooling at 30 degrees was obtained. L’actualité médicale du Centre Cardio-Thoracique de Monaco 3. Adhesions were tight and difficult to free. The aorta was X clamped just before the brachiocephalic trunk . Cold crystalloid cardioplegia was delivered into the aortic root ( 1 liter )enhanced by Figure 4. Postop continuous topical cooling cold saline. After having transected the aorta completely, the aorta and then the Dacron graft were dissected free from the pulmonary artery, the left and the right atrium. Both coronary artery ostia were individualized and separated from the Dacron graft. The aortic valve was separated from the aortic annulus. During the debridement 2 abscess cavities were found, debrided and left wide opened. Once the valve removed 3 abscess cavities were identified, there were deep especially one, going forwards the left atrium, but fortunately not perforated. The cavities were debrided, left opened, the annulus was also debrided, it was completely destroyed. Moreover the cavities would not have allowed any new prosthetic valve implantation. The aorto mitral curtain was elongated downwards into the left ventricle as often it happens in such infectious process. The first step was to suture a Dacron graft measured at the size of the outflow tract. It was 30 mm diameter sutured for one half of its circumference to the inter ventricular septum, 2 cm below the aortic annulus with a running 4/0 prolene suture. The other half was sutured to the aorto mitral curtain. Second step was then to have an end to end anastomosis with a mechanical Carbomedics 27 with a Dacron graft 30 mm, named Cardioseal. The suture between the first graft implanted into the outflow tract and the mechanical valve was achieved also with a running 4/0 prolene sutures. Then the mobilized ostia were anastomized in an end to side fashion with 5/0 prolene suture posteriorly for the left main and anteriorly for the right coronary. Finally, the distal anastomosis between the Dacron graft and the distal aorta was performed in an end to end fashion with 4/0 running prolene sutures. The patient was discharged on the 12 post operative day. He was in sinus rythm. One month after the operation, he stopped the antibiotic therapy. In these 6 months after surgery, the patient hasn’t had febrile episode. The echo and control scanner showed a normal prosthetic valve function and no sign of recurrence of infective endocarditis.
