All membership dues are subject to approval by the SDA Board. Upon approval, a properly
executed membership card will be issued to you.

Payment for the Spring and Fall Season is $30.00 (Hawk American and Hawk National
divisions) or $35.00 (Eagle division) per season, per player. Each season is to be paid prior to
the start of the season.

RELEASE
The undersigned, in consideration for being afforded the opportunity to participate in
Scottsdale Darts Association (hereafter “SDA”) events, including league matches and
tournaments, does hereby forever release, discharge, and hold harmless SDA and the
proprietors and managers of each establishment in which the undersigned plays darts from all
claims for injury (and any loss based on such injury) that have heretofore accrued or may
hereafter accrue to the undersigned, his or her heirs, administrators, representatives, assigns,
and anyone claiming through the undersigned or based upon any such injury or loss suffered
by the undersigned as a result of or related to past, present, or future participation in or
attendance at an SDA event by the undersigned.
This Release shall inure to the benefit of the members, directors, officers, employees, agents,
insurers, indemnitors, successors, and assigns of the proprietors and managers of the
establishments in which an SDA event takes place. This Release shall not release a person
from any claim based upon that person’s intentional misconduct or the intentional misconduct
of anyone for whom that person would otherwise be liable. Nor shall this Release release the
proprietors and managers of an establishment for any claim for injury or loss not directly
related to an SDA event and of a nature that could be suffered by a patron when the playing
darts was not involved.
This Release shall revoke and supercede any previous SDA Release signed by the
undersigned.
