Parmi ces 23 cas confirmés, 13 étaient des cas autochtones, les deux derniers cas autochtones datant de juin 1999 et juillet 2000. Ces résultats illustrent le succès du programme d’élimination de la rougeole mis en place en 1998 en Catalogne. From November 1998 to December 2000, 84 suspected measles cases were reported in Catalonia (6 090 040 inhabitants). Of the 73 laboratory tested cases (87%), 20 showed IgM antibodies specific to measles and 3 were epidemiologically linked to a confirmed case. Among these 23 confirmed cases, 13 were indigenous, the last two cases reported dating back from June 1999 and July 2000. These results confirm the success of the measles elimination programme implemented in 1998 in Catalonia. As already shown in the United States, the transmission of indigenous measles ceased in 1993, according to data from genetic analysis of isolated measles virus (2-4,15). En 1988, le Département de la santé et de la sécurité sociale de Catalogne, en Espagne, a initié un programme pour éliminer la rougeole de cette province en 2000 (5-9). L’incidence de cette maladie en Catalogne (population estimée à 6 090 040 habitants) est passée de 470 pour 100 000 habitants en 1983, à 1,01 pour 100 000 en 1997 (5) et à 0,5 pour 100 000 en 1999 (figure 1). En conséquence, les autorités sanitaires ont décidé, en 1997, de mettre en place un système de déclaration individualisée pour la rougeole (11-13). Le formulaire de déclaration fournit des renseignements sur les signes cliniques et les coordonnées de chaque patient (nom, adresse et téléphone), permettant ainsi de mener une étude épidémiologique complète des cas notifiés. ➤ In 1988 the Department of Health and Social Security of Catalonia, a region of Spain, started a programme for the elimination of measles in the region by the year 2000 (5-9). The incidence of measles infection in Catalonia (estimated population 6 090 040) declined from 470 per 100 000 inhabitants in 1983 to 1.01/100 000 in 1997 (5) and 0.5/100 000 inhabitants in 1999 (figure 1). In light of this, the public health authorities decided in 1997 to introduce an individualised reporting status for measles (11-13). Ainsi, le rappel est administré à 4 ans au lieu de 11 ans (comme il avait été fixé en 1988). Une planification sélective prévoyait parallèlement un rappel aux enfants nés entre 1990 et 1993, afin de s’assurer que tous aient été correctement vaccinés (10,14). Le Département de santé publique contrôle sur les certificats de vaccination que les deux doses de vaccin ROR ont bien été administrées et détermine le taux de couverture de la deuxième dose. Un cas suspect correspond à un patient présentant les symptômes cliniques de la rougeole (éruption cutanée durant plus de trois jours, température supérieure à 38,5°C, toux et/ou coryza et/ou conjonctivite), chez qui le diagnostic de la maladie n’a pas été encore confirmé par des tests sérologiques ou par l’établissement d’une relation avec un cas confirmé en laboratoire. Un cas confirmé en laboratoire est un cas suspect séropositif pour la rougeole selon les tests sérologiques et/ou la mise en évidence du virus dans les urines. Un cas est considéré comme confirmé épidémiologiquement s’il présente une symptomatologie clinique évocatrice suite à un contact avec un cas de rougeole confirmé en laboratoire. Les informations épidémiologiques recueillies portent sur les caractéristiques cliniques, le statut vaccinal, les voyages effectués, l’école fréquentée et tout autre renseignement qui pourrait s’avérer utile pour déterminer la source de la transmission et les contacts susceptibles d’avoir été infectés. Du 4e au 11e jour après l’apparition des éruptions cutanées, des prélèvements sanguins et urinaires sont recueillis par le personnel du programme d’élimination de la rougeole, ou celui du centre hospitalier ou de l’établissement de soins primaires. La détection en laboratoire d’anticorps IgM spécifiques étant réalisée immédiatement par un test d’immunofluorescence indirecte, les cas peuvent être confirmés rapidement. Si ce test se révèle positif, l’échantillon d’urine est mis en culture sur des cellules B95a pour isoler le virus et permettre des analyses génétiques ultérieures (3,4). L’origine de la maladie est définie de la façon suivante : on considère qu’un cas confirmé est importé si le patient a séjourné hors de la Catalogne 7 à 18 jours avant l’apparition de l’exanthème, ou s’il a été en contact avec un cas suspect venant d’un pays étranger. En revanche, un cas est défini comme autochtone s’il n’existe pas de preuve épidémiologique d’une transmission par une source étrangère. c) Vacciner la population à risque (camarades d’école, membres de la famille) et entreprendre des enquêtes pour détecter l’existence éventuelle de cas non déclarés. On considère que les contacts à risque sont les jeunes de moins de 25 ans qui ne peuvent pas justifier avoir reçu deux doses de ROR. Les résultats sont évalués par la surveillance du taux de morbidité pendant les années d’activité du programme, c’est-à-dire de 1998 à 2000. 114 35 EUROSURVEILLANCE VOL. 6 - N°7/8 JUILLET-AOÛT / JULY-AUGUST 2001 a) Improvement of the immunisation status of the susceptible population by giving the second dose of the measlesmumps-rubella (MMR) vaccine at age 4 in the 1999 vaccine calendar instead of at age 11 as was set in 1988, and by a selective plan for administration of the second dose to the cohorts born between 1990 and 1993, to ensure that all children were properly vaccinated (10,14). Vaccine certificates are checked by the public health department to verify the first and second doses of MMR vaccine and to determine coverage of the second dose. b) Enhanced epidemiological surveillance of suspected cases of the disease, which includes prompt notification of the case by the attending physician (within 24 hours of onset) to the public health department and active follow up of the case and his or her contacts, with concise and exhaustive collection of all epidemiological information relevant to the case. A suspected case would be defined as a patient with clinical features of measles virus infection (rash for more than three days, fever above 38,5°C, cough and/or coryza and/or conjunctivitis) for whom the diagnosis has not yet been confirmed by serological tests or by a relation to a laboratory confirmed case. A laboratory confirmed case would be a suspected case confirmed by specific serological test and/or isolation of measles virus from urine specimens. An epidemiologically confirmed case would be a suspected case related epidemiologically to a laboratory confirmed case of measles. The epidemiological information collected includes clinical features, immunisation status, travel, school attendance, and any other information that might be useful to determine the source of transmission and probable susceptible contacts. Blood and urine specimens are collected from the 4th to 11th day after the appearance of the rash either by staff belonging specifically to the programme or by hospital or primary care staff. Laboratory determination of specific IgM type antibodies by an indirect immunofluorescence method is carried out immediately, and the case can thus be confirmed promptly. If positive, the urine specimen is cultured on B95a cells to isolate the virus for genetic analysis (3,4). With regard to its origin, a confirmed case would be considered imported if the patient had been outside Catalonia 7 to 18 days before the onset of the exanthem or had had contact with a suspected case from abroad. A case is defined as indigenous if there is no epidemiological evidence of transmission from a foreign source. c) Vaccination of the susceptible population who had contact with the case (schoolmates, household contacts) and inquiries to see whether there are unreported cases. Susceptible contacts would be all those younger than 25 years who can not prove they have received two doses of MMR vaccine. Results are assessed by surveillance of the morbidity during the years in which the programme has been operative, from 1998 to 2000. Résultats Results Les certificats de vaccination recueillis dans les écoles et les centres de soins primaires ont montré que 85% de la population avaient reçu leur rappel de ROR. Suivant la définition des cas, 84 cas suspects ont été déclarés entre novembre 1998 et décembre 2000 (figure 2). Des prélèvements ont été réalisés chez 73 de ces cas (73/84, 87%). Vingt seulement présentaient des anticorps IgM spécifiques et 3 autres avaient un lien épidémiologique avec un cas confirmé en laboratoire. Certifications of the second MMR vaccination dose collected from schools and primary care centres showed a registered coverage of 85%. According to the case definition, the number of suspected cases notified between November 1998 and December 2000 was 84 (figure 2). Clinical specimens were obtained from 73/84 (87%). Only 20 of these had positive IgM antibodies to the measles virus, with 3 more being confirmed by epidemiological relation to a laboratory confirmed case. Du début du programme, le 1er novembre 1998, au 31 décembre 1998, 10 cas ont été rapportés. Des tests ont pu être menés sur seulement 6 patients qui se sont révélés négatifs. De janvier à décembre 1999, 49 cas ont été déclarés, parmi lesquels 44 ont eu des tests, positifs pour les IgM spécifiques dans 17 cas. Un cas supplémentaire a été considéré positif sur l’association d’une symptomatologie évocatrice et d’une exposition à une épidémie de rougeole. Entre janvier et décembre 2000, 25 cas de rougeole ont été déclarés, 23 analysés, et 3 seulement confirmés ; 2 autres cas ont été considérés comme confirmés après un contact avec un rougeoleux confirmé. From the beginning of the programme on 1st November until 31st December 1998, 10 cases were reported. Only 6 of these could be analysed and were confirmed as negative by laboratory assessment. Of the 49 cases reported from January to December 1999, 44 were analysed and 17 confirmed as positive for to anti-measles IgM antibodies. In addition, one was confirmed by exposure to an outbreak. From January to December 2000, 25 cases were reported of whom 23 were analysed. Only 3 of these cases were confirmed, and 2 more considered confirmed because they were related to a laboratory confirmed case. En 2000, 4 cas confirmés sur 5 (80%) étaient importés et le cinquième cas habite dans un quartier où résident en majorité des immigrants du Maroc, du Pakistan et des Philippines, ce qui nous amène à penser que ce cas pourrait être importé aussi. Ainsi, aucun cas de rougeole autochtone n’a été confirmé de juin 1999 à juillet 2000. De juillet à décembre 2000, il n’y a eu qu’un seul cas d’origine incertaine, qui a été classé comme cas autochtone, faute de preuves suffisantes d’un contact avec un cas importé. Of the cases confirmed in 1999, 6/18 (33%) were proved by epidemiological evidence to be imported. Four (80%) of the five cases confirmed in 2000 were proved to be imported by epidemiological evidence, and one case lives in an urban area where most of the inhabitants are immigrants from Morocco, Pakistan, and the Philippines, which leads us to suspect that this case could also be imported. Thus no indigenous cases were confirmed from June 1999 to July 2000. From July to December 2000 there was only one doubtful case, who was, however, classified as indigenous owing to a lack of evidence of contact with an imported case. Sur la totalité des échantillons analysés, 27% seulement (20/73) ont révélé la présence d’IgM spécifiques. Pour les 73% restants (53/73), le diagnostic de rougeole a été écarté. Durant la durée du programme d’élimination de la rougeole, il est essentiel de mener une surveillance épidémiologique active et stricte de tous les cas déclarés, de les confirmer par des données de laboratoire et d’empêcher la transmission du virus. Pendant les 2e et 3e années du programme, des analyses de laboratoire ont été réalisées pour 89,7% (44/49) des cas déclarés en 1999 et 92% (23/25) en 2000, contre seulement 60% (6/10) pendant le dernier trimestre 1998. En prenant en compte les cas considérés positifs sur la preuve de contacts avec un rougeoleux confirmé en laboratoire (lien épidémiologique), nous pouvons considérer que notre but, qui était de tester tous les cas, a été atteint. No indigenous cases of measles occurred in Catalonia between June 1999 and July 2000. During the elimination programme for measles, it is crucial to carry out strict active epidemiological surveillance of all reported cases, to confirm them by laboratory data and to stop the transmission of the virus. In the second and third year of the elimination programme, 89.7% (44/49) in 1999 and 92% (23/25) in 2000 were laboratory tested, compared with only 60% (6/10) in the last quarter of 1998. The aim of the programme to undertake laboratory testing of all reported cases has been reached, if we consider those confirmed by epidemiological relation to a laboratory confirmed case (figure 2). En revanche, sur les 10 cas importés, 3 seulement avaient un statut vaccinal correspondant à leur âge (2 cas < 15 mois, 1 cas de 33 ans). Of the 10 confirmed imported cases, only three had a correct immunisation status for measles according to their age (two were younger than 15 months, and one was 33 years). Most of the Catalan population at risk of coming into contact with an imported measles case are corNo dose rectly immunised. It is 54 % (7/13) therefore probable that there will be few or even no confirmed indigenous cases in the future. Furthermore, whenever clinical features lead to a diagnosis of a suspected case of measles – if this is immediately reported and serologically confirmed – it is mandatory to investigate the possibility of toire d’enquêter sur une possibilité de contacts étrangers. Susceptibility to 15 antibiotics was determined in 1548 non-typhoidal salmonella strains isolated in Greece from l990 to l997. The overall prevalence of resistance of both Salmonella enterica serotype Enteritidis and Salmonella enterica serotype Typhimurium increased during the first years of the study. A decrease was observed from 1996, especially for S. Enteritidis, which showed the highest overall antibiotic resistance. S. Typhimurium was the serotype with the highest multiresistance to antibiotics. The rest of the serotypes had very low resistance prevalence compared with both S. Enteritidis and Typhimurium serotypes. Les animaux destinés à la consommation sont souvent porteurs de salmonelles, et contaminent ainsi la viande, les produits laitiers et les œufs (2). Dans les pays développés, les épidémies d’infection à salmonella paratyphi ont été provoquées principalement par les sérotypes Enteritidis et Typhimurium (3). Animals used for food production often carry salmonella, thus contaminating meat, dairy products, and eggs (2). In developed countries, outbreaks of non-typhoidal salmonella infection have been caused mainly by serotypes Enteritidis and Typhimurium (3). Enteritidis était impliquée dans plus de 70% des infections cliniques causées par l’espèce salmonella en Grèce, S. Typhimurium étant le second sérotype le plus fréquent (presque 17% des souches isolées de salmonelles). According to epidemiological data from the National Reference Centre for Sa1monella and Shigella for 1997, S. Enteritidis accounted for more than 70% of clinical infections caused by salmonella species in Greece, and S. Typhimurium is the second most frequent serotype (almost 17% of clinical salmonella isolates). Une augmentation brutale des taux de résistance aux antibiotiques, en particulier à l’ampicilline, chez les souches de S. Enteritidis a été observée entre 1987-93 (6). Dans cette étude, notre objectif était de déterminer les taux de résistance aux antibiotiques communéments utilisés en clinique, chez les différents sérotypes de salmonelles d’origine humaine, animale et environnementale. A sharp increase in the antibiotic resistance rates, especially to ampicillin, among S. Enteritidis isolates has been noticed during 1987-93 (6). Our aim in this study was to investigate the antimicrobial susceptibilities to several classes of antibiotics in clinical use, among various salmonella serotypes of human, animal, food, and environmental origin. Matériel et méthodes Materials and methods Un total de 1 548 souches de salmonelles d’origine humaine, animale, alimentaire et environnementale ont été sélectionnées de façon aléatoire à partir d’isolats recueillis au Centre national grec de référence des salmonelles et des shigelles ; 1027 isolats étaient d’origine humaine, et 521 ont été isolés chez des animaux, dans l’alimentation, les aliments destinés au bétail et dans l’environnement. Les souches provenaient d’un grand nombre d’hôpitaux et d’instituts vétérinaires de différentes régions du pays sur une période de huit ans (199097). Elles ont été sélectionnées de façon aléatoire et équitable tous les deux ans, la dernière année de l’étude incluse étant 1997. Ces isolats peuvent, dans une large mesure, être considérés comme représentatifs de l’ensemble du pays. A total of 1548 isolates of human, animal, food and environmental origin were randomly selected from the collection of Greece’s national reference centre for salmonella and shigella; 1027 isolates were of human origin, and 521 were isolated from animals, foods, animal feeds, and the environment. The isolates originated from a large number of hospitals and veterinary institutes, from various parts of the country, for an eight year period (1990-7). They were randomly selected and evenly distributed every two years, with 1997 as the last year of the study. To a large extent these isolates could be considered representative for the whole country. The sample was divided into three groups: Salmonella Enteritidis group (Enteritidis group) included 709 isolates (45.8% of the sample); S. Typhimurium group (Typhimurium group) included 363 isolates (23.4% of the sample); and the group of various other non-typhoidal salmonella serotypes (“others” group) included 476 isolates (30.8% of the sample). All isolates were identified biochemically using the API 20E system (BioMerieux SA, Marcy-l’Etoile, France), and serotyped by the methods of Kauffman (7) with commercial antisera (BioMerieux SA, France), for cell wall (0) and flagellar (H) antigen. Le contrôle qualité a été réalisé en utilisant Escherichia coli ATCC 25922 comme souche de référence. Quality control was performed by using Escherichia coli ATCC 25922 as a reference strain. The antibiotics tested were ampicillin, piperacillin, ampicillin-sulbactam, piperacillin-tazobactam, cefazoline, cefuroxime, ceftriaxone, tetracycline, gentamicin, amikacin, chloramphenicol, ciprofloxacin, norfloxacin, cotrimoxazole, and nitrofurantoin. Résultats Results Une augmentation notable de la résistance à la plupart des antibiotiques a été observée au cours des premières années de l’étude, particulièrement pour les isolats des groupes Enteritidis et Typhimurium. La prévalence la plus élevée et les augmentations les plus remarquables ont été notées pour l’ampicilline, la pipéracilline, l’ampicilline-sulbactam, et la tétracycline dans les groupes Enteritidis et Typhimurium, plus le chloramphénicol dans le groupe Typhimurium (tableau 1). Pourtant en 1996, la prévalence de la résistance à la plupart des antibiotiques a diminué dans le groupe Enteritidis, et en 1997 une baisse plus importante a été observée. Une baisse générale moindre a été observée en 1997 dans le groupe Typhimurium, ainsi qu’une autre baisse– moins évidente– pour le groupe « autres », qui a toujours présenté une prévalence de la résistance inférieure à celles des groupes Enteritidis et Typhimurium. An appreciable increase of resistance to the most of the antibiotics has been noticed during the first years of the study, especially for the isolates of the Enteritidis and Typhimurium groups. The highest prevalence of resistance and also the most noticeable increases were observed for ampicillin, piperacillin, ampicillin-sulbactam, and tetracycline in the Enteritidis and Typhimurium groups, and additionally for chloramphenicol, in the Typhimurium group (table 1). Nevertheless, during 1996 the prevalence of resistance to almost all antibiotics declined for the Enteritidis group, and in 1997 a sharper decrease was observed. An overall slighter decrease was noticed in 1997 for the Typhimurium group. A decrease again, but less evident, was observed for the “others” group, which always had a lower prevalence of resistance compared with isolates from the Enteritidis and Typhimurium groups. En conclusion, dans de nombreux cas, on note une augmentation générale de la résistance aux antibiotiques au cours des années étudiées, en particulier dans les groupes Enteritidis et Typhimurium, vis-à-vis de l’ampicilline, la pipéracilline, l’ampicilline-sulbactam, la tétracycline, et vis-à-vis du chloramphénicol pour le groupe Typhimurium. Cette augmentation globale du taux de résistance était pratiquement insignifiante pour le groupe « autres ». La prévalence de la résistance aux aminoglycosides et aux quinolones était très faible dans tous les groupes ; quelques isolats sporadiques étaient résistants à la gentamicine, l’amikacine, et la norfloxacine; et aucun n’était résistant à la ciprofloxacine (tableau 1). Les phénotypes de résistance les plus fréquents étaient ampicilline/ampicilline-sulbactam/pipéracilline dans le groupe Enteritidis (175 isolats), ampicilline/ampicilline-sulbactam/pipéracilline/tétracycline/chloramphénicol (55 isolats) dans le groupe Typhimurium – qui est compatible au lysophage S. Typhimurium (DT) 104 multi-résistant – et la résistance à la tétracycline dans le groupe « autres » (25 isolats) (tableau 2). Un seul isolat clinique de S. Typhimurium était résistant à la ceftriaxone. Le groupe Enteritidis s’est révélé être le plus résistant, et cela est principalement dû à la prévalence élevée de la résistance de ce sérotype à l’ampicilline, alors que le groupe Typhimurium était le plus multi-résistant. ➤ Therefore, in many cases the overall increase of drug resistance throughout the years studied was noteworthy, especially in the Enteritidis and Typhimurium groups, to ampicillin, piperacillin, ampicillin-sulbactam, tetracycline, and to chloramphenicol for the Typhimurium group. The overall increase of drug resistance was almost insignificant for the “others” group. The prevalence of resistance in all groups was very low to aminoglycosides and quinolones; a few sporadic isolates were resistant to gentamicin, amikacin, and norfloxacin; and none was resistant to ciprofloxacin (table 1). The most frequent resistance phenotypes were ampicillin/ampicillin-sulbactam/piperacillin for the Enteritidis group (175 isolates), ampicillin/ampicillin-sulbactam/piperacillin/tetracycline/chloramphenicol (55 isolates) for the Typhimurium group - which is compatible to the multidrug resistant S. Typhimurium definitive phage type (DT) 104 - and resistance to tetracycline for the “others” group (25 isolates) (table 2). Only one clinical isolate of S. Typhimurium was found to be resistant to ceftriaxone. The Enteritidis group was found to be the most resistant group, and this is mainly because of the high prevalence of resistance of the serotype to ampicillin, whereas the Typhimurium group was found to be the most multiresistant. ➤ EUROSURVEILLANCE VOL. 6 - N° 7/8 JUILLET-AOÛT / JULY-AUGUST 2001 119 ➤ Discussion et conclusion ➤ Discussion and conclusion L’étude montre une élévation importante de la fréquence des infections à salmonella résistantes aux antibiotiques en Grèce au cours de la dernière décennie. La diminution de la prévalence de l’antibiorésistance au cours de la dernière année de l’étude suggère la prédominance des germes pathogènes sensibles, en particulier pour S. Enteritidis, et peut s’expliquer par le mécanisme de la résistance aux antibiotiques, principalement d’origine plasmidique pour ce sérotype (6, 9, 10). This study documents large increases in the frequency of antimicrobial resistant salmonella infections in Greece over the past decade. The decrease of prevalence of antibiotic resistance during the last year of the study may indicate the predominance of sensitive microbe populations, especially for S. Enteritidis, and is possibly facilitated by the mechanism of antibiotic resistance that is principally plasmid mediated for S. Enteritidis (6,9,10). A l’inverse, pour S. Typhimurium, le principal mécanisme de résistance aux antibiotiques est situé au niveau chromosomique ; il est par conséquent plus difficile à contrôler (11). De plus, les souches S. Typhimurium se sont avérées les plus résistantes ; résultat également observé dans d’autres pays tels que l’Angleterre et l’Allemagne (12,13). On the contrary, the main mechanism of drug resistance is chromosomally located in S. Typhimurium and consequently more difficult to control (11). Furthermore, the isolates of S. Typhimurium have proved to be the most multiresistant ; a finding that has also been observed in other countries, such as England and Germany (12,13). L’apparition d’un isolat S. Typhimurium résistant aux céphalosporines de troisième génération (C3G) soulève une certaine inquiétude. La récente observation de trois isolats grecs de S. Typhimurium présentant une résistance aux C3G d’origine plasmidique (14) suggère qu’une transmission des gènes de résistance à d’autres souches de salmonella ou à d’autres entérobactéries est possible. The appearance of one isolate of S. Typhimurium resistant to third generation cephalosporins gives cause for concern. The recent observation of three Greek S. Typhimurium isolates with plasmid mediated transferable resistance to third generation cephalosporins (14) renders possible the transmission of these resistance genes to other strains of salmonella or other enterobacteria. Dans l’ensemble, les souches d’origine humaine étaient aussi résistantes que les autres ; 47,5% des isolats humains étaient résistants à un ou plusieurs antibiotiques, par rapport à 49,1% des isolats non humains (données non présentées). Ces résultats sont différents de ceux rapportés par la plupart des autres pays en Europe et ailleurs, où les isolats non humains étaient moins résistants que les isolats humains (15, 16). As a whole, the human isolates were almost equally resistant in respect to the non-human ones; 47.5% of the human isolates were resistant to one or more antibiotics, compared with 49.l% of the non-human ones (data not shown). These findings are different from those reported for most other countries in Europe and elsewhere, where non-human isolates were more susceptible than human ones (15, 16). La législation portant sur l’utilisation des antibiotiques par les vétérinaires n’étant pas appliquée de façon stricte, cela peut entraîner une augmentation des taux d’antibiorésistance chez des isolats non humains par la sélection de bactéries résistantes chez les animaux destinés à la consommation (4). Des mesures pratiques devraient être appliquées aux cheptels afin d’éviter la transmission à l’homme des gènes de résistance. Parallèlement, la politique de prescription des antibiotiques devrait être appliquée plus strictement et des mesures de santé publique devraient être instaurées aux niveaux national et international. Ces actions pourraient contribuer à réduire l’incidence des infections, ainsi que les taux de résistance aux antibiotiques, sur le cycle complet de transmission de salmonella paratyphi. ■ Since the existing legislation regarding the use of antibiotics by veterinarians is not applied strictly, this may be responsible for the increased antibiotic resistance rates of non-human isolates through selection of resistant bacteria among food producing animals because of the use of antimicrobials (4). Practical measures should be applied in livestock to prevent the spread of antibiotic resistance genes to humans. At the same time, antibiotic policy must be more strictly applied, and stricter public health control measures have to be implemented at a national and international level; this might contribute to further decreases of both the incidence of infection and antibiotic resistance rates for the integral cycle of non-typhoidal salmonella transmission. ■ References 1. Eleven cases of legionnaires´ disease, all patients living in the same municipality, were admitted to a district hospital in northern Portugal. Preliminary investigations suggested an association with the events of the annual festivities in that municipality. Legionella pneumophila was not isolated from any of the suspected sources, but evidence from a case control study suggested that an aerosol produced by a decorative fountain in the main square during the night of a rock concert was the likely vehicle of infection. The prevalence of smoking was higher among cases than controls. Introduction Introduction epuis le 1er janvier 1999, la légionellose fait partie des maladies à déclaration obligatoire au Portugal. A notre connaissance, seuls des cas sporadiques avaient été rapportés (1). La première semaine de septembre 2000, les autorités sanitaires locales ont été informées de la probabilité d’une épidémie de légionellose dans une petite ville du nord du Portugal, plusieurs cas ayant été admis à l’hôpital régional le plus proche. La diffusion de cet évènement par les médias a provoqué un certain émoi dans la communauté. egionnaires’ disease has been a notifiable disease in Portugal since 1 January 1999. As far as we know, only sporadic cases have been reported (1). During the first week of September 2000, a local health authority was informed that a probable outbreak of legionnaires’ disease was occurring in a small municipality in the north of Portugal and that several cases had been admitted to the nearest district hospital. This event attracted the attention of news media, whose coverage created some alarm in the community. Entre le 25 août et le 4 septembre, 13 cas de légionellose y avaient été admis. Une enquête a été menée auprès de tous ces cas afin de pouvoir procéder à une première évaluation des facteurs de risque potentiels. Onze d’entre eux vivaient dans la même commune, les deux autres n’y ayant pas séjourné en 2000. Une investigation détaillée a été menée auprès de ces deux patients, mais les résultats ne sont pas rapportés dans cet article. Nous considérons en effet que ces deux cas sont épidémiologiquement distincts des 11 autres. L’un des patients a dû être hospitalisé en service de soins intensifs. Les symptômes les plus souvent retrouvés étaient les suivants : fièvre (11/11), toux (5/11), céphalées (3/11), myalgie (3/11), fatigue (2/11), dyspnée (2/11), vomissements (2/11) et diarrhée (1/11). Aucun décès n’a été rapporté. D’après la courbe épidémique (figure), et la période d’incubation (de 2 à 12 jours) (2), on peut estimer que l’exposition a eu lieu entre le 12 et le 22 août. Les fêtes annuelles de la ville où résidaient les 11 patients avaient eu lieu entre le 10 et le 14 août 2000. Il semble que ces festivités aient été particulièrement fréquentées, plusieurs manifestations populaires ayant attiré des centaines de spectateurs. Le 6 septembre nous avons visité la commune et sa place principale, où s’est tenue la plupart des festivités. Deux fontaines proches l’une de l’autre décorent cette place. Des ➤ 29 30 The OCT visited the hospital on 6 September. Thirteen cases of legionnaires’ disease had been admitted between 25 August and 4 September. They were all interviewed in order to make a preliminary assessment of potential risk factors. The only common characteristic was that 11 of them lived in the same municipality; the other two had not visited that municipality during 2000. Though we investigated these two patients in detail, the results are not described in this report because we consider them as epidemiologically distinct from the other 11 cases. These 11 cases were men aged 33 to 73 years, 10 of whom were regular smokers. One of the cases was seriously ill and was treated in the intensive care unit. All 11 patients suffered fever, 5 had cough, 3 headache, 3 myalgia, 2 weakness, 2 shortness of breath, 2 vomiting, and 1 diarrhoea. None of the patients died. According to the epidemic curve (figure) and the reported incubation period of the disease (2 to 12 days) (2) we assumed that exposure had occurred between 12 and 22 August. The annual festivities of the municipality where the 11 patients lived had taken place between 10-14 August 2000. We were told that these festivities had been especially crowded, hundreds of people had attended several popular events. On 6 September, we visited the municipality and its main square, where most of the events had taken place. We conducted a case control study in order to investigate associations between the presence at particular events of the festivities and the risk of disease. We regarded the two decorative fountains in the main square as potential vehicles of infection. Méthodes Methods Définition et recherche de cas Conformément aux définitions de cas de l’Organisation Mondiale de la Santé (3) et du réseau EWGLI (European Working Group on Legionella Infection) (4), nous avons classé les cas comme probables (n=5) ou confirmés (n=6). Un cas est considéré comme probable lorsqu’une symptomatologie pulmonaire est associée à des signes focaux de pneumopathie à l’examen clinique ou radiologique, et à une séroconversion avec un taux d’anticorps spécifiques de L. pneumophila d’au moins 128. Un cas est considéré confirmé lorsqu’aux critères cliniques de pneumopathie, s’ajoute au moins l’un des critères biologiques suivants : isolement de L. pneumophila, ou détection d’antigènes urinaires de L. pneumophila ou encore une élévation de quatre fois du taux d’anticorps spécifiques de L. pneumophila. Les cas probables et confirmés ont tous deux été sélectionnés pour l’étude cas-témoins. Définition et sélection des témoins Un cas témoin devait répondre aux critères suivants : être de sexe masculin, âgé de 33 à 73 ans et résider dans la même ville que les cas. Il n’y avait pas d’information disponible pour évaluer précisément la taille de l’échantillon. Nous avons donc utilisé Epi Info (5) pour l’estimer, en prenant en considération les hypothèses suivantes : α=0,05, β=0,20, 50% des témoins exposés, et un odds ratio très élevé (OR >20). Puis nous avons sélectionné quatre témoins pour chaque cas. Ils ont été choisis parmi les personnes qui consultaient, pour une raison quelconque, le Centre de soins primaires (Primary Care Health Centre, PCHC), les 21 novembre et 7 décembre. Nous avons utilisé une méthode d’échantillonnage systématique : tous les habitants éligibles consultant au Centre de soins entre 10 heures et midi ont été invités à participer. Le recueil de données Des enquêtes ont été menées avec tous les cas et les témoins, par un questionnaire rassemblant des informations sur le tabagisme et la présence aux festivités. Les fontaines décoratives n’étaient jamais mentionées explicitement dans le questionnaire, pour ne pas influencer les réponses des participants. Nous avons listé tous les événements de la fête (en nous basant sur le programme officiel) et demandé aux cas et aux témoins s’ils y avaient participé. Comme nous connaissions l’emplacement de chaque événement, nous pouvions en déduire si les personnes avaient été au contact des fontaines. Pour la période des huit jours suivant les fêtes, comprise dans la fenêtre de contamination possible, nous avons demandé aux cas et aux témoins s’ils s’étaient rendus en ville à certains moments précis de la journée (le matin, l’après-midi et le soir). L’analyse de données Nous avons utilisé la version Epi-Info 6.04 (5) pour la saisie des données et l’analyse préliminaire et le logiciel Stata (6) pour l’analyse restante. Les odds ratios bruts ont été estimés pour chaque événement et chaque moment de la journée dans les jours suivant la fête. Pour vérifier les variables confondantes, nous avons utilisé un modèle de régression logistique avec toutes les variables associées statistiquement à la maladie dans l’analyse univariée. L’enquête microbiologique Case definition and case finding According to the case definitions of the World Health Organization (3) and the European Working Group on Legionella Infection (4), and considering clinical and laboratory criteria, we classified cases as probable and confirmed. A case was considered probable when an acute pulmonary infection with focal signs of pneumonia, either by clinical or radiological evidence, was associated with the presence of a single serum antibody titre of at least 1:128 against L. pneumophila. A case was classified as confirmed when besides the clinical criteria of pneumonia there was at least one of the following laboratory criteria: isolation of L. pneumophila or detection of L. pneumophila antigen in the urine or a fourfold rise in the titre of L. pneumophila antibody in serum. Both probable and confirmed cases were selected for the case control study. Control definition and selection The criteria for the definition of the controls were: male gender, age 33 to 73 years, and residence in the same municipality as the cases. We had no information to make a precise sample size estimate. Thus, we used Epi Info (5) to estimate the sample size, considering the following assumptions: α=0.05, β=0.20, 50% of the controls exposed, and a very high odds ratio (OR >20). We then selected four controls for each case. They were recruited amongst the population consulting the Primary Care Health Centre (PCHC), for any reason, on 21 November and 7 December. We used a systematic sampling method: all eligible citizens attending the PCHC between 10am and midday were approached and invited to participate. Data collection We conducted interviews with all cases and controls, using a questionnaire gathering information about smoking habits and presence at the events of the festivities. The decorative fountains were never mentioned explicitly in the questionnaire, in order to avoid leading participant's answers. We listed all events of the festivities (transcribed from the official programme) and for each of them, cases and controls were asked to answer if they had or had not attended that event. We knew where the events had taken place and were therefore able to find out if people had attended events next to the decorative fountains. For the period of eight days after the festivities, included in the period in which cases could have been infected, we asked cases and controls whether or not they had been in the town at particular times of the day (morning, afternoon, and night). Data analysis Epi-Info version 6.04 (5) was used for data entry and preliminary analysis and Stata (6) for the remaining analysis. Crude ORs were estimated for each festivity event and period of time in the days after the festivities. To control for confounding we used a logistic regression model with all variables statistically associated with the disease in the univariate analysis. Microbiological investigation Le laboratoire de l’hôpital de district a recherché les antigènes de L. pneumophila dans les prélèvements urinaires par une technique immuno-enzymatique (Binax®). Urine specimens of the cases were examined for L. pneumophila antigen, using an enzyme immunoassay technique (Binax®), at the district hospital laboratory. Sputum smears were sent to the National Institute of Health in Porto, for conventional bacteriological investigation. Aucune culture n’a permis de mettre en évidence des légionelles. Ainsi, cinq cas ont été classés comme probables et six confirmés. Epidémiologie analytique Two patients had a positive legionella urinary antigen (L. pneumophila serogroup 1, Binax ®), six patients had a single high serological titre for legionella antibody equal or above 128, and five patients had a fourfold or greater rise in the serological titre for legionella antibody. None of the cultures performed was positive. Thus, five cases were classified as probable and six as confirmed. The age and residence distribution of cases and controls was such that no relevant selection bias was likely to have occurred. The use of an exhaustive list of events in the questionnaire, avoiding explicit mention of the decorative fountains, minimised the possibility that participants would be led to suggest the suspected source of infection. Nous avons conclu que la source probable de l’infection à L. pneumophila est liée aux aérosols produits par la fontaine décorative sur la place principale pendant le concert de rock, ces aérosols ayant été inhalés par quelques personnes présentes au concert. D’autres épidémies de légionellose provoquées par des fontaines décoratives ont été décrites dans la littérature (8,9). Il est possible que d’autres infections, non détectées, soient dues à la même source de L. pneumophila. Only one case could not be explained by attendance at the rock concert, and the computed value of the aetiological fraction was very high (though with a wide confidence interval). The higher prevalence of smoking habits among cases is consistent with previous observations (2). It is likely that L. pneumophila was not isolated from the water samples from the decorative fountains because, by the time samples were collected, the fountains had already been cleaned and disinfected and fresh treated water had been added to the system. According to the municipal records, only one of the two decorative fountains was working during the rock concert. Curiously, it was not the fountain closest to the public and the stage. Direct observation and analysis of night photographs have shown that a huge amount of aerosol was produced while the fountain played. Thus, biological plausibility adds to the epidemiological evidence. We concluded that it is very likely that aerosol particles produced by the decorative fountain in the main square during the night of the rock concert were the vehicle for spread of L. pneumophila, and that these particles were inhaled by some people who were present at the concert. Other outbreaks of legionellosis spread by decorative fountains have been described (8,9). It is possible that the same source of L. pneumophila caused other infections that were not detected.
