I personally examined the above-named person who is an in-patient at the psychiatric facility named above on (day / month / year) 2. I personally observed the following facts indicating incapacity to manage property: (please be specific to incapacity to manage property) 3. The following facts, if any, indicating incapacity to manage property were communicated to me by others: 4. I certify that the above-named patient is incapable to manage his/her property as of the date below: (date) (day / month / year) (signature of physician) Note: The physician shall promptly advise the patient of the Certificate of Incapacity by giving the patient a Form 33 and shall notify a rights adviser. To the attending physician: The patient must be re-examined 21 days before discharge from a psychiatric facility to determine whether he or she is capable of managing property. If the patient continues to be incapable, you must complete Form 24 Notice of Continuance of Certificate of Incapacity to Manage One’s Property under Subsection 57 (2) of the Act. The Officer in Charge should forward the original or a PDF version of the original Certificate of Incapacity to the Public Guardian and Trustee (email address: [email protected]). Je déclare que la personne malade susmentionnée est incapable de gérer ses biens à compter de la date indiquée ci dessous : (date) (jour / mois / année) (signature du médecin) Remarque : Le médecin informe promptement la personne malade du certificat d’incapacité en lui remettant une Formule 33 et avise une conseillère ou un conseiller en matière de droits. Au médecin traitant : La personne malade doit être réexaminée 21 jours avant de recevoir son congé d’un établissement psychiatrique afin de déterminer si elle est capable de gérer ses biens. Si la personne malade est toujours incapable, vous devez remplir la Formule 24 Avis de prorogation du certificat d’incapacité de gérer ses biens en vertu du paragraphe 57 (2) de la Loi. La dirigeante ou le dirigeant responsable transmet au Tuteur et curateur public l’original du certificat d’incapacité ou une version PDF (adresse de courriel : [email protected]).
