The Savyon® SeroCT™ - IgG kit is a new generation qualitative ELISA test which is based on Chlamydia trachomatis specific synthetic peptides. SeroCT™ is used as an aid in the diagnosis of C. trachomatis specific infection. SeroCT™ - IgG is intended to be run and interpreted in conjunction with the Savyon® SeroCT™ - IgA kit. For In Vitro Diagnostic Use. Introduction Chlamydia is a gram negative obligate intracellular bacteria that causes acute and chronic disease in mammalian and avian species. The genus Chlamydia is comprised of four species: C. trachomatis, C. pneumoniae,C. psittaci and C. pecorum (1- 4). C. trachomatis is divided into 15 serovars (5-8). Serovars A, B, Ba and C are agents of trachoma(9), the leading cause of preventable blindness, endemic in third world countries. Serovars L1-L3 are the agents of lymphogranuloma venereum. Serovars D-K are the common cause of sexually transmitted genital infection worldwide: cervicitis, endometritis/salpingitis(10) in females and urethritis(11) in both males and females. Endometritis/salpingitis can lead to tubal occlusion with a higher risk of extrauterine pregnancy and infertility. Genital infection may cause an acute and persistent infection occasionally without any clinical signs. Generally, these infections are treatable once they are diagnosed. However, without any treatment the infection may progress to a severe chronic inflammation leading to infertility, ectopic pregnancy, induced abortion or child delivery. Moreover, infants to infected mothers can be infected during birth leading to conjunctivitis or pneumonia (12-14). The serology of C. trachomatis is more interesting in cases of chronic infections than in acute infections. C. pneumoniae is an important respiratory pathogen in humans and causes up to 10% of community-acquired pneumonia cases. It has been associated with acute respiratory diseases, pneumonia, asthma, bronchitis, pharyngitis, acute chest syndrome of sickle cell disease, coronary heart disease and Gullain-Barré syndrome (15-17). C. psittaci infects a diverse range of host species from molluscs to birds to mammals and also causes severe pneumonia. In animals, C. psittaci and C. pecorum are capable of inducing diverse disease syndromes, like pneumonia, enteritis, polyserositis, encephalitis and conjunctivitis. Serological testing, now an established approach in many countries, has been shown to provide a comprehensive answer for the detection of C. trachomatis infection. In suspected deep-seated infections, serum sampling reduces the necessity for invasive procedures which are required for direct antigen detection. In cases of lower urogenital infections, collection limitations such as effectiveness of scrape sampling procedure, -3- specimen handling and transportation difficulties have to be weighted. Above all, there remains the issue that most Chlamydial infections are asymptomatic. Therefore, an infection may persist for a long time, ascend the upper genital tract causing deep and chronic infection, and increase the probability of false negative results in direct antigen detection. Serological testing for C. trachomatis, through the detection of various specific antibodies, is today an effective and highly accepted method option(10,11,18,19). New and accurate technologies apply the immuno markers IgM, IgA and IgG to characterize the presence and stage of the infection. Specific IgM is indicative of acute Chlamydial infection. Absence does not, however, preclude the presence of on-going infection, especially in recurrent and chronic cases. The use of specific IgA as a marker for active Chlamydial infection has been shown to have an important role because of its short half life time, while persisting as long as antigenic stimulation exists. IgA, however, is more suitable for post therapy follow-up. IgG is a marker for Chlamydial positive immune-response in either current, chronic or past infections. Serological cross reactions occur between the three different species of Chlamydia. Most of the serological diagnostic assays for Chlamydia use either purified elementary bodies: microimmunofluorecence (MIF) and ELISA tests, lipopolysaccharide (LPS), or purified major outer membrane protein, (MOMP) as antigens. Genus specific epitopes are present in all the above antigens, therefore, low species specificity is observed. Moreover, a large proportion of the population has been exposed to C. pneumoniae (with no clinical signs), the prevalence of anti-Chlamydia antibodies is very high. Therefore, the differentiation between C. pneumoniae and C. trachomatis specific antibodies using conventional serological screening tests (MIF, ELISA, EIA etc.) is insufficient. Savyon® Diagnostics has developed an assay in which C. trachomatis species specific epitopes, derived from different serotypes, are used in an Enzyme - Linked Immunosorbent Assay (ELISA). The test excludes cross-species reactive epitopes and enables more accurate and more specific determination of C. trachomatis IgG and IgA antibodies. Principle of the Test • SeroCT™ plates are coated with C. trachomatis specific peptides. • Serum to be tested is diluted and incubated with the precoated SeroCT™ plate 1h at 37°C. In this step C. trachomatis specific antibodies are bound to the immobilized C. trachomatis specific peptides. • Non specific antibodies are removed by washing. • Anti-human IgG conjugated to horseradish peroxidase (HRP) is added and incubated 1h at 37°C. In this step the HRP-conjugate is bound to the prebound antigen-antibody complex. -4- E • Unbound conjugate is removed by washing. • Upon the addition of TMB-substrate, the substrate is hydrolyzed by the peroxidase, yielding a blue solution of the reduced substrate. • Upon the addition of the stop solution, the blue color turns yellow and should be read by an ELISA reader at a wavelength of 450nm. • The absorbance is proportional to the amount of the specific antibodies which are bound to the immobilized peptides. Disposable plastic vial for dilution of the concentrated HRP- conjugated anti human IgG. 3. Adjustable micropipettes, or multichannel pipettes (5-50, 50-200 and 200-1000µl ranges) and disposable tips. 4. One liter volumetric flask. 5. One 50ml volumetric cylinder. 6. Wash bottle. 7. Absorbent paper. 8. Vortex mixer. 9. A 37°C water bath with a lid, or a moisture chamber placed in an incubator at 37°C. 10. ELISA-reader with 450nm filter. 11. Distilled or double deionized water. Warning and Precautions For In Vitro Diagnostic Use 1. This kit contains human sera which have been tested by FDA and CE approved techniques, and found to be negative for HBV antigen and for HCV and HIV 1&2 antibodies. Since no known method can offer complete assurance that products derived from human blood do not transmit infection, all human blood components supplied in this kit must be handled as potentially infectious serum or blood, -7- according to the recommendations published in the CDC/NIH manual "Biosafety in Micro Biological and Biomedical Laboratories", 1988. 2. TMB-Substrate solution is an irritant material to skin and mucous membranes. Avoid direct contact. 3. Diluted Sulfuric acid (1M) is an irritant agent for the eyes and skin. In case of contact with eyes, immediatley flush area with water and consult a physician. Do not pour water into this product. In case of an accident or discomfort consult a physician (if possible present the label). 4. All the components of this kit have been calibrated and tested by lot. It is not recommended to mix components from different lots since it might affect the results. Storage and Shelf-Life of Reagents 1. All the reagents supplied should be stored at 2-8°C. The unopened reagent vials are stable until the expiration date indicated on the kit pack. Exposure of originally stoppered or sealed components to ambient temperature for a few hours will not cause damage to the reagents. DO NOT FREEZE ! 2. Once the kit is opened, it’s shelf life is 90 days. 3. Unused strips must be resealed in the aluminum pouch with the desiccant card, by rolling the open end and sealing tightly with tape over the entire length of the opening. 4. Crystals may form in the 20x concentrated Wash Buffer during cold storage, this is perfectly normal. Redissolve the crystals by warming the buffer to 37°C before diluting. Once diluted, the solution may be stored at 2-8°C for up to 21 days. Serum Collection Prepare sera from aseptically collected samples using standard techniques. Heat inactivated sera should not be used. The use of lipemic, turbidor contaminated sera is not recommended. Particulate material and precipitates in sera may cause erroneous results. Such specimens should be clarified by centrifugation or filtration prior to the test. Storage: Specimens should be stored at 2-8°C and tested within 7 days (adding of 0.1% Sodium Azide is highly recommended). If a longer storage period is anticipated, aliquot and store the specimens below -20°C. Avoid repeated thawing and freezing. Test Procedure A. Preparation of Reagents 1. Bring all components and clinical specimens to be tested to room temperature. Mix well the Positive Control, Negative Control and the clinical specimens before use. 2. Determine the total number of specimens to be tested. In addition to the specimens, the following must be included in each test: two wells of Negative Control and one well of Positive Control. -8- E 3. Withdraw the microtiter plate from its aluminum pouch by cutting one end near the seal. Leave the required number of strips (according to the number of specimens to be tested) in the 96 well frame. 4. Dilute the Concentrated Wash Buffer 1/20 with double-deionized or distilled water. For example, in order to prepare one liter of Wash Buffer, add 50ml of the Concentrated Wash Buffer to 950ml of double-deionized or distilled water. B. Incubation of sera samples and controls 5. Dilute each patient serum 1/21 with the supplied Serum Diluent as follows: Add 10µl of patient serum to 200µl of Serum Diluent. 6. Pipette 50µl of Positive Control, Negative Control and 1/21 diluted sera into separate wells of the test strip. The Negative Control should be pipetted into two separate wells. 7. Cover the strips with a plate cover and incubate for 1h at 37°C in a moisture chamber. 8. Discard the liquid content of the wells. 9. Washing step: Fill each well with wash buffer (300-350µl) up to the end of the well and discard the liquid, repeat this step two times, for a total of three washing steps. 10. Dry the strips and frame by gently tapping them over clean absorbent paper. C. Incubation with Conjugate 11. Concentrated HRP-Conjugate anti-human IgG should be diluted to working solution shortly before use. Dilute the concentrated HRP-conjugated 1/300 with Conjugate Diluent. For example, for two strips prepare a minimum of 3 ml of diluted HRP-Conjugate(10µl of Concentrated HRP-conjugate is mixed with 3ml of Conjugate Diluent). 12. Pipette 50µl of diluted conjugate into each well . 13. Cover the strips with a plate cover and incubate for 1h at 37°C in a moisture chamber. 14. Discard the liquid content and wash as described in steps 9-10. D. Incubation with TMB - Substrate 15. Pipette 100µl TMB-Substrate into each well, cover the strips with a plate cover and incubate at room temperature for 15 minutes. 16. Stop the reaction by adding 100µl of Stop Solution (1M H2SO4) to each well. E. Determination of Results 17. Determine the absorbance at 450nm and record the results. Determination should not exceed 30 minutes following stopping of chromogenic reaction. -9- Note: Any air bubbles should be removed before reading. The bottom of the ELISA plate should be carefully wiped. Test Validation For the test to be valid the following criteria must be met. If these criteria are not met the test should be considered invalid and should be repeated. 1. Positive Control: The absorbance value should be ≥ 0.8 at 450nm. 2. Negative Control: The average absorbance value of the Negative Control performed in duplicate should be 0.1< NC ≤ 0.4 at 450nm. Calculation of Cut-Off Value (COV) and Cut-Off Index (COI) The cut-off value is calculated according to the following formula: COV = NC x 2 NC = The average absorbance at 450nm of the Negative Control run in duplicate. No single serological test should be used for a final diagnosis. All clinical and laboratory data should be taken into account. 2. If Chlamydial infection is suspected, a second sample should be obtained 14-21 days later and tested in parallel with the original sample. Der SeroCT®- IgG Kit gehört zu einer neuen Generation qualitativer ELISA Tests, der auf Chlamydia trachomatis spezifischen, synthetischen Peptiden basiert. Der SeroCT®- IgG Kit ist zur Durchführung und Interpretation zusammen mit dem Savyon® SeroCT®- IgA Kit vorgesehen. Nur zur in vitro Diagnostik Einführung Chlamydia sind gram-negative, obligat intrazelluläre Bakterien, die akute und chronische Krankheiten in Säugern und Vögeln verursachen. Die Gattung Chlamydia beinhaltet vier Spezies: C. trachomatis, C. pneumoniae, C. psittaci und C. pecorum (1-4). C. trachomatis ist in 15 Serovare unterteilt (5-8). Die Serovare A, B, Ba und C verursachen Trachoma (9), eine unbehandelt zur Erblindung führende Infektion, besonders häufig in tropischen und subtropischen Regionen mit niedrigem Hygienestandard. Die Serovare L1-L3 sind Erreger des Lymphogranuloma venereum (LGV, Lymphopathia venerea). Die Serovare D-K sind weltweit häufig die Ursache von sexuell übertragenen Genitalinfektionen: Zervizitis, Endometritis/Salpingitis (10) bei Frauen und Urethritis (11) bei Männern als auch bei Frauen. Die Genitalinfektion kann als akute und bisweilen chronische Infektion ohne klinische Symptomatik verlaufen. Im allgemeinen sind diese Infektionen, wenn diagnostiziert, kurabel. Ohne Behandlung können sie sich zu schweren chronischen Entzündungen entwickeln, die zu Unfruchtbarkeit durch Tubenverschluss, ektopisher Schwangerschaft, Aborten oder Frühgeburten führen. Des weiteren können Neugeborene von infizierten Müttern während der Geburt infiziert werden, was zu Konjunktivitis und Pneumonie führen kann (12-14). Die Serologie von C. trachomatis ist bei chronischen Infektionen von höherem Interesse als bei akuten Entzündungen. C. pneumoniae ist ein wichtiges Pathogen der Atemwege bei Menschen und verursacht bis zu 10% der Fälle von Lungenentzündung. Es wurde akuten Krankheiten der Atemwege zugeordnet, wie Pneumonie, Asthma, Bronchitis, Pharingitis, akutes Brustsyndrom der Sichelzellkrankheit, Koronarherzleiden und Guillain-Barré Syndrom (15-17). C. psittaci infiziert eine Reihe verschiedenartiger Spezies von Weichtieren, Vögeln und Säugetieren und verursacht u.a.. schwere Pneumonien in Tieren. C. psittaci und C. pecorum können verschiedene Krankheitsbilder wie Lungenentzündung, Polyserositis, Enzephalitis und Konjunktivitis hervorrufen. - 15 - Infektionen des Menschen kommen vor, sind aber offenbar nicht häufig. Serologische Tests auf Chlamydia trachomatis AK sind heute eine akzeptierte Methode in vielen Ländern und haben gezeigt, daß sie für den Nachweis von Chlamydia trachomatis - Infektionen wertvolle Informationen liefern. Beim Verdacht auf tiefsitzende Infektionen vermindert die Serumentnahme die Notwendigkeit invasiver Prozeduren, die für den direkten Antigen-Nachweis erforderlich sind. In Fällen von Urogenitalinfektionen sind die Unsicherheit der Abstrichprozedur, sowie die Schwierigkeiten beim Handhaben und dem Transport der Proben in Betracht zu ziehen. Es ist seit langem bekannt, daß viele Chlamydia - Infektionen asymptomatisch verlaufen können. Eine Infektion kann daher lange Zeit bestehen, in den oberen Genitaltrakt aufsteigen und schwerwiegende, evtl. chronische Entzündungen verursachen und die Wahrscheinlichkeit falsch negativer Resultate beim direkten Antigen-Nachweis erhöhen. Serologische Tests zum Nachweis verschiedener spezifischer Antikörper sind eine effektive und häufig angewandte Methode zur Feststellung von C. trachomatisInfektionen (10,11,18,19), Neue Technologien nutzen die Immunomarker IgM, IgA und IgG, um Bestehen und Stadium einer solchen Infektion zu charakterisieren. Spezifisches IgM ist ein Hinweis auf akute Chlamydia-Infektionen. Sein Fehlen schliesst jedoch - besonders in rezidivierenden und chronischen Fällen - eine Infektion nicht aus. Der Einsatz von spezifischem IgA als Marker für aktive Chlamydia-Infektionen erwies sich als wichtig wegen der kurzen Halbwertzeit, wohingegen es wohl erhalten bleibt, solange eine ausreichende antigene Stimulation vorhanden ist. IgA ist darüber hinaus wichtig als posttherapeutische Kontrolle. IgG ist ein Marker für Chlamydia-positive Immunantworten bei akuten, chronischen oder früheren Infektionen. Serologische Kreuzreaktionen zwischen den drei verschiedenen Spezies von Chlamydia treten auf. Die meisten serologischen Tests auf Chlamydia-AK benützen entweder gereinigte (MIF) Elementarteilchen, Lipopolysaccharide (LPS), oder gereinigtes Protein der Aussenmembran (MOMP) als Antigene. Genus-spezifische Epitope sind in all diesen Antigenen vorhanden, geringe SpeziesSpezifität ist daher die Folge. Des weiteren ist ein Grossteil der Bevölkerung mit C. pneumoniae (ohne klinisches Erscheinungsbild) durchseucht und das Vorkommen von anti-Chlamydia Antikörpern ist sehr häufig. Daher ist die Differenzierung zwischen C. pneumoniae und C. trachomatis-spezifischen Antikörpern mit Hilfe der üblichen serologischen Screeningverfahren (MIF, ELISA, EIA usw.) noch mangelhaft. Savyon® Diagnostics hat einen Test entwickelt, in welchem C. trachomatis Speziesspezifische Epitope, aus verschiedenen Serotypen stammend, in einem Enzyme Linked Immunosorbent Assay (ELISA) verwendet werden. Der Test schliesst Speziesüberkreuzende, reaktive Epitope aus und ermöglicht so die spezifische Bestimmung von C. trachomatis IgA und IgG-Antikörpern. - 16 - G Grundprinzip des Tests • SeroCT® platten sind mit C. trachomatis spezifischen Peptiden beschichtet. • Das zu untersuchende Serum wird verdünnt und in der vorbeschichteten SeroCT® Platte bei 37°C inkubiert. In diesem Durchgang werden C. trachomatis-spezifische Antikörper an die immobilisierten C. trachomatis-spezifischen Peptide gebunden. • Nicht spezifische Antikörper werden durch waschen entfernt. • Anti-Human IgG konjugiert mit Meerrettich Peroxidase „MRP“ (Horseradish P. „HRP“) wird zugesetzt und 1 Stunde bei 37°C inkubiert. In diesem Durchgang wird das MRP-Konjugat an den vorher gebildeten Antigen/Antikörper-Komplex gebunden. • Nicht gebundenes Konjugat wird durch waschen entfernt. • Nach zufügen des TMB Substrats wird das Substrat durch die Peroxidase hydrolysiert, was eine blaue Farbe des reduzierten Substrats ergibt. • Nach zufügen der Stoplösung erfolgt ein Farbumschlag nach gelb. Mit einem ELISA Photometer wird bei einer Wellenlänge von 450nm die Absorption gemessen. • Die Absorption ist proportional zur Menge der spezifischen Antikörper, die an die immobilisierten Peptide gebunden sind. Menschliche Seren, die in diesem Testkit enthalten sind wurden nach FDA- und CE genehmigten Methoden geprüft und als negativ für HBsAg-, HCV- und HIV- 1 & 2 Antikörper befunden. Dennoch ist bei Produkten menschlichen Ursprungs mit keinem der derzeit verfügbaren analytischen Verfahren mit letzter Sicherheit - 19 - auszuschließen, dass diese Krankheiten übertragen können. Daher müssen alle in diesem Testkit enthaltenen Reagenzien menschlichen Ursprungs nach den Empfehlungen, veröffentlicht in der CDC/NIH Anleitung „Biosafety in Micro Biological and Biomedical Laboratories 1988", grundsätzlich als potenziell infektiöses Serum oder Blut angesehen und entsprechend behandelt werden. 2. TMB Substratlösung ist ein Stoff, der Haut und Schleimhäute reizt. Direkter Kontakt ist zu vermeiden. 3. Verdünnte Schwefelsäure (1M) ist ein Reizmittel für Augen und Haut. Im Fall eines Kontaktes mit den Augen, spülen Sie sofort mit viel Wasser, und konsultieren Sie einen Arzt. Gießen Sie kein Wasser in das Produkt. 4. Alle Bestandteile dieses Kits wurden in der Lieferserie kalibriert und getestet. Es wird empfohlen, keine Bestandteile aus verschiedenen Chargen zu vermischen, da es die Ergebnisse beeinträchtigen könnte. Lagerung und Lagerfrist der Reagenzien 1. Alle gelieferten Reagenzien sollten bei 2-8°C gelagert werden.Ungeöffnete Reagenzfläschchen sind bis zu der auf der Verpackung angegebenen Verfallszeit stabil. Reagenzien die noch original verschlossen sind und die für einige Stunden der Aussentemperatur ausgesetzt waren, werden keinen Schaden erleiden. NICHT EINFRIEREN ! 2. Einmal geöffnet, hat der Kit eine Lagerfrist von 90 Tagen. 3. Nicht verwendete Streifen müssen in der Alutasche mit der Exsikkatorkarte erneut versiegelt werden, indem das offene Ende gefaltet und dicht mit Klebeband über die ganze Länge der Öffnung verschlossen wird. 4. Kristalle können sich in dem 20x konzentrierten Waschpuffer während der Kühllagerung bilden. Das ist nicht ungewöhnlich. Lösen Sie die Kristalle durch erwärmen des Puffers auf 37°C vor dem Verdünnen auf. Wenn bereits verdünnt, kann die Lösung bei 2-8°C bis zu 21 Tage gelagert werden. Gewinnung der Seren Der vorliegende Test wurde zur Untersuchung Serum konzipiert, Hyperlipämische, hämolysische, kontaminierte oder hitzeinaktivierte Seren können zu falschen Ergebnissen führen, Solches Material sollte mit diesem Test möglichst nicht untersucht werden. Die Gewinnung der Seren sollte fachgerecht erfolgen. Lagerung Serumproben sollten bei einer Temperatur von 2-8°C gelagert und innerhalb von höchstens 7 Tagen untersucht werden. Für längere Lagerung sollte 0.1% Natriumazid zugefügt werden und die Proben sollten bei einer Temperatur unter -20°C eingefroren werden. Grössere Volumina sollten in praktikable Aliquots geteilt werden. Häufigeres einfrieren und auftauen sollte vermieden werden. Nach auftauen der Proben sollten diese gut gemischt werden. Testablauf A. Vorbereitung der Reagenzien 1. Alle Bestandteile und klinischen Proben, die zu prüfen sind, müssen auf - 20 - G Zimmertemperatur gebracht werden. Die Positiv Kontrolle, Negativ Kontrolle und die klinischen Proben vor Verwendung gründlich mischen. 2. Die Gesamtzahl der zu testenden Proben feststellen. Ausser den zu testenden Proben, müssen in jedem Test zwei Vertiefungen für die Negativ Kontrolle und eine Vertiefung für die Positiv Kontrolle vorgesehen werden. 3. Entnehmen Sie die Mikrotiterplatte aus ihrer Alutasche indem Sie ein Ende nahe dem Verschluss abschneiden. Fixieren Sie die benötigte Anzahl von Streifen (gemäss der Zahl der zu testenden Proben) im Rahmen. 4. Verdünnen Sie das Waschpufferkonzentrat 1/20 mit doppelt entionisiertem Wasser oder aqua dest. Zum Beispiel, um 1 Liter Waschpuffer zuzubereiten, fügen sie 50 ml des Waschpufferkonzentrats zu 950ml von doppelt entionisiertem Wasser oder aqua dest. B. Inkubation der Serumproben und Kontrollen 5. Verdünnen Sie jedes Patientenserum 1/21 mit der mitgelieferten Serumverdünnung wie folgt: 10µl Patientenserum zu 200µl Serumverdünnung hinzufügen. 6. 50µl der Positiv Kontrolle, Negativ Kontrolle und der 1/21 verdünnten Seren in separate Vertiefungen des Teststreifens pipettieren. Die Negativ Kontrolle soll in zwei separate Vertiefungen pipettiert werden. In den Vertiefungen sollten keine Luftblasen sein! 7. Plattendeckel auflegen und für eine Stunde bei 37°C in einer feuchten Kammer inkubieren. 8. Flüssigkeitsinhalt aus den Vertiefungen entfernen. 9. Waschschritt: jede Vertiefung bis unter den Rand (300-350µl) mit Waschpuffer füllen und dann Flüssigkeit verwerfen. Wiederholen sie diesen Schritt noch zwei weitere Mal, also insgesamt sollte der Waschschritt dreimal erfolgen. 10. Die Streifen im Rahmen durch leichtes abklopfen auf sauberem Absorbierpapier trocknen. C. Inkubation mit Konjugat 11. Konzentriertes MRP-konjugiertes Anti-Human IgG soll kurz vor der Verwendung zur einer Arbeitslösung verdünnt werden. Verdünnen Sie das konzentrierte MRPkonjugierte Anti-Human IgG 1/300 mit Konjugatverdünnung. Zum Beispiel: Für zwei Streifen bereiten Sie 3ml verdünntes MRP-konjugiertes Anti-human IgG (10µl des konzentrierten MRP-konjugierten Anti-Human IgG werden mit 3ml Konjugatverdünnung gemischt). 12. 50µl des verdünnten Konjugats in jede Vertiefung pipettieren. Luftblasen vermeiden! 13. Plattendeckel auflegen und für eine Stunde bei 37°C in einer feuchten Kammer inkubieren. - 21 - 14. Flüssigkeitsinhalt entfernen und waschen, wie unter Ziffer 9 beschrieben. 15. Die Streifen im Rahmen durch leichtes abklopfen auf sauberem Absorbierpapier trocknen. D. Inkubation mit TMB-Substrat Anmerkung: Das TMB-Substrat ist in gebrauchsfertiger Form. 16. 100µl TMB-Substrat in jede Vertiefung pipettieren, Plattendeckel auflegen und 15 Minuten bei Raumtemperatur inkubieren. 17. Reaktion durch zufügen von 100µl Stoplösung in (1µ H2SO4) jede Vertiefung unterbrechen. E. Bewertung der Ergebnisse 18. Absorption bei 450nm messen und Ergebnisse aufzeichnen. Die Messung sollte innerhalb von 30 Minuten nach stoppen der chromogenen Reaktion erfolgen. Anmerkung: Vor dem Ablesen sind Luftblasen aus den Vertiefungen zu entfernen. Der Boden der Reaktionsstreifen sollte vorsichtig von Kondenswasser und eventuellen Verunreinigungen befreit werden. Testbewertung Für die Gültigkeit des Tests müssen folgende Kriterien erfüllt sein. Falls diese nicht erfüllt sind, ist der Test als ungültig anzusehen und sollte wiederholt werden. 1. Positiv Kontrolle: Der Absorptionswert soll ≥ 0.8 bei 450nm. 2. Negativ Kontrolle: Der durchschnittliche Absorptionswert der Negativ Kontrolle, zweifach durchgeführt, soll 0.1< NC ≤0.4 bei 450nm sein. Berechnung des Cut-Off-Wertes (COV) und des Cut-Off-Index (COI) Der Cut-Off-Wert wird laut folgender Formel berechnet: COV = NC x 2 NC = die durchscnittliche Absorption der zwei Negativ Kontrollen bei 450nm. Kein einzelner serologischer Test sollte für eine endgültige Diagnose herangezogen werden. Alle klinischen und Laborbefunde sollten in Betracht gezogen werden. 2. Proben, die zu früh während einer Erstinfektion entnommen wurden, enthalten möglicherweise keine nachweisbaren Antikörper. Falls Verdacht auf eine Chlamydia-Infektion besteht, sollte eine weitere Probe 14-21 Tage später entnommen und parallel mit der Originalprobe getestet werden. Usage Diagnostique IN VITRO Introduction Les chlamydiae sont des bactéries intracellulaires essentielles à gram négatif qui provoquent des affections aiguës et chroniques chez les mammifères et les oiseaux. Le genre Chlamydiae se compose de quatre espèces: C. trachomatis, C. pneumoniae, C. psittaci et C. pecorum (1-4). C. trachomatis se divise en 15 serovars (5-8). Les serovars A, B, Ba et C sont les agents infectieux à l'origine du trachome (9), la première cause de cécité évitable dans le tiers monde. Les serovars L1-L3 sont les agents infectieux à l'origine de la lymphogranulomatose vénérienne. Les serovars D-K sont couramment en cause dans les infections génitales sexuellement transmissibles dans le monde entier, infections telles que cervicite, endométrite, salpingite (10) chez les femmes, et urétrite (11) chez les hommes et les femmes. L'endométrite ou la salpingite peuvent mener à l'oblitération des trompes, entraînant une élévation du risque de grossesse extra utérine et de stérilité. Les infections génitales peuvent aussi apparaître sous forme d'infections persistantes sans symptômes cliniques. En général, ces infections sont traitées une fois diagnostiquées. Lorsqu'aucun traitement n'intervient, elles peuvent évoluer vers une grave inflammation chronique et aboutir à la stérilité à une grossesse ectopique, à des avortements à répétition et à des naissances prématurées. De plus, les nourrissons nés d'une mère infectée peuvent être infectés à leur tour durant l'accouchement et contracter une conjonctivite ou une pneumonie (12-14). La sérologie de C. trachomatis est plus intéressante dans les infections chroniques que dans les infections aiguës. C. pneumoniae est un agent pathogène important chez l'être humain, à l'origine de 10% des pneumonies contractées en communauté. Il est associé à des affections respiratoires aiguës, à la pneumonie, à l'asthme, à la bronchite, la pharyngite, au syndrome pulmonaire aigu de la drépanocytose, aux maladies coronariennes et au syndrome de Gullain-Barre (15-17). C. psittaci affecte un large éventail d'espèces, des mollusques aux oiseaux et aux mammifères et provoque également de graves pneumonies. Chez les animaux, C. - 27 - psittaci et C. pecorum peuvent induire divers syndromes tels que la pneumonie, l'entérite, la polysérite, l'encéphalite et la conjonctivite. Les analyses sérologiques ont abouti à une réponse fiable au problème posé par la détection des infections à C. trachomatis. Lorsque l'on soupçonne une infection déclarée, le prélèvement d'échantillon de serum limite le besoin de procéder à des techniques agressives, nécessaires au diagnostic direct des agents pathogènes. En cas d'infection de la partie inférieure des voies uro-génitales, il faut tenir compte des limites propres aux techniques de prélèvement, telles que l'efficacité du prélèvement par frottis et les difficultés de manipulation et de transport des échantillons. Le problème reste avant tout que la plupart des infections à Chlamydia sont asymptomatiques. Une telle infection peut donc durer longtemps, progresser le long des voies génitales supérieures et y causer une infection profonde et chronique et, de ce fait, augmenter la probabilité de résultats faussement négatifs au diagnostic direct des agents pathogènes. Le diagnostic sérologique de C .trachomatis par la détection de divers anticorps spécifiques représente désormais une technique efficace et souvent mise en œuvre (10, 11, 18, 19). Des technologies nouvelles et affinées utilisent les marqueurs de l'immunoglobuline IgM, IgA et IgG afin de déceler la présence de l'agent pathogène et la phase de l'infection. L'IgM spécifique indique une infection aiguë à Chlamydiae. Cependant, l'absence d'IgM n'exclut pas la présence d'une infection continue, surtout dans les cas d'affection récurrente et chronique. On a pu prouver que l'IgA spécifique est un marqueur indiquant une affection active à Chlamydiae, étant donné la brièveté de sa durée de vie et le fait que l'IgA n'est présente que tant qu'elle est stimulée par un antigène. De plus, l'IgA permet également un suivi post thérapeutique. Un taux d'IgG élevé marque une réponse immunitaire positive provenant d'une infection à Chlamydiae en cours, chronique ou passée. Les réactions croisées sérologiques sont fréquentes entre les différentes espèces de Chlamydiae. La plupart des analyses sérologiques destinées au diagnostic des Chlamydiae utilisent soit des corps élémentaires purifiés (analyses par microimmunofluorescence (MIF) et analyses (ELISA), soit des lipo-polysaccharides (LPS) ou des protéines purifiées des membranes externes (MOMP) comme antigènes: puisque les épitopes spécifiques de genre sont présents dans tous les antigènes ci-dessus, la spécificité des espèces est généralement faible. De plus, puisqu'une forte proportion de la population a subi une exposition à C. pneumoniae sans toutefois présenter de symptômes cliniques, les anticorps spécifiques des Chlamydiae sont très largement répandus. La différenciation entre les anticorps spécifiques de C. pneumoniae et ceux spécifiques de C. trachomatis à l'aide de tests de dépistage sérologiques conventionnels (MIF, ELISA, EIA etc.) s'avère donc déficiente. Savyon Diagnostics Ltd. a mis au point un test permettant le dépistage spécifique de C. trachomatis à l'aide d'une méthode immunoenzymatique (ELISA), capable de distinguer entre les différents anticorps des infections aux Chlamydiae décrits. Ce test utilise comme antigène des peptides spécifiques de C. trachomatis, ce qui exclut les épitopes des réactions croisées entre les espèces et permet donc de déceler de façon plus précise et plus spécifique les anticorps IgG et des IgA des infections à C. trachomatis. - 28 - F Principe du test • • • • • • • • Des plaques de micro-titration SeroCT™ sont enduites de peptides spécifiques de C. trachomatis. Le sérum à tester est dilué puis mis à incuber à 37°C dans les plaques pré-enduites du test SeroCT™. Lors de cette étape, les anticorps spécifiques de C. trachomatis de l'échantillon de sérum vont se fixer aux peptides spécifiques de C. trachomatis immobilisées. Les anticorps non spécifiques sont éliminés par lavage. On introduit une IgG anti-humaine conjuguée à de la peroxydase de raifort (HRP). Durant cette étape, le conjugué HRP va se fixer au complexe antigène-anticorps fixé auparavant. Le conjugué non fixé est éliminé par lavage. L'introduction de substrat de TMB provoque l'hydrolyse du substrat par la peroxydase, ce qui donne une solution bleue de substrat réduit. On introduit la solution d'arrêt, la couleur bleue vire au jaune, puis on la lit à l'aide d'un lecteur ELISA à une longueur d'onde de 450nm. L'absorbance est proportionnelle à la quantité d'anticorps spécifiques fixés par les peptides immobilisées. Précautions D'emploi Usage Diagnostique IN VITRO 1. Ce kit contient des sérums humains qui ont été testés par des techniques approuvées par la FDA, et CE –Ils ont été trouvés dépourvus d’antigène HBs,d’ac - 31 - VIH 1 et 2, et anti HCV ,comme aucune technique ne peux garantir l’innocuité absolue des matériels testés ,ceux-ci doivent donc être manipulés comme potentiellement infectieux- selon les recommandations publiées dans le manuel “Sécurité biologique dans les laboratoires de microbiologie et de biologie médicale, 1988” de CDC/NIH (National Institute of Health - Institut National de la Santé américain). 2. La solution de substrat TMB est une substance irritante pour la peau et les muqueuses. Eviter tout contact direct. 3. L’acide sulfrique dilué (1M) est un agent irritant pour les yeux et la peau. En cas de contact avec les yeux, laver immédiatement et abandamment avec de l’eau et consulter un spécialiste. Ne jamais verser de l’eau dans ce produit. Ne pas mélanger les composants de différents lots et ne pas utiliser d'éléments provenant d'autres fournisseurs. Conservation et Stabilité des Réactifs 1. Tous les composants de la trousse doivent être conservés entre 2- 8°C. Dans ces conditions, les réactifs non ouverts sont stables jusqu' à la date de péremption indiquée sur l'étiquette. L’exposition des composants, munis de leur bouchon d’origine ou scéllés, à la température ambiante durant quelques heures, ne peut pas endommager les réactifs. NE PAS CONGELER! 2. Une fois le coffret ouvert, les éléments se conservent pendant 90 jours. 3. Le sachet d'aluminium contenant les barrettes doit être soigneusement refermé sans oublier le sachet déshydratant. 4. Diluer le tampon de lavage concentré au 1/20 dans l'eau distillée. Exemple: pour une barrette, préparer 100ml de tampon de lavage (5ml de tampon de lavage concentré + 95ml d'eau distillée). La solution reconstituée est stable 21 jours à 2-8°C. Prélèvement des échantillons Recueillir les échantillons de sérum en respectant l'asepsie. Ne pas utiliser de sérums inactivés par la chaleur. Il est recommandé de ne pas utiliser de sérums présentant une lipémie ou troubles. Les particules et les précipités contenus dans le sérum peuvent fausser les résultats. De tels échantillons doivent être clarifiés par centrifugation ou filtrage avant analyse. Conservation: Les échantillons à tester peuvent être conserver 7 jours à 2-8°C (rajouter de de l’azide de sodium 0.1%). Si le dosage est prévu dans un délai plus long, conserver les prélèvements à -20°C. Il est recommandé de ne pas effectuer des décongélations successives, pour cela fractionner le sérum en plusieurs aliquots. Réalisation du test A. Préparation des Réactifs 1. Ramener tous les réactifs à température ambiante avant utilisation. Bien mélanger le contrôle positif, contrôle négatif et les échantillons à tester. - 32 - F 2. Déterminer le nombre total d’échantillons à analyser. Outre les échantillons, inclure les réactifs suivants dans chaque analyse: deux puits de contrôle négatif et un puits de contrôle positif. 3. Retirer la plaque de micro-titration de son sachet d’aluminium en coupant l’extrémité du sachet près de la partie scellée. Laisser le nombre de barrettes voulues dans le portoir de 96 puits. 4. Diluer le tampon de lavage concentré au 1/20 dans l'eau distillée. Exemple : pour une barrette, préparer 100 ml de tampon de lavage (5ml de tampon de lavage concentré + 95ml d'eau distillée). B. Incubation des échantillons et des contrôles 5. Diluer les sérums de patients au 1/21 de la manière suivante: ajouter 10µl de sérum de patient à 200µl de diluant des sérums. 6. Déposer 50µl de contrôle positif, 2 contrôles négatifs et les échantillons dilués au 1/21 selon le schéma de dépôt. Le contrôle négatif doit être déposé dans deux puits separés. 7. Recouvrir les barrettes avec le couvercle de plaque et incuber pendant 1 heure à 37°C en chambre humide. 8. Vider les puits. 9. Etapes de lavage: remplir,à ras-bord, chaque puit avec du liquide de lavage (300350µl) puis éliminer ce liquide. Répéter encore deux fois, pour un total de trois étapes de lavage. 10. Sécher les plaques et le portoir en les tampotant sur du papier absorbant. Répéter cette opération trois fois. C. Incubation du Conjugué 11. HRP- conjugué anti-human IgG doit être dilué juste avant l'utilisation. Diluer le conjugué au 1/300 avec le diluant du conjugué. Exemple: pour deux barrettes, diluer 10µl de HRP-conjugué anti-human IgG concentré dans 3ml de diluant du conjugué. 12. Déposer 50µl de conjugué dilué par puits. 13. Recouvrir les barrettes avec le couvercle de plaque et incuber pendant 1 heure à 37°C en chambre humide. 14. Vider les puits, les laver 3 fois et les sécher comme indiquer précédemment: étapes 9-10. D. Incubation du TMB-Substrat/Chromogène 15. Déposer 100µl de TMB-Substrat/Chromogène par puits. Recouvrir les barrettes avec le couvercle de plaque et incuber pendant 15 minutes à température ambiante. 16. Arrêter la réaction en ajoutant 100µl de solution d’arrêt (H2SO4) 1M par puits.. - 33 - E. Lecture 17. Essuyer soigneusement le dessous de la plaque et lire à 450nm dans les 30 minutes qui suivent l'arrêt de la réaction. Note: Eliminer les éventuelles bulles d’air présentes dans les cupules et essuyer soigneusement le dessous de la microplaque avant la lecture au spectrophotomètre. Critères d’acceptabilite du test Le test peut être validé d’après les conditions suivantes; Si ces conditions ne sont pas remplies, le test doit être refait. 1. Contrôle Positif: La valeur d’absorbance du contrôle positif doit être ≥ 0.8 à 450 nm. 2. Contrôle Négatif: La valeur d’absorbance du contrôle négatif effectué en double doit être êntre 0.1< NC ≤0.4 à 450nm. El kit SeroCTMR IgG es una nueva generación de Ensayo Inmunoenzimático (ELISA) cualitativo, basado en péptidos sintéticos específicos de Chlamydia trachomatis, para la detección de anticuerpos IgG contra C. trachomatis en suero humano. SeroCTMR se utiliza como auxiliar en el diagnóstico de una infección específica por C. trachomatis. SeroCTMR IgG debe usarse e interpretarse en conjunto con el ensayo SeroCTMR IgA. Para uso en diagnóstico In Vitro. Introducción Las Chlamydia son bacterias intracelulares obligadas gram negativas, que causan enfermedades crónicas y agudas. El género de Chlamydia comprende cuatro especies: C. trachomatis, C. pneumoniae, C. psittaci y C. pecorum (1-4). C. trachomatis incluye 15 serotipos (5-8). Los serotipos A, B, Ba y C están asociados con tracoma (9), la principal causa de cegueras tratables, siendo endémicos en el tercer mundo. Los serotipos L1-L3 se asocian con el linfogranuloma venéreo. Serotipos del D al K son la causa común de infecciones por transmisión sexual como: cervicitis, endometritis\salpingitis (10) en mujeres y uretritis (11) tanto en hombres como en mujeres. Endometritis\salpingitis puden causar oclusión de las trompas con un alto riesgo de embarazo extrauterino e infertilidad. Infecciones en los genitales pueden ser agudas y persistentes sin presentar en muchas ocasiones ningún síntoma. Generalmente estas infecciones son tratables una vez que se diagnostican. Sin embargo sin tratamiento, la infeccion puede progresar a inflamaciones severas y crónicas causando infertilidad, embarazos extrauterinos, inducción de abortos o nacimientos anticipados. Además, bebés de madres infectadas pueden contagiarse al nacer, causando conjuntivitis o neumonía (12-14). La serología de C. trachomatis es especialmente interesante en casos de infecciones crónicas. C. pneumoniae es un patógeno importante en infecciones respiratiorias humanas siendo el causante de hasta un 10% de los casos de neumonia adquirida por la comunidad. Se asocia con enfermedades respiratorias agudas. neumonía, asma, bronquitis, faringitis, síndrome agudo de pecho en anemia de células falcifornes, enfermedades coronarias y síndrome de Gullain-Barré (15-17). C. psittaci infecta un diverso rango de hospederos desde moluscos, aves y hasta mamíferos y también es la causa de severas neumonías. En animales, C. psittaci y C. pecorum son capaces de inducir diversos síndromes como neumonía, enteritis, poliserrositis, encefalitis y conjuntivitis. Ensayos serológicos, establecidos en muchos paises, han demostrado proveer una respuesta comprensiva en la detección de infecciones por C. Trachomatis. Cuando hay sospecha de infección arraigada, muestras de suero reducen la necesidad de procedimientos invasivos, requeridos para la detección directa de antígeno. En casos de infecciones urogenitales, limitaciones en el muestreo tales como la efectividad en el proceso de raspado, manipulación de la muestra y dificultades de transporte, deben ser - 39 - considerados. Sobre todo, debe recordarse que la mayoría de las infecciones clamidiales son asintomáticas. Por lo tanto, una infección puede persistir por un largo período y ascender al tracto urinario superior, causando infecciones profundas y crónicas, e incrementar la posibilidad de resultados negativos falsos en detección directa de antígeno. El ensayo serológico para C. trachomatis, mediante la detección de varios anticuerpos específicos, es hoy en día un método efectivo muy aceptado (10,11,18,19). Tecnologías nuevas y precisas se aplican a los inmunomarcadores IgM, IgA e IgG para caracterizar la presencia y el estado de la infección. Anticuerpos IgM específicos son un indicativo de infección aguda por Chlamydia. Sin embargo, su ausencia no excluye la presencia de una infección en curso, especialmente en casos recurrentes y crónicos. Se ha demostrado que el uso de IgA específica como marcador de infección clamidial activa tiene un papel importante debido a su limitada vida media, que persiste mientras exista la estimulación antigénica. Sin embargo, IgA es más adecuada para el seguimiento después de terapia. IgG es un marcador en respuestas inmunes positivas frente a Chlamydia en infecciones actuales, crónicas o pasadas. Reacciones serológicas cruzadas ocurren entre las tres especies diferentes de Chlamydia. La mayoría de los diagnósticos serológicos para Chlamydia utilizan como antígenos, cuerpos elementales purificados (microinmunofluorescencia (MIF) y ensayos de ELISA), lipopolisacáridos (LPS), o proteína principal purificada de la membrana exterior (MOMP). Epítopos específicos para todo el género estan presentes en todos los antígenos mencionados; es por ello que se observa muy baja especificidad. Además, ya que una larga proporción de la población ha estado expuesta a C. pneumoniae (sin signos clínicos), la prevalencia de anticuerpos para Chlamydia es muy alta. Es por ésto, que para la diferenciación de anticuerpos específicos frente a C. pneumoniae y C. trachomatis, el uso de métodos serólogicos convencionales, (MIF, ELISA, EIA, etc.) es insuficiente. Savyon® Diagnostics ha desarrollado un ensayo inmunoenzimático (ELISA) en el cual diferentes epítopos específicos de C. trachomatis , son usados como antígenos, para detectar respustas inmunes en humanos. El ensayo excluye reacciones cruzadas de otras especies y permite una detección precisa y específica de anticuerpos IgG e IgA contra C. trachomatis. Principio del ensayo * * * * * * Las placas de SeroCTMR están recubiertas con péptidos específicos de C. trachomatis. El suero a probar es diluido e incubado en las placas de SeroCTMR por 1h a 37ºC. En este paso los anticuerpos contra C. trachomatis se adhieren a los péptidos inmobilizados de C. trachomatis. Anticuerpos no especifícos se remuven con lavados. Este kit contiene suero humano que ha sido analizado por técnicas aprobadas por la FDA y CE, y se ha encontrado que es negativo para AgsHB, y para anticuerpos frente a VHC y VIH 1 y 2. Sin embargo, ya que no se conoce ningún método capaz de ofrecer una garantía absoluta de que productos derivados de sangre humana no transmiten infección, todos los componentes de sangre humana que se proporcionan en el kit deben manipularse como muestras de suero o sangre potencialmente infecciosas, de acuerdo con las recomendaciones publicadas en el manual CDC/NIH de "Biosafety in Micro Biological and Biomedical Laboratories" (Bioseguridad en laboratorios Microbiológicos y Biomédicos), 1988. 2. Todos los materiales que se proporcionan deben almacenarse de 2-8ºC. Los reactivos en los frascos cerrados son estables hasta la fecha de caducidad indicada en ellos. La exposición a temperatura ambiente durante unas pocas horas, de los componentes del kit aún cerrados de origen, no produce ningún daño. NO CONGELAR LOS REACTIVOS. 2. El kit caduca a los 90 días de haber sido abierto. 3. Las tiras de la placa no utilizadas, deben guardarse selladas en su bolsa de aluminio con la tarjeta desecativa, enrollando la bolsa a todo lo largo de la apertura y sellando con cinta adhesiva a todo lo largo. 4. Es possible que haya formación de cristales en la solución de lavado concentrada (20X) durante su almacenamiento en frío; ésto es perfectamente normal. Redisuelva los cristales calentando la solución a 37ºC antes de diluir. Una vez diluida, la solución puede almacenarse de 2-8ºC hasta por 21 días. Extracción del Suero El suero deberá prepararse de muestras colectadas asépticamente utilizando las técnicas estándar. Muestras inactivadas con calor no deben utilizarse. El uso de suero lipémico, túrbido o contaminado no es recomendado. Partículas o precipitados en el suero pueden causar resultados erróneos. Estas muestras deberán clarearse centrifugando o filtrando antes de someterlas al ensayo. Almacenamiento de las muestras Las muestran deben almacenarse de 2-8ºC hasta por 7 días (se recomienda agregar 0.1% de azida sódica). Si se prevé almacenamiento por largos periodos, las muestras deberán almacenarse a –20ºC en alícuotas. Evite descongelar y congelar repetidamente. Procedimiento del Ensayo A. Preparación de los reactivos 1. Permita que todos los reactivos y especímenes clínicos alcancen la temperatura ambiente. Mezcle bien las muestras y los controles positivo y negativo antes de usarlos. 2. Determine el número de muestras a probar. Además de las muestras, debe agregarse en cada prueba dos pocillos con Control Negativo y un pocillo con Control Positivo. - 44 - S 3. Saque la microplaca de su bolsa de aluminio cortándola cerca del cierre de la orilla sellada. Deje el número de tiras requerido (de acuerdo con el número de muestras a probar) en el marco de la microplaca. 4. Diluya la solución de lavado concentrada 1/20 con agua destilada o doblemente desionizada. Por ejemplo, para preparar un litro de solución de lavado, agregue 50ml de la solución de lavado concentrada a 950ml de agua destilada o doblemente desionizada. B. Incubación de las muestras y controles 5. Diluya el suero de cada paciente 1/21 con Diluyente de Suero, de la siguiente manera: Agregue 10µl del suero del paciente a 200µl del Diluyente de Suero. 6. Agregue en pocillos separados, 50µl del Control Positivo, del Control Negativo y del suero diluido 1/21. El Control Negativo debe ser agregado a dos pocillos separados. 7. Cubra las tiras con la cubierta de placa e incube 1h a 37ºC en una cámara húmeda. 8. Descarte el líquido contenido en los pocillos. 9. Lavado: Llene completamente cada pocillo con solución de lavado (300-350µl) y descarte el líquido; repita este paso dos veces más, para un total de tres lavados. 10. Seque el marco y las tiras golpeando suavemente sobre papel absorbente. C. Incubación con el Conjugado 11. El Conjugado-HRP concentrado debe diluirse inmediatamente antes de usarse. Diluya el Conjugado-HRP concentrado 1/300 con Diluyente de Conjugado. Por ejemplo, para dos tiras prepare un mínimo de 3ml de conjugado-HRP diluido (10µl de Conjugado-HRP concentrado mezclado con 3ml de Diluyente de Conjugado). 12. Administre 50µl del conjugado diluido en cada pocillo. 13. Cubra las tiras con la cubierta de placa e incube 1h a 37ºC en una cámara húmeda. 14. Descarte el líquido contenido en los pocillos y lave como se describe en los pasos 9 y 10. D. Incubación con el Substrato-TMB 15. Administre 100µl del Substrato-TMB en cada pocillo, cubra las tiras con la cubierta de placa e incube a temperatura ambiente por 15 minutos. 16. Pare la reacción agregando a cada pocillo 100µl de solución final (H2SO4 1M). E. Determinación de los Resultados 17. Determine la absorbancia a 450nm y guarde los resultados. La lectura no debe ejecutarse después de 30 minutos de haber parado la reacción cromogénica. Nota: Toda burbuja de aire debe ser removida antes de la lectura. La base de la placa de ELISA debe limpiarse con cuidado. - 45 - Validacion Del Ensayo Para que el ensayo sea válido debe cumplir con los siguientes criterios. En caso de no cumplirse estos criterios, el ensayo debe considerarse inválido y deberá repetirse. 1. Control Positivo: La absorbancia deberá ser ≥ 0.8 a 450nm. 2. Control Negativo: El promedio de la absorbancia del Control Negativo (CN) efectuado en duplicado deberá ser 0.1<CN≤ 0.4 a 450nm. Cálculo del valor del punto de corte (VPC) y del índice del punto de corte (IPC) El valor del punto de corte se calcula de acuerdo con la siguiente fórmula: VPC=CN x 2 CN= Absorbancia promedio del Control Negativo a 450nm ensayado en duplicado. SeroCT™ è da usarsi come aiuto nella diagnosi di infezione da C.trachomatis. SeroCT™– IgG è progettato per essere eseguito e interpretato in congiunzione con il kit Savyon® SeroCT™ – IgA. Per uso Diagnostico in Vitro Introduzione Chlamydia è un batterio gram negativo intracellulare obbligato che causa malattie acute e croniche nei mammiferi negli uccelli. Il genere Chlamydia comprende quattro specie C.trachomatis, C.pneumoniae, C.psittaci e C.pecorum (1- 4). C.trachomatis si divide in 15 serotipi (5-8). I serotipi A, B, Ba e C sono agenti del tracoma (9), la maggiore causa di cecità prevenibile, endemica nei paesi del terzo mondo. I serotipi L1-L3 sono agenti di linfogranuloma venereo. I serotipi D-K sono causa comune in tutto il mondo di infezioni genitali sessualmente trasmesse: cervicite, endometrite/salpingite (10) nelle donne e uretrite (11) in uomini e donne. L’endometrite/salpingite può portare a occlusione tubarica con aumentato rischio di gravidanza extrauterina e infertilità. L’infezione genitale può causare un’infezione acuta e persistente occasionalmente senza alcun segnale clinico. Generalmente, queste infezioni sono trattabili una volta diagnosticate. Senza trattamento l’infezione può progredire fino a infiammazione cronica grave che porta a infertilità, gravidanza ectopica, aborto indotto o parto prematuro. Inoltre, i neonati di madri infette possono essersi infettati durante il parto con rischio di sviluppare congiuntivite o polmonite (12-14). La serologia di C.trachomatis è più interessante nei casi di infezioni croniche che nelle infezioni acute. C.pneumoniae è un patogeno importante delle vie respiratorie nell’uomo e causa fino al 10% dei casi di polmonite acquisita in comunità. È stata associata a malattie respiratorie acute: polmonite, asma, bronchite, faringite, ma anche a sindrome toracica acuta dell’anemia falciforme, malattia coronaria e Sindrome di Gullain-Barré (15-17). C.psittaci infetta una diversa gamma di specie ospiti dai molluschi agli uccelli ai mammiferi e causa anche polmoniti gravi. Negli animali, C.psittaci e C.pecorum possono indurre sindromi diverse, come polmonite, enterite, poliserosite, encefalite e congiuntivite. I test serologici, ora un approccio consolidato in molti paesi, si sono dimostrati capaci di fornire una risposta globale per la determinazione di infezione da C.trachomatis. Nel sospetto di infezioni profonde, il siero come cam-pione riduce la necessità di procedure invasive che sono richieste per il rilevamento diretto dell’antigene. In caso di infezioni del tratto urogenitale inferiore, vanno valutate le limitazioni al prelievo come la procedura di raschiatura del campione, e le difficoltà di manipolazione e di trasporto del campione stesso. Soprattutto, rimane il fatto che la maggioranza delle infezioni da clamidia sono asintomatiche. Quindi, un’infezione può - 51 - persistere per lungo tempo, può risalire il tratto genitale superiore causando infezioni profonde e croniche e aumentare la probabilità di risultati falsamente negativi nei test di rilevazione diretta dell’antigene. I test serologici per C.trachomatis, con la ricerca di vari anticorpi specifici, sono oggi una metodologia efficace e largamente accettata (10, 11, 18, 19). Nuove e accurate tecnologie impieganoi marker immunologici IgM, IgA e IgG per caratterizzare la presenza e lo stadio dell’infezione. Le IgM specifiche sono indice di infezione acuta da clamidia. L’assenza tuttavia non esclude un’infezione in atto specialmente nei casi ricorrenti e cronici. L’uso delle IgA specifiche come marker per le infezioni attive da clamidia si è rivelato svolgere un ruolo importante per la breve emivita delle IgA stesse, che persistono finchè esiste una stimolazione antigenica. Le IgA sono, comunque, più adatte per il controllo post terapia. Le IgG sono un marker di risposta immune positiva sia nelle infezioni correnti sia in quelle croniche o pregresse. Crossreazioni serologiche avvengono fra le tre specie di clamidia. La maggior parte dei test diagnostici serologici per clamidia usa ELISA o MicroImmunoFluorescenza (MIF) con corpi elementari purificati, lipopolisaccaridi (LPS), o proteine di membrana purificate (major outer membrane protein, MOMP) quali antigeni. Epitopi genere specifici sono presenti in tutti gli antigeni sopra menzionati, quindi, si osserva una bassa specie specificità. Inoltre una larga parte della popolazione è stata esposta a C.pneumoniae (senza evidenza clinica), quindi la prevalenza di anticorpi anti-clamidia è molto alta. Quindi la differenziazione tra anticorpi specifici per C.pneumoniae e per C.trachomatis usando test serologici tradizionali di screening (MIF, ELISA, EIA ecc.) risulta insufficente. Savyon® Diagnostics Ltd ha sviluppato un test in cui epitopi specie specifici per C.trachomatis , derivati da diversi serotipi sono impiegati in un saggio immunoenzimatico (ELISA). Principio del Test * * * * * * * Le piastre di SeroCT™ sono rivestite con peptidi C.trachomatis specifici. Il siero da analizzare viene diluito e incubato con le piastre rivestite 1h a 37°C. In questo passaggio gli anticorpi specifici per C.trachomatis si legano ai peptidi di C.trachomatis immobilizzati. Gli anticorpi non specifici vengono rimossi con un lavaggio. Si aggiungono Anti-IgG umane coniugate con perossidasi di rafano (HRP) e si incuba 1 h a 37°C. In questo passaggio il coniugato HRP si lega al complesso antigene-anticorpo preformatosi. Il coniugato non legato viene rimosso con dei lavaggi. Si aggiunge il substrato TMB che viene idrolizzato dalla perossidasi producendo una soluzione blu di substrato ridotto. Dopo aggiunta di soluzione d’arresto,il colore blu vira al giallo e deve essere letto in un lettore per ELISA a 450nm. Avvertenze e Precauzioni Per Uso Diagnostico in Vitro 1. Questo kit contiene siero umano che è stato testato con metodiche approvate dall’FDA e CE, ed è risultato negativo per HBsAg, e per anticorpi anti-HCV e antiHIV 1 & 2. Poiché nessun metodo conociuto può dare assicurazione totale che i prodotti derivati da sangue umano non trasmettano infezioni, tutte le componenti derivate da sangue umano fornite in questo kit devono essere maneggiate come siero o sangue potenzialmente infettante, secondo le raccoman-dazioni pubblicate nel manuale del CDC/NIH "Biosafety in Micro Biological and Biomedical Laboratories", 1988. 2. Alcuni reagenti contengono sodio azide la cui quantità risulta all'interno dei limiti di concentrazione consentiti. Tuttavia, a causa dell'alta tossicità di questa sostanza, manipolare con estrema cautela, evitando ogni contatto di-retto. 3. La soluzione Substrato-TMB è irritante per la pelle e per le membrane mucose. Evitare il contatto diretto. 4. La soluzione di acido solforico (H2SO4 1M) è irritante per gli occhi e per la pelle. In caso di contatto con gli occhi, sciacquare immediatamente e abbondantemente con acqua e consultare un medico. 5. Tutte le componenti di questo kit sono state calibrate e testate per lotto. Non è raccomandabile mescolare componenti da lotti diversi in quanto potrebbe influenzare i risultati. - 55 - Conservazione e durata dei reagenti 1. Tutti i reagenti forniti devono essere conservati a 2-8°C. I flaconi non aperti sono stabili fino alla scadenza indicata in etichetta. L’esposizione di componenti ancora sigillate a temperatura ambiente per alcune ore non causa danno ai reagenti contenuti. NON CONGELARE! 2. Una volta aperto il kit ha una scadenza di 90 giorni. 3. Le strisce di pozzetti non utilizzate vanno risigillate nella busta di alluminio con il dessiccante, arrotolando l’estremità aperta e sigillando l’intera larghezza dell’apertura con nastro adesivo. 4. Cristalli possono formarsi nel Tampone di Lavaggio concentrato (20X) durante la conservazione refrigerata, ciò è perfettamente normale. Ridisciogliere i cristalli riscaldando il tampone a 37°C prima di diluire. La soluzione può essere conservata a 2-8°C fino a 21 giorni. Raccolta dei campioni di siero Preparare i sieri da campioni raccolti asetticamente usando metodiche standard. Non usare siero inattivato al calore. L’uso di siero lipemico, torbido o contaminato non è raccomandabile. Materiali particolati e precipitati nel siero possono causare risultati erronei. Tali campioni dovrebbero essere chiarificati per centrifugazione o filtrazione prima del test. Conservazione I campioni devono essere conservati a 2-8°C e saggiati entro 7 giorni (si raccomanda di aggiungere sodio azide 0,1%). Se si prevede una conservazione più lunga, aliquotare e conservare i campioni sotto -20°C. Evitare di congelare e scongelare ripetutamente. Procedimento del test A. Preparazione dei Reagenti 1. Portare tutte le componenti e i campioni a temperatura ambiente. Mescolare bene il Controllo Positivo, il Controllo Negativo e i campioni prima dell’uso. 2. Determinare il numero totale di campioni da analizzare. In aggiunta ai campioni, includere ogni volta due pozzetti per il Controllo Negativo e uno per il Controllo Positivo. 3. Estrarre la micropiastra dalla sua busta di allumino tagliando un’estremità vicino al sigillo. Lasciare le strisce necessarie (secondo il numero di campioni da ana-lizzare) nel portastrisce da 96 pozzetti. 4. Diluire il Tampone di Lavaggio Concentrato 1/20 con acqua bi-deionizzata o distillata. Ad esempio, per preparare un litro di Soluzione di Lavaggio, aggiungere 50 ml del Tampone di Lavaggio Concentrato a 950 ml di acqua distillata o bideionizzata. - 56 - I B. Incubazione dei sieri campione e dei controlli 5. Diluire ogni campione 1/21 con il Diluente del Siero fornito aggiungendo 10µl di siero del paziente a 200µl di Diluente del Siero. 6. Dispensare 50µl di Controllo Positivo, Controllo Negativo e sieri diluiti 1/21 in pozzetti separati. I controlli negativi vanno dispensati in doppio. 7. Coprire le strisce con un copripiastre e incubare 1h a 37°C in una camera umida. 8. Eliminare il liquido contenuto nei pozzetti. 9. Lavaggio: Riempire ciascun pozzetto con tampone di lavaggio (300-350µl) ed eliminare il liquido, Ripetere questo passaggio due volte per un totale di tre lavaggi. 10. Asciugare strisce e portastrisce sbattendole con attenzione su carta assorbente pulita. C. Incubazione con il coniugato 11. Il Coniugato-HRP Concentrato deve essere diluito a soluzione di lavoro subito prima dell’uso. Diluire 1/300 il coniugato-HRP concentrato anti-IgG con diluente del coniugato. Per esempio, per due strisce preparare un minimo di 3ml di coniugato-HRP diluito (10µl di Coniugato-HRP Concentrato mescolato con 3ml di Diluente del Coniugato). 12. Dispensare 50µl di coniugato diluito in ciascun pozzetto. 13. Coprire le strisce con un copripiastre e incubare 1h a 37°C in una camera umida. 14. Eliminare il liquido e lavare come descritto ai punti 9-10. D. Incubazione con il Substrato-TMB 15. Dispensare 100µl di Substrato-TMB in ogni pozzetto coprire le strisce e incubarea temperatura ambiente per 15 minuti. 16. Fermare la reazione aggiungendo 100µl di Soluzione d’Arresto (H2SO4 1M) in ogni pozzetto. E. Determinazione dei Risultati 17. Determinare l’assorbanza a 450nm e registrare i risultati. La lettura non dovrebbe protrarsi oltre i 30 minuti dall’arresto della reazione cromogena. Nota: Qualsiasi bolla d’aria deve essere rimossa prima della lettura. Il fondo della piastra ELISA deve essere pulito con cura. Validazione del Test Perchè il test sia valido devono essere rispettati i seguenti criteri. Se i seguenti criteri non sono rispettati il test va considerato non valido e dovrebbe essere ripetuto. 1. Controllo Positivo Il valore dell’assorbanza dovrebbe essere ≥0,8 a 450nm. 2. Controllo Negativo La media delle assorbanze dei controlli negativi dovrebbe essere 0,1<NC≤0,4 a 450nm. - 57 - Calcolo del Cut-Off (COV) e del Cut-Off Index (COI) Il valore di cut-off è calcolato in base alla formula seguente: COV = NC x 2 NC = La media delle assorbanze del Controllo Negativo a 450nm usato in duplicato. Per normalizzare I risultati ottenuti in sedute analitiche diverse, si calcola il cut-off index secondo la seguente formula: COI = Assorbanza del campione a 450nm COV. I campioni ottenuti troppo precocemente durante un’infezione primaria possono non contenere anticorpi rilevabili. Se si sospetta infezione da clamidia, si dovrebbe fare un secondo prelievo 14-21 giorni più tardi per analizzarlo in parallelo al campione originale.
