Public Salary Disclosure 2013 for Mississauga Halton CCAC

The Mississauga Halton CCAC has 15 employees on the Public Sector Salary
Disclosure (PSSD) 2013 list, compared to 14 employees in 2012.

Safe, effective health care at home is what Ontarians want. The Mississauga
Halton CCAC is the only organization in the region to coordinate the full
range of health and social services that residents need.

In fiscal 2012/13 (April 1, 2012 to March 31, 2013), Mississauga Halton
CCAC care coordinators designed personalized care plans and organized
needed services for 41,172 patients (caring for, on average, 14,143 patients
per week). Care coordinators arranged for a total of 2,437,224 care visits, an
increase of 13.5% from the previous fiscal year, in response to the higher
health care needs of our older patients.

In fiscal 2012/13, the Mississauga Halton CCAC supported patients and
families in the region, while keeping administration costs to 4.1 per cent of
total budget, and plant operations and IT at 4.3 per cent.

The Mississauga Halton CCAC provides quality care to patients, in fiscal year
2012/13, in an independent provincial survey, 96.25 per cent of patients and
caregivers “would recommend our services to family and friends.”

To support the shift from hospital-based care to the community, which is
what Ontarians want and is safer and more cost-effective, we are designing
new programs to help meet the current and future needs of residents
o In fiscal 2012/13, Mississauga Halton CCAC care coordinators designed
personalized care plans and coordinated services for patients so they
could leave hospital sooner, achieving an alternate level of care (ALC)
rate of 7 per cent. That means 93 out of 100 hospital spaces were
available for patients requiring hospital care.

The Mississauga Halton CCAC launched new Rapid Recovery services
to enable patients to leave hospital earlier and start rehabilitation
treatment at home sooner. In fact, when Rapid Recovery services
were first introduced, between December 2013 and January 2014, the
Mississauga Halton CCAC reduced the ALC rehabilitation rate from 13
per cent to 7 per cent, opening more spaces for new patients needing
hospital care.
Over the past year, the Mississauga Halton CAC was given responsibility for
additional programs to support residents.
o By working in partnership with hospitals and physicians in
HealthLinks, the Mississauga Halton CCAC provides patients with
intensive support to live at home safely and remain out of hospital.
The Mississauga Halton CCAC is a key partner to creating a sustainable
health system for residents in the region.
