The horseradish peroxidase-conjugated antibody preparation that is included in the test system is specific for human IgG heavy and light chains. Intended Use: This is an enzyme immunoassay (EIA) test system for the detection of antibodies to the extractable nuclear antigens Sm (Smith), RNP, SSA/Ro, SSB/La, Scl-70, and Jo-1 in human serum. The results from this assay can be used as an aid in the diagnosis of autoimmune diseases. After incubation with horseradish peroxidase conjugate, if results are positive, a stable three part complex is formed. This complex consists of horseradish peroxidase-conjugated anti-human antibody bound to human anti-ENA antibody, which is bound to the antigen stabilized on the plastic surface. Antibodies to extractable nuclear antigens (ENA) have been associated with several autoimmune syndromes, and appear to be of diagnostic and/or prognostic significance in systemic sclerosis (1, 2), mixed connective tissue disease (3-5), Sjögren’s syndrome (6, 7), polymyositis (8), dermatomyositis (9), systemic lupus erythematosus (5), and rheumatoid arthritis (10). The antinuclear antibody (ANA) test has been used as a screen for these antibodies, but the ANA does not indicate the specificity of the antibody, and antibodies to some ENA do not show a positive ANA test (11, 12). Thus, secondary confirmatory testing for antibodies to ENA is highly recommended (13). After another washing step, this complex is detected by adding a solution of tetramethylbenzidine (TMB) and H2O2 as a chromogenic substrate. The degree of color development in each well is proportional to the concentration of anti-ENA antibodies in each serum sample. Each microwell is read in a spectrophotometer at 450 nm. Sm RNP SSA SSB Scl-70 Jo-1 MULTIPARAMETER ANTIBODY SCREENING TEST FOR IN VITRO DIAGNOSTIC USE SYSTEM COMPONENTS (MATERIALS PROVIDED) Storage: All components should be stored under refrigeration between 2-10°C. Do not freeze. Stability: All components remain stable at least 12 months from date of manufacture. Do not use any component beyond its expiration date. The Sm (Smith) antigen was identified in 1966 by Tan and Kunkel as a saline soluble, non-histone glycoprotein, which is not dependent on DNA or RNA for its antigenicity (14). Antibodies to the Sm antigen are considered a specific serologic marker due to their high degree of specificity for systemic lupus erythematosus (SLE). These antibodies are seen in up to 30% of SLE patients, and have been associated with active renal disease and cerebritis (15-17). REACTIVE REAGENTS Extractable nuclear antigen coated microwell strips: Catalog No. 7008-09. Eight well strips coated with stabilized solutions of affinity purified extractable nuclear antigens. One eight well strip is sealed in each individual foil pouch. Antibodies to Sm antigen are frequently found in conjunction with antibodies to U1-RNP in the sera of patients with SLE (18, 19). In contrast to antibodies to Sm antigen, antibodies to RNP are not considered a specific serologic marker because they are found in patients with a variety of rheumatic diseases including SLE, scleroderma, Sjögren’s syndrome, and rheumatoid arthritis. However, high levels of antibody to RNP are highly associated with an overlap syndrome called mixed connective tissue disease (MCTD). Patients with MCTD are characterized by a combination of clinical features seen in SLE, scleroderma, and polymyositis. These patients frequently demonstrate a good response to corticosteroid treatment and have a lower incidence of renal disease when compared to patients with SLE (20, 21). SSA and SSB were originally described as nuclear RNAprotein antigens in patients with Sjögren’s syndrome (6, 7). Ro and La were described as cytoplasmic RNAprotein antigens in patients with SLE (22, 23). It is now widely accepted that SSA and Ro are analogous, SSB and La are analogous, and these antigens are found in both the nucleus and cytoplasm. Antibodies to SSA/Ro alone or SSA/Ro and SSB/La are found in up to 62% of patients with subacute cutaneous lupus (24), and in 85% of patients with Sjögren’s syndrome who develop vasculitis (25). Antibodies to SSA/Ro alone occur in patients who have a homozygous deficiency of the C2 complement fraction (26), in primary biliary cirrhosis patients who develop Sjögren’s syndrome (27), and in up to two thirds of patients with “ANA negative SLE” (28). The Scl-70 antigen has been identified as a cellular enzyme, DNA topoisomerase I, an auxiliary protein to DNA polymerase delta (29). Antibodies to Scl-70 have been reported in up to 56% of patients with progressive systemic sclerosis (PSS), particularly the subset of patients with diffuse scleroderma (30). These autoantibodies are considered a marker for PSS, since they are not seen in other connective tissue diseases. Antibodies to Jo-1, which is the cellular enzyme histidyl tRNA synthetase, are found in 25-30% of patients with polymyositis or dermatomyositis, but not in other myopathies (11, 31). Anti Jo-1 antibodies have also been shown to have a high association with interstitial lung disease seen in conjunction with myositis (31). Sample Diluent: Catalog No. 7015 (15 ml). Proprietary buffered sample diluent used to dilute patient samples. Enzyme Antibody Reagent - Human IgG heavy and light chain specific: Catalog No. 7009-09 (6 ml). Goat antihuman IgG (H&L) conjugated to horseradish peroxidase (HRP). Reagent is ready to use. Substrate Solution: Catalog No. 7035 (6 ml). HRP-specific enzyme substrate solution, containing stabilized 3,3’,5,5’-tetramethylbenzidine (TMB) and hydrogen peroxide (H2O2). Reagent is ready to use. Stopping Reagent: Catalog No. 7033 (6 ml). Proprietary stopping reagent for Immuno Concepts EIA test systems. Reagent is ready to use. CAUTION: Corrosive. This reagent contains hydrochloric and sulfuric acids (less than 3% each, by volume), and should be handled with care. Keep out of the reach of children. In case of contact with eyes, flush immediately and thoroughly with water and consult a physician. Never add water to this reagent. Multiparameter ENA Positive Control: Catalog No. 702109 (1 ml). Human positive control serum that contains antibodies to Sm, RNP, SSA/Ro, SSB/La, Scl-70, and Jo-1 antigens. This serum is at working dilution and is ready to use. This reagent contains 0.1% sodium azide as a preservative. ENA Negative Control: Catalog No. 7031 (1 ml). Human negative control serum that does not contain antibodies to Sm, RNP, SSA/Ro, SSB/La, Scl-70, or Jo-1 antigens. This serum is at working dilution and is ready to use. This reagent contains 0.1% sodium azide as a preservative. NON-REACTIVE COMPONENTS Holder for microwells Wash Buffer Solution: PBS Buffer: Catalog No. 1011. Phosphate-buffered saline powder (0.01 M, pH 7.4 ± 0.2). Each pouch contains sufficient buffer powder to make one liter. (Two pouches of buffer powder are supplied for each 96-microwell plate in complete test kits). PRINCIPLE OF THE TEST Wash Buffer Concentrate: Catalog No. 1031 (10 ml). 5% Tween 20 solution to be used in the wash buffer. (Two vials of buffer concentrate are supplied for each 96microwell plate in complete test kits). This test is a qualitative indirect EIA. Stabilized preparations of affinity-purified extractable nuclear antigens have been coated onto the surface of the microwells to serve as an antigenic substrate in this system. Dilutions of the patient samples are placed in the microwells and incubated, allowing specific antibodies in the sample to react with the antigen on the solid phase. Preparation: Dissolve one pouch of buffer powder in one liter of deionized or distilled water. Add the entire contents of one bottle of Wash Buffer Concentrate 2 to the dissolved PBS. Mix well and store refrigerated between 2-10°C for up to 4 weeks or until signs of contamination or other visible changes occur. Wash buffer solution must be at room temperature (19-23°C) before use. of hemolysis, icterus, lipemia, or microbial growth should not be used because these conditions may cause aberrant results. Specimens containing visible particulate matter should be clarified by centrifugation before testing. ADDITIONAL MATERIAL REQUIRED (BUT NOT PRO- Storage: Sera may be stored at 2-10°C up to one week. GENERAL PROCEDURAL NOTES 1. It is extremely important to have all kit components and serum samples at room temperature (19-23°C) before use. A full liter of wash buffer may require several hours to warm to 20°C after removal from the refrigerator. Incubation temperatures above or below the stated range may cause inaccurate results. Return unused samples and reagents to refrigerated storage after use. 2. Mix reagents well before use by gentle inversion. Do not vortex or shake reagents. Avoid foaming. 3. When preparing sample dilutions, pipette tips should be wiped prior to dispensing serum into specimen diluent. Excess sample adhering to the outside of the pipette tip will affect results. 4. The use of a multichannel pipettor is recommended because it provides more uniform reagent dispensing, incubation times, and reaction times. 5. Adequate washing of wells is extremely important. Inadequately washed wells will exhibit high background values, and may show false positive values. For manual washing, aspirate the contents of the wells, then fill each well with wash buffer solution. Avoid cross-contamination of the wells, particularly in the first wash after aspiration. Drain all of the wash buffer from the wells by inverting, then shaking residual wash buffer from the wells with a sharp “snapping” motion of the wrist. Repeat the filling and draining steps for a total of 3 to 5 washes. The wells should then be rapped vigorously on a paper towel or other absorbent material to remove all traces of residual wash buffer. The use of an automated microwell washing system will assure consistent washing of the wells, and is recommended. NOTE: Due to the various types of wash techniques and automated systems, the number of washes may be adjusted to obtain optimal results. Each laboratory should determine the most efficient number of washes for their washing system. 6. Inadequate removal of residual wash buffer can cause inconsistent color development. Microwell strips should be blotted on absorbent paper or towels to minimize residual wash buffer. 7. Timing of all steps is critical. All serum samples should be diluted before beginning the procedure, and they must be dispensed into the microwells in as short a period of time as possible (not more than five minutes). Batch sizes should be set so that specimen handling can be accomplished comfortably within this time period. The use of a multichannel pipettor facilitates the handling of samples and reagents, and is recommended. 8. With the exception of the last incubation (substrate solution), the start of each incubation period begins with completion of sample or reagent dispensing. The substrate solution incubation must be exactly five minutes for each well. All samples and reagents should be dispensed in the same sequence and at a constant rate. PRECAUTIONS 1. All human source materials used for this product have been tested and found negative (not repeatedly reactive) for antibodies to Human immuno-deficiency virus-1 (HIV-1), Human immunodeficiency virus-2 (HIV-2), hepatitis C virus (HCV), and for hepatitis B surface antigen (HBsAg) by FDA approved methods. However, no test method can offer complete assurance that HIV-1, HIV-2, hepatitis C, hepatitis B, or other infectious agents are absent. Thus, all kit materials should be handled in the same manner as potentially infectious materials. 2. All patient samples should be handled at the Biosafety Level 2 as recommended for any potentially infectious human serum or blood specimen in the Centers for Disease Control/National Institutes of Health Manual: Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. Dilution of the components or substitution of components other than those provided in this system may yield inconsistent results. 4. Sodium azide (0.1%) is used as a preservative in the control sera. Sodium azide may react with lead or copper plumbing and form highly explosive metal azides. When disposing of reagents, flush with ample volumes of tap water to prevent potential residues in plumbing. Sodium azide is a poison and may be toxic if ingested. 5. This kit is for in vitro diagnostic use. 6. Never pipette by mouth and avoid contact of reagents and specimens with skin and mucous membranes. If contact occurs, wash with a germicidal soap and copious amounts of water. 7. Do not smoke, eat, or drink in areas where specimens or kit reagents are handled. 8. Avoid splashing or generation of aerosols at all times. 9. Incubation times and temperatures other than those specified may give erroneous results. 10. Cross contamination of reagents or samples may give false results. Samples must remain confined to microwells during testing. 11. Reusable glassware must be washed and thoroughly rinsed free of detergents prior to use. All glassware must be clean and dry before use. 12. Bring all reagents, microwells, and specimens to room temperature (19-23°C) prior to use. 13. Wear disposable latex gloves when handling specimens and reagents, and wash hands thoroughly afterwards. 14. Microbial contamination of reagents or samples may give false results. 15. The stopping reagent is corrosive and may cause burns. This reagent contains hydrochloric and sulfuric acids (less than 3% each, by volume), and should be handled with care. Keep out of the reach of children. In case of contact with eyes, flush immediately and thoroughly with water and consult a physician. Never add water to this reagent. INTERPRETATION OF RESULTS QUALITY CONTROL 1. The blank control well should have an absorbance value of less than 0.250. Blank absorbance values greater than 0.250 indicate inadequate washing, or contamination of reagents. 2. The absorbance reading of the blank control well (row G) is subtracted from the absorbance readings of the RELISA® Procedure Check (RPC) well (row H) and each of the antigen wells (rows A through F). Net values less than zero are considered to be zero values. 3. The RELISA® Procedure Check (RPC) well (row H) should have a net absorbance greater than 0.300. Absorbance values less than 0.300 indicate inadequate color development, and an invalid run. Inadequate color development is usually due to the use of cold reagents or incorrect timing of one SPECIMEN COLLECTION Collection: Serum is the preferred specimen. Approximately 5 ml of whole blood should be collected aseptically by venipuncture using a sterile vacuum collection tube or other suitable collection system. Allow blood to clot at room temperature (19-23°C). Serum should be separated from the clot by centrifugation as soon as possible to minimize hemolysis. Allow reagents to warm to room temperature (19-23°C), and repeat the run with particular attention paid to the timing of all steps. Net absorbance values are each multiplied by 100 to obtain the value for each antigen in ENA Units. The positive control serum is a pool of human serum that contains antibodies to all six autoantigens in this test. Each antigen well should show a value of 30 ENA Units or more. The negative control serum is a pool of human serum that does not contain antibodies to any of the six autoantigens in this test. Each antigen well should show a value of less than 20 ENA Units. Each laboratory should establish the frequency for running positive and negative control sera, based on the frequency of patient tests and the laboratory’s experience with this assay. negative for antibodies to the other antigens. Based on these data, the normal cut-off values were established as less than 20 ENA Units. Good laboratory practice dictates that each laboratory must establish its own normal ranges based on its patient population and other local factors. LIMITATIONS OF TEST INTERPRETATION OF PATIENT RESULTS 1. This is a qualitative assay. The levels of antibody detected have no known clinical significance and the Unit values obtained in this assay are designed merely to separate patients into the following three broad groups. Patient sample wells that have calculated values greater than 30 ENA Units are considered to be positive. Patient sample wells that have calculated values less than 20 ENA Units are considered to be negative. Values between 20 Units and 30 Units are considered to be borderline positive and should be repeated. Each laboratory must establish its own reference range and cut-off values based on the population of patients that are being tested. Unit values are affected by patient factors, mechanical considerations (such as pipetting precision and accuracy), and assay conditions (such as temperature and timing of steps.) 2. The Sm and Sm/RNP wells are used together to determine the presence of these two autoantibodies. If the Sm/RNP well is positive, and the Sm well is negative, the patient has antibodies to RNP. If both wells are positive, with approximately equal values, the patient has antibodies to Sm. If both wells are positive, and the Sm/RNP well is 30 ENA Units or more higher than the Sm well, it is indicative of the presence of both Sm and RNP autoantibodies. The presence of both antibodies can be confirmed using Immuno Concepts AUTO-ID® immunodiffusion system, catalog number 6030. 3. The other wells are coated with affinity purified monospecific antigens and react only with the homologous antibody. However, patient sera with multiple antibody specificities are commonly seen in SLE and other autoimmune syndromes, so an individual serum may show a positive reaction in more than one well. 4. The wells are coated with affinity purified antigens in the following order, from top (well A, and the solid tab end of the strip) to bottom (well H, and the notched tab end of the strip): REPORTING OF RESULTS PERFORMANCE CHARACTERISTICS The Immuno Concepts RELISA® Screening Assay was compared to other similar ELISA assays in commercial distribution; to double immunodiffusion and counterimmunoelectrophoresis tests done in reference laboratories; and to immunoblotting (Western Blot) results obtained with an in-house method. The results of all methods and the clinical diagnosis of the patient were considered in determining the expected or “correct” results for each sample tested. Based on these comparisons, the data in Table 2 were obtained. The discrepancies in specificity are attributed to the increased sensitivity of ELISA assays in comparison to “traditional” methods such as counterimmuno-electrophoresis and immunodiffusion. CROSSREACTIVITY Seven samples were used for crossreactivity studies. These samples were well-characterized by Western Blot, CIE, and immunodiffusion as monospecific sera for each of the autoantibodies in the RELISA® Screening Test. No cross-reactivity was noted in any of these samples. See table 3. REPRODUCIBILITY For each of the six antigen specificities, six samples were run on three different lot numbers of antigen strips, on three different occasions. Two of the samples were negative, but close to the 20 ENA Unit cut-off value; two samples were positive, but close to the 20 ENA Unit cut-off value; and two samples were clearly positive above the 30 ENA Unit level. Diagnosis cannot be made on the basis of antibodies to extractable nuclear antigens alone. The physician must interpret these results in conjunction with the patient’s history and symptoms, the physical findings, and other diagnostic procedures. 2. Treatment should not be initiated on the sole basis of a positive test for antibodies to extractable nuclear antigens. Clinical indications, other laboratory findings, and the physician’s clinical impression must be considered before any treatment is initiated. 3. Some patients with autoimmune diseases may have undetectable or insignificant levels of antibodies to extractable nuclear antigens, and some individuals may have high levels of antibodies to extractable nuclear antigens, but little or no evidence of clinical disease. The physician must interpret the results of tests for antibodies to extractable nuclear antigens in conjunction with the patient’s history and symptoms, the physical findings, and other diagnostic procedures. 4. Prepare Wash Buffer Solution (PBS-Tween): Dissolve contents of one PBS buffer pouch in one liter of deionized or distilled water. Add the entire contents of one bottle of Wash Buffer Concentrate to the one liter container of dissolved PBS. Mix well. The wash buffer solution may be covered and stored at 2-10°C up to four weeks. 3. Dilute Patient Samples: Dilute patient samples 1:40 by adding 25 µl of serum to 975 µl of Sample Diluent. Mix well. Place the frame holder on a solid surface, with the letters A through H on the left and the numbers 1-12 on the top edge of the frame holder. Place the strips in the frame holder so the notched end of the strip fits over the small peg at the bottom edge of the frame holder. Press down firmly on both ends of the strips so that they securely snap into the frame holder. Strips that are properly seated in the frame holder will not fall out when the frame holder is inverted. Label the frosted top end of the strip to indicate the patient identification and position in the frame holder. 5. TEST DE DÉPISTAGE DES ANTICORPS À PARAMÈTRES MULTIPLES Sm RNP SSA SSB Scl-70 Jo-1 POUR UTILISATION DIAGNOSTIQUE IN VITRO RÉSUMÉ ET EXPLICATION DU TEST UTILISATION PRÉVUE : Il s’agit d’un système de test par immunodosage enzymatique (EIA) pour la détection des anticorps dirigés contre les antigènes nucléaires solubles Sm (Smith), RNP, SSA/Ro, SSB/La, Scl-70 et Jo-1 dans le sérum humain. Les résultats de ce dosage peuvent être utilisés comme aide au diagnostic des maladies auto-immunes. Les anticorps anti-antigènes nucléaires solubles (ENA) ont été associés à plusieurs syndromes auto-immuns et semblent avoir une valeur diagnostique et/ou pronostique dans la sclérodermie (1, 2), la connectivité mixte (3-5), le syndrome de Sjögren (6, 7), la polymyosite (8), la dermatomyosite (9), le lupus érythémateux disséminé (5) et la polyarthrite rhumatoïde (10). Le test ANA (anticorps anti-nucléaires) a été utilisé pour dépister ces anticorps mais il n’indique pas la spécificité de l’anticorps, et les anticorps dirigés contre certains antigènes nucléaires solubles ne sont pas positifs au test ANA (11, 12). Par conséquent, un second test de confirmation de la présence d’anticorps anti-antigènes nucléaires solubles est fortement recommandé (13). Après incubation avec le conjugué de peroxydase de raifort, si les résultats sont positifs, on observe la formation d’un complexe tripartite stable. Ce complexe se compose d’un anticorps anti-humain conjugué à de la peroxydase de raifort lié à un anticorps anti-ENA humain, lui-même lié à l’antigène stabilisé sur la surface en plastique. Après un deuxième lavage, ce complexe est détecté par ajout d’une solution de tétraméthylbenzidine (TMB) et de H2O2 servant de substrat chromogène. Le degré de développement de la couleur dans chaque puits est proportionnel à la concentration en anticorps anti-ENA dans chaque échantillon de sérum. Chaque micropuits est lu à l’aide d’un spectrophotomètre à 450 nm. L’antigène Sm (Smith) a été identifié en 1966 par Tan et Kunkel. Il s’agit d’une glycoprotéine non histone soluble saline, non dépendante de l’ADN ou de l’ARN quant à son antigénicité (14). Les anticorps anti-Sm sont considérés comme un marqueur très spécifique en raison de leur degré élevé de spécificité au lupus érythémateux disséminé (LED). Ils sont présents chez plus de 30 % des patients atteints d’un LED et ont été associés à la néphropathie évolutive et à la cérébrite (15-17). COMPOSITION DES SYSTÈMES (MATÉRIELS FOURNIS) Conservation : Tous les composants doivent être conservés au réfrigérateur entre 2 et 10 °C. Ne pas congeler. Stabilité : Tous les composants sont stables pendant 12 mois à partir de la date de fabrication. Ne pas utiliser les composants au-delà de leur date de péremption. L’anticorps Sm est souvent associé à l’anticorps U1-RNP dans les sérums de patients atteints d’un LED (18, 19). À l’inverse de l’anticorps Sm, l’anticorps RNP n’est pas considéré comme un marqueur sérologique spécifique car on observe sa présence chez les patients atteints de diverses maladies rhumatismales, notamment d’un LED, du syndrome de Sjögren et de polyarthrite rhumatoïde. Un taux élevé d’anticorps anti-RNP est toutefois souvent associé à un syndrome de chevauchement appelé connectivité mixte (MCTD). Les patients atteints d’une MCTD présentent la plupart du temps une association de signes cliniques observés dans le LED, la sclérodermie et la polymyosite. Ces patients réagissent souvent bien à un traitement corticostéroïde et présentent une incidence de néphropathie plus faible que les patients atteints de LED (20, 21). RÉACTIFS Bandelettes de micropuits recouvertes d’antigènes nucléaires solubles : Référence catalogue 7008-09. Bandelettes à huit puits recouvertes de solutions stabilisées d’antigènes nucléaires solubles purifiés par affinité. Chaque sachet métallisé contient une bandelette de huit puits emballée hermétiquement. Diluant pour échantillon : Référence catalogue 7015 (15 ml). Diluant tamponné propriétaire, utilisé pour diluer les échantillons de patient. Réactif immuno-enzymatique – Spécifique aux chaînes lourdes et légères d’IgG humaine : Référence catalogue 7009-09 (6 ml). Globuline de chèvre anti-IgG humaine (chaînes lourdes et légères) couplée à de la peroxydase de raifort (HRP). Le réactif est prêt à l’emploi. Les SSA et SSB ont initialement été décrits comme des antigènes nucléaires protéine-ARN chez les patients atteints du syndrome de Sjögren (6, 7). Les Ro et La ont été décrits comme des antigènes cytoplasmiques protéine-ARN chez les patients atteints de LED (22, 23). Il est maintenant largement reconnu que, d’une part, le SSA et le Ro et, d’autre part, le SSB et le La sont analogues et que ces antigènes se trouvent à la fois dans le noyau et le cytoplasme. Les anticorps anti-SSA/Ro seuls ou anti- SSA/Ro et SSB/La sont observés chez 62 % des patients souffrant de lupus cutané subaigu (24) et chez 85 % des patients atteints du syndrome de Sjögren qui développent une vascularite (25). Les anticorps anti-SSA/Ro seuls apparaissent chez les patients ayant un déficit homozygote de la fraction C2 (26), chez les patients souffrant de cirrhose biliaire primitive qui développent le syndrome de Sjögren (27) et chez deux tiers des patients atteints de « SLE négatif aux ANA » (28). Solution de substrat : Référence catalogue 7035 (6 ml). Solution de substrat enzymatique spécifique à la HRP, contenant de la 3,3’,5,5’-tétraméthylbenzidine (TMB) et du peroxyde d’hydrogène (H2O2) stabilisés. Le réactif est prêt à l’emploi. Réactif d’arrêt : Référence catalogue 7033 (6 ml). Réactif d’arrêt propriétaire pour les systèmes de test EIA d’Immuno Concepts. Le réactif est prêt à l’emploi. ATTENTION : Corrosif. Ce réactif contient des acides chlorhydrique et sulfurique (moins de 3 % chacun par volume) et doit être manipulé avec précaution. Conserver hors de portée des enfants. En cas de contact avec les yeux, laver immédiatement et abondamment avec de l’eau et consulter un spécialiste. Ne jamais ajouter d’eau à ce réactif. L’antigène Scl-70 a été identifié comme une enzyme cellulaire, l’ADN topoisomérase de type I, une protéine auxiliaire de l’ADN-polymérase delta (29). Les anticorps anti-Scl-70 ont été observés chez 56 % de patients souffrant de sclérodermie généralisée (PSS), particulièrement le sous-groupe de patients atteints de sclérodermie diffuse (30). Ces auto-anticorps sont considérés comme un marqueur de la sclérodermie car ils ne sont pas observés dans d’autres collagénoses. Contrôle positif ENA à paramètres multiples : Référence catalogue 7021-09 (1 ml). Sérum de contrôle positif humain qui contient des anticorps anti-antigènes Sm, RNP, SSA/Ro, SSB/La, Scl-70 et Jo-1. Ce sérum est prédilué et prêt à l’emploi. Ce réactif contient 0,1 % d’azide de sodium (conservateur). Les anticorps anti-Jo-1, qui est l’enzyme cellulaire histidyl-ARNt-synthétase, sont observés chez 25 à 30 % des patients atteints de polymyosite ou de dermatomyosite et non dans les autres myopathies (11, 31). Il a également été démontré que les anticorps anti-Jo-1 sont fortement corrélés à la pneumopathie interstitielle associée à la myosite (31). Contrôle négatif ENA : Référence catalogue 7031 (1 ml). Sérum de contrôle négatif humain qui ne contient pas d’anticorps anti-antigènes Sm, RNP, SSA/Ro, SSB/La, Scl70 ou Jo-1. Ce sérum est prédilué et prêt à l’emploi. Ce réactif contient 0,1 % d’azide de sodium (conservateur). COMPOSANTS NON RÉACTIFS PRINCIPE DU TEST Support pour micropuits 7 que celles spécifiées peuvent donner des résultats erronés. 10. La contamination croisée des réactifs ou des échantillons peut donner de faux résultats. Pendant le test, les échantillons doivent rester confinés dans les micropuits. 11. Concentré tampon de lavage : Référence catalogue 1031 (10 ml). Solution Tween 20 à 5 % à utiliser dans le tampon de lavage. Chaque système complet contient deux flacons de concentré tampon pour chaque plateau de 96 micropuits. Préparation : Dissoudre un sachet de poudre tampon dans un litre d’eau désionisée ou distillée. Ajouter tout le contenu d’une bouteille de concentré tampon de lavage au PBS dissous. Bien mélanger et conserver au réfrigérateur entre 2 et 10 °C pendant 4 semaines maximum ou jusqu’à ce que des signes de contamination ou de modifications visibles apparaissent. La solution tampon de lavage doit être à température ambiante (19-23 °C) avant utilisation. PRÉLÈVEMENT D’ÉCHANTILLONS MATÉRIEL SUPPLÉMENTAIRE REQUIS (MAIS NON FOURNI) Prélèvement : Le sérum est l’échantillon préférentiel. Environ 5 ml de sang entier doivent être prélevés de manière aseptique par ponction veineuse à l’aide d’un tube à prélèvement sous vide stérile ou tout autre système de prélèvement adapté. Laisser le sang coaguler à température ambiante (19-23°C). Le sérum doit être séparé du caillot par centrifugation aussi rapidement que possible, de façon à limiter l’hémolyse. Les échantillons contenant des particules visibles doivent être clarifiés par centrifugation avant de procéder au test. PRÉCAUTIONS 1. Tous les matériels d’origine humaine utilisés dans la composition de ce produit ont été testés et se sont révélés négatifs (non-réactivité répétée) vis-à-vis des anticorps des virus de l’immunodéficience humaine 1 et 2 (vih 1 et 2), de l’anticorps du virus de l’hépatite C (hcv) et de l’antigène de surface de l’hépatite B (hbsag), selon les méthodes approuvées par la fda. Néanmoins, aucune méthode de test ne peut assurer totalement l’absence de vih-1, vih-2, hcv, hbv ou d’autres agents infectieux. Par conséquent, tous les matériels du kit doivent être manipulés de la même manière que des matériels considérés comme potentiellement infectieux. 2. Tous les échantillons de patient doivent être manipulés conformément aux recommandations du niveau de biosécurité 2, comme pour tout échantillon de sérum ou de sang humain potentiellement infectieux, telles qu’indiquées dans le manuel du Centers for Disease Control/National Institutes of Health : Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. La dilution des composants ou l’utilisation de composants autres que ceux fournis dans ce système peut produire des résultats incohérents. 4. L’azide de sodium (0,1 %) est utilisé comme conservateur dans les sérums de contrôle. Il est possible que l’azide de sodium réagisse au contact des canalisations en plomb ou en cuivre et forme des azides métalliques extrêmement explosifs. Lors de l’élimination des réactifs, rincer abondamment les canalisations avec de l’eau afin d’éviter toute accumulation de résidus. L’azide de sodium est un poison et peut être toxique en cas d’ingestion. 5. Ce kit est destiné à une utilisation diagnostique in vitro. 6. Ne pas pipeter avec la bouche et éviter tout contact de la peau et des muqueuses avec les réactifs et les échantillons. En cas de contact, laver abondamment avec un savon germicide et de l’eau. 7. Ne pas fumer, manger ni boire dans les zones où des échantillons ou des réactifs du kit sont manipulés. 8. Éviter toute éclaboussure ou pulvérisation d’aérosols à tout moment. 9. Les durées et températures d’incubation autres 8 Conservation : Les sérums peuvent être conservés entre 2 et 10 °C pendant une semaine maximum. Si le test est reporté, ils doivent être congelés à -20 °C minimum. Le sérum ne doit pas être conservé dans un réfrigérateur ou un congélateur à dégivrage automatique. ATTENTION : Les congélations et décongélations successives des échantillons de patient peuvent induire des résultats faussement positifs ou faussement négatifs. REMARQUES GÉNÉRALES RELATIVES À LA PROCÉDURE 1. Il est extrêmement important que tous les composants du kit et les échantillons de sérum soient à température ambiante (19-23 °C) avant utilisation. Il faut à un litre entier de tampon de lavage plusieurs heures pour atteindre 20 °C à sa sortie du réfrigérateur. Les températures d’incubation au-dessus ou en dessous de la plage indiquée peuvent donner des résultats inexacts. Remettre les échantillons et les réactifs non utilisés au réfrigérateur après utilisation. 2. Avant utilisation, bien mélanger les réactifs en les retournant doucement de haut en bas. Ne pas créer de tourbillon dans les réactifs ou les secouer. Éviter la formation de mousse. 3. Lors de la préparation des dilutions d’échantillon, essuyer les embouts des pipettes avant de distribuer le sérum dans le diluant pour échantillon. Si un excès d’échantillon adhère à l’embout de la pipette, les résultats s’en trouveront affectés. 4. L’utilisation d’un pipeteur multicanaux est recommandée car il permet d’uniformiser la distribution du réactif ainsi que les durées d’incubation et de réaction. 5. Un lavage adéquat des puits est extrêmement important. Des puits mal lavés afficheront des valeurs de bruit de fond élevées ainsi que des valeurs faussement positives. Pour le lavage manuel, aspirer le contenu des puits puis remplir ceux-ci de solution tampon de lavage. Éviter la contamination croisée des puits, en particulier lors du premier lavage suivant l’aspiration. Vider toute la solution tampon de lavage des puits en les renversant puis en les secouant d’un geste sec du poignet afin d’éliminer tout résidu. Répéter ces étapes (remplissage/élimination) jusqu’à effectuer 3 à 5 lavages au total. Frapper ensuite vigoureusement les puits sur une serviette en papier ou une autre matière absorbante pour éliminer toute trace résiduelle de tampon de lavage. Pour un lavage homogène des puits, il est recommandé d’utiliser un système automatisé. REMARQUE : En raison des divers types de techniques de lavage et de systèmes automatisés, le nombre de lavages peut être adapté pour obtenir des résultats optimums. Chaque laboratoire doit déterminer le nombre de lavages le plus efficace pour son système de lavage. 6. Une élimination inadaptée des résidus de tampon de lavage peut conduire à un développement inadapté de la couleur. Sécher les bandelettes de micropuits sur du papier ou des serviettes absorbants afin d’éliminer au maximum les résidus de tampon de lavage. 7. Le respect du minutage de toutes les étapes est essentiel. Tous les échantillons de sérum doivent être dilués avant le début de la procédure et déposés dans les micropuits aussi rapidement que possible (moins de cinq minutes). La taille des lots doit être définie de manière à ce que la manipulation des échantillons puisse être accomplie sans précipitation dans ce laps de temps. Il est recommandé d’utiliser un pipeteur multicanaux qui facilite la manipulation des échantillons et des réactifs. 8. Les valeurs unitaires sont affectées par des facteurs liés aux patients, des considérations mécaniques (précision et exactitude du pipetage, etc.) et les conditions de dosage (ex. : température et minutage des étapes). 2. Les puits Sm et Sm/RNP sont utilisés ensemble pour déterminer la présence de ces deux auto-anticorps. Si le puits Sm/RNP est positif et si le puits Sm est négatif, le patient a des anticorps anti-RNP. Si les deux puits sont positifs, avec des valeurs quasiment égales, le patient a des anticorps anti-Sm. Le relevé de densité optique du puits de contrôle à blanc (ligne G) est soustrait des relevés de densité optique du puits de vérification de procédure (PRC) RELISA® (ligne H) et de chacun des puits d’antigène (lignes A à F). Les valeurs nettes inférieures à zéro sont considérées comme étant égales à zéro. 3. Le puits de vérification de procédure (RPC) RELISA® (ligne H) doit avoir une densité optique nette supérieure à 0,300. Les valeurs de densité optique inférieures à 0,300 indiquent un développement de la couleur inadéquat et une analyse non valide. Un développement inadéquat de la couleur est généralement dû à l’utilisation de réactifs froids ou au non respect du minutage d’une ou plusieurs étapes du dosage. Laisser tous les réactifs atteindre la température ambiante (19-23 °C) et répéter l’analyse en veillant à respecter le minutage de toutes les étapes. 4. Les valeurs de densité optique nettes sont toutes multipliées par 100 pour obtenir la valeur de chaque antigène en unités ENA. 5. Préparation du formulaire : Étiqueter le formulaire inclus dans le kit afin d’indiquer l’identification de chaque bandelette de micropuits à huit puits. 2. Préparation de la solution tampon de lavage (PBS-Tween) : Dissoudre le contenu d’un sachet de tampon PBS dans un litre d’eau désionisée ou distillée. Ajouter tout le contenu d’une bouteille de concentré tampon de lavage au récipient d’un litre de PBS dissous. Bien mélanger. La solution tampon de lavage peut être couverte et conservée à 2-10 °C pendant quatre semaines. 3. Dilution des échantillons des patients : Diluer les échantillons des patients à 1: 40 en ajoutant 25 µl de sérum à 975 µl de diluant pour échantillon. Bien mélanger. Les contrôles sont prédilués et ne doivent pas être dilués de nouveau. 4. Préparation des bandelettes de micropuits : Sortir le nombre requis de bandelettes de micropuits de leurs sachets et les placer dans le support. Les bandelettes de micropuits doivent être correctement placées sur le support. Placer le support sur une surface stable, les lettres A à H à gauche et les chiffres 1 à 12 sur le bord supérieur. Placer les bandelettes dans le support de sorte que l’extrémité dentelée de la bandelette s’enclenche sur la petite fiche à la base du support. Appuyer fermement sur les deux extrémités des bandelettes pour les enclencher correctement dans le support afin qu’elles ne tombent pas lors du retournement du support. Étiqueter l’extrémité supérieure glacée de la bandelette pour identification du patient et position dans le support. 5. Distribution des dilutions sériques : Déposer 50 µl d’échantillon de patient dilué dans chacun des huit puits d’une bandelette à paramètres multiples. 6. Incubation des bandelettes (30 minutes à température ambiante, soit 19-23 °C) : Mettre à incuber à température ambiante pendant 30 minutes. Les bandelettes doivent être protégées des courants d’air ou des variations de température pendant l’incubation. Recouvrir, le cas échéant, les bandelettes d’un film transparent ou d’une serviette en papier pour les protéger de la poussière ou d’autres corps étrangers. 7. 8. Lavage des bandelettes (voir Remarques générales relatives à la procédure 5 et 6) : Laver les puits 3 à 5 fois avec la solution tampon de lavage PBS-Tween. Pour le lavage manuel, aspirer le contenu des puits puis remplir ceux-ci de solution tampon de lavage. Éviter la contamination croisée des puits, en particulier lors du premier lavage suivant l’aspiration. Vider toute la solution tampon de lavage des puits en les renversant puis en les secouant d’un geste sec du poignet afin d’éliminer tout résidu. Répéter ces étapes (remplissage/élimination) jusqu’à effectuer 3 à 5 lavages au total. Frapper ensuite vigoureusement les puits sur une serviette en papier ou une autre matière absorbante pour éliminer toute trace résiduelle de tampon de lavage. 9. Incubation des bandelettes (30 minutes à température ambiante, soit 19-23 °C) : Mettre à incuber à température ambiante pendant 30 minutes. Les bandelettes doivent être protégées des courants d’air ou des variations de température pendant l’incubation. Recouvrir, le cas échéant, les bandelettes d’un film transparent ou d’une serviette en papier pour les protéger de la poussière ou d’autres corps étrangers. 10. Lavage des bandelettes : Laver les puits 3 à 5 fois avec la solution tampon de lavage PBS-Tween. Pour le lavage manuel, aspirer le contenu des puits puis remplir ceux-ci de solution tampon de lavage. Éviter la contamination croisée des puits, en particulier lors du premier lavage suivant l’aspiration. Vider toute la solution tampon de lavage des puits en les renversant puis en les secouant d’un geste sec du poignet afin d’éliminer tout résidu. Répéter ces étapes (remplissage/élimination) jusqu’à effectuer 3 à 5 lavages au total. Frapper ensuite vigoureusement les puits sur une serviette en papier ou une autre matière absorbante pour éliminer toute trace résiduelle de tampon de lavage. 11. Distribution de la solution de substrat : Utiliser un chronomètre pour respecter le minutage et déposer 50 µl de solution de substrat dans chacun des puits. La solution de substrat doit être ajoutée aux puits à un rythme constant, de manière à ce que l’incubation de chaque puits ait la même durée (cinq minutes). La solution de substrat mise à incuber avec des échantillons positifs se colore en bleu et celle mise à incuber avec des échantillons négatifs variera d’incolore à bleu très pale. 12. Incubation des bandelettes (exactement 5 minutes à température ambiante, soit 19-23 °C) : Mettre à incuber à température ambiante pendant exactement cinq minutes. Les bandelettes doivent être protégées des courants d’air ou des variations de température pendant l’incubation. 13. Distribution du réactif d’arrêt : Après incubation du premier puits pendant exactement cinq minutes, ajouter 50 µl de réactif d’arrêt dans chaque puits, dans le même ordre et au même rythme que pour l’ajout de solution de substrat. Dès l’ajout de réactif d’arrêt, la solution de substrat bleue devient jaune et la solution incolore demeure incolore. 14. Lecture de la densité optique des puits : Dans les 30 minutes suivant l’ajout de réactif d’arrêt, les puits doivent être lus à l’aide d’un spectrophotomètre lecteur de microplaques. Les puits sont lus à 450 nm par rapport au puits de contrôle à blanc. Si un spectrophotomètre à double longueur d’onde est disponible, la longueur d’onde pour le filtre de référence doit être réglée sur 600-650 nm. La lecture des micropuits sans filtre de référence donne des valeurs de densité optique plus élevées. I risultati di questa analisi possono essere usati come aiuto nella diagnosi delle autoimmunopatie. Gli anticorpi contro gli ENA (Extractable nuclear antigens, antigeni nucleari estraibili) sono stati associati a parecchie sindromi autoimmuni e sembrano avere un significato diagnostico e/o prognostico nella sclerosi sistemica progressiva,(1, 2) nella malattia mista del tessuto connettivo, (3-5) nella sindrome di Sjögren,(6, 7) nella polimiosite,(8) nella dermatomiosite,(9) nel lupus eritematoso sistemico(5) e nell’artrite reumatoide.(10) Il test ANA (Antinuclear antibody, anticorpi antinucleari) è stato usato come screening per questi anticorpi, ma l’ANA non indica la specificità dell’anticorpo e gli anticorpi contro alcuni ENA non mostrano test ANA positivi.(11, 12) Quindi è altamente raccomandata un’analisi secondaria di conferma per gli anticorpi contro gli ENA.(13) L’antigene Sm (Smith) fu identificato nel 1966 da Tan e Kunkel come soluzione fisiologica solubile, glicoproteina non istone non dipendente da DNA o RNA per la sua antigenicità.(14) Gli anticorpi anti-antigeni Sm sono considerati come uno specifico marcatore sierologico grazie al loro elevato grado di specificità per l’SLE (Systemic Lupus Erythematosus, lupus eritematoso sistemico). I livelli osservati di questi anticorpi che sono stati associati a malattia renale attiva e ad encefalite raggiungono fino al 30% dei pazienti.(15-17) Gli anticorpi anti-Sm si trovano di frequente in connessione con gli anticorpi diretti contro il complesso U1-RNP nel siero di pazienti affetti da SLE.(18, 19) A differenza degli anticorpi anti-antigene Sm, gli anticorpi anti-RNP non sono considerati un marcatore sierologico specifico in quanto si trovano in pazienti con una serie di malattie reumatiche compresi SLE, sclerodermia, sindrome di Sjögren e artrite reumatoide. Comunque, elevati livelli di anticorpi diretti contro l’RNP sono altamente associati ad una sindrome coincidente denominata MCTD (Mixed Connective Tissue Disease, malattia mista del tessuto connettivo). I pazienti con MCTD tipicamente mostrano un insieme di caratteristiche cliniche osservabili nell’SLE, nella sclerodermia e nella polimiosite. Questi pazienti spesso mostrano una buona risposta al trattamento corticosteroide ed hanno una minore incidenza di malattia renale in confronto ai pazienti affetti da SLE.(20, 21) SSA e SSB furono originariamente descritti come antigeni nucleari RNA-proteina nei pazienti con sindrome di Sjögren.(6, 7) Ro e La furono descritti come antigeni citoplasmatici RNA-proteina nei pazienti con SLE.(22, 23) È ora comunemente accettato che SSA e Ro siano analoghi, SSB e La siano analoghi e che questi antigeni si trovino nel nucleo e nel citoplasma. Anticorpi diretti verso l’SSA/Ro soltanto o l’SSA/Ro e l’SSB/La sono stati rilevati nel 62% di pazienti con lupus cutaneo subacuto,(24) e nell’85% di pazienti con sindrome di Sjögren che sviluppano vasculite.(25) Anticorpi diretti solamente verso l’SSA/Ro si presentano nei pazienti che hanno una deficienza a livello omozigotico della frazione del complemento C2,(26) nei pazienti con cirrosi biliare primaria che sviluppano la sindrome di Sjögren(27) e nei due terzi di pazienti con “SLE ANA negativo”.(28) L’antigene Scl-70 è stato identificato come enzima cellulare, topoisomerasi I del DNA, una proteina ausiliaria del DNA polimerasi delta.(29) Anticorpi anti-Scl-70 sono stati segnalati nel 56% dei pazienti con sclerosi progressiva sistemica (PSS), ed in particolare nel sottogruppo di pazienti con sclerodermia diffusa.(30). Questi anticorpi sono considerati come un marcatore per PSS, poiché non sono osservati in altre patologie del tessuto connettivo. Gli anticorpi anti-Jo-1, che è l’istidil tRNA sintetasi dell’enzima cellulare, sono presenti nel 25-30% dei pazienti con polimiosite o dermatomiosite, ma non con altre miopatie.(11, 31) È stato anche dimostrato che gli anticorpi diretti contro Jo-1 sono fortemente associati all’enfisema interstiziale osservato in connessione con la miosite.(31) Questa analisi è un EIA (Enzime Immunoassay, dosaggio immunoenzimatico) qualitativo indiretto. In questo sistema, la superficie dei micropozzetti è stata rivestita con preparati stabilizzati di antigeni nucleari estraibili purificati per affinità che fungono da substrato antigenico. Le diluizioni dei campioni del paziente sono messe nei micropozzetti ed incubate lasciando reagire gli specifici anticorpi presenti nel campione con l’antigene in fase solida. Dopo il lavaggio per rimuovere l’anticorpo non legato e le altre proteine del siero, i pozzetti sono incubati con anticorpi anti-umani di capra etichettati con perossidasi di rafano. Il preparato di anticorpi coniugati con perossidasi di rafano incluso in questo sistema di analisi è specifico per catene anti-IgG umane pesanti e leggere. Se i risultati sono positivi, dopo l’incubazione col coniugato di perossidasi di rafano si forma un complesso stabile in tre parti. Questo complesso consiste di anticorpo antiumano coniugato con perossidasi di rafano legante l’anticorpo anti-ENA umano che è legato all’antigene stabilizzato sulla superficie di plastica. Dopo un’altra fase di lavaggio, questo complesso viene rilevato aggiungendo una soluzione di tetrametilbenzidina (TMB) e H2O2 come substrato cromogenico. Il grado di viraggio del colore in ciascun pozzetto è proporzionale alla concentrazione di anticorpi anti-ENA in ciascun campione di siero. Ciascun micropozzetto viene letto con uno spettrofotometro a 450 nm. COMPONENTI DEL SISTEMA (materiali forniti) Conservazione: tutti i componenti vanno conservati alla temperatura di 2-10°C. Non congelare. Stabilità: tutti i componenti restano stabili per almeno 12 mesi dalla data di produzione. Non usare alcun componente oltre la data di scadenza. Otto strisce di pozzetti rivestiti di soluzioni stabilizzate di antigeni nucleari estraibili purificati per affinità. In ciascun sacchetto di alluminio è sigillata una striscia con otto pozzetti. Reagente enzimatico degli anticorpi (catena pesante e leggera anti-IgG umane): numero di catalogo 7009-09 (6 ml). Anti-IgG umane di capra (H&L) coniugate con perossidasi di rafano (HRP). Il reagente è pronto per l’uso. Soluzione di substrato: numero di catalogo 7035 (6 ml). Soluzione di substrato enzimatica HRP-specifica, contenente 3,3’,5,5’-tetrametilbenzidina (TMB) stabilizzata e perossido di idrogeno (H2O2). Il reagente è pronto per l’uso. Reagente bloccante: numero di catalogo 7033 (6 ml). Esclusivo reagente bloccante per sistemi di analisi EIA della Immuno Concepts. Il reagente è pronto per l’uso. ATTENZIONE: corrosivo. Questo reagente contiene acidi cloridrico e solforico (meno del 3% ciascuno in volume), e deve essere maneggiato con cura. Tenere fuori della portata dei bambini. Siero di controllo umano negativo che non contiene anticorpi diretti contro gli antigeni Sm, RNP, SSA/Ro, SSB/La, Scl-70 o Jo-1. Questo siero è alla diluizione operativa ed è pronto per l’uso. Questo reagente contiene sodio azide allo 0,1% come conservante. COMPONENTI NON REATTIVI Supporto per micropozzetti PRINCIPIO DEL TEST Soluzione tampone di lavaggio 12 pletamente liberata da ogni residuo di detergente. Prima dell’uso, tutta la vetreria di laboratorio deve essere pulita e asciutta. 12. Prima dell’uso, portare tutti i reagenti, i pozzetti e i campioni a temperatura ambiente (19-23°C). 13. Indossare guanti a perdere in lattice quando si maneggiano campioni e reagenti e dopo lavare accuratamente le mani. 14. Polvere salina tamponata al fosfato (0,01 M, pH 7,4 ± 0,2). Ciascuna busta contiene polvere tamponata sufficiente a fare 1 litro. (Nei kit per analisi completi, per ogni piastra con 96 micropozzetti sono forniti due sacchetti di polvere tamponata). Concentrato tampone di lavaggio: numero di catalogo 1031 (10 ml). Soluzione al 5% di Tween 20 da usare nel tampone di lavaggio. (Nei kit per analisi completi, per ogni piastra con 96 micropozzetti sono fornite due fiale di concentrato tamponato). Preparazione: sciogliere il contenuto di una busta di tampone in un litro di acqua deionizzata o distillata. Aggiungere l’intero contenuto di un flacone di concentrato di tampone di lavaggio al PBS sciolto. Mescolare bene e tenere alla temperatura di 2-10°C fino a 4 settimane o fino a che non si presentino segni di contaminazione o altri cambiamenti visibili. Prima dell’uso, la soluzione tampone di lavaggio deve essere a temperatura ambiente (19-23°C). ALTRI MATERIALI NECESSARI (MA NON FORNITI) Pipette volumetriche di precisione per l’erogazione di volumi da 25-1000 µl. Flacone morbido per erogare la soluzione tampone di lavaggio nei micropozzetti o un sistema di lavaggio automatico o semiautomatico per micropozzetti. Contenitore da un litro per soluzione tampone di lavaggio PBS. Acqua deionizzata o distillata. Spettrofotometro per la lettura della piastra, capace di rilevare la misurazione dell’assorbanza a 450 nm. Provette per preparare le diluizioni dei sieri. Carta bibula o assorbente. Pipettatrice multicanale in grado di erogare i volumi necessari in 8 pozzetti. Guanti a perdere in lattice. Timer da laboratorio. PRECAUZIONI 1. Tutti i materiali di origine umana usati per questo prodotto sono stati analizzati e trovati negativi (non ripetutamente reattivi) per gli anticorpi del virus dell’immunodeficienza umana tipo 1 (HIV-1), del virus della immunodeficienza umana tipo 2 (HIV-2), del virus dell’epatite C (HCV) e per l’antigene di superficie dell’epatite B (HBsAg) con metodi approvati dalla FDA. Nessun metodo di analisi può garantire con completa sicurezza che siano assenti HIV-1, HIV-2, epatite C, epatite B o altri agenti infettivi. Quindi, tutti i componenti del kit vanno maneggiati secondo le stesse modalità utilizzate per i materiali potenzialmente infettivi. 2. Tutti i campioni dei pazienti devono essere maneggiati osservando le precauzioni di sicurezza biologica di livello 2, come raccomandato per ogni siero umano potenzialmente infettivo o per campioni di sangue nel manuale pubblicato per i CDC/NIHM (Centers for Disease Control/National Institutes of Health Manual, Centri per il Controllo delle Infezioni/ Istituti Nazionali per la Sanità): Biosafety in Microbiological and Biomedical Laboratories, Edizione 1984. 3. La diluizione di componenti o la sostituzione di componenti diversi da quelli forniti in questo sistema può dare risultati non coerenti. 4. Il sodio azide (0,1%) viene usato come conservante nei sieri di controllo. Il sodio azide può reagire con le tubature in piombo o rame formando azoturi metallici altamente esplosivi. Quando si eliminano i reagenti, far scorrere grandi quantità di acqua del rubinetto per evitare la formazione di potenziali residui nelle tubature. Il sodio azide è un veleno e può essere tossico se ingerito. 5. Questo kit è per uso diagnostico in vitro. 6. Non pipettare mai con la bocca ed evitare il contatto dei reagenti e dei campioni con la cute e le mucose. In caso di contatto, lavare abbondantemente con un sapone germicida e acqua. 7. Non fumare, non mangiare o non bere nelle aree in cui sono maneggiati i campioni o i reagenti del kit. 8. Evitare sempre gli spruzzi e la formazione di aerosol. 9. Tempi e temperature di incubazione diversi da quelli specificati può dare risultati errati. 10. La contaminazione incrociata dei reagenti o dei campioni può dare origine a risultati falsi. Durante l’analisi i campioni devono rimanere confinati nei micropozzetti. 11. Prima dell’uso, la vetreria di laboratorio riutilizzabile deve essere lavata e sciacquata a fondo e com- RACCOLTA DI CAMPIONI Raccolta: il siero è il campione preferito. Devono essere prelevati con tecnica asettica circa 5 ml di sangue intero per venopuntura usando una provetta di raccolta sterile a vuoto o un altro sistema di raccolta adatto. Lasciare che il sangue si coaguli a temperatura ambiente (19-23°C). Non appena possibile, il siero deve essere separato dal coagulo per centrifugazione in modo da minimizzare l’emolisi. Sostanze interferenti: non vanno usati sieri che mostrano un alto grado di emolisi, ittero, lipemia o crescita microbica, perché queste condizioni possono provocare risultati atipici. Campioni contenenti sostanze particellari visibili vanno chiariti per centrifugazione prima dell’analisi. Conservazione: i sieri possono essere conservati a 2-10°C fino ad una settimana. Se l’analisi viene ulteriormente rimandata, i sieri devono essere conservati congelati a -20°C o a una temperatura inferiore. Il siero non deve essere conservato in frigoriferi autosbrinanti o in freezer. ATTENZIONE: il ripetuto congelamento e scongelamento dei campioni dei pazienti può dare origine a falsi risultati positivi o a falsi risultati negativi. NOTE PROCEDURALI GENERALI 1. Prima dell’uso è estremamente importante avere tutti i componenti del kit e i campioni di siero a temperatura ambiente (19-23°C). Per portare alla temperatura di 20°C un intero litro di tampone di lavaggio dopo averlo tolto dal frigo, possono essere necessarie parecchie ore. Temperature di incubazione superiori o inferiori alla gamma stabilita possono essere causa di risultati non accurati. Dopo l’uso conservare al freddo campioni e reagenti non usati. 2. Mescolare bene i reagenti prima dell’uso capovolgendoli con delicatezza. Non far vorticare i reagenti e non scuoterli. Evitare la formazione di schiuma. 3. Quando si preparano le diluizioni dei campioni, le punte delle pipette devono essere asciugate prima di erogare il siero nel diluente per campioni. L’eccesso di campione adeso alla parte esterna della punta della pipetta influenza i risultati. 4. Si consiglia l’uso di una pipettatrice multicanale perché garantisce erogazione, tempi di incubazione e tempi di reazione più uniformi. 5. Un adeguato lavaggio dei pozzetti è estremamente importante. Pozzetti non lavati adeguatamente mostreranno valori di fondo alti e possono dare falsi valori positivi. Per il lavaggio manuale, aspirare il contenuto dei pozzetti, poi riempirne ciascuno con soluzione tampone di lavaggio. Evitare la contaminazione incrociata dei pozzetti, particolarmente nel primo lavaggio dopo l’aspirazione. Drenare tutto il tampone di lavaggio dai pozzetti capovolgendoli, poi eliminare i residui con un brusco movimento “secco” del polso. Ripetere le fasi di riempimento e di drenaggio per 3-5 lavaggi in tutto. I pozzetti devono poi essere passati con forza su carta assorbente o altro materiale assorbente in modo da eliminare ogni traccia di residuo di tampone di lavaggio. L’uso di un sistema di lavaggio automatico dei micropozzetti ne assicurerà il giusto lavaggio ed è consigliato. NOTA: a causa dei vari tipi di tecniche di lavaggio e di sistemi automatici, il numero dei lavaggi può essere adeguato in modo da ottenere risultati ottimali. Ciascun laboratorio deve decidere il numero di lavaggi più efficace per il proprio sistema di lavaggio. 13 6. Una inadeguata rimozione dei residui del tampone di lavaggio può causare un viraggio incoerente del colore. Per ridurre al minimo i residui di tampone di lavaggio, le strisce di micropozzetti devono essere tamponate su carta assorbente. 7. I tempi di tutte le fasi sono cruciali. Tutti i campioni di siero devono essere diluiti prima di iniziare la procedura e devono essere distribuiti nei micropozzetti nel minor tempo possibile (non più di cinque minuti). Le dimensioni dei lotti vanno fissate in modo che la manipolazione del campione possa essere realizzata comodamente entro questo lasso di tempo. L’uso di una pipetta multicanale agevola il maneggiamento dei campioni e dei reagenti ed è consigliato. 8. Con la sola eccezione dell’ultima incubazione (soluzione di substrato), ogni periodo di incubazione inizia con il completamento dell’erogazione del campione o del reagente. L’incubazione della soluzione di substrato deve durare esattamente cinque minuti per ciascun pozzetto. Tutti i campioni e i reagenti devono essere distribuiti con la stessa sequenza e a tasso costante. INTERPRETAZIONE DEI RISULTATI CONTROLLO DELLA QUALITÀ 1. Il pozzetto di controllo del bianco deve avere un valore di assorbanza inferiore a 0,250. Valori di assorbanza del bianco superiori a 0,250 indicano un lavaggio inadeguato o contaminazione dei reagenti. 2. La lettura di assorbanza del pozzetto di controllo in bianco (fila G) viene sottratta dalle letture di assorbanza dei pozzetti di controllo della procedura RELISA® (RPC) (fila H) e di ciascun dei pozzetti dell’antigene (file da A ad F). I valori netti inferiori a zero sono considerati come valori zero. 3. Il pozzetto di controllo della RELISA® (RPC) (fila H) dovrebbe avere una assorbanza netta superiore a 0,300. Valori di assorbanza inferiori a 0,300 indicano un viraggio inadeguato del colore ed una sequenza non valida. Il viraggio inadeguato del colore in genere è dovuto all’uso di reagenti freddi o ad un tempo non esatto di una o più fasi dell’analisi. Portare i reagenti a temperatura ambiente (19-23°C) e ripetere la fase prestando particolare attenzione al tempo di tutte le fasi. 4. Per ottenere il valore di ciascun antigene in unità ENA, ciascun valore di assorbanza netto viene moltiplicato per 100. 5. Il siero di controllo positivo è un pool di siero umano contenente gli anticorpi diretti contro tutti i sei autoantigeni di questo test. Ciascun pozzetto di antigene dovrebbe avere un valore di 30 ENA unità o più. 6. Il siero di controllo negativo è un pool di siero umano che non contiene gli anticorpi diretti contro uno dei sei autoantigeni di questa analisi. Ciascun pozzetto di antigene dovrebbe avere un valore inferiore a 20 unità ENA. 7. Ciascun laboratorio deve fissare la frequenza di esecuzione dei sieri di controllo positivi e negativi in base alla frequenza dei test dei pazienti e all’esperienza dello stesso laboratorio con questa analisi. Interpretazione dei risultati del paziente 1. Questa è un’analisi qualitativa. I livelli di anticorpi rilevati non hanno alcun significato clinico noto e i valori delle unità ottenuti in questa analisi tendono semplicemente a dividere i pazienti nei tre ampi gruppi che seguono. I pozzetti di campioni di pazienti che hanno valori calcolati superiori a 30 unità ENA sono considerati positivi. I pozzetti di campioni di pazienti che hanno valori calcolati inferiori a 20 unità ENA sono considerati negativi. I valori tra 20 e 30 unità sono considerati al limite (borderline) della positività e vanno ripetuti. Ciascun laboratorio deve fissare il propria range di riferimento e i propri valori di cut-off sulla base dei gruppi di pazienti oggetto del test. I valori delle unità sono influenzati da fattori inerenti ai pazienti, da situazioni meccaniche (come ad esempio, la precisione e l’accuratezza della pipettatura) e dalle condizioni di analisi (come ad esempio, la temperatura e il tempo delle fasi). 2. I pozzetti Sm e Sm/RNP sono usati assieme per definire la presenza di questi due autoanticorpi. Se il pozzetto Sm/RNP è positivo e quello Sm è negativo, il paziente ha anticorpi diretti contro l’RNP. Se entrambi i pozzetti sono positivi, con valori quasi uguali, il paziente ha anticorpi anti-Sm. Il medico deve interpretare i risultati delle analisi degli anticorpi anti-antigeni nucleari estraibili confrontandoli con l’anamnesi e i sintomi del paziente, i dati fisici e altre procedure diagnostiche. 4. I livelli di anticorpi rilevati con questo sistema di analisi non indicano necessariamente la gravità o la durata della patologia. CARATTERISTICHE PRESTAZIONALI Il test di screening RELISA® della Immuno Concepts fu confrontato con altre analisi ELISA simili presenti sul mercato, con test doppi ad immunodiffusione e controimmunoelettroforesi eseguiti in laboratori di riferimento e con i risultati del metodo immunoblot (Western Blot) ottenuti con un metodo in-house. Preparazione del modulo di protocollo: specificare nel modulo di protocollo accluso nel kit l’identificazione di ciascuna delle otto strisce di micropozzetti. 2. Preparazione della soluzione di lavaggio tampone (PBS-Tween): sciogliere il contenuto di una busta di tampone PBS in un litro di acqua deionizzata o distillata. Aggiungere l’intero contenuto di un flacone di concentrato di tampone di lavaggio al contenitore di PBS sciolto. Mescolare bene. La soluzione tampone di lavaggio può essere coperta e conservata a 2-10°C fino a quattro settimane. 3. Diluizione dei campioni dei pazienti: diluire i campioni del paziente a 1:40 aggiungendo 25 µl di siero a 975 µl di diluente per campioni. Mescolare bene. I controlli sono forniti alla diluizione di lavoro e non ne richiedono una ulteriore. 4. Preparazione delle strisce di micropozzetti: rimuovere i micropozzetti necessari dalle rispettive buste e metterli nel supporto. I micropozzetti devono essere sistemati saldamente nel supporto. Mettere il supporto su una superficie piana con le lettere da A a H sulla sinistra e i numeri 1-12 sul bordo superiore del supporto. Mettere le strisce nel supporto in modo che l’estremità dentellata della striscia coincida col piccolo cavicchio al margine inferiore del supporto. Premere con forza su entrambe le estremità delle strisce in modo che si blocchino correttamente nel supporto. I pozzetti ben sistemati nel supporto non cadranno quando il supporto si capovolge. Mettere l’etichetta all’estremità congelata della striscia per indicare l’identificazione e la posizione nel supporto. 5. Erogazione delle diluizioni del siero: erogare 50 µl di campione diluito del paziente in ciascuno degli otto pozzetti di una striscia multiparametro. 6. Incubazione delle strisce (30 minuti a temperatura ambiente, ovvero 19-23°C): incubare a temperatura ambiente per 30 minuti. Durante l’incubazione, le strisce devono essere protette da correnti d’aria o variazioni di temperatura. Le strisce possono essere eventualmente coperte con nastro trasparente o con carta assorbente per proteggerle dalla polvere o da altri corpi estranei. 7. Lavaggio delle strisce (consultare le note procedurali generali 5 e 6): lavare i pozzetti da 3 a 5 volte con soluzione di lavaggio tampone PBS-Tween. Per il lavaggio manuale, aspirare il contenuto dei pozzetti, poi riempirne ciascuno con soluzione tampone di lavaggio. Evitare la contaminazione incrociata dei pozzetti, particolarmente nel primo lavaggio dopo l’aspirazione. Drenare tutto il tampone di lavaggio dai pozzetti capovolgendoli, poi eliminare i residui con un brusco movimento “secco” del polso. Ripetere le fasi di riempimento e di drenaggio per 3-5 lavaggi in tutto. I pozzetti devono poi essere passati con forza su carta assorbente o altro materiale assorbente in modo da eliminare ogni traccia di residuo di tampone di lavaggio. 8. Erogazione del reagente anticorpo enzimatico: erogare 50 µl di reagente anticorpo enzimatico in ciascun pozzetto. 16 9. Incubazione delle strisce (30 minuti a temperatura ambiente, ovvero 19-23°C): incubare a temperatura ambiente per 30 minuti. Durante l’incubazione, le strisce devono essere protette da correnti d’aria o variazioni di temperatura. La soluzione substrato deve essere aggiunta a tasso regolare in modo che ciascun pozzetto sia incubato esattamente per lo stesso tempo (cinque minuti). La soluzione substrato nei pozzetti incubati con campioni positivi virerà al blu e la soluzione nei pozzetti incubati con campioni negativi sarà da incolore a blu molto chiaro. 12. Incubazione delle strisce (5 minuti esatti a temperatura ambiente, ovvero 19-23°C): incubare a temperatura ambiente per cinque minuti. Durante l’incubazione, le strisce devono essere protette da correnti d’aria o variazioni di temperatura. 13. Erogazione del reagente bloccante: dopo che il primo pozzetto è stato incubato per cinque minuti esatti, aggiungere 50 µ di reagente bloccante nello stesso ordine e allo stesso tasso con cui era stato aggiunto ai pozzetti la soluzione substrato. Al momento dell’aggiunta del reagente bloccante, la soluzione di substrato blu vira al giallo e quella incolore resta incolore. 14. Lettura dell’assorbanza dei pozzetti: entro 30 minuti dall’aggiunta del reagente bloccante, i pozzetti devono essere letti in uno spettrofotometro per la lettura della piastra. I pozzetti vengono letti a 450 nm rispetto al pozzetto di controllo del bianco. Se è disponibile uno spettrofotometro a lunghezza d’onda doppia, la lunghezza d’onda per il filtro di riferimento deve essere 600-650 nm. La lettura dei micropozzetti a 450 nm senza filtro di riferimento avrà come risultato valori di assorbanza più alti. Los resultados de esta prueba se pueden utilizar como complemento del diagnóstico de enfermedades autoinmunitarias. Los anticuerpos anti-ENA se han asociado a diversos síndromes autoinmunitarios, y parecen tener un significado diagnóstico y/o pronóstico en la esclerosis sistémica (1, 2), las enfermedades mixtas del tejido conjuntivo (3-5), el síndrome de Sjögren (6, 7), la polimiositis (8), la dermatomiositis (9), el lupus eritematoso sistémico (5) y la artritis reumatoide (10). Se ha utilizado el análisis de anticuerpos antinucleares (ANA) para hacer una detección selectiva de estos anticuerpos, pero en dicho análisis no se especifica el anticuerpo, y algunos anticuerpos contra determinados ENA no dan positivo en el análisis de ANA (11, 12). Por eso es muy recomendable realizar una evaluación secundaria de confirmación de los anticuerpos anti-ENA (13). El antígeno Sm (Smith) fue identificado en 1966 por Tan y Kunkel como una glucoproteína no histona soluble en suero salino, que no depende del ADN ni del ARN para su poder antigénico (14). Se considera que los anticuerpos contra el antígeno Sm son marcadores serológicos específicos, debido a su elevado grado de especificidad por el lupus eritematoso sistémico (LES). Estos anticuerpos se observan en hasta el 30% de los pacientes con LES, y se han asociado a nefropatías activas y a cerebritis (15-17). En el suero de pacientes con LES es frecuente encontrar anticuerpos anti-Sm junto con anticuerpos anti-U1-RNP (18, 19). Al contrario que los anticuerpos anti-Sm, los anticuerpos anti-RNP no se consideran marcadores serológicos específicos, pues se encuentran en pacientes con diversas enfermedades reumáticas como LES, esclerodermia, síndrome de Sjögren y artritis reumatoide. No obstante, los niveles elevados de anticuerpos anti-RNP presentan una elevada asociación a un síndrome intermedio denominado enfermedad mixta del tejido conjuntivo (EMTC). Los pacientes con EMTC se caracterizan por una combinación de datos clínicos que se observan en el LES, la esclerodermia y la polimiositis. A menudo, estos pacientes demuestran una buena respuesta al tratamiento con corticosteroides, y presentan una incidencia menor de enfermedades renales que los pacientes con LES (20, 21). Originalmente, SSA y SSB fueron descritos como antígenos nucleares de proteínas del ARN en pacientes con síndrome de Sjögren (6, 7). Ro y La fueron descritos como antígenos citoplásmicos de proteínas del ARN en pacientes con LES (22, 23). En la actualidad se suele aceptar que SSA y Ro son análogos, que SSB y La también lo son, y que estos antígenos se encuentran tanto en el núcleo como en el citoplasma. Se encuentran anticuerpos contra SSA/Ro solo o contra SSA/Ro y SSB/La en hasta el 62% de los pacientes con lupus cutáneo subagudo (24), y en el 85% de los pacientes con síndrome de Sjögren que presentan vasculitis (25). Se observan anticuerpos contra SSA/Ro solo en pacientes con déficit homozigótico de la fracción C2 del complemento (26), en pacientes con cirrosis biliar primaria que desarrollan síndrome de Sjögren (27) y en hasta dos tercios de los pacientes con “LES sin ANA” (28). Se ha descubierto que el antígeno Scl-70 es una enzima celular, la ADN topoisomerasa I, una proteína auxiliar de la ADN polimerasa delta (29). Se han observado anticuerpos anti-Scl-70 en hasta el 56% de los pacientes con esclerosis sistémica progresiva (ESP), sobre todo en el subgrupo que presenta esclerodermia difusa (30). Se considera que estos autoanticuerpos son marcadores de la ESP, pues no se observan en otras enfermedades del tejido conjuntivo. Los anticuerpos anti-Jo-1, que es la enzima celular histidil ARNt sintetasa, se encuentran en el 25-30% de los pacientes con polimiositis o dermatomiositis, pero no en otras miopatías (11, 31). También se ha demostrado que los anticuerpos anti-Jo-1 presentan una elevada asociación con las neumopatías intersticiales que se observan en asociación a las miositis (31). PRINCIPIO DE LA PRUEBA Se trata de un EIA cualitativo indirecto. La superficie de los micropocillos ha sido recubierta con un preparado estabilizado de antígenos nucleares extraíbles purificados en función de su afinidad, que actúa como antígeno en este sistema. Se disponen muestras diluidas de los pacientes en los micropocillos, y se incuban permitiendo que los anticuerpos específicos de la muestra reaccionen con el antígeno de la fase sólida. Tras lavar para retirar el anticuerpo no ligado y otras proteínas del suero, se incuban los pocillos con anticuerpos humanos de cabra, marcados con peroxidasa de rábano. El preparado de anticuerpos conjugados con peroxidasa de rábano que se incluye en sistema de evaluación es específico de las cadenas ligeras y pesadas de la IgG humana. Tras la incubación con los conjugados de peroxidasa de rábano, y si los resultados son positivos, se forma un complejo estable con tres partes, a saber: Este complejo esta compuesto por anticuerpo antihumano conjugado con peroxidasa de rábano unido al anticuerpo anti-ENA humano, unido a su vez al antígeno estabilizado en la superficie de plástico. Tras otro paso de lavado, se detecta el complejo añadiendo una solución de tetrametilbenzidina (TMB) y H2O2 como sustrato cromógeno. El grado de aparición del color en cada pocillo es proporcional a la concentración de anticuerpos anti-ENA en cada muestra de suero. Cada micropocillo es analizado en un espectrofotómetro a 450 nm. COMPONENTES DEL SISTEMA (MATERIALES SUMINISTRADOS) Conservación: Todos los componentes deben conservarse en refrigerador a 2-10 ºC. No congelar. Estabilidad: Todos los componentes son estables al menos durante 12 meses a partir de la fecha de fabricación. No utilice los componentes pasada la fecha de caducidad. Todos los materiales de procedencia humana utilizados en este producto han sido analizados en busca de anticuerpos con el virus de la inmunodeficiencia humana-1 (VIH-1), el virus de la inmunodeficiencia humana-2 (VIH-2), el virus de la hepatitis C (VHC) y el antígeno de superficie de la hepatitis B (HBsAG) con métodos homologados por la FDA, obteniendo resultados negativos (no reactivos en varias ocasiones) en todos los casos. Pero no existe ningún método de análisis que pueda garantizar por completo la ausencia de VIH-1, VIH-2, hepatitis C, hepatitis B u otros agentes infecciosos. Por eso, todos los materiales del kit deben ser manipulados como si fueran infecciosos. 2. Todas las muestras de paciente deben ser manipuladas según el nivel 2 de bioseguridad, según se recomienda en el manual de los Centers for Disease Control/National Institutes of Health para toda muestra de suero o sangre humana potencialmente infecciosa: Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. La disolución de los componentes o su sustitución por otros distintos de los suministrados con el sistema puede arrojar resultados incoherentes. 4. En los sueros de control se emplea azida sódica (0,1 %) como conservante. La azida sódica puede reaccionar con las conducciones de plomo o cobre y formar sales de azidas metálicas muy explosivas. Al eliminar los reactivos, lavar con grandes volúmenes de agua del grifo para evitar que queden residuos en las tuberías. La azida sódica es venenosa y puede ser tóxica en caso de ingestión. 5. Este kit es para uso diagnóstico in vitro. 6. No pipetee nunca con la boca y evite el contacto de los reactivos y las muestras con la piel y las mucosas. En caso de contacto, lávese con un jabón germicida y agua abundante. 7. Esta prohibido fumar, comer o beber en las zonas de manipulación de las muestras o los reactivos del kit. 8. Evite salpicaduras y la generación de aerosoles en todo momento. 9. Si los tiempos de incubación y las temperaturas no son los especificados, los resultados pueden ser erróneos. 10. La contaminación cruzada de los reactivos o de las muestras puede dar resultados falsos. Las 18 muestras deben permanecer en los micropocillos durante el análisis. 11. Los elementos de vidrio reutilizables deben ser lavados y enjuagados a fondo para eliminar los detergentes antes de su uso. Todos los elementos de vidrio deben estar limpios y secos antes de su uso. 12. Todos los reactivos, micropocillos y muestras deben estar a temperatura ambiente (19-24 ºC) antes de su uso. 13. Para manipular las muestras y los reactivos debe utilizar guantes de látex desechables, y cuando acabe deberá lavarse bien las manos. 14. La contaminación microbiana de los reactivos o de las muestras puede dar resultados falsos. 15. El reactivo de parada es corrosivo y puede producir quemaduras. Este reactivo contiene ácido clorhídrico y ácido sulfúrico (menos del 3% de cada uno, por volumen) y debe ser manipulado con precaución. Manténgase fuera del alcance de los niños. En caso de contacto con los ojos, lavar inmediatamente con agua abundante y acudir al médico. No añadir agua a este reactivo en ningún caso. OBTENCIÓN DE MUESTRAS Obtención: La muestra ideal es suero. Se obtendrán aproximadamente 5 ml de sangre por venipunción aséptica, con un tubo de vacío estéril u otro sistema de obtención adecuado. Deje que la sangre coagule a temperatura ambiente (19-23 ºC). Se separará el suero del coágulo por centrifugado cuanto antes, para que la hemólisis sea mínima. Sustancias que interfieren: No se utilizarán sueros que muestren grados elevados de hemólisis, ictericia, lipemia o crecimiento microbiano, pues en todas esas circunstancias se pueden producir resultados aberrantes. Si la muestra presenta partículas visibles, deben ser eliminadas por centrifugado antes de la prueba. Conservación: Los sueros se pueden conservar a 2-10 ºC durante una semana como máximo. Si el análisis se posterga, los sueros deben conservarse congelados a una temperatura de –20 ºC o inferior. No se utilizarán refrigeradores ni congeladores con sistema “auto-frost” (eliminación automática de la escarcha). PRECAUCIÓN: Si las muestras son sucesivamente congeladas y descongeladas, se pueden obtener resultados falsos positivos y negativos. OBSERVACIONES GENERALES SOBRE EL PROCEDIMIENTO 1. Es muy importante que todos los componentes del kit y las muestras de suero estén a temperatura ambiente (19-23 ºC) antes de utilizarlos. Para que un litro entero de tampón de lavado sacado del refrigerador alcance los 20 ºC, puede ser necesario calentar durante varias horas. Si las temperaturas de incubación son superiores o inferiores a las recomendadas puede que los resultados no sean precisos. Las muestras y reactivos no utilizados deben ser devueltos al refrigerador. 2. Mezcle bien los reactivos antes de utilizarlos, dando la vuelta al envase con suavidad. No agite ni remueva los reactivos. Evite que se forme espuma. 3. Para preparar las diluciones de muestras, limpie la punta de las pipetas antes de utilizarlas para poner suero en el disolvente. Si queda muestra adherida al exterior de la punta de la pipeta, los resultados pueden variar. 4. Se recomienda utilizar una pipeta multicanal porque con ella la dispensación del reactivo y los tiempos de incubación y reacción son más uniformes. 5. Es extremadamente importante lavar bien los pocillos. Si no es así, los valores de fondo serán muy elevados, lo que se puede traducir en resultados falsos positivos. Para el lavado manual, aspire el contenido de los pocillos y a continuación lave cada uno con solución tampón de lavado. Evite la contaminación cruzada de los pocillos, sobre todo en el lavado siguiente a la aspiración. Elimine todo el tampón de los pocillos dándoles la vuelta; los residuos de tampón de lavado se quitan con un movimiento brusco de la muñeca. Repita estos pasos hasta un total de 3 a 5 lavados. A continuación, se pasará con fuerza una toalla de papel u otro material absorbente por los pocillos para retirar todo vestigio de tampón de lavado. Se recomienda utilizar un sistema de lavado automático de los micropocillos, ya que así su limpieza queda garantizada. NOTA: Como existen diversas técnicas de lavado ����� � � ������ ����� � � ������ aproximadamente ����� � � ����iguales, el paciente tiene anticuerpos anti-Sm. Si los������� dos pocillos son positivos y ����� � � ����� el pocillo Sm/RNP presenta 30 unidades ENA o una ����� � � ������� ��������� cifra superior a la del pocillo Sm, es indicativo de la ����� �����anti-Sm y anti-RNP. presencia de autoanticuerpos y de sistemas automáticos, hay que ajustar el número de lavados para que los resultados sea óptimos. Cada laboratorio determinará el número de lavados más eficaz para su sistema de lavado. 6. Si no se elimina todo el tampón puede que la aparición de color no sea homogénea. Para ello, hay que limpiar con fuerza las tiras de micropocillos, secándolas con papel secante o toallas absorbentes. 7. El tiempo de cada paso es fundamental. Antes de iniciar el procedimiento se disolverán las muestras de suero, que deben ser dispuestas en los micropocillos en el menor tiempo posible (no más de cinco minutos). Los tamaños de lote deben determinarse de forma que se puedan manipular las muestras en este período de tiempo con comodidad. Se recomienda utilizar pipetas multicanal, pues facilitan la manipulación de las muestras y los reactivos. 8. A excepción de la última incubación (solución de sustrato), cada período de incubación empieza al finalizar la dispensación de la muestra o el reactivo. La incubación de la solución de sustrato debe durar exactamente cinco minutos para cada pocillo. Todas las muestras y reactivos deben dispensarse con la misma secuencia y a velocidad constante. La presencia de ambos anticuerpos se puede confirmar con el sistema de inmunodifusión AUTO-ID® de Immuno Concepts, número de catálogo 6030. 3. Los demás pocillos están recubiertos con antígenos �������� � � �� monoespecíficos purificados en función de su �������� � � ������con el anticuerpo afinidad, y sólo reaccionan �������� ������ homólogo. Pero � en� el LES y otros síndromes auto�������� � � ������ inmunitarios es frecuente ver sueros de pacientes con múltiples anticuerpos, �������� � � ������ por lo que un suero individual puede�dar una reacción positiva en más �������� � ���� de un�������� pocillo. � � ����� ������� 4. Los pocillos están recubiertos con antígenos puri�������� � � ������� ��������� ficados en función de su afinidad, en el siguiente ����� ����� orden, de arriba (pocillo A, y extremo de la tira continuo) a abajo (pocillo H, y extremo de la tira con escotaduras): ������� � � �� ������� � � ������ ������� � � ������ ������� � � ������ ������� � � ������ ������� � � ���� ������� � � ����� ������� ������� � � ������� ��������� ����� ����� INTERPRETACIÓN DE LOS RESULTADOS CONTROL DE CALIDAD 1. El pocillo de control vacío debe presentar un valor de absorbancia inferior a 0,250. Si los valores de absorbancia en el pocillo vacío son superiores a 0,250, ello indica que el lavado no ha sido adecuado o que se han contaminado los reactivos. 2. Se resta la absorbancia del pocillo de control vacío (fila G) de la absorbancia del pocillo de verificación del procedimiento RELISA® (RPC) (fila H) y de cada pocillo con antígeno (filas A a F). Los valores netos inferiores a cero se consideran cero. 3. La absorbancia neta del pocillo de verificación del procedimiento RELISA® (RPC) (fila H) debe ser superior a 0,300. Si es inferior, la aparición del color no es adecuada y el ciclo no es válido. Si la aparición del color no es adecuada suele deberse a que los reactivos están fríos o a que la duración de uno o más pasos de la prueba no ha sido correcta. Deje que los reactivos alcancen la temperatura ambiente (19-23 ºC) y repita el ciclo prestando especial atención a la duración de todos los pasos. 4. Los valores netos de la absorbancia se multiplican por 100 para obtener el valor de cada antígeno en unidades ENA. 5. El suero de control positivo es una mezcla de sueros humanos que contiene anticuerpos contra los seis autoantígenos de este análisis. Cada pocillo con antígeno debe mostrar un valor de 30 unidades ENA o más. 6. El suero de control negativo es una mezcla de sueros humanos que no contiene anticuerpos contra ninguno de los seis autoantígenos de este análisis. Cada pocillo con antígeno debe mostrar un valor inferior a 20 unidades ENA. 7. Cada laboratorio establecerá la frecuencia con la que debe utilizar los sueros de control positivo y negativo, en función de la frecuencia con la que se analicen muestras de pacientes y de la experiencia del laboratorio con esta prueba. NOTIFICACIÓN DE RESULTADOS Se indicará si���������� el resultado � es�positivo o negativo para �� los anticuerpos correspondientes a los antígenos nucle���������� � � ������ ares extraíbles. Los niveles de anticuerpos carecen de ���������� � � ������ significado clínico conocido. ���������� � � ������ ���������� � � ������ VALORES ESPERADOS ���������� � � ���� La incidencia de autoanticuerpos los diversos ���������� � � �����contra ������� antígenos nucleares varían función ��������� de la población ���������� � en � ������� de pacientes y de la incidencia de enfermedades ����� ����� reumáticas clínicas en esa población. En tabla 1 se resume la asociación de los anticuerpos a las enfermedades reumáticas concretas. ����� � � �� LÍMITES DE ����� REFERENCIA � � ������ ����� � � ������ Los límites de referencia se han establecido a partir de ����� � � ������ sueros de 206 donantes de sangre sanos, 105 mujeres ����� � � ������ y 101 varones, ninguno de los cuales presentaba ante� � ���� reumáticas. Además, se cedentes de����� enfermedades obtuvieron datos de�143 pacientes con enfermedades ����� � ����� ������� reumáticas que presentaban anticuerpos contra uno ����� � � ������� ��������� o más de los antígenos de����� estos análisis, ����� pero no para los demás. Basándose en estos datos, se establecieron los valores de corte normales en <20 unidades ENA. La buena práctica exige que cada laboratorio establezca sus propios límites de normalidad en función de su población de pacientes y de otros factores locales. LIMITACIONES DE LA PRUEBA 1. No es posible hacer un diagnóstico basándose sólo en la detección de anticuerpos contra antígenos nucleares extraíbles. El médico debe interpretar estos resultados en el contexto de la historia y los síntomas del paciente, los hallazgos físicos y otros procedimientos diagnósticos. 2. No se debe iniciar un tratamiento basándose exclusivamente en un resultado positivo del análisis de anticuerpos con antígenos nucleares extraíbles. Antes de iniciar un tratamiento hay que tener en cuenta las indicaciones clínicas, otros hallazgos de laboratorio y la impresión clínica del médico. 3. Algunos pacientes con enfermedades autoinmunitarias pueden presentar niveles indetectables o insignificantes de anticuerpos contra los antígenos nucleares extraíbles, y algunos individuos pueden presentar niveles elevados con escasos o nulos datos de enfermedad. El médico debe interpretar estos resultados en el contexto de la historia y los síntomas del paciente, los hallazgos físicos y otros procedimientos diagnósticos. 4. El nivel de anticuerpos detectado con este sistema de análisis no indica necesariamente la intensidad o la duración de la enfermedad. INTERPRETACIÓN DE LOS RESULTADOS DEL PACIENTE 1. Se trata de un análisis cualitativo. Los niveles de anticuerpos detectados carecen de significado clínico, y las cifras obtenidas en unidades sólo sirven para separar a los pacientes en los siguientes tres grandes grupos: Los pocillos con muestras de pacientes que presentan valores calculados superiores a 30 unidades ENA se consideran positivos. Los pocillos con muestras de pacientes que presentan valores calculados inferiores a 20 unidades ENA se consideran negativos. Los valores entre 20 y 30 unidades se consideran positivos en el límite, y obligan a repetir la prueba. Cada laboratorio deberá establecer sus propios límites de referencia y valores de corte, en función de la población de pacientes evaluada. Los valores resultan afectados por factores de los pacientes, consideraciones mecánicas (como la exactitud y precisión del pipeteado) y las condiciones del análisis (como la temperatura y la cronología de los pasos). 2. Los pocillos de Sm y Sm/RNP se utilizan juntos para determinar la presencia de estos dos autoanticuerpos. Si el pocillo Sm/RNP es positivo y el Sm es negativo, el paciente tiene anticuerpos anti-RNP. Preparación de la hoja de trabajo: Marque la hoja de trabajo que se adjunta con el kit, para indicar la identificación de cada tira de ocho micropocillos. 2. Preparación de la solución tampón de lavado (PBS-Tween): Disuelva el contenido de una bolsa de tampón PBS en un litro de agua desionizada o destilada. Añada todo el contenido de un frasco de concentrado de tampón de lavado al envase de un litro de PBS disuelto. Mezcle bien. La solución tampón de lavado se puede tapar y conservar a 2-10 ºC durante cuatro semanas como máximo. 3. Muestras de pacientes diluidas: Disuelva las muestras de pacientes a 1:40 añadiendo 25 µl de suero a 975 µl de disolvente de muestras. Mezcle bien. Los controles se entregan a la dilución de trabajo, por lo que no es necesario disolverlos. 4. Preparación de las tiras de micropocillos: Saque de sus bolsas las tiras de micropocillos necesarias y colóquelas en el soporte. Deben quedar firmemente sujetas en él. Coloque el soporte sobre una superficie sólida, de forma que las letras A-H queden a la izquierda y los números 1-12 en el borde superior del soporte. Ponga las tiras en el soporte de forma que el extremo con escotaduras se acople a la pequeña clavija del borde inferior del soporte. Apriete suavemente ambos lados de la tira, de forma que quede bien sujeta en el soporte. Si las tiras están bien colocadas en el soporte, no se caen al darle la vuelta. Marque el borde mate de la tira para indicar la identificación del paciente y la posición en el soporte. 5. Dispensación de las diluciones de suero: Ponga 50 µl de muestra del paciente diluida en cada uno de los ocho pocillos de cada tira multiparamétrica. 6. Incubación de las tiras (30 minutos a temperatura ambiente, es decir, 19-23 °C): Incube a temperatura ambiente durante 30 minutos. Durante la incubación, la temperatura no podrá sufrir cambios bruscos. Si se desea se pueden tapar las tiras con cinta transparente o con toallas de papel, para protegerlas del polvo u otros cuerpos extraños. 7. Lavado de las tiras (véanse las Observaciones generales sobre el procedimiento 5 y 6): Lave los pocillos de 3 a 5 veces con solución tampón de lavado PBS-Tween. Para el lavado manual, aspire el contenido de los pocillos y a continuación lave cada uno con solución tampón de lavado. Evite la contaminación cruzada de los pocillos, sobre todo en el lavado siguiente a la aspiración. Elimine todo el tampón de los pocillos dándoles la vuelta; los residuos de tampón de lavado se quitan con un movimiento brusco de la muñeca. Repita estos pasos hasta un total de 3 a 5 lavados. A continuación, se pasará con fuerza una toalla de papel u otro material absorbente por los pocillos para retirar todo vestigio de tampón de lavado. 8. 9. Evite la contaminación cruzada de los pocillos, sobre todo en el lavado siguiente a la aspiración. Elimine todo el tampón de los pocillos dándoles la vuelta; los residuos de tampón de lavado se quitan con un movimiento brusco de la muñeca. Repita estos pasos hasta un total de 3 a 5 lavados. Dispensación de la solución sustrato: Utilizando un cronómetro para que los intervalos sean homogéneos, ponga 50 µl de solución de sustrato en cada pocillo. La solución de sustrato se incorporará a los pocillos a velocidad constante, de forma que todos se sometan al mismo tiempo de incubación (cinco minutos). La solución de sustrato incubada en pocillos con muestras positivas se volverá de color azul, y si son muestras negativas, serán incoloras o de azul muy claro. 12. Incubación de las tiras (5 minutos exactamente a temperatura ambiente, es decir, 19-23 °C): Incube a temperatura ambiente durante 5 minutos exactamente. Durante la incubación, la temperatura no podrá sufrir cambios bruscos. 13. Dispensación del reactivo de parada: Cuando el primer pocillo haya sido incubado durante cinco minutos exactos, añada 50 µl de reactivo de parada a cada pocillo, en el mismo orden y a la misma velocidad en que se añadió la solución de sustrato a los pocillos. Sämtliche für dieses Produkt verwendeten Materialien menschlichen Ursprungs wurden nach von der FDA anerkannten Methoden negativ (nicht wiederholt reaktiv) auf Antikörper gegen HIV-1, HIV-2, Hepatitis-C (HCV) und Hepatitis-B-Oberflächenantigen (HBsAg) getestet. Keine Testmethode kann jedoch mit absoluter Sicherheit nachweisen, dass keine HIV-1, HIV-2, Hepatitis-C oder Hepatitis-B-Viren oder andere infektiöse Agenten vorhanden sind. Eine Kreuzkontamination der Reagenzien oder Proben kann ebenfalls zu falschen Ergebnissen führen. Während der Tests müssen die Proben in den Vertiefungen der Mikrotiterplatten verbleiben. 11. Wiederverwendbare Glasartikel müssen vor Gebrauch gewaschen und gründlich ausgespült werden, um sämtliche Waschmittelrückstände zu entfernen. Die Glasartikel müssen vor Gebrauch sauber und trocken sein. 12. Vor der Testdurchführung müssen alle Reagenzien, PROBENGEWINNUNG Probenentnahme: Nach Möglichkeit sollten Serumproben hergestellt werden. Dazu durch Venenpunktion in ein steriles Vakuumröhrchen oder durch ein anderes geeignetes Blutentnahmesystem aseptisch ca. 5 ml Vollblut entnehmen. Das Blut bei Zimmertemperatur (1923 °C) gerinnen lassen. Danach muss das Serum so bald wie möglich durch Zentrifugation abgetrennt werden, um Hämolyse zu vermeiden. Störsubstanzen: Stark hämolytische, lipämische oder durch Mikrobenwachstum verunreinigte Seren sowie Seren von Ikteruspatienten dürfen nicht verwendet werden, weil diese Zustände zu falschen Ergebnissen führen können. Proben mit sichtbaren Verunreinigungen müssen vor Verwendung zentrifugiert werden. Aufbewahrung: Serumproben können bei einer Temperatur von 2-10 °C maximal eine Woche lang aufbewahrt werden. Sollen die Proben länger aufbewahrt werden, müssen sie bei mindestens –20 °C eingefroren werden. Serum darf nicht in einem Kühlschrank oder Gefrierschrank mit Abtauautomatik gelagert werden. ACHTUNG: Wiederholtes Einfrieren / Auftauen von Patientenproben ist zu vermeiden. Andernfalls können falsch-positive oder falsch-negative Ergebnisse auftreten. ALLGEMEINE HINWEISE ZUM VERFAHREN 1. Vor Verwendung müssen unbedingt alle Serumproben und Kitbestandteile auf Zimmertemperatur (19-23 °C) gebracht werden. Ein Liter Waschpuffer benötigt mehrere Stunden, um nach Entnahme aus dem Kühlschrank auf 20 °C zu kommen. Bei Inkubationstemperaturen außerhalb des hier angegebenen Bereichs können die Ergebnisse verfälscht werden. Unverbrauchte Proben und Kitmaterialien müssen nach Verwendung wieder gekühlt werden. 2. Reagenzien vor dem Test durch vorsichtiges Umdrehen der Flasche gut durchmischen. Auf keinen Fall schütteln oder auf einen Mischer stellen. Schaumbildung vermeiden. 3. Beim Herstellen der Probenverdünnungen müssen die Pipettenspitzen vor der Zugabe von Serum in den Probenverdünner abgewischt werden. Außen an der Pipette anhaftendes überschüssiges Probenmaterial kann die Ergebnisse beeinträchtigen. 4. Das Arbeiten mit einer Mehrkanalpipette wird empfohlen, weil dadurch die Zugabe, die Inkubationszeit und die Reaktionszeit gleichmäßiger ausfallen. 5. Sorgfältiges Waschen der Vertiefungen ist von entscheidender Bedeutung. Unzureichendes Waschen der Vertiefungen führt zu hohen Hintergrundwerten und unter Umständen zu falsch-positiven Ergebnissen. Beim Auswaschen von Hand den Inhalt der Vertiefungen aspirieren, anschließend die Vertiefungen mit Waschpuffer füllen. Kreuzkontamination zwischen den Vertiefungen vermeiden, vor allem beim ersten Waschgang nach dem Absaugen. Die Platten umdrehen und in der Luft kräftig ausschlagen, um alle Waschlösung aus den Vertiefungen zu entfernen. Der Waschvorgang mit Füllen und Entfernen muss drei bis fünf Mal wiederholt werden. Danach die Platten auf einem Papierhandtuch oder vergleichbarem Material ausklopfen, so dass der Waschpuffer vollständig entfernt wird. Die Verwendung eines automatischen Waschgeräts für Mikrotiterplatten wird empfohlen, weil dies zu gleichmäßigeren Waschergebnissen führt. HINWEIS: Auf Grund der verschiedenen Waschtechniken und Automatiksysteme muss die Anzahl der Waschgänge evtl. angepasst werden, um optimale Ergebnisse zu erzielen. Jedes Labor muss für sein Waschsystem die Anzahl von Waschgängen mit der höchsten Effizienz selbst bestimmen. 6. Unzureichende Entfernung von Waschpufferrückständen kann zu inkonsistenter Farbentwicklung 23 führen. Die Mikrotiterstreifen sollten auf Papierhandtüchern oder vergleichbarem Material ausgelegt werden, um Waschpufferrückstände zu minimieren. 7. Die bei den Inkubationsschritten angegebene Zeitdauer muss unbedingt eingehalten werden. Alle Serumproben müssen vor Versuchsbeginn verdünnt und möglichst schnell hintereinander (in maximal fünf Minuten) in die Vertiefungen dispensiert werden. Die Testserien dürfen nur so groß sein, dass diese Zeit bequem eingehalten werden kann. Die Verwendung einer mehrkanaligen Pipette zur leichteren Handhabung von Proben und Reagenzien ist zu empfehlen. 8. Mit Ausnahme des letzten Inkubationsschritts (Substratlösung) beginnt jede Inkubationsperiode nach beendigter Proben- oder Reagenzienzugabe. Die Inkubation mit Substratlösung muss für jede Vertiefung genau fünf Minuten dauern. Daher müssen Proben und Reagenzien in derselben Reihenfolge und im selben zeitlichen Abstand zugegeben werden. Der Zusammenhang zwischen dem Vorhandensein von Antikörpern und spezifischen Rheumaerkrankungen ist in der folgenden Tabelle 1. REFERENZBEREICH Der Referenzbereich wurde durch Tests an Seren von 206 gesunden Blutspendern, 105 Frauen und 101 Männer, ermittelt. Bei keinem der Spender war eine rheumatische Erkrankung bekannt. Zusätzlich wurden Daten von 143 Rheumapatienten mit Antikörpern gegen ein oder mehrere Antigene dieses Assays gesammelt, die jedoch negativ für Antikörper gegen die anderen Antigene waren. Auf Basis dieser Daten wurden die normalen Cutoff-Werte, weniger als 20 ENA-Einheiten, berechnet. Die gute Laborpraxis schreibt vor, dass jedes Labor seine eigenen normalen Wertebereiche auf Basis der Patientenpopulation und anderer lokaler Faktoren ermitteln muss. EINSCHRÄNKUNGEN DES TESTS INTERPRETATION DER PATIENTENERGEBNISSE 1. Dies ist ein qualitatives Assay. Die Anzahl der nachgewiesenen Antikörper besitzt keinerlei bekannte klinische Relevanz und die mit diesem Assay erzielten Einheitsgrößen dienen lediglich der Aufteilung der Patienten in die folgenden drei Hauptgruppen. Die Patientenprobevertiefungen, deren errechnete Werte höher als 30 ENA-Einheiten sind, sind als positiv zu werten. Die Patientenprobevertiefungen, deren errechnete Werte niedriger als 20 ENA-Einheiten sind, sind als negativ zu werten. Werte zwischen 20 und 30 Einheiten liegen im positiven Grenzbereich und sollten wiederholt werden. Jedes Labor muss seinen eigenen Referenzbereich und eigene Cutoff-Werte auf Basis der getesteten Patientenpopulation ermitteln. Die Einheitswerte sind abhängig von Patientenfaktoren, mechanischen Gegebenheiten (wie Präzision der Pipetten), und Assay-Umständen (wie Temperatur und zeitliche Abfolge der Schritte.) 2. Die Sm und Sm/RNP Vertiefungen werden zusammen verwendet, um das Vorhandensein dieser beiden Autoantikörper zu bestimmen. Falls die Sm/ RNP Vertiefung positiv ist und die Sm Vertiefung negativ, hat der Patient Antikörper gegen RNP. Wenn beide Vertiefungen positiv sind, mit annähernd gleichen Werten, hat der Patient Antikörper gegen 24 1. Es ist nicht möglich, lediglich auf der Basis des Nachweises von Antikörpern gegen extrahierbare nukleäre Antigene eine Diagnose zu stellen. Der Arzt muss die Ergebnisse im Zusammenhang mit Krankengeschichte und Symptomen des Patienten, den Ergebnissen der körperlichen Untersuchung und anderen diagnostischen Methoden interpretieren. 2. Lediglich auf Grund eines positiven Testergebnisses für Antikörper gegen extrahierbare nukleäre Antigene sollte keine Behandlung initiiert werden. Für eine Behandlung müssen auch klinische Symptome, andere Laborergebnisse und der Gesamteindruck des Patienten auf den behandelnden Arzt herangezogen werden. 3. Bei einigen Patienten mit Autoimmunerkrankungen können nicht nachweisbare oder unbedeutende Mengen Antikörper gegen extrahierbare nukleäre Antigene vorhanden sein; bei anderen Patienten mit großen Mengen an Antikörpern gegen extrahierbare nukleäre Antigene sind u.U. nur geringfügige oder keine Anzeichen einer klinischen Erkrankung festzustellen. Arbeitsblatt ausfüllen: In das mit dem Kit gelieferte Arbeitsblatt zu Identifikationszwecken die Lage der Proben in den acht Vertiefungen der Mikrotiterplatte eintragen. 2. Waschpufferlösung (PBS-Tween) vorbereiten: Inhalt eines Beutels mit PBS-Pufferpulver in einem Liter deionisiertem oder destilliertem Wasser auflösen. Den Inhalt einer ganzen Flasche Waschpufferkonzentrat in den aufgelösten PBS-Puffer (1 Liter) hinzugeben und gut durchmischen. Die Waschpufferlösung kann verschlossen und gekühlt (2-10 °C) bis zu vier Wochen aufbewahrt werden. 3. Patientenproben verdünnen: Patientenproben durch Zugabe von 25 µl Serum zu 975 µl Probenverdünner 1:40 verdünnen und gut durchmischen. Die Kontrollseren sind gebrauchsfertig und müssen nicht weiter verdünnt werden. 4. Mikrotiterstreifen vorbereiten: Benötigte Streifenzahl aus den Beuteln entnehmen und in den Halter einsetzen. Die Streifen müssen fest im Halter sitzen. Den Halter auf eine feste Oberfläche stellen, wobei die Buchstaben A bis H auf der linken Seite und die Nummern 1-12 am oberen Rand des Halters liegen sollen. Die Streifen so in den Halter einsetzen, dass das gekerbte Streifenende auf dem kleinen Haken am unteren Rand des Halters sitzt. Kräftig auf beide Enden drücken, so dass die Streifen fest im Halter einrasten. Streifen, die korrekt im Halter sitzen, können beim Umdrehen des Halters nicht herausfallen. Beim Auswaschen von Hand den Inhalt der Vertiefungen aspirieren, anschließend die Vertiefungen mit Waschpuffer füllen. Kreuzkontamination zwischen den Vertiefungen vermeiden, vor allem beim ersten Waschgang nach dem Absaugen. Die Platten umdrehen und in der Luft kräftig ausschlagen, um alle Waschlösung aus den Vertiefungen zu entfernen. Der Waschvorgang mit Füllen und Entfernen muss drei bis fünf Mal wiederholt werden. Danach die Platten auf einem Papierhandtuch oder vergleichbarem Material ausklopfen, so dass der Waschpuffer vollständig entfernt wird. Enzym-Antikörper-Reagens zugeben: 50 µl Enzym-Antikörper-Reagens in jede Vertiefung dispensieren. 26 9. Die Substratlösung muss in gleichmäßigen Zeitabständen zugegeben werden, damit sichergestellt ist, dass jede Vertiefung exakt fünf Minuten inkubiert wird. In Vertiefungen, die positive Proben enthalten, verfärbt sich die Substratlösung blau. In den anderen Vertiefungen bleibt die Lösung farblos oder wird nur schwach blau. 12. Streifen inkubieren (genau 5 Minuten bei Zimmertemperatur, 19-23 °C): Die Streifen genau fünf Minuten bei Zimmertemperatur inkubieren. Während dieser Zeit müssen die Streifen vor Luftzug oder Temperaturschwankungen geschützt sein. 13. Stoppreagens zugeben: Nachdem die erste Vertiefung genau 5 Minuten inkubiert wurde, mit dieser anfangen und im selben Rhythmus wie bei der Substratlösung in jede Vertiefung 50 µl Stopplösung zugeben. Bei Zugabe der Stopplösung verfärbt sich blaue Substratlösung gelb, während farblose Lösung farblos bleibt. 14. Absorption messen: Die Absorption der Vertiefungen muss innerhalb von 30 Minuten nach Zugabe des Stoppreagens mit einem Mikrotiterplatten-Spektrophotometer gemessen werden. Die Vertiefungen werden bei 450 nm um den Leerwert bereinigt gemessen. Wenn die Möglichkeit besteht, bei einer zweiten Wellenlänge zu messen, auch bei 600-650 nm als Referenzwellenlänge messen. Werden die Werte in den Vertiefungen ohne Referenzfilter berechnet, so führt dies zu höheren Absorptionswerten. Resultaten av denna analys kan användas som ett hjälpmedel vid diagnostisering av autoimmuna sjukdomar. Antikroppar mot extraherbara nukleära antigener (ENA) har associerats med flera autoimmuna syndrom och verkar vara av diagnostisk och/eller prognostisk betydelse vid systemisk skleros (1, 2), blandad bindvävssjukdom (3-5), Sjögrens syndrom (6, 7), polymyosit (8), dermatomyosit (9), systemisk lupus erytematosus (5) och reumatoid artrit (10). Det antinukleära antikropptestet (ANA) har använts som ett filter för dessa antikroppar, men ANA anger inte antikroppens specificitet, och antikroppar mot vissa ENA ger inte positivt ANA-svar (11, 12). Därför rekommenderas bestämt en andra bekräftande analys av antikroppar mot ENA (13). Sm-antigenen (Smith) identifierades 1966 av Tan och Kunkel som ett i saltlösning lösligt icke-histonglykoprotein som inte är beroende av DNA eller RNA för sin antigena potential (14). Antikroppar mot Sm-antigen betraktas som en specifik serologisk markör på grund av sin höga specificitet för systemisk lupus erytematosus (SLE). Dessa antikroppar har noterats hos upp till 30 procent av SLEpatienter och associerats med aktiv njursjukdom och cerebrit (15-17). Antikroppar mot Sm-antigen påträffas ofta tillsammans med U1-RNP-antikroppar i sera hos patienter med SLE (18, 19). Till skillnad från antikroppar mot Sm-antigen betraktas antikroppar till RNP inte som en specifik serologisk markör, eftersom de påträffas hos patienter med olika reumatiska sjukdomar, t ex SLE, sklerodermia, Sjögrens syndrom och reumatoid artrit. Emellertid associeras hög antikroppnivå av RNP med ett överlappande syndrom som kallas blandad bindvävssjukdom (MCTD). Patienter med MCTD karaktäriseras av en kombination av sjukdomssymptom liknande de som påträffas vid SLE, sklerodermia och polymyosit. Sådana patienter reagerar ofta väl på kortikosteroidbehandling och har färre njursjukdomar jämfört med SLE-patienter (20, 21). SSA och SSB beskrevs ursprungligen som nukleära RNA-proteinantigener hos patienter med Sjögrens syndrom (6, 7). Ro och La beskrevs som cytoplasmiska RNA-proteinantigener hos patienter med SLE (22, 23). Det är i dag vida accepterat att såväl SSA och Ro som SSB och La är likartade, och att dessa antigener påträffas i både kärnan och cytoplasman. Antikroppar mot enbart SSA/Ro eller SSA/Ro och SSB/La påträffas hos 62 procent av patienter med subakut kutan lupus (24) och hos 85 procent av patienter med Sjögrens syndrom som utvecklar vaskulit (25). Antikroppar mot enbart SSA/Ro påträffas hos patienter som har homozygot brist på C2komplementfragment (26), patienter med primär biliär cirros som utvecklar Sjögrens syndrom (27) samt hos högst två tredjedelar av patienter med ”ANA-negativ SLE” (28). Scl-7o-antigenen har identifierats som ett cellformigt enzym, DNA topoisomeras I, ett hjälpprotein till DNA polymerasdelta (29). Antikroppar mot Scl-70 har rapporterats hos upp till 56 procent av patienter med progressiv systemisk skleros (PSS), framför allt den delmängd av patienterna som har diffus sklerodermia (30). Dessa autoantikroppar betraktas som en markör för PSS, eftersom de inte påträffas vid andra bindvävssjukdomar. Antikroppar mot Jo-1, vilket är den cellformiga enzymhistidyl-TRNA-syntestasen, påträffas hos 25-30 procent av patienter med polymyosit eller dermatomyosit, men inte vid andra myopatier (11, 31). Anti-Jo1-antikroppar har också visat sig vara starkt associerade med interstitiell lungsjukdom, t ex i samband med myosit (31). TESTPRINCIP Detta test är en kvalitativ indirekt EIA. Mikrobrunnarnas yta har täckts med stabiliserade preparat av affinitetsrenade extraherbara nukleära antigener och fungerar som ett antigensubstrat i detta system. Spädningar av patientproven placeras i mikrobrunnarna och odlas så att de specifika antikropparna i provet reagerar med antigenen i den fasta fasen. Efter tvättning för att avlägsna obunden antikropp och annat seraprotein odlas brunnarna med getantihumana antikroppar märkta med pepparrotperoxidas. Det pepparrotperoxidaskonjugerade antikroppreparat som ingår i detta testsystem är specifikt för humana IgG-tunga och -lätta kedjor. Om resultaten är positiva efter inkubation med pepparrotperoxidaskonjugat bildas ett stabilt komplex bestående av tre delar. Detta komplex består av antihuman antikropp konjugerad med pepparrotperoxidas bunden till human anti-ENA-antikropp, vilken i sin tur är bunden till den antigen som stabiliserats på plastytan. Efter ännu ett tvättsteg detekteras detta komplex genom att tillsättande av en lösning av tetrametylbensidin (TMB) och H2O2 som kromogent substrat. Graden av färgutveckling i varje brunn står i relation till koncentrationen av anti-ENA-antikroppar i respektive serumprov. Varje mikrobrunn avläses i spektrometer vid 450 nm. ICKE-REAKTIVA KOMPONENTER Hållare för mikrobrunnar Tvättbuffertlösning: PBS-buffert: Katalognummer 1011: Fosfatbuffrat saltlösningspulver(0.01 M, pH 7.4 ± 0.2). Varje påse innehåller tillräckligt med buffertpulver för att ge en liter. (Två påsar buffertpulver medföljer varje 96-mikrobrunnsplatta i kompletta testsatser). Allt material av humant ursprung som använts i denna produkt har testats med FDA-godkända metoder och visade sig reagera negativt (inte upprepat reaktivt) på antikroppar mot humant immunbristvirus-1 (HIV-1), humant immunbristvirus-2 (HIV-2), hepatit C-virus (HCV) och hepatit B ytantigen (HBsAg). Ingen testmetod kan emellertid helt garantera att det inte förekommer HIV-1, HIV-2, hepatit-C, hepatit-B, eller andra smittämnen. Därför skall allt satsmaterial hanteras som potentiellt smittsamt. 2. Alla patientprover på biosäkerhetsnivå 2 skall hanteras enligt rekommendationerna för potentiellt smittsamt humanserum eller blodprov i Centrum för Sjukdomskontroll/Nationella hälsoinstitutets manual: Biosäkerhet i mikrobiologiska och biomedicinska laboratorier, 1984 års upplaga. 3. Spädning av komponenter eller byte till andra komponenter än de som medföljer detta system kan ge motsägande resultat. 4. Natriumazid (0,1 %) används som konserveringsmedel i kontrollserat. Natriumazid kan reagera med ledningsrör av bly eller koppar och bilda högexplosiva metallazider. När reagenser kasseras skall man spola med rikliga mängder kranvatten för att skölja bort eventuella rester i avloppsledningarna. Natriumazid är ett gift och kan vara toxiskt vid förtäring. 5. Denna sats är avsedd för diagnostisk användning in vitro. 6. Pipettera aldrig med munnen och undvik att komma i kontakt med reagenser och prov med hud eller slemhinnor. Tvätta med bakteriedödande tvål och rikligt med vatten om sådan kontakt inträffat. 7. Undvik att röka, äta eller dricka i områden där prov eller satsreagenser hanteras. 8. Undvik alltid stänk och alstring av aerosoler. 9. Andra inkubationstider och temperaturer än de angivna kan ge felaktiga resultat. 10. Korskontamination mellan reagenser och prover kan ge felaktiga resultat. Proverna måste hållas instängda i mikrobrunnarna under analysen. 11. Återanvändningsbart glas måste tvättas och noggrant sköljas från rengöringsmedel innan det används. Allt glas måste rengöras och torkas före användning. 12. Placera alla reagenser, mikrobrunnar och prover i rumstemperatur (19-23 °C) före användning. 13. Använd engångshandskar av latex vid hantering av prover och reagenser, och tvätta händerna noggrant efteråt. 14. Mikrobisk kontamination av reagenser eller prover kan ge felaktiga resultat. 15. Stoppreagensen är frätande och kan orsaka brännskador. Denna reagens innehåller hydrokloriska och svavelhaltiga syror (mindre än 3 % vardera, per volym) och skall hanteras med varsamhet. PROVTAGNING Provtagning: Serum rekommenderas som prov. Cirka 5 ml helblod skall tas aseptiskt genom venpunktion med hjälp av ett sterilt vakuumblodtagningsrör eller annat lämpligt blodtagningssystem. Låt blodet koagulera i rumstemperatur (19-23° C). Serum skall så snart som möjligt separeras från koagler genom centrifugering för att minimera hemolys. Störande substanser: Sera som uppvisar en hög grad av 28 1. Det är ytterst viktigt att alla satskomponenter och serumprov står i rumstemperatur (19-23 °C) före användning. En hel liter tvättbuffert kan ta flera timmar att värma till 20 °C sedan den har tagits ur kylskåpet. Inkubationstemperaturer som ligger högre eller lägre än det angivna intervallet kan leda till felaktiga resultat. Sätt tillbaka oanvända prover och reagenser i kylförvaring efter användning. 2. Blanda reagenserna väl före användning genom försiktig inversion. Snurra eller rotera inte reagenserna. Undvik skumning. 3. När provspädningarna framställs skall pipettspetsarna torkas av innan serum dispenseras i en provspädning. Överflödigt prov som sitter fast på utsidan av pipettspetsen påverkar resultaten. 4. Vi rekommenderar att en multikanalspipett används eftersom detta ger mer enhetliga reagensdispenseringar, inkubationstider och reaktionstider. 5. Det är ytterst viktigt att brunnarna tvättas ordentligt. Otillräckligt tvättade brunnar leder till höga bakgrundsvärden och kan ge falskt positiva resultat. Manuell tvätt: Aspirera brunnarnas innehåll och fyll därefter varje brunn med tvättbuffertlösning. Undvik korskontamination mellan brunnarna, särskilt i första tvätten efter aspirationen. Töm all tvättlösning från brunnarna genom att vända dem upp och ned och skaka därefter resterande tvättbuffert från brunnarna med en bestämd ”knyck” med handleden. Upprepa fyll- och torkstegen för totalt tre till fem tvättar. Brunnarna skall sedan knackas bestämt mot en pappershandduk eller annat absorberande material för att avlägsna alla spår av kvarvarande tvättbuffert. Om det automatiserade tvättsystemet för mikrobrunnar används blir brunnarna blir ordentligt tvättade, varför detta rekommenderas. OBSERVERA: Då det finns olika tvättekniktyper och automatiserade program kan antalet tvättar behöva justeras för optimala resultat. Varje laboratorium bör bestämma det mest effektiva antalet tvättar för sitt tvättsystem. 6. Om den resterande tvättbufferten inte avlägsnas ordentligt kan detta leda till ojämn färgutveckling. Mikrobrunnsremsorna skall läskas på absorberande papper eller handdukar för att minimera mängden kvarvarande tvättbuffert. 7. Det är av avgörande betydelse att samtliga steg sker i rätt tid. Samtliga serumprover skall spädas innan proceduren påbörjas och de måste dispenseras till mikrobrunnarna på så kort tid som möjligt (högst fem minuter). Batcharnas storlek skall väljas så att provhanteringen går så smidigt som möjligt under denna tidsperiod. Prov- och reagenshanteringen underlättas med hjälp av en multikanalspipett, varför detta rekommenderas. 8. Med undantag för den sista inkuberingen (substratlösning) börjar varje inkubationstid med att prov eller reagenser dispenseras. Inkubationen av substratlösningen måste vara exakt fem minuter för varje brunn. Alla prover och reagenser skall dispenseras i samma ordningsföljd och med en konstant hastighet. TOLKNING AV RESULTAT KVALITETSKONTROLL 1. Ämneskontrollbrunnen skall ha ett absorptionsvärde som understiger 0,250. Ämnesabsorptionsvärden större än 0,250 tyder på otillräcklig tvättning eller kontamination av reagenser. 2. Absorptionsavläsningen av ämneskontrollbrunnen (rad G) dras ifrån absorptionsavläsningarna för RELISA® procedurkontrollbrunn (RPC) (rad H) och respektive antigenbrunn (raderna A till och med F). Nettovärden som är lägre än noll betraktas som nollvärden. 3. RELISA® procedurkontrollbrunn (RPC) (rad H) skall ha en nettoabsorption större än 0,300. Absorptionsvärden som är lägre än 0,300 betyder att färgutvecklingen varit otillräcklig och serien är ogiltig. 4. 5. 6. 7. Varje ����� � skall � ������ antigenbrunn uppvisa ett värde på mindre än ����� � � ������ 20 ENA-enheter. ����� � � ������ Varje laboratorium skall fastställa en frekvens för positiva och �negativa ����� � ������kontrollsera baserat på förekomsten patienttester och laboratoriets ����� �av� ���� erfarenhet �����av� denna � �����analys. ������� REFERENSINTERVALL Referensintervallet upprättades genom att testa sera från 206 friska blodgivare, 105 kvinnor och 101 män, av vilka ingen veterligen hade haft någon reumatisk sjukdom tidigare. Dessutom hade data erhållits från 143 patienter med reumatiska sjukdomar som hade antikroppar mot en eller flera av antigenerna i denna analys, men som reagerade negativt på antikroppar mot andra antigener. Baserat på dessa data upprättades de normala frånslagsvärdena som mindre än 20 ENA-enheter. God laboratoriepraxis föreskriver att varje laboratorium skall fastställa sina egna normala spridningsområden baserat på patientpopulation och andra lokala faktorer. TESTETS BEGRÄNSNINGAR 1. Diagnos kan inte ställas enbart på grundval av antikroppar mot extraherbara nukleära antigener. Läkaren måste tolka dessa resultat med hänsyn till patientens tidigare sjukdomar och symptom, de fysiska upptäckterna och andra diagnostiska metoder. 2. Behandling bör inte påbörjas enbart på grundval av ett positivt test för antikroppar mot extraherbara nukleära antigener. Kliniska indikationer, andra laboratorieupptäckter och läkarens kliniska intryck måste beaktas innan behandling påbörjas. 3. Vissa patienter med autoimmuna sjukdomar kan ha obetydliga nivåer av antikroppar eller nivåer som är omöjliga att upptäcka, medan andra personer kan ha höga nivåer av antikroppar mot extraherbara nukleära antigener, men få eller inga tecken på klinisk sjukdom. Läkaren måste tolka testresultaten för antikroppar mot extraherbara nukleära antigener med hänsyn till patientens tidigare sjukdomar och symptom, de fysiska upptäckterna och andra diagnostiska metoder. 4. Antikroppnivåerna har ingen känd klinisk betydelse. PRESTANDA Immuno Concepts screeninganalys RELISA® jämfördes med andra liknande ELISA-analyser på marknaden, med dubbla immundiffusions- och kontraimmunelektroforestester som gjorts i referenslaboratorier och med immunoblot (Western Blot)-resultat som erhållits med en intern metod. Resultaten av alla metoder och den kliniska diagnosen av patienten togs med i beräkningen när förväntade eller ”korrekta” resultat testades för varje prov. Följande data erhölls med utgångspunkt från jämförelser. Tabell 2. Skillnaderna i specificitet beror på den ökade sensitiviteten hos ELISA-analyser jämfört med ”traditionella” metoder, exempelvis kontraimmunelektrofores och immundiffusion. KORSREAKTIVITET Sju prover användes för studier av korsreaktivitet. Dessa prover karaktäriserades väl av Western Blot, CIE och immundiffusion som monospecifika sera för var och en av autoantikropparna i screeningstestet RELISA®. Ingen korsreaktivitet noterades i något av dessa prov. Tabell 3. REPRODUCERBARHET För var och en av de sex antigenspecificiteterna kördes sex prover med tre olika lotnummer av antigenremsor vid tre olika tillfällen. Två av proverna var negativa, men låg nära frånslagsvärdet 20 ENA. Två prover var positiva, men låg nära frånslagsvärdet 20 ENA och två prover låg klart positivt över nivån 30 ENA . I inget av fallen gav ett negativt prov positiva resultat. Alla ”gränsfallspositiva” prover gav överlag resultat mellan 20 och 30 ENA-enheter och de klart positiva proven gav klart positiva svar. FÖRVÄNTADE VÄRDEN Förekomsten av autoantikroppar mot olika nukleära antigener varierar beroende på patientpopulation och förekomsten av kliniska reumatiska sjukdomar i denna. Iordningställande av arbetsblad: Märk det arbetsblad som medföljer satsen för att ange identifieringen för varje åttabrunnarsremsa med mikrobrunnar. 2. Framställning av tvättbuffertlösning (PBS Tween): Lös upp innehållet i en PBS-buffertpåse i en liter avjoniserat eller destillerat vatten. Tillsätt hela innehållet i en flaska med tvättbuffertkoncentrat i den upplösta fosfatbuffrade saltlösningen i en enlitersbehållare. Blanda väl. Tvättbuffertlösningen kan täckas och förvaras i 2-10 °C i högst fyra veckor. 3. Spädning av prover: Späd patientproverna till 1:40 genom att tillsätta 25 µl serum till 975 µl provspädningsvätska. Blanda väl. Kontrollerna tillhandahålls färdigspädda och behöver inte spädas ytterligare. 4. Iordningställande av mikrobrunnsremsor: Avlägsna nödvändigt antal mikrobrunnsremsor från deras påsar och placera dem i ramhållaren. Mikrobrunnsremsorna måste sitta ordentligt fast i ramhållaren. Placera ramhållaren på ett stadigt underlag, med bokstäverna A till H till vänster och siffrorna 1-12 på ramhållarens övre kant. Placera remsorna i ramhållaren så att den naggade änden på remsan passar in över det lilla stiftet på ramhållarens nedre kant. Tryck ned remsans båda ändar ordentligt så att de sitter säkert fast i ramhållaren. Remsor som sitter ordentligt fast i ramhållaren faller inte ut om ramhållaren vänds upp och ned. Märk den frostade övre änden av remsan med patientidentifikation och placering i ramhållaren. 5. Dispensering av serumspädningar: Dispensera 50 µl spätt patientprov i var och en av de åtta brunnarna i en multiparameterremsa. 6. Inkubation av remsor (30 minuter i rumstemperatur, det vill säga 19-23 °C): Odla i rumstemperatur i 30 minuter. Remsorna skall skyddas mot drag och temperaturväxlingar under inkubationstiden. Remsorna kan om så önskas täckas med genomskinlig tejp eller pappershandduk för att skydda dem mot damm och andra främmande ämnen. 7. Tvättning av remsor (se „Allmänna proceduranvisningar“ 5 och 6): Tvätta brunnarna tre till fem gånger med PBS-Tween tvättbuffertlösning. Manuell tvätt: Aspirera brunnarnas innehåll och fyll därefter varje brunn med tvättbuffertlösning. Undvik korskontamination mellan brunnarna, särskilt i första tvätten efter aspirationen. Töm all tvättlösning från brunnarna genom att vända dem upp och ned och skaka därefter resterande tvättbuffert från brunnarna med en bestämd ”knyck” med handleden. Upprepa fyll- och torkstegen för totalt tre till fem tvättar. Brunnarna skall sedan knackas bestämt mot en pappershandduk eller annat absorberande material för att avlägsna alla spår av kvarvarande tvättbuffert. 8. 9. Substratlösningen måste tillsättas i brunnarna i en jämn hastighet så att varje brunn inkuberas exakt lika länge (fem minuter). Substratlösningen i brunnar som inkuberas med positiva prover blir blå, medan lösningen i brunnar som inkuberas med negativa prover blir färglösa eller mycket svagt blå. 12. Inkubation av remsor (exakt fem minuter i rumstemperatur, dvs 19-23 °C): Odla i rumstemperatur i exakt fem minuter. Remsorna skall skyddas mot drag och temperaturväxlingar under inkubationstiden. 13. Dispensering av stoppreagens: När den första brunnen har inkuberats i exakt fem minuter skall 50 µl stoppreagens tillsättas i varje brunn, i samma ordningsföljd och med samma hastighet som substratlösningen tillsattes i brunnarna. När stoppreagens tillsätts skiftar den blå substratlösningen till gult, medan den färglösa lösningen förblir färglös. 14. Läsning av brunnarnas absorption: Brunnarna måste avläsas i plattläsande spektrofotometer inom 30 minuter efter tillsättning av stoppreagensen. Brunnarna avläses vid 450 nm mot ämneskontrollbrunnen. Om spektrofotometer med dubbla våglängder används skall referensfiltrets våglängd ställas in på 600-650 nm. Avläsning av mikrobrunnarna utan referensfilter ger högre absorptionsvärden.
