I confirm that I have arranged for the minor to be accompanied to the airport on departure and to be met on arrival by the persons named (Escort on arrival must present an
identity document with photo). These persons will remain at the airport until the flight has departed and will be available at the airport at the scheduled time of arrival of the
flight. Should the minor not be met as stated above, I authorize the carrier(s) to take whatever action they consider necessary to ensure the minor’s safe custody including a
return of the minor to the airport of departure, and I agree to indemnify and reimburse the carrier(s) for the necessary and reasonable costs and expenses incurred by them in
taking such action. In any case, I shall hold Thomas Cook Airlines Belgium their personnel and agents harmless against any damages sustained and costs incurred by
reason of or in connection with the minor’s journey. I, the undersigned parent or guardian of the minor named above, agree to and request the unaccompanied carriage of the
minor named above.
I certify that the minor is in possession of all travel documents required for the journey.
Je confirme que le mineur sera accompagné à l'aéroport de départ et qu’à l’arrivée, il sera accueilli à l'aéroport par la personne mentionnée ci-dessus. A l'arrivée,
l'accompagnateur doit être en mesure de présenter une pièce d'identité avec photo. Avant le départ, nous resterons à l'aéroport jusqu'au décollage de l'avion et à l'arrivée la
personne indiquée sera présente à l'aéroport afin d'accueillir le mineur à l'heure précise prévue de l'atterrissage. Si le mineur n'est pas accueilli, selon ce qui été convenu cidessus, je donne l'autorisation à la compagnie aérienne d'entreprendre toute action nécessaire afin de garantir la garde du mineur, y compris un vol retour vers le lieu de
départ. J'accepte de rembourser à la compagnie aérienne tous les frais nécessaires et raisonnables que cette procédure impliquera: En aucun cas, je ne ferai répercuter les
dommages et les frais encourus suite au voyage du mineur à Thomas Cook Airlines Belgium, son personnel et/ou ses agents. Je, soussigné, parent ou tuteur du mineur
susmentionné, donne l'autorisation et prie de: Transporter sans accompagnement le mineur susmentionné.
Je confirme que le mineur possède tous les documents nécessaires pour ce voyage. Je certifie que les informations données sont correctes.
Ik bevestig dat de minderjarige zal begeleid worden naar de luchthaven van vertrek en zal afgehaald worden op de luchthaven van aankomst door bovenvermelde personen.
De begeleider na aankomst dient een identiteitsbewijs met foto voor te leggen. Wij zullen aanwezig zijn op de respectievelijke luchthavens tot na het vertrek van het vliegtuig
(luchthaven van vertrek) en op het voorziene uur van aankomst van de vlucht (luchthaven van aankomst). Indien de minderjarige niet wordt afgehaald zoals hierboven
vermeld, verleen ik hierbij de toelating aan de luchtvaartmaatschappij om elke actie te ondernemen welke zij nodig acht opdat de bescherming van de minderjarige wordt
gegarandeerd, inbegrepen een terugvlucht naar de luchthaven van vertrek. Ik aanvaard de nodige onkosten welke deze maatregelen met zich meebrengen te vergoeden
aan de luchtvaartmaatschappij. Ik zal de schade en onkosten welke hieruit voortvloeien niet verhalen op Thomas Cook Airlines Belgium, zijn personeel en/of zijn agenten. Ik,
ondergetekende, ouder of voogd van het minderjarig kind hierboven vermeld, ben akkoord en vraag om: Het vervoeren zonder begeleiding van bovenvermelde minderjarige:
Ik bevestig dat de minderjarige in het bezit is van alle nodige documenten welke nodig zijn voor deze reis. Ik verklaar dat de door mij gegeven informatie correct
