About Us
Community Transition Services provides
students with an intellectual disability, a
Transition Plan that is person-centered
and is used to facilitate a smooth and
successful transition from school to
community life.
Transition Coordinators can assist high
school students and their families to plan
for their future and set personal goals.
Personal Goal Setting
Students will receive support to
pursue individual goals, including:
 Continuing or post-secondary
education
 Volunteer opportunities
 Employment opportunities
 Employment training
 Independent living skills
 Community participation and
awareness
 Recreation and leisure
 Health and safety

secondaires dans la communauté.
À propos de nous
Les services de transition communautaire
offrent aux étudiants ou étudiantes ayant
une déficience intellectuelle, un plan de
transition qui est axé sur la personne. Le
plan promeut une transition harmonieuse
et une réussite de l'école à la vie
communautaire.
Les coordonnateurs ou coordinatrices de
transition aident les étudiants et leurs
familles à planifier leur avenir et à fixer des
objectifs personnels.
Établir objectifs personnel
Les étudiants dans ce programme
recevront de l’appui afin de poursuivre
des objectifs individuels, y compris:
 Formation continuelle ou postsecondaire
 Possibilités de bénévolat
 Possibilités d'emploi
 Préparation à l'emploi
 Compétences nécessaires à une vie
autonome
 Participation et sensibilisation à la vie
communautaire.
 Récréation et loisir
 Santé et sécurité
