Health System In France - GP Years 2-4 – June 2015 HEALTH SYSTEM IN FRANCE GP YEARS 2-4 As soon as you have signed your doctoral contract, the social security ("sécurité sociale") is not supported by ENP anymore but by your employer. Indeed, as soon as you have signed your doctoral contract (contrat de travail), your employer, proceeds to all the necessary registrations. It is automatically deduced from your payslip. When you register at the university, do not subscribe to the student social security. If you want to subscribe to a complementary health insurance which is optional, it is your responsibility to find a provider and to pay the chosen complementary health insurance. In PhD, you have to register to the national social security, more precisely at your local CPAM (Caisse Primaire d’Assurance Maladie). How to proceed? Download the form entitled « Formulaire S1104 - Déclaration de changement de situation » available on Ameli’s website. Required items: - The Cerfa form filled in triple copies - A recent certified copy of your birth certificate with an official translation - A copy of your first payslip + 3 last payslips - A copy of your doctoral contract - A copy of your DUE (Déclaration Unique d’Embauche)1 - Bank account details (RIB) - A copy of your passport - A copy of your titre de séjour - A proof of address All these documents have to be send to the your local CPAM office.
