HOW DO I MAKE A DATE REQUEST? 2013 CAMP SEASONS ONLINE at www.campcourageous.org - follow link provided Monday 10 AM - Friday 12:30 PM. Adults with mild/moderate cognitive and/or physical disabilities. Fee: $400.00 with $75.00 non-refundable deposit due 1/18/2013. (100 waiver units) 1.Select your top three camping dates. Every effort will be made to place in the first date you request, but this is not always possible. Please provide your second and third options. 2.Select method to make date request on appropriate date/time: for “date requests.” Link will not open until the specified time. Read and follow all instructions. Provide all contact information, camper information, and top three camping dates. PHONE the camp at 319-465-5916 - enter extension for the request that you are making. Phone messages will not be accepted until specified time. Listen carefully to instructions provided. Provide all contact info, camper info, and the top three camping dates. 3.Once you have submitted your online request or recorded your voice mail message then you are done. Please refrain from making duplicate submissions online or additional phone calls since they will only delay the request process. FREQUENTLY ASKED QUESTIONS: Should I make requests online or by phone? The online system is recommended. Regardless of which you use, be prepared to make your request as promptly as you can on the date and time listed. Please only make one request using either system (not both). After I make my date request, how long before I am notified if I got the date I wanted? It may take two to three weeks to get campers scheduled. Each camper requires special consideration to make sure that their stay will be fun and safe. Please be patient during this time since hundreds of requests are submitted. Is there a waiting list? Yes. If all three of your dates are already filled and an alternate date will not work then you can be put on a waiting list. You will be notified about being put on a waiting list. When do I fill out the camper’s application, physical, and medical forms? Paperwork will be made available once a camper has been placed. After being notified that a date has been assigned, the application and physical forms will be mailed to you or you can fill them out online. If you have Internet access, the camp prefers that you use the online forms system to reduce paper, printing, mailing, and other costs. Who can answer other questions? Experienced staff, who plan each detail of the trip, accompany travel campers. Every Camp Courageous trip is catered to each camper group and aids campers in functioning successfully in a variety of public settings. A typical trip is created for campers that thrive in a 3:1 or greater ratio and are more independent. Camp also offers trips for campers who benefit from more enhanced supervision and care, which are indicated as Wheeling (1:1) and Assisting (2:1) trips. All participants must provide a NON-REFUNDABLE DEPOSIT within two-weeks of sign-up. If a deposit is not received within two weeks of signing-up for a trip then the camper’s name will be removed from the trip. Unless otherwise noted, deposits are $300 for week-long trips and $100 for weekend trips. Balances for non-flight trips are due at least 30 days prior to travel. For trips requiring flight, the balance is due at least 95 days prior to travel. Campers are welcome to pay their trip(s) balances at any time and are encouraged to set-up travel accounts to plan for future travel. REQUIREMENTS FOR INDIVIDUALS WHO PARTICIPATE IN TRIPS: • Be on-the-go A LOT. (Assisting & Wheeling trips are more relaxed paced) • Use stairs, elevators, and/or escalators. • Attend sporting and cultural events in large stadiums or venues (schedule permitting). • Cross city streets. • Get in and out of a 15-passenger motor coach. (With the exception of the “Wheeling” and week-long December trips, there is no lift on our vehicle. Please register accordingly.) • Ride in a motor coach for 3 or more hours. • Stay with the whole group. (wheeling trips plan for the most 1:1 time comparatively.) • Share a quad room with two other campers and one staff person. • Be independent in personal care (exceptions being our “Assisting” and “Wheeling” trips). • Have appropriate social skills. We will travel by Duck around the city, visit the Philadelphia Zoo, and many other historic sites. 8 am Check-In (10 people) $975 ALL ABOARD!! Take a train ride to our nation’s capital. We will visit Lincoln Memorial and tour many museums. Check-In TBA. $2,000 w/$500 deposit (30 people) - Sign-up before Dec. 13, 2013 Travel Program Policies — Prior to participating in the Camp Courageous Travel Program you should review all Travel Program Policies. They include important details about deposits, cancellations, and refunds. Travel Cancellation and Weather Policy – Camp Courageous does not provide campers, volunteers, or staff with traveler’s insurance. Therefore, if you would like to purchase individual travel insurance for your trip, you should contact your current insurance broker and ask about their travel plans/policies or seek a reputable insurer online. General Cancellation Policy: 1) All travel campers must submit a nonrefundable/ nontransferable deposit within two weeks after the camper has received notification from the Travel Director, confirming his/her spot on a Camp Courageous trip. Failure to submit the deposit within two weeks may result in the camper losing his/her spot on the trip. 2) There are no monetary refunds. We offer a Travel Program Credit which must be used toward future Camp Courageous travel. 3) If you cancel your trip within 95 days prior to the trip’s departure, you can apply the whole amount you have paid, to a future trip through our Travel Program Credit, with the exception of the deposit. 4) If the registration date has passed and not enough campers have signed up for the trip, the Travel Director has the right to cancel the trip and offer the camper a choice of another open trip (of equal cost) within the calendar year. 5) A prorated amount will be assigned as a Travel Program Credit for cancellations prior to 30 days before your scheduled departure date for driving trips and 95 days for flying trips. 6) No Travel Program Credit will be awarded if you cancel your trip for any reason within 30 days prior to your trip’s scheduled departure date (95 days for flying trips). Under special circumstances, determined on an individual basis by the Travel Director, a Travel Program Credit may be awarded for use on future Travel Program trips for that particular camper. Such circumstances may include camper hospitalization or diagnosis of a serious illness, a pro-rated amount will be assigned to your Travel Program Credit if appropriate documentation of illness or hospitalization is provided to the Travel Director within 1 week of cancellation. 7) Failure to complete and return a completed Camper Application with an updated medication list and the travel camper’s prepackaged oral medications, including the required, additional full day’s dose(s) at least 14 days prior to departure, will be considered a cancellation and a forfeiture of the total amount of the trip’s cost. (Each Camper Application is used by all Camp Courageous programs- Residential, Respite, and Travel. Therefore, only one application is required annually.) 8) Campers who, without at least 2 hours advance notice, fail to arrive before or during check-in on the planned departure date, will forfeit the trip. 9) If a participant cannot continue on with a trip, the camper’s guardian or facility will be contacted. Arrangements will be made to send the camper home by whatever means of transportation is deemed appropriate. This may mean that a camper would need to be picked up, or be sent home by plane or bus and a Camp Courageous staff member would accompany the camper, at the camper’s expense. Reasons campers may need to be sent home include, but are not limited to illness, injury, behavior not conducive to harmony of the group, and other extenuating circumstances. There are no refunds or credits issued if any of these circumstances occur. 10) Prices are subject to change, although all effort will be made to maintain the prices as listed for the calendar year. If for some reason changes in price are necessary, advance notice will be given, as seen fit by the Travel Director. 11) If while traveling, any concerns or problems arise, please see the itinerary for phone numbers of our lodging and the Travel Director’s emergency contact number. For additional information regarding trip cancellation and weather policies please visit the Camp Courageous website, www.campcourageous.org and click on Camper Info. 12) If a camper must cancel from going on a trip then their guardian/group home MUST CONTACT THE TRAVEL DIRECTOR at 319465-5916, ext. 2330. Only the TRAVEL DIRECTOR can officially cancel a camper from a trip (no other staff person is authorized to cancel a travel camper). Due to the time sensitivity of a cancelation all correspondence will be closely logged with times and dates. A guardian/group can not assume their camper is cancelled until confirmation is given directly by the TRAVEL DIRECTOR - no exceptions. Weather Policies: 1) The Camp Courageous Travel Program will run in any weather, including conditions of precipitation (i.e. rain, hail, sleet, snow) EXCEPT when the weather is deemed unsafe. 2) In order to properly evaluate the actual weather conditions anticipated, Camp makes weather-based decisions according to the 3-day forecast prior to departure. 3) Cancellations and rescheduling due to unsafe weather are at the discretion of the Camp Courageous Program Directors and we reserve the right to modify the itinerary for your safety. 4) If a trip is canceled by the Camp Courageous Program Directors before departure due to unsafe weather, you can apply the pro-rated amount to a future trip through our Travel Program Credit. 5) If a third-party vendor is involved in our travel program and cancels an activity or event due to unsafe weather, we must adhere to their cancellation policy. We will credit you according to their policy, with the credit to be used for future travel with Camp Courageous. 2013 RESPITE CARE WEEKENDS — Respite Care Weekends are provided for any individual with a disability that is living at home with their guardian/parent. Weekends run from Friday, 7:00 PM to Sunday, 1:00 PM. •Fee: $175 with $50 non-refundable deposit (43 waiver units). •Limit of one registration space per phone call/web request. •Limit of six respites per year. •Request dates for each weekend occurs two to three weeks prior to the scheduled weekend (see schedule). Pictures of each type are shown below. Prepackaged medications have a set of compartments or “bubbles” for each day of the week. The medications are packaged together according to the time they are to be given. FAILURE TO SEND MEDS TWO WEEKS PRIOR TO ARRIVAL WILL DELAY YOUR CHECK-IN. “Bubble pack” medications prepared by a pharmacist. Medication cassettes can be purchased at many pharmacies and drug stores. • Request the pharmacist to pack the designated amount of medication for the camper’s duration at camp plus one extra dose. The extra dose, if not needed, will be returned to you at the end of the week. • Use the multi-dose system when packaging medications. In a multi-dose system, each med time for each day has one container/compartment for all meds to be administered at that time. • Label the medication bubble pack or cassette with the camper’s name. • Provide a current list of the camper’s medications. Please include: name of medication, dosage (mg. and number of tabs), and times for administration. • Package and label separately any medication that requires a blood pressure, pulse check, or other nursing intervention prior to med administration. “Odd-Time” medications should also be identified separately. • Use caution before sending non-FDA approved herbal or homeopathic remedies to camp. Contact the Nursing Department for specific packaging instructions. • Bring liquid and powder medications, inhalers, eye drops, and topical treatments the day of arrival and bring to nursing during check-in. The Camp Courageous Nursing Staff is not liable for any errors in “pre-packaged” medications sent to camp. The Nursing Department appreciates your cooperation in handling camper medications. Please call Sharon Roller with any questions at 319-465-5916, ext. 2200 or e-mail [email protected] CHECK-IN PROCESS ON DAY OF ARRIVAL — (EVERYONE MUST CHECK-IN) Camp Courageous is located at 12007 190th Street, Monticello, IA 52310, which is approximately five miles southwest of the city of Monticello. For those using Highway 151, use exit 65 and follow the signs. When arriving at Camp Courageous, turn into the main gate and follow the signs to the arrival area. Staff will greet you and connect your camper with their counselor and group. Campers will go with counselors to unpack in the cabins while guardians/staff will go to nursing to checkin all medications. Even if a camper does not have medications, everyone must check-in at nursing. Check-in is done on a first-come, first-serve basis. However, priority will be given to those that have sent in their medications two weeks prior to the arrival date. The entire check-in process can take well over an hour to ensure that each camper’s needs are met as much as possible. Please allow enough time for this process. Arriving earlier than the stated arrival time will not guarantee you a shorter waiting time. During the nursing check-in process, you will: • Verify that all necessary paperwork has been submitted and signed. • Update and verify contact information. • Review dietary concerns and bowel assessments. • Update camper’s recent medical history (physicals are required every two years). • Update current medications taken by the camper to be entered in the camp’s computer system. Sending or faxing changes in medications to the camp two weeks prior to camper’s arrival will greatly enhance the check-in process.
