ATTENTION Non-pyrogenic. Do not use the device in case of open or damage package. Avoid exposure to high temperature. Protect from light. This products has been designed to be used only once. Therefore DO NOT RE-USE OR RESTERILIZE. GTA Srl - Società Unipersonale substract itself from any responsibility in the event the product is used more than once. Only a physician familiar with its application may use this product. CONTRAINDICATIONS ECS EVOLUTION cannot be prescribed if there is some kind of infection, specially genital or related to the urinary tract. ECS EVOLUTION must not be used with patients under anticoagulation therapy and during pregnancy. ECS EVOLUTION should be used with precaution in diabetic women. INFORMATION FOR THE PATIENT The surgeon has the responsibility of informing the patient about the possible complications related to the implantation of ECS EVOLUTION before the surgery. GTA Srl - Società Unipersonale and its distributors delegate to the surgeon the responsibility of informing to the patient about the advantages and the possible risks related to the implantation of sling. The patient should be advised that the implantation of ECS EVOLUTION might loose effectiveness with future pregnancies. It is convenient that the patient avoids weight lifting or intense exercises that involve effort (cycling, aerobics, etc …) for the first three to four weeks after the surgery, and sexual relationships for at the least one month. It is essential that the patient contacts the surgeon immediately in case of: - pain or difficulty during urination. - suprapubic or vaginal pain. - presence of serous, bleeding or purulent secretion. - fever. - bleeding or other problems. COMPLICATIONS The possible complications associated with the use of sling must be discussed with the patient before surgery. The use of this sling may result in some complications associated with the medication and the methods used in the surgical procedure as well as the reaction of the patient or the degree of intolerance to any foreign body implanted in the body. Other complications reported with this or other sling include the following: wound infection including a secondary necrosis. urethral or vaginal erosion. - suprapubic/vaginal pain or inflammation. - serous or blood purulent flow. - injuries of vessels, nerves or bladder. - presence of vaginal fistula. - vesical instability. - vaginal dehiscence. - urinary obstruction. SURGICAL PROCEDURE General chirurgical indications: Prepare the patient for the surgery as usual. ECS EVOLUTION is implanted in a vaginal approach under regional, local or general anaesthesia. The patient must remain in litotomic position. Chirurgical specific indications: The sling is positioned under the urethra being under the form of an U and maintaining the extremities anchored to the endopelvic band and to the adjacent tissues (Free Endopelvic anchorage) 1. Will be positioned on the vaginal mucus suburethral 2 pincers Allis: the first one of 1 cm form the outer urethral meatus and the second one 2 cm. lower, and after you proceed with a scalpel and make a sagittal incision on the vaginal mucosa 2. The vaginal mucosa is separated with blunt scissors, from the urethra and bladder dall'uretra, dissecting the right paraurethral space with the scissors headed to the right shoulder of the patient, about 45 º, until you cross the bottom edge of the pubic bone branch and the same is repeated on the left side. 3. Hook one end of the mesh ECS EVOLUTION with a small curve Pean forceps, leaving 1 inch of free space, it is introduced through the paraurethral space right dissected; then placed in a vertical position the Pean forceps and striving on the back of the pubis, puts pressure to pierce the urogenital diaphragm, through the endopelvic band; with the other end of the mesh you do the same, allowing the mesh to position itself in perfect grip of the urethra. 4. Suture the vaginal incision by continuous absorbable point. DESCRIPTION Le système ECS EVOLUTION est indiqué comme un ruban sous urétral pour le traitement chirurgical de l'incontinence urinaire à l'effort féminin par hyper mobilité urétrale et par déficience sphinctérienne intrinsèque urétral. Dans ce cas, une régulation post chirurgicale de la tension de la maille est requise par le biais de deux fils tenseurs suprapubien. ATTENTION Apyrogène. Ne pas utiliser le mécanisme si le paquet est ouvert ou défaillant. Eviter l'exposition à de hautes températures. Protéger de la lumière. Ce produit a été conçu pour une utilisation unique. Par conséquent, NE PAS RÉUTILISER NI RESTÉRILISER. GTA Srl - Società Unipersonale décline toute responsabilité en cas de réutilisation. Ce produit ne peut être utilisé que par du personnel médical familiarisé avec cette application. CONTRE INDICATIONS ECS EVOLUTION ne peut être prescrit en cas d'infection, spécialement génitale ou ayant un lien avec la partie urinaire. ECS EVOLUTION ne doit pas être utilisé sur des patients en thérapie avec des anticoagulants ou chez des patientes enceintes. ECS EVOLUTION doit être utilisé avec précaution sur des patients diabétiques. 1. Sub-urethral incision 1. Incision sub-urétrale AVERTISSEMENTS La prothèse ne doit pas être manipulée avec des objets pointus, dentés ou aiguisés car n'importe quel enfoncement, détérioration, perforation ou déchirure peuvent être la cause de complications ultérieures. ECS EVOLUTION doit être implanté sans tension, c'est-à-dire que la maille centrale perforée doit restée appuyée sur l'urètre sans aucune tension. L'implantation en tension implique, comme conséquence, des problèmes d'érosion urétrale et de rétention urinaire. En passant l'aiguille, il pourrait y avoir une perforation ou une lésion dans les vaisseaux sanguins, la vessie, les organes ou les nerfs, une autre intervention chirurgicale de réparation serait nécessaire. INFORMATION POUR LA PATIENTE Le Chirurgien a la responsabilité d'informer la patiente avant la chirurgie sur les éventuelles complications liées avec l'implantation de ECS EVOLUTION. GTA Srl - Società Unipersonale et ses distributeurs délèguent au Chirurgien la responsabilité d'informer la patiente des avantages et des possibles risques liés à l'implantation et de l'utilisation du ruban. La patiente doit être prévenue que des grossesses futures pourraient invalider les effets chirurgicaux de l'implantation de ECS EVOLUTION. Il serait convenable que la patiente ne lève pas de poids et évite les exercices intenses qui impliquent des efforts (aller en bicyclette, courir, etc..) au cours des trois ou quatre semaines suivant la chirurgie, et d'avoir des relations sexuelles qu'un mois après la réalisation de l'opération au minimum. La patiente devra recourir immédiatement au Chirurgien en cas de : Douleur ou difficulté à uriner. Douleur supra pubienne ou vaginale. Présence de sécrétions séreuses, sanguinolentes ou purulentes. Fièvre. Hémorragies ou autres gènes. 2. Dissection of para-urethral space 2. Dissection de l’espace para-urétral COMPLICATIONS Les éventuelles complications associées à l'utilisation du ruban doivent être commentées avec la patiente avant la chirurgie. L'utilisation de ce ruban peut apporter des complications découlant de la médication ainsi que des méthodes utilisées au cours du procédé chirurgical, et également des complications liées à la réaction de la patiente ou du degré d'intolérance par rapport à un corps étranger implanté dans son corps. Autres complications rapportées avec ce ruban ou autres incluent : Infection dans la plaie, en comprenant nécrose secondaire. - Erosion urétrale ou vaginale. - Douleur ou inflammation suprapubienne/vaginale. - Décharges purulentes, séreuses ou sanguines. - Lésions sur des vaisseaux, nerfs ou sur la vessie. - Présence de fistule vaginale. - Instabilité vésicale. - Déhiscence vaginale. - Obstruction urinaire. TECHNIQUE CHIRURGICALE Technique chirurgicale générale On prépare la patiente pour une chirurgie générale habituelle. La technique chirurgicale peut être réalisée sous anesthésie locale, loco régionale ou générale. On place la patiente en position de lithotomie. Technique chirurgicale spécifique Ce ruban se place sous l'urètre en forme de U, ses extrémités restent ancrées au fascia endopélvien et des tissus adjacents (Ancrage Libre Endopélvien) 1. On maintient la muqueuse vaginale sous urétrale avec 2 pinces d'Allis : l'une à un centimètre du méat urétral externe et l'autre à 2cm plus bas et on réalise, à l'aide d'un bistouri, une incision sagittale dans la muqueuse vaginale. 2. On sépare à l'aide d'un ciseau mousse la muqueuse vaginale de l'urètre et de la vessie, en disséquant l'espace paraurétral droit avec le ciseau dirigé jusque l'épaule droite de la patiente (envir 45º) jusqu'à atteindre le bord inférieur de la branche de l'os pubien, on répète l'opération du coté gauche. 3. On attrape une extrémité de la maille ECS EVOLUTION avec une petite pince courbe de Pean en laissant 1cm libre, on y introduit, par l'espace para urétral droit disséqué, et en plaçant en situation verticale la pince de Pean et collé en arrière du pubis, on presse jusqu'à perforer le diaphragme urogénital en traversant le fascia endopélvien, avec l'autre extrémité de la maille, on réalise la même manœuvre, en laissant ainsi la maile adhérée à l'urètre. 4.
