Abstract In their clinical practice, the observation of walking is very important to osteopaths. They consider that walking can be altered by osteopathic disfunctions of the lowers limbs or remote disfunctions.As far as we know, there is no survey existing showing that osteopathic disfunctions, wherever they are localized,can have an impact on walking. We contacted the «Centre Hospitalier Universitaire de Nantes» (CHU) in the site «Hôpital Saint Jacques», Department Physiatrics and Rehabilitation where the analysis laboratory of movement is situated.This laboratory is equipped with a system of Instrumented Gait Analysis (IGA) which includes : - a video equipment : 2 cameras (100 images / second); - a system of passive markers allowing to capture in three dimensions the information situated on the limbs of the patient, making it possible to calculate the articular angles ; - three plateforms to register the reaction forces on the ground ; - one electromyographic system to register the dynamic muscle activity on moving. The IGA is a rigorous method for measuring the parameters of walk : it allows analysing every movement establishing a relation among anatomic, kinematic, kinetic, energetic and electromyographic information. It specifies every gait dysfunction, considering all articulations and muscles, one by one. Résumé Dans leur pratique clinique, les ostéopathes attachent une grande importance à l’observation de la marche. Ils considèrent qu’elle peut être perturbée par des dysfonctions ostéopathiques des membres inférieurs ou à distance. À notre connaissance, il n’existe pas d’étude montrant que des dysfonctions ostéopathiques, quelles que soient leur localisation, aient un impact sur la marche. Nous avons contacté le Centre Hospitalier Universitaire de Nantes (CHU) dans le site Hôpital Saint Jacques, pôle médecine physique et réadaptation où se trouve le laboratoire d’analyse du mouvement. Ce laboratoire est équipé d’un système d’Analyse Quantifiée de la Marche (AQM) qui comprend : - du matériel vidéo : 2 caméras (100 images à la seconde) ; - un système de marqueurs passifs permettant de capturer en trois dimensions les coordonnées de marqueurs situés sur les membres du patient afin de calculer les angles articulaires ; - trois plates-formes pour enregistrer les forces de réaction au sol ; - un système électromyographique pour enregistrer l’activité musculaire dynamique lors du déplacement. L’AQM mesure de manière rigoureuse les paramètres de la marche : il permet d’analyser chaque mouvement en mettant en relation les données anatomiques, cinématiques, cinétiques, énergétiques et électromyographiques. Il spécifie tous les dysfonctionnements de la marche, articulation par articulation, muscle par muscle. Des études sont nécessaires pour vérifier si l’AQM est un moyen de mesurer l’impact de dysfonctions ostéopathiques des membres inférieurs ou à distance sur la marche, et si ce système permet de mettre en évidence les effets d’un traitement ostéopathique. n 28 S i =1 x x x x x x La Revue de l'Ostéopathie x x2 (p=0,05) x xx x x x a Numero 1-2: 2012
