I, the undersigned parent or guardian of the minor named above,
agree to the unaccompanied carriage of the minor named above.
Je confirme avoir organisé l‘accompagnement de l‘enfant à
l‘aéroport du départ et sa prise en charge à l‘arrivée par les
personnes mentionnées ci-dessous.
Ces personnes resteront à l‘aéroport jusqu‘au départ du vol et/
ou seront présentes à l‘aéroport à l‘heure prévue d‘arrivée.
I confirm that I have arranged for the minor to be accompanied
to the airport on departure and to be met on arrival by the persons
named below. These persons will remain at the airport until
the flight has departed and will be available at the airport at the
scheduled time of arrival of the flight.
Au cas où, à l‘arrivée, l‘enfant ne devrait pas être pris en
charge comme prévu, j‘autorise par la présente la ou les
compagnie(s) aérienne(s) à prendre les mesures qui seront
jugées nécessaires pour assurer la sécurité de l‘enfant, celles-ci
pouvant éventuellement comprendre son retour à l‘aéroport
de départ. Je m‘engage à indemniser et rembourser au transporteur les frais nécessaires qu‘il aura engagés.
Should the minor not be met as stated above, I authorize the
carrier(s) to take whatever action is necessary to ensure the minor’s
safe custody including a return of the minor to the airport of
departure, and I agree to indemnify and reimburse the carrier(s) for
the necessary costs and expenses incurred by them in taking such
action.
J‘assume néanmoins toute responsabilité en cas de dommages
et de coûts qui seraient causés à Germania et/ou autres compagnies aériennes, leur personnel ou leurs agents d‘exécution
en raison ou dans le cadre du voyage de l‘enfant.
