In this newsletter, we would like to review a new type of sinus procedure that we are currently offering our patients, commonly referred to as Balloon Sinuplasty. Traditionally, patients with rhinosinusitis have been limited to two treatment options: medical therapy or conventional sinus surgery. Medical therapy can alleviate symptoms for some patients. For approximately 20-25% of sufferers, however, medical management alone is not adequate, and sinus surgery is the next hope in finding relief. Conventional sinus surgery, however, requires bone and tissue removal in order to open up blocked sinus passageways. Currently, many patients are benefiting from Balloon Sinuplasty, a minimally invasive, endoscopic procedure for treating rhinosinusitis. The Balloon Sinuplasty technology, developed by Acclarent, utilizes a small, flexible sinus balloon catheter to open up blocked sinus passageways, restoring normal sinus drainage and function. When the sinus balloon is inflated, it gently restructures and widens the walls of the sinus passageway while maintaining the integrity of the sinus lining. These balloons, once properly 1 positioned using fluoroscopic guidance, gently open blocked sinus openings. In many cases, no tissue or bone removal is necessary. • Reduced Bleeding Because in many instances no tissue or bone is removed, there may be reduced bleeding associated with the procedure. As a result, the need for uncomfortable nasal packing may also be eliminated. • Improved Recovery Time While recovery time varies with each patient, many people can return to normal activities within 24 hours. • Does Not Limit Additional Treatment Options This technology is an endoscopic tool and may be used with other medical therapies or sinus surgery techniques. It does not limit future treatment options for individuals with progressive disease. Balloon Sinuplasty: Surgical Technique Balloon Sinuplasty is performed by a qualified Otolaryngologist-Head and Neck Surgeon specifically trained in this technique. The procedure requires fluoroscopy to properly guide the balloon tipped catheters into position within the sinus osteum. The diagrams below outline the steps in this surgical procedure. To gain initial sinus access, the Sinus Guide Catheter is introduced into the target sinus under endoscopic visualization. A flexible Sinus Guidewire is introduced through the Sinus Guide Catheter and gently advanced into the target sinus under fluoroscopic guidance. The Sinus Balloon Catheter tracks smoothly over the Sinus Guidewire and positioned across the blocked ostium. Using fluoroscopy, the position of the Sinus Balloon Catheter is confirmed. It is gradually inflated to gently restructure the blocked ostium. The Balloon SinuplastyTM system is removed, leaving the ostium open and allowing the return of normal sinus drainage and function. There is little to no disruption to mucosal lining. 2 Who is a Candidate for Balloon Sinuplasty? Balloon Sinuplasty is a safe and effective treatment for most patients with chronic and recurring acute rhinosinusitis that have not responded to medical treatment. Unfortunately, patients with nasal polyposis will still require traditional sinus surgery in order to remove the polyps. As with any surgical approach, there are risks, and results may vary from patient to patient. Summary Balloon Sinuplasty is a new, minimally invasive surgical procedure for treating patients with chronic rhinosinusitis. A new alternative to rhinosinusitis surgery, Balloon Sinuplasty is an outpatient procedure that gently stretches and widens the sinus openings, thereby improving sinus drainage. Balloon Sinuplasty differs from traditional surgery for rhinosinusitis in several ways. Traditional sinus surgery involves the removal of tissue from around and within the paranasal sinuses. This requires longer time in the operating room. Traditional sinus surgery can also cause scarring of tissue, resulting in the potential need for additional sinus surgery in up to 20% of patients. Balloon Sinuplasty utilizes catheters and inflatable balloons that widen the sinus ostea, improving drainage, and thereby providing relief from rhinosinusitis. As a result of this minimally invasive approach, Balloon Sinuplasty minimizes patient discomfort, bleeding and scarring. It also results in quicker recovery times, with most patients resuming normal activities within 24 hours of the procedure. If you suffer from chronic or recurrent acute rhinosinusitis, and if you have not obtained adequate relief from medical therapy, you may be a candidate for this minimally invasive surgical approach.
