In order to initiate the claim process, fill out the attached CLAIM REPORT, and send it by e-mail or fax (see next page).

If the product needs to be returned, our Customer Technical Support department will provide you with an RMA number
and the necessary information to return the product to the factory.

Our Customer Technical Support department will also check for the warranty status of the product.
