BERLE Clara, MARSOLLIER Estelle, MICHEL Aurélie Résumé: Autrefois, la lutte contre le dopage visait à limiter les actes frauduleux dans le monde du sport. Aujourd’hui elle est également un enjeu de santé publique étant donnée la méconnaissance des dangers et l’usage non encadré des produits dopants. On peut alors se demander : Quels sont les risques associés aux effets des produits dopants ? A la suite de l’affaire Festina en 1998, l’Agence Mondiale Antidopage (AMA) a été créée, et a bouleversé la réglementation sur le dopage. Depuis, les scandales associés au dopage se multiplient. En effet, le sport est devenu un enjeu politico-financier soumettant de fortes pressions aux athlètes. Malheureusement, les vrais dangers du dopage sont souvent méconnus. C’est pourquoi, de nombreux sportifs ont recours à l’utilisation abusive de médicaments normalement destinés à soigner des malades. Il existe de nombreuses classes de substances et méthodes de dopage interdites par l’AMA. Les stéroïdes anabolisants androgéniques (AAS) représentent la première classe des agents dopants interdits en et hors compétition sportive. Ils sont dérivés de la testostérone (Figure-1) et caractérisés par un noyau hydrophobe. Ces molécules sont utilisées pour augmenter la masse et la force musculaire. En effet, les anabolisants augmentent la population de cellules souches musculaires en inhibant l’apoptose ou en augmentant leur division. De plus, la testostérone augmente la synthèse du facteur de croissance IGF-1, qui permet la synthèse protéique du muscle squelettique[1]. Les AAS ont également un effet sur l’oxygénation des muscles en augmentant d’une part l’hématocrite, proportion d’érythrocytes dans le sang, à travers plusieurs mécanismes. La testostérone stimule la production d’érythropoïétine (EPO). Ensuite la testostérone favorise la libération du fer (essentiel à la production des érythrocytes) en inhibant l’hepcidine, molécule inhibant la libération du fer. D’autre part, la testostérone améliore la distribution de l’oxygène aux tissus en favorisant son relargage par les érythrocytes[2]. Cependant, en utilisant ces substances, les sportifs s’exposent à des dangers pour leur santé mentale et physique. L’exposition chronique aux AAS augmente l’agressivité. D’autre part, les AAS ont des effets neurotoxiques similaires à ceux impliqués dans la pathogénèse de certaines pathologies neurodégénératives (maladie d’Alzheimer, Parkinson, etc.). Ces effets sont : une augmentation du stress oxydatif préexistant de la cellule, de la mort neurale par apoptose, et une toxicité due à une surexcitation des neurones par le glutamate[3]. Enfin, les AAS présentent des dangers au niveau vasculaire. Ils induisent une hypertension artérielle due à la perturbation de la dynamique mitochondriale qui inhibe la production du vasodilateur, le monoxyde d’azote, dans le cadre d’une activité physique. En conclusion, la toxicité induite par les AAS est difficile à étudier car les informations données par le sportif sont basées sur son honnêteté. De plus, la polymédication rend difficile la distinction entre les effets causés par les AAS et ceux causés par d’autres drogues. Figure 1: Structure en 3D de la testostérone. Indeed, sport has become a political and financial stakes subjecting intense pressure to athletes. Unfortunately, the real dangers of doping are often unknown. Therefore, many athletes rely on the misuse of drugs normally used to treat patients . There are many classes of substances and method of doping forbid by AMA. Androgenicanabolic steroids (AAS) represent the first class of doping agents forbid in and out of tournament. They are derived from testosterone (Figure-1) and characterised by a hydrophobe nucleus. These molecules are used for increase the muscular masse and strength. Indeed, anabolic steroid increase the population of muscular stem cells by inhibiting cells death and increasing synthesis of growth factor IGF-1, which allow the production of protein of skeletal muscle[1]. AAS have also an effet on oxygenation by rising on one hand hematocrit, which is the proportion of erythrocytes in blood, through several mecanisms. Testosterone can cause a significant increase on the production of eythropoietin (EPO). Then, testosterone boost the iron release (essential in erythrocyte production) by inhibiting hepcidin, molecule repressing the iron release. On the other hand, testosterone enhance oxygen distribution to tissus by enhancing its release by erythrocytes[2]. However, the use of these substances in athletes exposed them to dangers for their physical and mental health. Chronic exposure to AAS increases aggressiveness. On the other hand, AAS have neurotoxic effects similar to those involved in the pathogenesis of neurodegenerative diseases (Alzheimer's, Parkinson's, etc.). These effects are: an increase in the pre-existing oxidative stress of the cell and neural death by apoptosis, and toxicity due to overstimulation of neurons by glutamate[3]. Finally, AAS show dangers at vasculary level. They induce high blood pressure due to a perturbation of the mitochondrial dynamic which inhibit the production of oxyde nitric ( a vasodilatator), in the context of physical activity In conclusion, toxicity induced by AAS is difficult to study because of the fact that information provided by the sportsman are based on their honesty. Moreover, polypharmacy makes difficult distinction between effects caused by AAS and those caused by other drugs. Figure 2: 3D structure of testosterone.
