Interpretation of results must be made within the context of the patient’s clinical history and other diagnostic tests by a certified professional. Introduction The human BCR (breakpoint cluster region) gene consists of 23 exons spanning 135 kb on chromosome 22 band q11 (1, 2). The BCR protein contains a unique serine/threonine kinase activity and at least two SH2 binding sites encoded by the first exon of the gene and a C-terminal domain that functions as a GTPase-activating protein for p21rac (1, 3). The BCR translocation has been identified with different chromosome partners (4). The two DNA probes used for splitsignal FISH detection of translocations involving the BCR gene are designed to hybridize upstream and downstream of the common breakpoint cluster region. Co-localization of the probes results in a yellow signal, whereas translocation events in the region will split the signal in separate green (fluorescein) and red (Texas Red) signals. Target (red) The Texas Red-labeled DNA-probe (BCR-TR) anneals to a 333 kb area centromeric to the BCR breakpoint cluster region on chromosome 22q11. Target (green) The fluorescein-labeled DNA-probe (BCR-Flu) anneals to a 408 kb area telomeric to the BCR breakpoint cluster region on chromosome 22q11. Probe gap The gap between BCR-TR and BCR-Flu is 155 kb. Reagent provided BCR FISH DNA Probe, Split Signal, is a mixture of two fluorochrome-labeled DNA probes and unlabeled PNA blocking probes. The DNA probes in the mixture consist of a fluorescein-labeled probe (BCR-Flu) and a Texas Red-labeled probe (BCR-TR), and should be used in combination with Dako Cytology FISH Accessory Kit, code K5499, on cytology specimens. The reagent is provided in liquid form in hybridization solution containing 45% formamide, 10% dextran sulphate, 300 mmol/L NaCl, 5 mmol/L phosphate, and blocking agent. Each vial contains 200 µL corresponding to 20 tests (10 µL per test). Specificity 98.8%. Defined as the mean of 6 independent countings of 1050 cells from normal peripheral blood. Sensitivity 97.3% Defined as the mean of 6 independent countings of 1050 cells in a translocation-positive cell line BV173 (5). Cut-off value When counting 50 cells, specimens with more than 4 split signal events should be considered BCR translocation positive. Based on the specificity and sensitivity of the probe, this will ensure that 99% of all specimens containing less than 2% cells with split signal events will be characterized as translocation negative, while 99% of all specimens containing more than 12% cells with split signal events will be characterized as translocation positive. In case of doubt, the specimen should be re-counted. Les deux sondes ADN utilisées pour la détection du signal FISH dédoublé des translocations impliquant le gène BCR sont conçues pour une hybridation en amont et en aval de la région BCR commune. La colocalisation des sondes donne un signal jaune alors que les événements de translocation dans la région dédoublent le signal en un signal vert (fluorescéine) et un signal rouge (Texas Red). Valeur de coupure Dans un comptage de 50 cellules, les échantillons avec plus de 4 signaux dédoublés doivent être considérés comme étant translocation BCR positifs. Sur la base de la spécificité et de la sensibilité de la sonde, cela assure la caractérisation de 99 % de tous les échantillons contenant moins de 2 % de cellules avec des signaux dédoublés comme étant translocation-négatifs, alors que 99 % de tous les échantillons contenant plus de 12 % de cellules avec des signaux dédoublés seront caractérisés comme étant translocation-positifs.
