Block 1.
Enter the COMSEC Account No. and name of the account ordering the key.
Case 1.
Entrer le numéro et le nom du compte COMSEC à l’origine de la commande.
Block 2.
Enter the transaction ID for the order. It includes the COMSEC Account No. of the OPM or
STAR that is submitting the order, the current date in YYYYMMDD format and a one up sequence
number from the current month starling back at 1 each month.(e.g. 0001, 0002, etc)
Case 2.
Entrer l’ID de transaction de Ia commande, y compris le numéro de compte COMSEC du gestionnaire des
privilèges de commande (OPM) ou du demandeur d’attribution de titre abrégé (STAR) qui
présente Ia demande, Ia date au format AAAAMMJJ et un numéro séquentiel correspondant au mois en
cours en recommençant à 1 chaque mois (p. ex. 0001, 0002, etc.).
Block 3.
Enter a one up number identifying the line item.
Case 3.
Entrer un numéro ascendant correspondant à la ligne d’article.
Block 4.
Enter the Short title of the key being ordered. If this is a continuing requirement,
obtain the short title from your previous order. If a new short title is required, please
enter "New" as the short title.
Case 4.
Entrer le titre abrégé de la clé qui est commandée. S’il s’agit d’un besoin permanent,
utiliser le titre abrégé inscrit sur votre dernière commande. Pour obtenir un
nouveau titre abrégé, indiquer « Nouveau ».

Enter the quantity of key required. No more than 200 keys can be delivered per CD-ROM order. If you
require more than 200 keys, please submit on a separate key order.

Select one classification of TOP SECRET, or SECRET for KEY MAT.
The User Data File is UNCLASSIFIED.
Case 7.
Sélectionner Ia classification TRÈS SECRET ou SECRET pour le MATÉRIEL DE
CHIFFREMENT. Le fichier de données utilisateur est NON CLASSIFIÉ.
Block 8.
Enter the date that you would like to receive the material. Key orders take 2-3 weeks to process.

For CSEC use.
Entrer Ia date à laquelle vous aimeriez recevoir le matériel. II faut prévoir un délai de deux
à trois semaines pour le traitement d’une demande de clé.

Enter any remarks for this line item.

The ordering COMSEC Account’s OPM or STAR must sign this form.
