Dear Costco Member,
Costco records indicate that you may have purchased a Monarch Bunk Bed between November 2009 and
April 2010 from www.costco.ca
Monarch Specialties Inc., in cooperation with Health Canada, is announcing a voluntary recall of a
Monarch Bunk Bed identified by the model number I5032 and UPC 021032203542. The model number
appears on a label affixed to one of the long bed rails.
The wooden side rails that run from the headboard to the footboard and hold the bunk bed's mattress in
place can split and cause the bunk bed to collapse, posing a fall hazard to consumers. Health Canada has
received two reports of the side rails cracking or breaking, which lead to minor bruises and abrasions.
Consumers should immediately stop using the recalled bunk beds and return it to Costco for a full
refund.
