La police pour laquelle vous avez soumis une proposition contient une garantie complÈmentaire relative au
versement d'une prestation de dÈcÉs anticipÈe.
La garantie complÈmentaire relative au versement d'une prestation de dÈcÉs anticipÈe permet  l'assurÈ de
recevoir une partie du capital-dÈcÉs prÈvu dans la police sur rÈception d'une preuve satisfaisante Ètablissant
qu'il est atteint d'une maladie en phase terminale.
DÈfinition de maladie en phase terminale: Ce terme dÈsigne le diagnostic d'une maladie incurable d'un assurÈ
conduisant, avec un degrÈ de certitude mÈdicale raisonnable, Â un pronostic de dÈcÉs dans les 12 mois suivant
la date de demande de la prestation.
Montant de la prestation: Le montant de la prestation anticipÈe correspondra  cinquante pour cent (50%) du
capital-dÈcÉs moins cinquante pour cent (50%) de toute avance sur police et de tout intÈrÊt sur avance.
Exemple: L'exemple suivant prÈsente le calcul du montant de la prestation anticipÈe ainsi que l'incidence de
son versement sur les valeurs de la police:

ACCELERATED DEATH BENEFIT DISCLOSURE
AND ACKNOWLEDGEMENT
The policy you have applied for contains an Accelerated Death Benefit rider.
The Accelerated Death Benefit rider on this policy allows the Insured to receive a portion of the policy's
Death Benefit upon receiving due proof that the Insured has a Terminal Illness.
Definition of Terminal Illness: The Insured is diagnosed as having an incurable medical condition that
with reasonable medical certainty will result in the death of the Insured within 12 months from the date
the benefit is requested.
Amount of the Benefit: The amount of the Accelerated Benefit will be equal to fifty percent (50%) of
the Death Benefit less fifty percent (50%) of any outstanding policy loan and loan interest.
"Sample Illustration." The calculation of the Accelerated Benefit Amount and the effects on the
remaining policy values are shown in the "sample illustration" below:
