To be exempted from the obligation to hold the Small Vessel Operator
Proficiency training certificate to perform the Masters duties on board a
fishing vessel of up to 15 gross tonnage or not more than 12rn in overall
length, pursuant to subsection 212(8) of the Marine Personnel
Regulations, I declare that I have acquired at least seven fishing seasons
as Master of a fishing vessel of up to 15 gross tonnage or not more than 12 rn in overall length with no two of those seasons occurring in the same
year, prior to July 1, 2007 and that to the best of my knowledge and belief,
the particulars contained in this declaration and described at Section 3 are
correct and it is a true statement of my experience.
This declaration, upon validation, is also considered an equivalency to proof
of competency under the Competency of Operators of Pleasure Craft
Regulations.

Of the Canada Shipping AC; 2001. This information Is collected for a Seafarer to declare
at least seven fishing seasons as Master of a fishing vessel of up to 15 gross tonnage or
not more than 12 m in overall length, pursuant to subsection 272(8) of the Marine
Perso nnel Regulations. The information is then used to issue a Canadian Maritime
Document and other marine documents in order to comply with the Canada Shipping ACT,
2001 and the Marine Personnel Regulations. The information may also be used to
validate and issue the equivalent of proof of competency in accordance wah the
Competency or Operators of Pleasure Craft Regulations. The personal information
collected is described in Personal Information Bank entitled Seafarer's certificates and
Documents (TC Ppu 030). The information will be retained until the nearer reaches the
age of 100 years or two years after the de portment has been notified Mat the seafarer Is
deceased. The information will then be destroyed at the expiry period of Conservation.
