Individuals serving as Joint Accreditation Committee Members
I, as a Joint Accreditation Committee (JAC) Member, hereby:

Acknowledge and agree to abide by the OTA & PTA EAP’s Values;

Understand that some aspects of the OTA & PTA EAP’s business are highly
confidential in nature;

Agree to keep confidential all materials and discussions with respect to the
accreditation status of education programs (as per Policies ACC-05 Confidentiality,
and ACC-06 Electronic Storage of Confidential Data)

Agree to dispose of confidential materials as directed by the OTA & PTA EAP.
