Main objective
L'objectif général du programme COSET (cohortes pour la surveillance épidémiologique en lien avec le
travail) est de décrire l'état de santé des actifs et anciens actifs en lien avec les facteurs professionnels.
ce programme contribuera à identifier les métiers et les conditions de travail à risque pour la santé, et à
proposer des recommandations en matière de prévention. Pour cela, ce programme s'appuie sur des
données issues de plusieurs cohortes représentatives des actifs des trois principaux Régimes. Le DST met
en œuvre les cohortes concernant les actifs affiliés à la MSA (COSET-MSA) et au RSI (voir fiche COSET-RSI).
Les objectifs de la cohorte COSET-MSA sont ceux du programme COSET, déclinés aux travailleurs inclus
au Régime agricole.
Objectifs spécifiques :
- Décrire et surveiller les liens entre la morbidité/mortalité des actifs et les expositions professionnelles
qu'elles soient d'origine physique, chimique, psychosociale ou organisationnelle ;
- Calculer des fractions de morbidité/mortalité attribuables aux facteurs d'exposition professionnelle ;
- Faciliter la mise en place d'études ad hoc en cas de repérage de problèmes émergents ou mal
documentés sur le plan scientifique ;
- Identifier et surveiller à grande échelle des sous-populations soumises à des risques spécifiques ;
- Réaliser des comparaisons entre les travailleurs issus des différents régimes de sécurité sociale et suivis
dans le cadre du programme Coset.

Main objective
Created by the Law of 13 August 2004, the Electronic Health Record is a dematerialized health booklet,
under the responsibility of its owner, accessible via Internet, with fast and easy access for patients and
their caregivers. It allows health workers to share all the medical data considered as useful for the
coordination of care (hospital reports, medical imaging, biological analysis, allergies, antecedents, ongoing treatments, etc). These data are integrated in the Electronic Health Record by health workers during
care and treatment, with the consent of the patient. The patient can also add documents that he wishes
to share with health workers. Data are stored in a highly secure manner by a body approved by the
Ministry of Health. To reinforce security, all data contained in the Electronic Health Records are crypted.
The Electronic Health Record is created with the informed consent of the patient. It is free and optional for
the patient and it doesn't affect his/her entitlement to reimbursement. It is accessible 24 hours a day at
all points of the territory. Access to Electronic Health Records is protected by the Law and all access is
recorded so as to protect individual freedoms and health workers responsibility.
The Electronic Health Record enables:
- to facilitate the description of antecedents of each patient each time that they consult a new health
worker;
- not to forget important information;
- not to need to bring paper documents for consultation or during hospitalization: laboratory results,
prescriptions, radiographs, etc.;
- to avoid unnecessary exams, in case of duplication;
- to prevent the risk of interaction between different medications;
- in case of emergency, to increase patient's odds by saving valuable time.
Inclusion criteria
All beneficiaries of the Assurance Maladie can have an Electronic Health Record. For the moment, entitled
beneficiaries can't benefit from it because of technical reasons.

Study on the increased risk of cancer, including lung cancer, after chronic exposure to ionising radiation,
including alpha emitters (radon descendants, uranium dust). Professional cofactors and smoking are
taken into account. Secondary objective: The cohort study benefits from high quality individual registration
of exposure to various carcinogens on an annual basis, as well as long-term follow-up (over 20 years on
average). All causes of death are recorded regularly. It is also possible to study risks other than cancer in
this cohort, including the risk of cardiovascular or respiratory death. The extrapolation of risk per unit of
exposure enables the assessment of low-dosage risk, at levels regularly encountered by some of the
French population. This is especially true for the estimation of cancer risk per unit of radon exposure in the
home.

Main objective
The French National Occupational Diseases Surveillance and Prevention Network (RNV3P) groups together
the 32 Occupational Disease Consultation Centres (CCPPs) in mainland France and a sample of 10
occupational health services (SSTs) associated with the network. This network aims to collect data from
each consultation in a permanent national database on occupational diseases (patient's demographic
data, diseases, exposures, business sector and profession). The RNV3P has two interrelated components:
"RNV3P-CCPP" and "RNV3P-SST". The former is a surveillance system for CCPP patient consultations while
the latter is a survelliance system for the working population. One of the original aims of RNV3P is to
record CCPP expertise on the causal nature of relations between occupational exposure and diseases
observed in patients. It is up to the network's university-hospital experts to investigate the diseases and
attribute them, if necessary, to an occupational origin (this causality is also registered in the database).
The RNV3P is not a health surveillance system that gives a representative picture of actual disease
frequency in a given field. Its main objectives are to identify and describe at-risk occupational situations in
France, and to research new etiologies and emerging occupational health risks. As a network operator,
ANSES is responsible for coordinating all activities associated with the network and to participate in related
scientific work in partnership with the State Health Insurance Fund for Salaried Workers (CNAM-TS), the
Central Health and Social Security Insurance Fund for Agricultural Employees (CCMSA), the French
Institute for Public Health Surveillance (InVS), and the French Occupational Medicine Society (SFMT).
Inclusion criteria
All patients who attended a CCPP consultation identified in the network. All salaried workers with a disease
noted by a sentinel physician where a relationship between disease and occupation (current or past) is
suspected or has not been ruled out.

Main objective
GAZEL was set up in 1989 among workers of Électricité de France-Gaz de France (EDF-GDF). It was
designed as an ?open epidemiologic laboratory? characterized by a broad coverage of health problems
and determinants and accessible to the community of researchers. At study inception in 1989, the GAZEL
Cohort Study included 20,625 volunteers (15,011 men and 5,614 women) then aged from 35 to 50 years.
The data routinely collected cover diverse dimensions and come from different sources: annual selfadministered questionnaire (morbidity, lifestyles, life events, etc.); personnel department of EDF-GDF for
social, demographic, and occupational characteristics; EDF-GDF special social insurance fund (for sickness
absences and cancer and ischemic heart disease registries), occupational medicine (occupational
exposure and working conditions), Social Action Fund (healthcare utilization), Health Screening Centres for
standardized health examination and the National Death Register (causes of death); a biobank including
more than 8,000 blood samples was also set up. Today, more than 40 ancillary projects on diversified
themes have been set up in the GAZEL Cohort Study by some 30 French and foreign teams from
Denmark, Germany, Belgium, Canada, Great Britain, Sweden, Finland, and USA. Health problems as
diverse as migraine, postmenopausal osteoporosis, ischemic heart disease, depression, musculoskeletal
diseases, or traffic accidents have been the object of research projects in this cohort. They take into
account risk factors that are behavioural, social, psychological, occupational, and medical. A substantial
proportion of the research work has focused on the problem of social inequalities in health and their
occupational, personal, and social determinants using a life course perspective.

Main objective
PMSI is a tool for medico-economic analysis of hospital activity. Its main purpose is to describe hospital
activity to enable a better division of budgetary envelopes, according to the medical and economic
complexity of the pathologies treated in each establishment. By its medical-administrative content, it
allows to study hospital morbidity and its caring (diagnosis and actions), and the analysis of regional and
interregional fluxes of hospitalizations. It's finally used as an internal management tool for some health
establishments. In 2004, a systematic collection of medical information has been set up for all the health
establishments, publics or privates, having an authorization for hospitalization at home activities, since
the 1st of January 2005. The specific of this collection, in comparison with the ones realized in other
domains of hospitalization, concern the nature of the information collected: caring methods, main and
associated, Karnofsky index. These variables define the classification into homogenous caring groups
(GHPC). On the other hand, a certain degree of flexibility is accorded to the establishment, concerning the
PMSI for HAD data production rhythm. Indeed, the collection is not necessarily made by stay or calendar
week, but by medical treatment sequence: during an hospitalization at home, each sequence of medical
treatment is defined as long as a combination of three classifying variables continues; the establishments
have the freedom to create one or more summaries by RPSS, to cover the duration of each sequence.

Main objective
The SUMER survey aims to:
- describe all types of exposure linked to work stations (physical constraints and atmospheres,
organizational and relational constrains, biological agents or chemical products) to which employees are
exposed to;
- characterise these exposure types (duration and intensity);
- describe the collective and personal protection established by businesses.
Inclusion criteria
The reference population consists of French employees covered by occupational medicine or prevention
and by the general scheme, the agricultural scheme (MSA), as well as Air-France, SNCF, EDF GDF, The
National Post Office and Public Hospital employees. Some civil servants working for the French State and
regional authorities were included in 2009-2010.

Le premier objectif européen de l'enquête est de connaître le nombre d'accidents du travail ainsi que le
nombre de jours de travail perdus du fait de ces accidents du travail. L'enquête permettra aussi de
connaître le nombre de maladies causées ou aggravées par le travail ainsi que le nombre de jours de
travail perdus du fait de ces maladies.
Le second objectif européen est d'analyser les différencesde résultats selon :
- les caractéristiques de l'employeur (activité économique, taille de l'entreprise?)
- les caractéristiques de l'emploi du salarié (statut professionnel, profession, temps complet, temps
partiel, durée de la carrière, ?)
Ce questionnement complémentaire visait notamment à rendre possible des comparaisons avec les
résultats du module ad hoc 2002 sur l'emploi des personnes handicapées, afin de pouvoir évaluer les
effets de la « loi pour l'égalité des droits et des chances, la participation et la citoyenneté des personnes
handicapées » adoptée en 2004. Le questionnement a été divisé en trois parties. La partie A traite des
problèmes de santé des douze derniers mois

Enquête 2002 : L'enquête a pour objectif de fournir une information détaillée sur la situation d'emploi ou
d'inactivité des personnes handicapées ou souffrant de problèmes de santé depuis au moins 6 mois.
Enquête 2011 : Ce module a pour objectif l'évaluation du plan d'action européen sur le handicap, lancé en
2003. Ce plan vise d'une part à réduire les inégalités auxquelles font face les personnes handicapées,
mais aussi à intégrer la question du handicap dans toutes les politiques concernées. En outre, cette

Il s'agit de recherches qualitatives privilégiant le point de vue de personnes atteintes de cancer ou du VIH
sur leur vie au travail, les difficultés rencontrées comme les stratégies déployées pour construire et

nuancent bon nombre de résultats d'enquêtes quantitatives portant sur l'impact d'une pathologie sur
l'emploi ou la trajectoire professionnelle qui montrent invariablement l'effet négatif potentialisé selon la
