Thank you for your email and your interest in working for Plastic Surgeon.
If you are applying for a position with us it’s important that you complete this application form in full in
your own hand writing. You may send your CV in support of your application form along with anything
else you think may be of interest. We do not require you to send your driving licence, CRB check,
passport or birth certificate with your application form either in original format or copies. We will gladly
review this information if you are selected for a face to face interview.
In the event that you are successful please ensure you supply an active telephone number and email
address on your application form, this will ensure you receive updates on your application.

9DS. Please pay particular attention to postage instructions otherwise we may not receive your
application form.
We receive a large number of applications for employment each week so regret that we are unable to
reply to each application personally. However, we do review every application individually and yours
will go through the selection process detailed below. Due to the amount of applications we receive
we will only be in contact should we wish to take your application further. If you have not heard
