Polyphenole und Gesundheit Polyphenols and Health Berlin 25. 2. 2006 Editorial Die Polyphenole gehören zweifellos zu den „hot topics“ der aktuellen Präventivmedizin. Die Liste ihrer potenziell gesundheitsfördernden Eigenschaften ist beeindruckend: Sie wirken antioxidativ, tumorhemmend, entgiftungsfördernd und immunmodulierend. Mehr und mehr beginnt man zu erkennen, dass ein komplexes Zusammenwirken Dr. med. Peter Prock vieler Einzelsubstanzen für diese Wirkungen notwendig ist. Die so genannten „Vitamine des 21. Jahrhunderts“ sind also echte Teamplayer. Als Teamplayer verstehen sich auch die Mitglieder unserer Fachgesellschaft ENA. Gemeinsam wollen wir die Erforschung und den Einsatz von Nahrungsergänzungsmitteln voranbringen. Gerne möchten wir mit diesem Kongressjournal auch alle Without a doubt, the polyphenols are one of the hot topics in current preventative medicine. The list of their potential health-promoting properties is impressive: they have antioxidative, anticancer, detoxifying and immunomodulating activities. More and more, we are starting to recognize that a complex interplay of many individual substances is necessary for these effects. The so-called “vitamins of the 21st century“ are therefore true team players. We also regard the members of our specialist organisation, the European Nutraceutical Association, as team players. Together we want to advance research into food supplements and promote their use. Through the medium of this congress journal, we invite all those who were unfortunately unable to come to the most interesting meeting in Berlin, to also gain from the information it conveyed. Sie konnten wertvolle Informationen zum aktuellen Stand des Wissens über die gesundheitsfördernden Effekte der Polyphenole und nicht zuletzt auch praktische Tipps für die Vermittlung sinnvoller Präventionsmaßnahmen mit nach Hause nehmen. In dieser Ausgabe können Sie die wichtigsten Punkte nachlesen. Over 400 participants – medical practitioners, nutritional experts, pharmacists, natural scientists and sports scientists – from ten European Countries took part in the ENA Congress in February 2006 in Berlin. They were able to take home valuable information about the current state of knowledge concerning the health-promoting effects of polyphenols and - not least – also practical tips for passing on sensible preventative measures. Jahrhunderts Zahlreiche Aspekte der Polyphenole kamen am Kongress in Berlin zur Sprache. Sie reichten von den biochemischen Grundlagen der gesundheitsfördernden Wirkung über eine weltweit einmalige prospektive Studie bis hin zu der Frage, wie man als Arzt die (noch) Gesunden für präventivmedizinische Themen sensibilisieren kann. „Polyphenole sind die reaktivsten Substanzen der Pflanze“, sagte Professor Roland Bitsch vom Institut für Ernährungswissenschaften der Friedrich-Schiller-Universität Jena. Allen gemein ist eine hohe antioxidative Wirkung. Diese ist in vielen Fällen beträchlich höher als bei den als Antioxidantien bekannten Vitaminen A und E; beispielsweise ist sie für das in Äpfeln enthaltene Flavanoid Quercetin 4.7-mal, für das Rutin aus Zitrusfrüchten 2.4-mal und für das Cyanidin aus dunklen Beerenfrüchten 4.4-mal höher. Biochemisch werden die Polyphenole in verschiedene Klassen unterteilt, wo2 bei die Flavanoide die größte Gruppe stellen. Flavanoide entstehen unter Lichteinfluss und sind darum meist in der Schale der Früchte und Gemüse besonders hoch konzentriert. Der Polyphenolgehalt kann je nach Reifungsgrad und Anbaubedingungen stark schwanken. So enthält beispielsweise ein Kilogramm unreife Schwarze Johannisbeeren zirka 9 mg Quercetin, während es bei reifen Früchten 41 mg sind. Obstsaft als „functional food“ Die Resorptions- und Metabolisierungsrate im Organismus ist für die verschiedenen Substanzen unterschiedlich. Anhaltspunkte über die zu erwartenden Größenordnungen geben Studien mit Quercetin. Im Durchschnitt werden hiervon 30 bis 50% resorbiert, von denen bis zu 80% vollständig metabolisiert und letztlich mit der Atemluft als CO2 abgegeben werden. Die antioxidative Wirkung von Polyphenolen ist im menschlichen Plasma nach dem Genuss von Apfel- oder Fruchtsaft nachweisbar, berichtete Bitsch aufgrund eigener Untersuchungen. Demnach könnte man Fruchsaft gar als „functional food“ definieren, da er nun nachgewiesenermaßen eine Wirkung hat, die „potenziell gesundheitsfördernd“ ist. Die Isolierung und Identifizierung einzelner Polyphenole mit hoher antioxidativer Potenz in vitro dürfe allerdings nicht dazu verführen, hohe Dosen an Einzelsubstanzen im Grammbereich zu verabreichen. Dies könne durchaus negative Folgen haben, warnte Bitsch (mehr dazu im Interview auf Seite 6). Spätestens seit den negativen Studien mit Vitaminen zur Krebsprävention bei The Vitamines of the 21st century Many aspects of the polyphenols were talked about at the Berlin Congress, ranging from the biochemical principles of their health-promoting action, to a worldwide unique study and finally to the question of how, as a doctor, one can make (still) healthy people more aware of matters relating to preventative medicine. “Polyphenols are the most reactive substances of plants“, said Professor Roland Bitsch of the Institute for Nutritional Sciences of the FriedrichSchiller University Jena. They all have a high antioxidative effect, which in many cases considerably exceeds that of the well-known antioxidants vitamins A and E. For example, that of the flavonoid quercetin contained in apples is 4.7 times higher, of rutin from citrus fruits 2.4 times higher and of cyanidine from dark-coloured berries, 4.4 times higher than that of vitamins A and E. Biochemically, polyphenols are subdivided into various classes of which the flavanoids form the largest group. Flavanoids are produced under the influence of light and are therefore generally especially highly concentrated in the peel of fruits and vegetables. The polyphenol content can vary markedly depending on the degree of ripeness and growing conditions. For example, one kilogram of unripe blackcurrants contains about 9 mg of quercetin, whereas ripe fruits contain 41 mg. Fruit juice as a functional food The rate of absorption and metabolism in the body of the various substances differs. Studies with quercetin provide some indication of the expected orders of magnitude. On average, 30 to 50% Congress Report Rauchern hat sich die Erkenntnis durchgesetzt, dass ein pharmakologischer Ansatz mit hoch dosierten Einzelsubstanzen für die Vitamine und andere Mikronährstoffe nicht sinnvoll ist. Vielmehr sollten diese in einer möglichst natürlichen Mischung konsumiert werden. Über eine auf den ersten Blick ungewöhnliche Quelle gesundheitsfördernder Substanzen berichtete Professor Helmut Sies vom Institut für Biochemie und Molekularbiologie I der Heinrich-Heine-Universität Düsseldorf. Nach dem Genuss eines Kakaogetränks mit hohem Flavanolgehalt konnte sein Team eine gefäßerweiternde Wirkung nachweisen. „Allerdings hat das übliche Kakaopulver aus dem Supermarkt einen viel zu geringen Flavanolgehalt“, sagte Sies. Um potenziell gesundheitsrelevante Wirkungen zu erhalten, braucht es sehr hochwertigen Kakao, der schonend gewonnen wurde. Die gefäßerweiternde Wirkung läuft vermutlich über eine Steigerung der NOAktivität im Plasma, berichtete Sies (mehr dazu im Interview auf Seite 7). Wirkmechanismen Die zellschützende, antioxidative Wirkung der Polyphenole beruht auf zahlreichen Reaktionswegen. Dazu gehören ihre Aktivität als Radikalfänger durch Fortsetzung auf Seite 4 of quercetin is absorbed, of which up to 80% is completely metabolised and finally excreted in the expired air as CO2. Based on his own investigations, Bitsch reported that the antioxidative action of polyphenols can be demonstrated in human plasma after the ingestion of apple or fruit juice. Accordingly, even fruit juice could be defined as a functional food since it has now been proved to have an effect that is „potentially health-promoting“. However, the isolation and identification of individual polyphenols with high antioxidative potency in vitro should not encourage the administration of high doses of individual substances in the gram region. This could have thoroughly negative consequences, warned Bitsch (more about this in the interview on page 6). At the latest following the negative studies with vitamins for cancer prevention in smokers, it has become generally acknowledged that a pharmacological approach with high doses of individual substances is not worthwhile for vitamins and other micronutrients. On the contrary, these should be consumed in as natural a mixture as possible. What at first sight appears an unusual source of health-promoting substances was described by Professor Helmut Sies of the Institute of Biochemistry and Molecular Biology I of the Heinrich-Heine University, Düsseldorf. His team was able to demonstrate a vasodilating effect after the consumption of a cocoa drink with a high content of flavanols. “However, the flavanol content in the normal cocoa powder from the supermarket is far too low!“ said Sies. In order to obtain potentially health-relevant effects, a very high quality cocoa is needed that has been carefully obtained. He reported that the vasodilating action probably occurs through an increase in plasma NO activity (more about this in the interview on page 7). Mechanisms of action The cytoprotective, antioxidative effect of polyphenols is based on numerous reaction pathways. These include their activity as radical scavengers through electron and proton donation, the prevention or delay of the oxidation of biomolecules and the formation of chelates with metal ions. These effects are expressed biologically by an antiinflammatory or immunomodulating action, for example. The modulation of signal transduction and of gene expression in the cell, as well as the inhibition of enzyme activities, are based on the above mechanisms of plant secondary metabolites, also known as the “vitamins of the 21st century“. Incidentally, this willingly used catchphrase is nothing new. As long ago as 1936, the subsequent Nobel prizewinner Albert Szent-Györgi described the flavanols as “Vitamin P“. Unlike vitamins, a lack of secondary plant metabolites does not directly lead to visible damage to health. However, there is evidence from epidemiological studies that a suboptimal supply of these substances increases the risk of cancer and cardiovascular diseases, whereas a high intake of fruit and vegetables reduces the risk. In this connection, Helmut Sies warned out that epidemiological studies can admittedly provide interesting indications, but no cast-iron proof of cause-effect relationships. To illustrate this point, he presented a study published in Nature back in 1988, showing an absolutely perfect correlation between the fall in numbers of white storks and the decline in human births in this region....... Auch die Modulation der Signaltransduktion bzw. der Genexpression in der Zelle sowie die Hemmung von Enzymaktivitäten beruhen auf den oben genannten Mechanismen der auch als „Vitamine des 21. Jahrhunderts“ bezeichneten sekundären Pflanzenstoffe. First prospective study Dieses gerne gebrauchte Schlagwort ist übrigens keine neue Erfindung. Bereits 1936 beschrieb der spätere Nobelpreisträger Albert Szent-Györgi die Flavanole als „Vitamin P“. Anders als bei den Vitaminen führen Mangelerscheinungen an sekundären Pflanzenstoffen nicht unmittelbar zu sichtbaren Gesundheitsschäden. Epidemiologische Studien geben jedoch Hinweise darauf, dass eine suboptimale Versorgung mit diesen Stoffen das Risiko für Krebs- und Herz-Kreislauferkrankungen erhöht, während ein hoher Konsum an Obst und Gemüse das Risko vermindert. Helmut Sies wies in diesem Zusammenhang darauf hin, dass epidemiologische Studien zwar interessante Hinweise, jedoch keine hieb- und stichfesten Beweise für Ursache-Wirkungsbeziehungen liefern können. Um dies zu verdeutlichen, präsentierte er seine bereits 1988 in Nature publizierte Studie, die absolut eine perfekte Korrelation zwischen dem Rückgang der Weißstörche und dem Rückgang der Neugeborenen in derselben Region zeigte... A study carried out to investigate whether there is an actual cause-effect relationship between fruit and vegetable intake and the reduction in risk of cardiovascular disease was presented to the Berlin Congress by Dr. Mark C. Houston of the Vanderbilt University in Nashville, Tennessee. For the first time, he has been able to demonstrate a cause-effect relationship between the consumption of a plant concentrate made from fruit, vegetable and berry juices* and health-promoting effects on the blood vessels. In a 2-year study with a total of 51 participants, Houston showed that the calcification of blood vessels as well as other parameters, such as blood pressure and arterial compliance, followed a far more favourable course in the study subjects compared with a historical control group. “No-one has ever shown this before in a prospective study“, said Houston. You can read about the results of his study and a detailed interview with Mark C. Houston on page 8 of this issue. Polyphenols and Anti-Aging “The aging of society will pose great problems for all of us“, said Professor Johannes Huber of the Vienna University Department of Gynaecology. For this reason alone, the discovery of substances with cytoprotective effects is of central importance. Resveratrol from red grapes, which first came to our attention due to its effect known as the French paradoxon, is a very promising candidate. High concentrations of the substance are present in the grapes *Juice PLUS+® capsules, NSA International Inc., Memphis Tennessee 4 used to make red wine. Huber reported that it has been shown to activate similar biochemical mechanisms in the cell as fasting, (more about this in the interview on page 8). Even though, according to Huber, “all these theories still have to be proved“, fasting is regarded as a potentially lifeprolonging measure. Consideration of Dr. Mark C. Houston biorhythms plays a key role here. Low levels of blood sugar during the night promote the cellular repair of DNA damage. Under the influence of the circadian-regulated nightly release of growth hormone, it reaches its maximum point between midnight and 2 am. In this sense, dinner-cancelling i.e. no further meal after 4 pm, would be a thoroughly logical measure. If, about 10 pm, one then also added resveratrol – as plant concentrate and not as red wine – then this could further improve the nocturnal DNA repair. However this would not have to be done night after night; Huber speculated that even two “Anti-Aging Days“ per week would probably be sufficient. Erste prospektive Studie Um einen tatsächlichen Ursache-Wirkungsnachweis zwischen Obst- und Gemüsekonsum und der Verringerung des KHK-Risikos ging es in der Studie, die Dr. Mark C. Houston von der Vanderbilt University in Nashville, Tennessee, am Kongress in Berlin präsentierte. Er konnte erstmals eine Ursache-Wirkungsbeziehung zwischen dem Verzehr eines Pflanzenkonzentrats aus Obst-, Gemüse- und Beerensäften* und gesundheitsfördernden Effekten für die Blutgefäße dokumentieren. In einer 2Jahresstudie mit insgesamt 51 Teilnehmern konnte Houston zeigen, dass die Kalzifizierung der koronaren Blutgefäße sowie andere Parameter wie Blutdruck und arterielle Compliance bei den Probanden gegenüber einer historischen Kontrollgruppe deutlich günstiger verliefen. konnte nachweisen, dass sie in der Zelle ähnliche biochemischen Mechanismen aktiviert wie das Fasten, berichtete Huber (mehr dazu im Interview auf Seite 8). Auch wenn „all diese Theorien ihren Wahrheitsbeweis noch antreten müssen“, so Huber, gilt das Fasten als potenziell lebensverlängernde Maßnahme. „Niemand hat dies zuvor in einer prospektiven Studie gezeigt“, sagte Houston. Die Resultate seiner Studie und ein ausführliches Interview mit Mark C. Houston finden Sie ab Seite 8 in dieser Ausgabe. Die Beachtung der Biorhythmen spielt dabei eine entscheidende Rolle. Ein niedriger Blutzuckerspiegel in der Nacht fördert demnach die zelluläre Reparatur von DNA-Schäden. Sie erreicht unter dem Einfluss der zirkadian geregelten Ausschüttung des Wachstumshormons Nacht für Nacht zwischen 0 und 2 Uhr ihren Höhepunkt. „Dinner-cancelling“, d.h. keine Mahlzeit mehr nach 16 Uhr, sei in diesem Sinne eine durchaus sinnvolle Maßnahme. Füge man dann noch gegen 22 Uhr das Resveratrol hinzu – als Pflanzenkonzentrat und nicht als Rotwein –, so könnte dies die nächtliche DNA-Regeneration noch verbessern. Dies müsse man allerdings nicht Nacht für Nacht tun, zwei „Anti-Aging-Tage“ pro Woche seien vermutlich bereits ausreichend, spekulierte Huber. Polyphenole und Anti-Aging Marketing in der Arztpraxis „Die Alterung der Gesellschaft wird uns alle vor große Probleme stellen“, sagte Professor Johannes Huber von der Universitätsklinik für Frauenheilkunde Wien. Nicht zuletzt aus diesem Grund sei die Erforschung von Substanzen mit zellschützender Wirkung von zentraler Bedeutung. Das Resveratrol aus roten Trauben, dem man zunächst wegen seines als „French Paradoxon“ genannten Effekts auf die Spur kam, ist hier ein sehr vielversprechender Kandidat. Die Substanz ist in hoher Konzentration in roten Weintrauben enthalten. Man Ein auf den ersten Blick exotisches Thema für einen medizinischen Fortbildungskongress präsentierte Professor Michael Kunze vom Institut für Sozialmedizin der Medizinischen Universität Wien. In einem sehr kurzweiligen und lebendigen Vortrag versuchte er die Anwesenden zu überzeugen, dass Marketing „nichts Unanständiges“ sei, sondern vielmehr notwendig, um sich mit fachlich kompetenten Informationen im bunten Medien-Markt der Präventionsund Wellnesstrends durchzusetzen. Nach den Reaktionen des Publikums zu urteilen, hatte er damit Erfolg. Doch lesen Sie am besten selbst das Interview mit Michael Kunze auf Seite 11. *Juice PLUS+® Kapseln, NSA International Inc., Memphis Tennessee Marketing in medical practice A subject that at first glance might be considered somewhat exotic for a congress devoted to medical continuing education was presented by Professor Michael Kunze of the Institute of Social Medicine at Vienna University. Professor Michael Kunze In a highly entertaining and lively lecture, he attempted to convince the audience that marketing „is not a dirty word“ but instead necessary in order to gain acceptance and exert a positive influence, through the provision of expert professional information, in the somewhat confused media market of prevention and wellness trends. Judging by the reaction of his audience, he was very successful - but it is best if you read the interview with Michael Kunze yourself on page 11. 5 Interviews Professor Roland Bitsch Polyphenole schützen vor Zivilisationskrankheiten wie Krebs und Herz-KreislaufErkrankungen Polyphenols protect against diseases of civilisation such as cancer and cardiovascular disorders Aufnahme von Obst und Gemüse ist die Häufigkeit von Krebs und HerzKreislauf-Erkrankungen signifikant verringert. Professor Roland Bitsch Herr Professor Bitsch, was sind die wesentlichen Wirkungen der Polyphenole? Aufgrund ihrer Reaktionsfähigkeit sind sie ausgesprochen antioxidativ wirksam. Das heisst, sie fangen radikalischen Sauerstoff ab und wirken darum in der Pflanze bakterizid, fungizid und zum Teil auch viruzid. Beim Menschen ist bisher nur die antioxidative Wirkung nachgewiesen. Wir haben in eigenen Untersuchungen gezeigt, dass die antioxidative Aktivität im Plasma und Harn nach dem Genuss von Apfel- oder Beerensaft ansteigt. Welchen Effekt hat die antioxidative Wirkung der Polyphenole für die Gesundheit? Man kann davon ausgehen, dass diese antioxidative Wirkung wesentlich dazu beiträgt, den Menschen vor Zivilisationskrankheiten wie Herz-Kreislauf-Erkrankungen oder Krebs zu schützen. Tierexperimentell ist das schon belegt und es gibt Hinweise aus epidemiologischen Studien, dass das auch für den Menschen zutrifft. Bei einer hohen 6 Wie sicher ist man, dass dieser Effekt nicht nur darauf beruht, dass diese Personen sowieso gesünder leben, sich mehr bewegen oder nicht rauchen? Das ist eine sehr gute Frage. Genau an diesem Punkt sind wir noch ein bisschen unsicher. Was wir bisher wissen, stammt aus tierexperimentellen Untersuchungen und man schließt daraus auf ähnliche Mechanismen beim Menschen. Sicher trägt aber die allgemein gesündere Lebensweise von Personen, die viel Obst und Gemüse essen, mit dazu bei, dass diese weniger häufig erkranken. Was halten Sie von Präparaten mit sekundären Pflanzenstoffen? Bei Untersuchungen mit isolierten Substanzen im Grammbereich zeigte sich bei Quercetin und anderen Flavonoiden eine Hemmung von bestimmten Enzymen. Dies könnte zu einem verzögerten Abbau körpereigener, potenziell schädlicher Substanzen führen und damit zur Entwicklung neurodegenerativer Erkrankungen beitragen. Auch wurde in neueren Untersuchungen gezeigt, dass der Homocysteinspiegel im Plasma nach konzentrierter Chlorogensäurezufuhr ansteigt und Homocystein gilt als Risikofaktor für Herz-KreislaufErkrankungen. Man sollte darum mit Präparaten vorsichtig sein, die isolierte Flavonoide in hoch konzentrierter Form enthalten und diese Substanzen lieber in ihrer natürlich vorliegenden Form mit der täglichen Nahrung aufnehmen. Professor Bitsch, what are the important effects of the polyphenols? Polyphenols are highly reactive, which makes them extremely effective antioxidants. That means they scavenge free oxygen radicals and thereby have bactericidal, fungicidal and sometimes also virucidal effects in the plant. In humans, only the antioxidative effect has been demonstrated to date. We have shown in our own experiments that the antioxidative activity in plasma and urine increases after drinking apple or berry juice. What effect does the antioxidative action of polyphenols have on health? It can be assumed that this antioxidative action makes a significant contribution to protecting humans from diseases of civilisation such as cardiovascular diseases or cancer. This has already been proved in animal experiments and there is evidence from epidemiological studies that this also applies to humans. The incidence of cancer and cardiovascular diseases is significantly reduced with a high intake of fruit and vegetables. How certain is it that this effect isn‘t merely due to the fact that these persons live healthier lives, take more exercise or don‘t smoke? That‘s a good question and it‘s this point that we are still a bit uncertain about. What we know so far comes from animal experiments and we have concluded that similar mechanisms apply in humans. However, the generally healthier lifestyle of persons who eat a lot of fruit and vegetables definitely contributes to them being ill less often. Professor Helmut Sies What do you think about products with secondary plant metabolites? In studies with isolated substances in the gram range, quercetin and other flavonoids inhibited certain enzymes. This might result in a delayed breakdown of endogenous, potentially harmful substances and hence to the development of neurodegenerative diseases. More recent studies have also shown that the levels of homocysteine in plasma increase after administration of concentrated chlorogenic acid, and homocysteine is a risk factor for cardiovascular diseases. One should therefore be careful with products that contain isolated flavonoids in highly concentrated form and it is better to ingest these substances in their naturally occurring form with daily food. Kakaoflavanol erweitert die Blutgefäße Herr Professor Sies, welche Effekte konnten Sie für das Kakaoflavanol nachweisen? Bisher kennen wir als Kurzzeiteffekt eine Gefäßerweitung der Armarterie, die vermutlich durch Stickoxid im Plasma vermittelt wird. Die Wirkung ist nach zirka zwei Stunden maximal und nach sechs Stunden wieder verschwunden. Man müsste nun eine Studie über mehrere Monate durchführen, um zu sehen, ob dies auch längerfristig gesundheitliche Vorteile bringt. Welche Rolle spielen die Flavonoide für das Endothel? Mikronährstoffe wie die Flavonoide sind ein Faktor, um die Endothelauskleidung der Gefässe funktionell in gutem Zustand zu halten. Man darf aber nicht glauben, dass man mit einer einzelnen Substanz das gesamte Herz-KreislaufSystem ins richtige Lot bringen kann. Hier wirken sehr viele verschiedene Faktoren und Mechanismen. Für wie bedeutend halten Sie den Einsatz isolierter Nahrungsmittelbestandteile für die Gesundheit? Insgesamt gilt hier hauptsächlich das Prinzip Hoffnung. Zunächst entdeckt man einen molekularen Mechanismus, der interessant sein könnte. Dann folgen Experimente mit Tieren, wobei sich immer die Frage nach der Übertragbarkeit der Ergebnisse auf den Menschen stellt. Schließlich folgen Versuche mit Menschen, wie wir sie gerade durchführen. In unserem Fall messen wir eine Erweiterung der Armaterie und schließen daraus auf einen günstigen kardiovaskulären Effekt. Allerdings wissen wir nicht, ob das bei den Herkranzgefäßen tatsächlich genauso funktioniert. Dazu laufen Studien, deren Ergebnisse aber noch nicht bekannt sind. What role do flavonoids have for the endothelium? Micronutrients such as flavonoids are one factor to maintain the endothelial lining of blood vessels in a well-functioning condition. However, we shouldn‘t believe that a single substance can sort out the entire heart and circulatory system. Very many different factors and mechanisms are involved. Professor Helmut Sies Cocoa flavanol dilates blood vessels Professor Sies, what effects have you been able to demonstrate for the cocoa flavanol? So far, we have shown a short-term effect of dilating the brachial (arm) artery, which is probably mediated by nitric oxide in plasma. The effect is greatest after about two hours and has disappeared after six hours. A study now needs to be undertaken over several months in order to see whether it also has long-term health benefits. How important do you regard the use of isolated food constituents for health? All in all, the principle here is largely one of hope. First of all, a molecular mechanism is discovered that might be interesting. This is followed by animal experiments – with the usual question of the applicability of the results to humans. Finally, studies in humans are carried out, as we have just done. In our case, we measured a dilatation of the arm artery and this led us to conclude a favourable cardiovascular effect. However we don‘t know whether it works the same way in coronary blood vessels. Studies are currently underway, but the results are not yet known. 7 Interview Pflanzenkonzentrate – eine Versicherungspolice für die Blutgefäße Herr Dr. Houston, waren Sie überrascht über die Ergebnisse Ihrer Studie zu den Wirkungen der Nahrungsergänzung mit einem Pflanzenkonzentrat*? Ich war nicht überrascht. Eine Fülle an Daten zeigt, dass sich das Risiko für einen Herzinfarkt oder Schlaganfall reduzieren lässt, wenn man acht bis zehn Portionen Obst und Gemüse pro Tag verzehrt. Das wusste man bereits zuvor, aber nur aus epidemiologischer Sicht. Niemand hatte dies bislang im Rahmen einer prospektiven Studie nachgewiesen. In früheren Studien wurden gezielt Mikronährstoffe oder Vitamine angewendet, zum Beispiel Vitamin E, C, Betakarotin und Selen. Die Ergebnisse waren nicht gut, in manchen Studien verschlechterte sich der Zustand der Patienten unter den Ergänzungspräparaten sogar. Bei diesen Studien wurden jedoch keine kompletten Nahrungsmittel untersucht, sondern es wurden vier isolierte Einzelsubstanzen ausgewählt – warum sollte man denn erwarten, dass diese tatsächlich wirken würden! Es war also keine außergewöhnliche Idee, dass man eine prospektive Studie mit Obst und Gemüse durchführen sollte. Ich habe geahnt, dass ich eine Wirkung nachweisen könnte, die Herausforderung bestand jedoch darin, die Wirksamkeit in einem kurzen Zeitraum nachzuweisen. Wie haben Sie die Wirksamkeit nachgewiesen? Wir nutzten die Technologie der Elektronenstrahltomographie (EBT). Ein EBT-Scan ermöglicht es, die Menge an verkalkten Plaques in einer Koronararterie nicht-invasiv festzustellen. Es ist wie bei einem Eisberg. Wenn man einen Eisberg sieht, kann man nur 20% davon sehen, 80% liegen unter der Oberfläche. Dieser Anteil der Plaque ist hart und verkalkt, den anderen Anteil von 80% kann man jedoch nicht sehen, weil er weich und nicht verkalkt ist. Wenn man bei der EBT also eine zunehmende Menge an Plaques in einer Koronararterie feststellt, ist das ein schlechtes Zeichen. Darüber hinaus führten wir Messungen von sekundären Indikatoren durch, wie Blutdruck, arterielle Elastizität und Compliance, Homocystein, anti-oxidative Aktivität und glykosyliertes Hämoglobin – und nahezu alle diese Parameter fielen positiv aus. Die Studie war klein, und es gab keinen Plazebo-Arm. Könnte man also unterstellen, dass die Ergebnisse möglicherweise andere Gründe hatten als die Anwendung des Pflanzenkonzentrats? Wir wussten von Anfang an, dass das Fehlen einer Plazebo-Kontrollgruppe kritisiert werden würde. Andererseits wäre es nahezu unmöglich gewesen, diese Art von Studie plazebokontrolliert zu konzipieren, da es so gut wie unmöglich war, vergleichbare Populationen prospektiv zu etablieren. So entschieden wir uns zu einer historischen Kontrolle als zweitbeste Möglichkeit. Im weiteren Verlauf unserer Studie hielten wir alle anderen Variablen konstant. Wir gaben keine Anweisungen hinsichtlich der Ernährung und wiesen die Probanden an, während der gesamten Studie ihre Gewohnheiten nicht zu ändern. Wir kontrollierten dies anhand von Tagebüchern und Interviews und stellten für keinen Parameter, der das Risiko für Herzkrankheiten oder das Blutdruckrisiko hätte beeinflussen können, eine Veränderung fest. Die meisten Probanden verhielten sich genau so wie vor der Studie. So können wir also durchaus sagen, dass der einzige Faktor, der das Ergebnis beeinflusste, die Einnahme des Pflanzenkonzentrats war. Ich hoffe, diese Studie weckt genügend Interesse, um Fördermittel aufzubringen, die eine weitaus größere, plazebokontrollierte Studie ermöglichen. Was würden Sie den Menschen empfehlen? Sollten sie nicht einfach mehr Obst und Gemüse verzehren, statt eine Kapsel einzunehmen? Nun, das ist eine Frage der menschlichen Natur. Es wäre sicher schön, tatsächlich eine Änderung des Lebensstils zu erreichen, das heisst täglich zehn Portionen Obst und Gemüse zu essen, jeden Tag 30 oder sogar 60 Minuten körperlich aktiv zu sein und das Körpergewicht zu halten. Aber das ist ein Idealfall. Keiner hält sich wirklich daran. Daher ist es besser, den Menschen anzuraten, zumindest ein Pflanzenkonzentrat als eine Art Versicherungspolice für die Blutgefäße einzunehmen, ihnen gleichzeitig aber unbedingt auch all die sinnvollen Lebensstiländerungen zu empfehlen. Damit wird man mehr erreichen. Phytonutrients are an insurance policy for the blood vessels Doctor Houston, were you surprised about the results of your study on the effects of supplementation with a phytonutrient concentrate powder*? I was not surprised. There is a huge amount of data that you will reduce your risk of heart attack or stroke if you eat your eight to ten servings of fruits and vegetables every day. That was known before, but only in an epidemiologic sense. No one had ever proven that in prospective ways before. Previous studies had used targeted micro*Juice PLUS+® capsules, NSA International Inc., Memphis Tennessee nutrients or vitamins, for example Vitamin E, C, beta carotene and selenium. The results were not good, in some studies the patients even got worse on supplements. However, in these kind of studies you were not studying food, but you picked out four isolated substances – why should anyone really expect that they would work! So it didn‘t take a genius to figure out that we should take literally fruit and vegetables to do a prospective trial. I really felt deep down that I could demonstrate an effect, but the challenge was to document that effect in a short period of time. How did you monitor the effects? We used Electron Beam Tomography technology. An EBT-scan allows you to determine the amount of calcified plaque in an coronary artery non-invasively. This calcification is an indirect measure of the plaque burden of the coronary artery. It is like an iceberg. When you seen an iceberg, you see 20% of it, and 80% of it is below the surface. When you see a calcified plaque it tells you that you are seeing only 20% of the plaque. This part of the plaque is hard and calcified, but there is another 80% that you cannot see because it is soft and non-calcified. So when you see increasing plaque in a coronary artery in EBT you know that you are in trouble. We also measured secondary indicators like blood pressure, arterial elasticity and compliance, homocysteine, antioxidant levels, and glycosylated hemoglobins – and virtually all of them were positive. The study was small, and there was no placebo arm. So one could suggest that the results may have had other reasons than the phytonutrient concentrate? We knew before that not having a placebo control would create criticism. On the other hand, it would have been almost impossible to design this type of study placebo-controlled, because it was almost impossible to match the populations prospectively. So we decided to do a historical control as the second best option. When we went forward with our study, we kept every confounding variable stable throughout the study. We did not give any dietary instructions, and we told them not to change anything throughout the entire study. We checked this in diaries and interviews, and we saw no difference in any parameter that would have affected their heart disease risk or their blood pressure risk. Most of the people did exactly what they were doing before the trial. So as best we can tell, the only factor that chan- Dr. Mark C. I hope that this study will create enough interest to raise funds that will allow a much more larger trial with a placebo-controlled group. Das ist eine Frage, die momentan von David Sinclair von der Harvard Medical School in den Vereinigten Staaten untersucht wird. Es ist aber noch zu früh, um dazu etwas zu sagen. Resveratrolpräparate werden in einer Dosis angeboten, die ungefähr einer halben Flasche Rotwein entspricht. Es gibt bisher zwar viele experimentelle Studien – wenn Sie in der Medline Resveratrol eingeben, sehen Sie, dass die Publikationen in den letzten fünf Jahren massiv angestiegen sind – aber klinische Studien sind sicher noch notwendig. 10 Gibt es andere sekundäre Pflanzenstoffe, denen man eine ähnliche Wirkung zuschreibt? Resveratrol mimics the way that the SIRT-protein works in the body. This protein promotes DNA repair by deacetylating DNA and stimulating the FOXO protein. In turn, this repairs DNA on the one hand and also increases the resistance of cells to oxidative stress on the other. Resveratrol thus stimulates DNA repair. We can also say that resveratrol imitates the effects of fasting. Fasting is probably the only way to prolong life and this occurs via the same mechanism as just outlined above. How much resveratrol does the body need and is overdosage possible? That is a question currently being investigated by David Sinclair of the Harvard Medical School in the United States. However it is too early to say anything about it. Resveratrol products are available in a dose that roughly corresponds to the amount in half a bottle of red wine. Many experimental studies have already been carried out – if you enter „resveratrol“ in Medline you will see that the number of publications has risen enormously in the last five years – but clinical trials are certainly still needed. Are there other secondary plant metabolites to which a similar effect has been ascribed? That I don‘t know, but there are also other plant substances which are extremely interesting for medicine. You just have to think of Tamiflu; in principle that is actually also a natural product, or anticancer drugs. Mother nature holds many treasures. Marketing bedeutet die Orientierung an den Bedürfnissen der Zielgruppe. Das ist für viele Ärzte ein neues Denken. Sie müssen zunächst die Grenze im Kopf überwinden und erkennen, dass Marketing im Sinne einer Orientierung an den Bedürfnissen der Zielgruppe nichts Unanständiges ist. Wir stehen in einem ungeheuren Wettbewerbsumfeld. Früher war der Arzt mehr oder weniger der einzige Anbieter in Gesundheitsfragen. Heute aber gibt es eine enorme Wellness-Bewegung, selbst ernannte Fachleute und einen Medienrummel, in dem ein akademisch ausgebildeter Arzt zunehmend an Einfluss bei der Zielgruppe verliert, nämlich den Kranken – und den Gesunden! Aber ein Arzt darf sich doch schon rein rechtlich nicht anbieten... Natürlich darf er das! Marketing ist in erster Linie Kommunikation und kommunizieren kann er auf allen Kanälen. Er kann zum Beispiel in der Praxis mit den Menschen sprechen und Informationsmaterial abgeben, er kann Vorträge halten – all das darf er. Er hat nur Einschränkungen in der klassischen Werbung. Leider glauben viele, Marketing sei Werbung, doch das ist ein Missverständnis. Werbung kann ein Teil des Marketings sein, muss es aber nicht unbedingt. Man stellt mir oft die Frage, ob die Praxis ein „point of sale“ sein kann. Ich sage, dass sie das schon immer war. Wir haben schon immer Leistungen erbracht und dafür einen Gegenwert erwartet, egal ob den die Krankenkasse bezahlt oder der Patient selbst. Professor Kunze, what barriers need to be overcome in marketing in medical practice? Marketing means basing the sales strategy on the needs of the target group. For many medical practitioners, that is a new way of thinking. First of all you need to overcome the barriers in the mind and recognize that marketing – in the sense of basing it on the needs of the target group – is not a dirty word. We live and work in a tremendously competitive environment. Previously, the medical practitioner was more or less the only source of information about health. But today, there is an enormous wellness movement, selfproclaimed experts and a media hype in which an academically trained doctor is increasingly losing influence with the target group, namely patients – and with healthy people as well! Ärzte machen schon immer auch Werbung für sich. Sie besuchen zum Beispiel gesellschaftliche Ereignisse wie den Wiener Opernball und werden damit in bestimmten Kreisen bekannt. Es gibt natürlich eine Grenze, aber wenn man ein vernünftiges Produkt hat, das wissenschaftlich seriös und klinisch geprüft ist – warum sollten sie es nicht empfehlen?
