For twenty years, PPS® Pharmacy Lighthouse has evolved to cater to its niche audience of pharmacists and stakeholders. We celebrated our 21st birthday with somewhat of a revolution. We said goodbye to Pharmacy Lighthouse and introduced LINK. It is in these challenging times that we must work harder to create value for our markets. At PPS®, we started by rebuilding our brand and creating a new image. In July 2012, we issued our first LINK newsletter. It is a fresher and more contemporary look. We also added highlights of the PPS® Buyers Guide (at right). With a simple glance, you now have the new product introductions from the Guide. Many of you have commented positively on seeing this section in LINK. As the expression goes; “Rome was not built in a day” and PPS® is here for the long term. I will continue with our rebranding projects in the new year. Our main focus for 2013 will be to reinforce the Guide, add more value to LINK and build stronger ties between pharmaceuticals and you, pharmacist and industry partners. A non-inferiority, prospective, double-blind, double-dummy, two-arm, parallel-group, 12 week trial in OAB patients. After a 2 week, single-blind, placebo run-in, patients were randomized to receive either Vesicare 5 mg OD or tolterodine ER 4 mg OD. After 4 weeks of treatment, patients had the option of either continuing with their original dose or requesting a dose increase. However regardless of dose decision (dummied throughout), only Vesicare treated patients were given an increase from 5 mg to 10 mg OD based on product labeling (dose increase was not approved for tolterodine ER). INDICATIONS: Vesicare (solifenacin succinate) is indicated for the treatment of overactive bladder in adults with symptoms of urge urinary incontinence, urinary urgency and urinary frequency. Safety and effectiveness in children have not yet been established. incidence of these side effects was higher in the 10 mg (27.6% and 13.4%, respectively) compared to the 5 mg (10.9% and 5.4%, respectively) dose group (4.2% and 2.9% for placebo, respectively). CONTRAINDICATIONS: Patients with urinary retention, dependent on dialysis, gastroparesis or narrow angle glaucoma. ADVERSE EVENTS: Expected side effects of antimuscarinic agents are dry mouth, constipation, blurred vision (accommodation abnormalities), urinary retention, and dry eyes. The most common adverse events reported in patients treated with Vesicare were dry mouth and constipation and the *For patients with urinary frequency, urgency or urge incontinence who have failed to respond to behavioral techniques AND an adequate trial of oxybutynin with gradual dose escalation has shown to be either ineffective or resulted in unacceptable side effects. NOTE: If after a trial of 2 weeks patients continue to experience similar side effects and no greater efficacy than oxybutynin, continued therapy with this more costly agent should be reassessed. Antimuscarinic agents should be used with caution in the elderly due to potentially serious adverse effects (e.g. confusion, psychosis, acute urinary retention, constipation). Antimuscarinic agents should be avoided in older adults with pre-existing cognitive impairment (e.g. dementia) and those who are already using other drugs with significant anticholinergic effects (e.g. tricyclic antidepressants) in order to avoid a high overall anticholinergic drug burden. Essai de non-infériorité de 12 semaines, prospectif, à double insu, contre double placebo, à deux bras, en groupes parallèles, mené sur des patients atteints d’HV. Après une période de pré-inclusion de 2 semaines à simple insu avec placebo, les sujets ont été randomisés pour recevoir soit Vesicare 5 mg x 1/j, soit de la toltérodine LP 4 mg x 1/j. Après 4 semaines de traitement, les patients pouvaient soit continuer de recevoir leur dose initiale, soit demander une augmentation de dose. Cependant, quelle qu’ait été la décision prise au sujet de la dose (la technique du double placebo ayant été appliquée du début à la fin), seuls les patients traités par Vesicare ont reçu une dose plus forte, soit de 10 mg au lieu de 5 mg x 1/j, selon les indications relatives à l’administration du produit (une augmentation de la dose n’a pas été approuvée dans le cas de la toltérodine LP). INDICATIONS : Vesicare (succinate de solifénacine) est indiqué dans le traitement de l’hyperactivité vésicale chez l’adulte, caractérisée par des symptômes d’incontinence par impériosité, d’urgence mictionnelle et de pollakiurie. L’innocuité et l’efficacité de Vesicare chez l’enfant n’ont pas été établies. CONTRE-INDICATIONS : Patients souffrant de rétention urinaire, de gastroparésie ou d’un glaucome à angle fermé, ou sous dialyse. Patients présentant une hypersensibilité au médicament ou à l’un quelconque de ses ingrédients ou de l’un des composants du contenant. MISES EN GARDE : Chez les femmes en âge de procréer, le traitement ne peut être envisagé que si elles utilisent une contraception adéquate. ÉVÉNEMENTS INDÉSIRABLES : Les événements indésirables attendus des antimuscariniques sont la sécheresse de la bouche, la constipation, une vision trouble (anomalies de l’accommodation), la rétention urinaire et la sécheresse oculaire. Les événements indésirables les plus fréquemment signalés chez les patients traités par Vesicare sont la sécheresse de la bouche et la constipation; leur incidence a été la plus élevée dans le groupe ayant reçu la dose de 10 mg (27,6 % et 13,4 %, respectivement), comparativement au groupe ayant reçu la dose de 5 mg (10,9 % et 5,4 %, respectivement) ou un placebo (4,2 % et 2,9 %, respectivement).
