Pharmaceutical Chemistry Bernd Clement, Pharmazeutisches Institut Die Bedeutung der Biotransformation in der Arzneistoffentwicklung und Therapie Viele Verbindungen, die mit hoher Affinität an ein Zielmolekül binden, werden durch High-Throughput Screening-Verfahren (HTS) und andere Methoden der Arzneistoffentwicklung identifiziert. Es ist mittlerweile aber auch klar erkannt worden, dass ein erfolgreicher Arzneistoffkandidat nicht nur Wirkstärke und Selektivität, sondern auch ein geeignetes pharmakokinetisches Profil einschließlich der Biotransformation (Arzneistoffmetabolismus) aufweisen muss. Von daher sind gute metabolische Eigenschaften ein Ziel der Arzneistoffentwicklung. In meiner Gruppe werden Enzyme untersucht, die in den Metabolismus von Arzneistoffen und Arzneistoffkandidaten eingreifen. Insbesondere wurden Enzyme identifiziert, die N-hydroxylierte Strukturen reduzieren. Diese Enzymsysteme sind sowohl an der Reduktion von genotoxischen N-hydroxylierten Verbindungen als auch an der Aktivierung von Amidoximprodrugs beteiligt. Dieses Prodrug-Prinzip „Amidoxime an Stelle von Amidinen“ wurde neu entwickelt und ist mittlerweile auf zahlreiche Arzneistoffkandidaten übertragen worden. The Importance of Drug Metabolism in Drug Development and Therapy Many active compounds binding with high affinity to a target are identified by high-throughput screening and other methods of drug design. However, it is now clearly recognized that a successful drug candidate requires not only potency and selectivity, but also a suitable pharmacokinetic profile including metabolism. Thus, highly favorable metabolic properties are one goal of drug development. In my group enzymes are studied which are involved in the metabolism of drugs and new drug candidates. In particular new enzymes systems have been identified which are capable of reducing N-hydroxylated structures. These systems are involved in the reduction of genotoxic N-hydroxylated compounds and also in the activation of amidoximes used as prodrugs of amidines. This prodrug principle was newly developed and has been applied to several drug candidates. Arzneistoffmetabolisierende Enzyme Drug Metabolizing Enzymes Arzneistoffmetabolismus hat biochemische, pharmakologische und toxikologische Aspekte (Bild 1). Arzneistoffe werden durch fremdstoff(xenobiotika)-metabolisierende Enzyme, hauptsächlich das Cytochrom P450 Enzymsystem, welches aus verschiedenen Isoenzymen besteht (Multigenfamilie, Tabelle 1), umgesetzt. Drug metabolism has biochemical, pharmacological and toxicological aspects (Figure 1). Drugs are transformed by foreign-compound (xenobiotic) metabolizing enzymes, in particular the cytochrome P450 enzyme system, consisting of several isoenzymes (multigene family, Table 1). Polymorphic enzymes (CYP2D6, CYP2C19) have been identified and their participation in the metabolism of a drug or a drug candidate has to be realized. Pharmacokinetic interactions were the reason that many drugs were withdrawn from the market during recent years (f. ex. mibefradil, terfenadin). In vitro studies with isoenzyme selective substrates (marker-substrates, see Table1) or inhibitors have to be conducted during an early phase of drug development in order to see if interactions are feasible. The induction potential of many drugs is well known and should also be checked for drug candidates. Pig and human hepatocyte culture systems were also developed for phase 2 biotransformation studies in vitro. The viability of the cultured hepatocytes was characterized during cultivation by lactate dehydrogenase release into the supernatant and albumin synthesis of the cells. The metabolic activity of the hepatocyte cultures was measured by 7-ethoxycoumarin (ECOD) and 7-ethoxy-resorufin O-deethylation (EROD). The viability and metabolic activity of these pig hepatocytes were preserved for several days by culturing the cells in a monolayer culture system. With these systems we were able to identify several new metabolic pathways. The enzymatic basis of N-oxidative processes is very well understood whereas reductions have been less extensively investigated. We purified an enzyme system from pig and human liver consisting of cytochrome b5, its reductase, and a P450 enzyme which is involved in the reduction of the N-hydroxylated products. Similar activities were found in all species studied so far. Furthermore, comparable reductive reactions could also be demonstrated with microsomal fractions from organs other than liver. Also mitochondria are highly capable of performing the reductions of these N-hydroxylated compounds. Um die Bedeutung von solchen metabolischen Zyklen und insbesondere die Reduktion von N-hydroxylierten Verbindungen zu studieren, wurde der reversible Metabolismus von Amidin-, Guanidin- und Amidinohydrazonstickstoffen innerhalb verschiedener Arzneistoffe und Modellverbindungen untersucht. Die eigenen Studien ergaben, dass diese N-oxygenierten Metaboliten sehr leicht zu den Ausgangsverbindungen rückreduziert werden. Ein Vergleich der kinetischen Daten der N-Hydroxylierung mit denen der Reduktion lassen eine Bevorzugung der Reduktion in vivo vermuten. Dies konnte durch in vivo Studien bestätigt werden. Im allgemeinen sind die enzymatischen Grundlagen von N-oxidativen Prozessen sehr gut verstanden. Auf der anderen Seite sind Reduktionen weniger intensiv untersucht worden. Ein Enzymsystem, bestehend aus Cytochrom b5, der zugehörigen Reduktase und einem P450 Enzym, wurde von uns aus Schweineleber und humaner Leber gereinigt. Dieses System ist an der Reduktion von N-hydroxylierten Verbindungen beteiligt. Ähnliche Aktivitäten konnten in allen Spezies, die bisher untersucht worden sind, nachgewiesen werden. Darüberhinaus wurden vergleichbare reduktive Aktivitäten auch in Mikrosomen von anderen Organen nachgewiesen. Mitochondrien zeigten ebenfalls hohe Reduktionsraten von N-hydroxylierten Verbindungen. Toxicological Aspects of Drug Metabolism In most cases inactive metabolites are formed during biotransformation. However, certain structures can also be transformed into reactive metabolites such as electrophiles and radicals during a process known as toxification. Responsible functional groups should be identified and not be included in new chemical entities. The accepted concept of the metabolic activation of arylamides, arylamines, and aminoazoheterocycles involves the N-hydroxylation of the amine and amide function. In the case of primary arylamines, hydroxylamines are formed, which can be metabolized by conjugation reactions to various esters which can be unstable and decompose to arylnitrenium ions forming DNA adducts. The possibility of reduction of hydroxylamines and similar N-hydroxylated metabolites back to the starting non-toxic nitrogen containing functional groups, the easiest way of detoxification for an organism, has been neglected. Enzyme systems are studied in my laboratory which are capable of catalyzing such reductive reactions. A number of successful drugs are on the market in which oral absorption and pharmacokinetics have been improved through the design of prodrugs. Amidines and guanidines are functional groups, which are very often incorporated into active molecules because the cations formed after protonation are necessary for interactions with the negatively charged carboxylates of target molecules. Very often, benzamidines are imitating the guanidine functional group of arginine. Es sind eine Reihe von erfolgreichen Arzneistoffen zugelassen worden, bei denen die orale Absorption und die pharmakokinetischen Eigenschaften durch die Entwicklung von Prodrugs entscheidend verbessert worden sind. Schema 1 / Scheme 1 Amidines like guanidines are protonated at the double bonded nitrogen atom and thus form a highly mesomerically stabilized cation. They are the strongest known organic bases, protonated under physiological conditions, very hydrophilic, usually not absorbed from the gastrointestinal tract, and thus not orally available. The N-hydroxylated derivatives (amidoximes, N-hydroxyguanidines) are less basic because of the introduction of the oxygen atom. They are not protonated under physiological conditions and should lead to sufficient oral absorption and therefore to improved bioavailability. Thrombininhibitoren, Glykoprotein IIb/IIIa-Rezeptorantagonisten und Faktor Xa-Inhibitoren, aber auch in alten Arzneistoffen wie Verbindungen mit trypanociden und leishmaniciden Aktivitäten (Pentamidin) und mit Wirkungen gegen Lungenentzündungen, die durch Pneumocystis Carinii hervorgerufen werden (Schema 1). Sie sind unter physiologischen Bedingungen nicht protoniert und sollten von daher zu einer ausreichenden oralen Verfügbarkeit und somit zu einer verbesserten Bioverfügbarkeit führen. Sibrafiban, ein Glykoprotein IIIb/IIIa Rezeptorantagonist, wies in diesen Studien eine sehr gute orale Verfügbarkeit mit einer geringen individuellen Variabilität der pharmakokinetischen Parameter und keine Zeichen von Toxizität auf. Die gleichen Eigenschaften wurden auch für Schema 2 / Scheme 2 Ximelagatran, dem ersten oralen direkten Thrombininhibitor, nachgewiesen. Sibrafiban und Ximelagatran sind beide doppelte Prodrugs (Monoamidoxime und Estergruppierungen), bei denen in den Wirkformen die Amidinfunktion durch ein N-Hydroxyamidin (Amidoxim) und die Carbonsäure durch eine Estergruppierung ersetzt worden sind. amidine functional groups are replaced by N-hydroxyamidines and the carboxyclic acids by ester functionalities. Nitric Oxide Donors, NO-Synthases Nitric oxide synthases (NOSs), expressed as inducible (iNOS), endothelial (eNOS), and neuronal (nNOS, bNOS) isoenzymes, oxidize L-arginine in two steps via N-hydroxy-L-arginine to nitric oxide (NO) and the urea derivative L-citrulline. The second step is reported to be also catalyzed by P450 enzymes. The discovery of new (NO) liberating compounds and structure-activity relationships are desirable both for the development of new drugs and to advance our understanding of the metabolism catalyzed by NOSs and P450s. Die Entdeckung von neuen NOfreisetzenden Verbindungen und Struktur-Aktivitäts-Beziehungen ist sowohl für die Entwicklung von neuen Arzneistoffen als auch zur Verbesserung des Verständnisses der durch NO-Synthasen und P450-Enzymen katalysierten Reaktionen äußerst wünschenswert. In my group several N-hydroxyguanidines, azides and hydrazines were tested for their capability to release NO by cytochrome P450 enzymes. Non-physiological N-hydroxyguanidines are capable of producing NO in the presence of microsomal enzymes like P450. However, in vivo the reduction of these Nhydroxyguanidines is the predominant reaction (see above). Inhibitors of NO synthases are also developed which might be beneficial for the treatment of several diseases. In meiner Gruppe sind viele N-Hydroxyguanidine, Amidoxime, Azide und Hydrazine hinsichtlich ihrer Fähigkeit zur Freisetzung von NO, katalysiert durch P450-Enzyme, untersucht worden. Nichtphysiologische N-Hydroxyguanidine und Amidoxime sind in der Lage in Gegenwart von mikrosomalen Enzymen wie P450 NO freizusetzen. Allerdings ist in vivo die Reduktion (siehe oben) dieser N-hydroxylierten Verbindungen die weitaus dominierende Reaktion. 29 Pharmaceutical Chemistry Darüberhinaus werden zur Zeit auch Inhibitoren von NO-Synthasen entwickelt, die zur Behandlung verschiedener Krankheitsbilder von grossem Vorteil sein könnten.
