premiums subsidised under the Riester scheme (§§ 10a, 82 et seqq. of the Income Tax Act [EStG]) in relation
to the above policy.

Please note that under the Retirement Income Act of 11/06/2004, once the one-off lump sum payment has been applied for, the
conditions for tax-free payment of premiums pursuant to § 3 No. 63 EStG from shall no longer apply 01/01/2005 – here: date of
application.

Annex to “Application for Lump Sum Payment” form
Information
Based on the law modernising statutory health insurance (Statutory Health Insurance Modernisation Act –
GMG) of 19/11/2003, health insurance funds’ full premium rates for pensions, used for calculating the health
insurance premiums payable from pensions, shall also apply to pensioners with statutory health insurance
with effect from 01/01/2004. PKDW will deduct these premiums from ongoing pension payments.
Moreover, under the revised version of § 229 Para. 1 Sentence 3 of the Social Security Statutes
Book (SGB) Vol. 5 of 01/01/2004, all lump sum payments for the purposes of retirement, survivor’s
or disability pensions are subject to mandatory premiums.
For one-off lump sum payments, 1/120 of the total amount will be added to the pensioner’s income that
is subject to premiums every month for ten years.
Under the current legal situation, the following documents must be submitted approximately
8 weeks before the lumpsum is due to be paid:

your original insurance certificate,
a copy of your birth or marriage certificate,
the first page of the social security insurance holder’s Notice of Pension Entitlements (copy),
your Tax ID Number,
the address of your health insurance fund and your health insurance number,
your bank account details and
if the lump sum is being paid before the age of 65, additional proof of loss of income.
(Note: In accordance with the legal definition of a pension fund, PKDW may only insure for loss of income. A copy of the first
page of your Notice of Pension Entitlements or Notice of Unemployment, along with written confirmation that your
unemployment benefit plus pension payments will not exceed your previous income, will suffice as proof. The same applies
to the non-working phase of semi-retirement.)
