Vorgehen bei neuen Patienten: Anamnese: Fokus Schmerz, Infektionen und Begleiterkrankungen Körperlicher Untersuchungsbefund inklusive Größe und Gewicht: Fokus Polyneuropathien, Infektionen und Schmerzlokalisation Laboruntersuchungen (Blut): Blutbild mit Differentialblutbild, Gesamtprotein, Albumin, Kreatinin, Harnstoff, Natrium, Calcium, Kalium, GOT, GPT, γ-GT, Bilirubin, LDH, AP, Harnsäure, CRP, Beta2-Mikroglobulin, Serum-Elektrophorese mit Immunfixation, freier Leichtkettentest Laboruntersuchungen (Urin): Kreatinin-Clearance, Gesamtproteinurie, Albuminurie, quantitative Leichtkettenausscheidung, Immunfixation Knochenmarkdiagnostik: Aspirations-Zytologie sowie 80 ml Heparinblut für FISH-Diagnostik und GEP (Tel. Nr. GEP-Labor: 39098), Histologie (z. Hd. Suitable for patients up to 65 years of age. The RP regimen includes Revlimid (lenalidomide) and prednisone. Regimen Schedule: Revlimid 25 mg p.o. days 1 - 21 (Administered after a meal) Prednisone 2 mg/kg/day p.o. day 1 - 4 (Administered after the morning meal) Necessary Measures: optional Anopyrin 400mg 1x1 Standard Prophylaxis: omeprazole 1-2x20mg/day along with prednisone, optional growth factor. Regimen Measures: Blood count check on the 15th day after the administration of lenalidomide, at the facility closest to the patient’s home. Suitable for patients up to 65 years of age. The CVD regimen includes cyclophosphamide, Velcade (bortezomib) and dexamethasone. If the regimen (4 cycles) is applied before the collection of blood stem cells, Velcade should be discontinued 6 weeks prior to stimulation. Regimen Schedule: Cyclophosphamide 500 mg/sq.m 625 mg/sq.m - i.v. - p.o. day 1 and 15 (Administered after the morning meal) Velcade 1.5 mg/sq.m - i.v. bolus days 1, 8, and 15 Dexamethasone 40 mg/day p.o. days 1-4 and 15-18 (Administered after the morning meal) Recommended Measures: omeprazole 1-2x20mg/day and optinal fluconazole 100mg 1x1 along with dexamethasone, cotrimoxazole 480mg 0-0-1, acyclovir 200mg 1-0-0, loperamide up to 3x1 if diarrhoea occurs after Velcade. Antiemetics: Determined by the physician Regimen Measures: If possible, 2-3 liters of liquids per day, hydration before administering Velcade (e.g., saline 1/1 500 ml before Velcade is administered; after application, flush with saline 1/1 100-250 ml).
