The Hôpital Erasme is the teaching hospital of the Université libre de Bruxelles. As such, it takes on the university’s values as its own: tolerance, humanism and freedom of thought. The hospital welcomes all patients and does not discriminate on the grounds of origin, gender or religion. Its objectives are to provide care, teaching and medical research. Since 1977, the Hôpital Erasme has had over 800 beds spanning all areas of specialist medical practice and run by various services. Almost 3000 members of staff perform an extremely varied range of jobs which contribute to its threefold mission. With its highly sophisticated equipment, the hospital provides care to patients from near and far, including those who have travelled from abroad. The aim of this brochure is to provide you with all of the information you need to make your stay as comfortable as possible. At the same time you will receive a feedback form. We would be very grateful if you could fill in this form and return it to a member of nursing staff at the end of your stay. Hospital staff is on hand to provide any additional information you require. We wish you a speedy recovery to full health. Before your stay > Access The Hôpital Erasme can be accessed: By car From the motorways Namur (E411/A4), Liège (E40/A3), Antwerp (E19), Ostende (A10), Charleroi (E19/A9), take the ring RO, look for exit “ULB Erasme” (exits 16 or 15a) By metro The metro station ‘‘Erasme’’, directly facing the hospital is where line 5 terminates (Hermann-Debroux - Erasme). Depending on the time of day, metros run every 6 to 10 minutes. Upon leaving the station, look for the exit towards the same direction of travel, turn left once in the tunnel and you will find yourself facing the hospital’s main entrance. From the same platform opposite, take line 5 in the direction “Erasme” and get off at Erasme station (where the metro terminates). Travelling from the Gare Centrale: Take the metro (line 5) towards “Erasme” and get off at the station “Erasme” (where the metro terminates). Travelling from the Gare Bruxelles-Schuman: Take the metro (line 5) towards “Erasme” and get off at “Erasme” (where the metro terminates). Travelling from the Gare du Nord: Take the pre-metro 4 (Stalle) or 3 and 33 (Churchill) for two stops and get off at “De Brouckère”. Airport information: www.brusselsairport.be Alternatively, take the train from the airport to the Gare Centrale 4 A guide to your hospital stay - Hôpital Erasme > What to pack We recommend that you bring pyjamas, nightshirts, a dressing gown, slippers, underwear and any toiletries you may need (toothbrush, toothpaste, razor, soap, comb, brush), including towels and washcloths. > Estimating the cost of your stay An estimate of how much your stay is likely to cost can be obtained from the Admissions Service (T 02 555 39 30 or [email protected]). > Parking A number of independently run pay-and-display car parks are within close proximity of the hospital. The “Erasme” car park can be found to the left of the main entrance and is connected to a shopping centre. The “Lennik” car park is on the Route de Lennik, opposite the campus, close to the metro station. Preferential rates are available for long-term stays. For any information, please contact the company in charge of managing the car parks. (T 02 217 98 86 - F 02 219 33 31) You may also be dropped off in front of the hospital. If using this option, stops at the main entrance must be very brief. > Security The hospital actively pursues an anti-theft policy but cannot be held responsible for any loss or theft of objects occurring during your stay. Avoid bringing objects of value (jewellery, watches, cash, wallets, bank cards, MP3 players, etc.). Use of mobile phones is permitted except in the Intensive care Unit, the Dialysis Unit, recovery room or wherever a sign indicates that mobile phone use is not allowed. Any objects of value can be left in the safety deposit box at the cash desk in the main hall. This desk is open on weekdays, from 9:00 am to 12:00 am and from 1:00 pm to 4:30 pm. It will not be possible to deposit or retrieve objects outside of these hours. The desk is closed at weekends and on public holidays 6 2. Admission The Admissions Service is open on weekdays from 8:00 am to 6:00 pm, on Saturdays from 9:00 am to 12:00 am and from 1:00 pm to 4:00 pm and on Sundays from 9:00 am to 6:30 pm. As soon as a hospital doctor has decided to admit you, you will be asked to take a hospital stay request form to the Admissions Service, located in the main hall. You will be given a number of documents which you must read carefully. Some of them will require a signature. To complete the administrative formalities, you will also be asked to produce your identity card or passport, your social security card (SIS card), any documents relating to your hospital insurance policy including your insurance card. If an accident has occurred at work or whilst practising sport, it is essential to provide proof of your insurance policy. Finally, please also provide us with the contact details for your GP so that we can inform him/her of your progress. > Paying your hospital costs in advance You will be charged an advance sum of money upon admission. This can be renewed each week. Rates vary according to the type of room and details of these can be found in the annex. > Insurance All of the hospital’s practitioners are covered by professional indemnity insurance. Further information on this subject can be obtained upon request from the Medical Director’s secretariat (T 02 555 32 02 or [email protected]). > Use of medical data and residual bodily materials for the purposes of scientific medical research The Hôpital Erasme is a teaching hospital where scientific research is carried out. As such, unless prior refusal has been expressed on your part, our teams may be called upon to consult your medical records or preserve fragments of tissues or organs which have been extracted as samples (biopsies, samples from operations, etc.) as part of drawing up a diagnosis or therapy. Anonymity is always respected. 7 A guide to your hospital stay - Hôpital Erasme You may at any moment, even after your stay, oppose any use for these purposes by signing and sending back the appended form. Your stay from A to Z From the very moment you arrive, a dedicated team of professionals will be on hand to cater for your every need and answer any questions you may have. > Arrival Admissions personnel are on hand to welcome you in the main hall on the ground floor and can provide you and your visitors with any information you require. They are available every day on 02 555 33 32 or [email protected]. > Arrival on the hospital ward As soon as you arrive, a member of the care team will be on hand to welcome you and explain what will happen during your stay and who the doctor(s) in charge of the ward will be. You will be given an explanation of how your bed works, how to call a nurse and how the lighting, telephone and television work. All rooms are (or are soon to be in the near future) equipped with a fridge, a toilet cubicle and a plug socket (220V). A bath or shower room is available on the ward. Individual rooms all come equipped with these as standard. Should you need to leave your room temporarily, do not forget to: • inform the nurse; • ensure you are wearing a night dress, dressing gown and over garment; • close the door to your room. Unless medical permission has been granted, it is not possible to leave the hospital. In single-person rooms, it is possible for a close relative to stay overnight. All conditions and rates can be found in the appendix. For hygiene reasons, animals are not permitted in the hospital. We kindly ask you to refrain from feeding the birds outside the window to your room. > Moral, religious or philosophical assistance Your philosophical and religious convictions will be respected at all times. Only patients who request the assistance of a religious minister or secular advisor will be receiving a visit. If you would like a visit, please speak to the hospital secretary. More detailed information can be found in the appendix. 9 A guide to your hospital stay - Hôpital Erasme > Banking A branch of BNP PARIBAS FORTIS (T 02 520 24 40 - F 02 520 44 79) is located on the ground floor of the hospital. It is open from Monday to Friday, from 9:00 am to 1:00 pm and from 2:00 pm to 4:00 pm. It has an ATM (Bancontact /MasterCard/Visa). Another ATM is available in the shopping centre. > Volunteers A team of volunteers from the Belgian Red Cross is on hand to assist you to your room, help you as you move around the hospital and pay you visits. To find out more about the services available to you, simply ask the hospital secretary or call 02 555 34 33. > Library Thanks to support from the Belgian Red Cross, a library is available, free of charge. Inroom book loaning takes place on Monday and Thursday afternoons. The library is also open on the first basement level (01) on Monday from 10:30 am to 4:00 pm, Tuesday from 2:00 pm to 4:00 pm and on Thursday from 10:30 am to 4:00 pm (T 02 555 36 02). Please remember to return your books to the hospital secretary before you leave the hospital. > Wheelchairs There are wheelchairs available in the area off to the right of the main entrance and in the volunteers’ office nearby the information office in the main hall of the hospital. Volunteers are on hand to assist you back to your room. > Hairdresser A hair salon can be found on the ground floor (T 02 555 43 44). Volunteers are on hand to help those experiencing mobility difficulties to get there. > Shops Snacks, newspapers, confectionery, drinks, fresh fruit, flowers, games, films and gifts are all available for purchase on the ground floor of the hospital. There are other shops available in the shopping centre. 10 > Mail service Your mail will be brought to you each day. Ask anyone wishing to send mail to you to mention your first name, surname and room number on any correspondence. (Once again, the hospital’s address is: Hôpital Erasme - Route de Lennik 808 - 1070 Brussels). To send mail, deliver your stamped envelopes to the nurse or the secretary of the care unit. There is also a letterbox in the main hall. > Your rights and responsibilities as a patient The rights of a patient that must be respected when being admitted to hospital are now enshrined in law. Further details can be found in the appendix. The law also provides a definition of mediation. At the Hôpital Erasme, the patients’ rights ombudsman is Alain Heselwood (T 02 555 44 91 - F 02 555 43 76 - mediateur@ erasme.ulb.ac.be - www.patientsrights.be). It is free to consult the ombudsman and confidentiality will be respected at all times. The ombudsman is bound by professional secrecy. > Robert Dubois School To avoid falling behind at school during their hospital stay, children may attend lessons which are personalised and free of charge. The school is made up of several different classes spanning nursery, primary and secondary level as well as a workshop for crafts. Bedside lessons can also be arranged for children if required by their level of health. The school is located at the 3rd floor offices 3010 and 3015 (T 02 555 68 69) > Shopping centre A shopping centre can be accessed from the ground floor of the hospital. There, you will find a range of different shops (selling food, books, toiletries...) as well as a branch of a health insurance provider. > Visiting hours Family and friends are welcome to visit. Visits are allowed during the week, at weekends and on public holidays from 3:00 pm to 8:00 pm. In single-occupancy rooms, visiting hours run from 1:00 pm to 8:00 pm. Intensive care, the Coronary care Unit and Psychiatric Service have special visiting hours which are on display. 11 A guide to your hospital stay - Hôpital Erasme In order to guarantee high-quality care, please make the following recommendations to your visitors: • please leave the room whenever care is being dispensed or a doctor is visiting; • avoid excessively long visits; • abide strictly by the visiting hours. The hospital closes its doors at 8:30 pm. > Smoke-free hospital Out of concern for the overall health of the population, the Hôpital Erasme is a member of the Smoke-free Hospitals network. Resulting from and in keeping with the law, smoking in the hospital is not allowed, including in patients’ rooms. Only patients on the psychiatry ward have access to a smoking room, under certain conditions. For all other patients, a smoking area is available outside the hospital, close to the main entrance. To avoid having withdrawal symptoms during your stay, nicotine substitute products may be prescribed to you. Smoking experts and a Smokers’ Assistance Centre are on hand to help. For any information, call 02 555 34 20. If your hospital stay is being planned a long time in advance, we recommend you raise the subject with your GP. Your stay may be an opportunity to stop smoking. > Hotel The Hôtel Erasme is located on the Route de Lennik 790 - 1070 Brussels T 02 523 62 82 - F 02 523 62 83 For further information, please consult the website www.hotelerasme.com > Internet Each room is equipped with internet access. There is a charge for this service and it is controlled through the television (see television section). In order to access the internet, you must bring your own laptop. > Homestay for family members The homestay for family members (“Maison d’accueil des familles”) is simple accommodation, representing good value and a warm setting. It allows families who live a long way from the hospital to stay close to their relative and comfort them with their presence. Patients cannot stay here. Information can be obtained from the Maison d’accueil des familles, Postweg 13, 1602 Vlezenbeek (T 02 567 09 69), from Monday to Friday from 8:30 pm to 10:00 pm, on Saturdays from 10:00 am to 12:00 am and from 8:30 pm to 10:00 pm, on Sundays and public holidays from 4:00 pm to 6:00 pm and from 8:30 pm to 10:00 pm. 12 Information can also be obtained from the hospital’s social services (4th floor), from Monday to Friday between 8:00 am and 12:00 am and from 1:30 pm to 5:00 pm (T 02 555 34 73) as well as at the hospital’s main reception. > Lost and found All lost and found objects are dealt with at the Conciliation Service, which can be contacted on weekdays on 02 555 59 59 and at weekends and public holidays on 02 555 43 00. > Chiropodist You may request a visit from the chiropodist by arranging this with the nurse on your ward. You will be charged for this appointment at the end of your stay. > Meals Breakfast is served at approximately 9:00 am, lunch at 12:30 am and dinner at 6:00 pm. A wide range of food and drink is available. If necessary, a dietician is on hand to offer dietary advice. > Social services Sickness and being admitted to hospital may have consequences that require you to call upon social services. They are there to assist you with any social or administrative matters (health insurance, unemployment, ...) and can supply you with information or help you prepare for the time you leave hospital (domestic assistance, convalescence, rehabilitation, revalidation, placements, ...). To arrange a meeting with the social assistants, contact the hospital secretary or the social services secretariat on 02 555 34 73 from 8:00 am to 4:30 pm. The social services offices are located on the 4th floor. > Telephone The hospital equips each room with a telephone handset. You will have a unique access code which will be provided to you upon admission. It is activated upon request by the staff of your ward and will be automatically cancelled at the end of your stay. To place a call, enter your code and once you hear the dialling tone, enter the number of the person you are trying to reach. Anyone wishing to call you must dial 02 555 7 followed by the 3 figures of your room number. 13 A guide to your hospital stay - Hôpital Erasme > Television - Internet Each room is equipped with a television set. The rental formalities are carried out at the Admissions Service upon arrival. Flatscreen televisions with Wi-Fi capability are currently being installed. You can either select the whole package (58 channels plus internet) or a mini-package (two national channels in fr-nl + two children’s channels fr-nl). If you would like internet access, you also have to bring your own laptop. Rental rates can be found in the appendix. Headphones, which are compulsory in rooms with two beds, and internet cables can be bought from the Admissions Service. If you are in an individual room, the cost of television channel rental is included in the price of your room. For further information concerning television and internet, contact the Admissions Service (T 02 555 39 30 - [email protected]). > Restaurants The cafeteria (staff canteen) and sandwich shop, located on the first basement level (01), are accessible to visitors. They are open from 8:30 am to 5:30 pm for the cafeteria and from 11:30 am to 2:30 pm for the restaurant. They are closed on Saturdays and Sundays. The café-restaurant “Le Jardin d’Erasme”, located on the ground floor of the hospital, is open from Monday to Friday from 7:00 am to 8:00 pm, on Saturdays and Sundays from 10:00 am to 7:00 pm. www.lejardinderasme.be The café-restaurant “Le Pré Vert”, located in the shopping centre, is open from Monday to Friday from 10:00 am to 3:00 pm. The sandwich shop “Le Nouveau Matin”, located in the shopping centre, is open from Monday to Friday from 7:30 am to 5:00 pm. > Travel There is a travel agency located on the first basement level (01), close to the cafeteria. Your departure Your doctor will inform you of the likely date of your departure as soon as possible. Unless medical conditions dictate otherwise, you will leave the hospital before lunchtime. On the day of departure you will receive a provisional hospital report for your GP and all of the necessary medical prescriptions and information for your ongoing treatment. Depending on your needs you will receive a full dossier containing the relevant information for the various service providers you may require (physiotherapist, nursing staff, etc.). You will receive CD-ROMs containing the results of your diagnostic imaging. It is recommended to keep these somewhere safe. If there are any documents (certificates, medical documents, private insurance documents, etc.) which have to be filled in by the doctor on the ward, hand them in at the hospital secretariat so that they can be filled in by the day of departure at the latest. If you have to return for any consultations, the hospital secretary will organise the date and time of your appointments. If you wish to leave the hospital without the prior approval of a doctor, you must first fill in and sign the document entitled “sortie exigée” (requested release form) which will be provided to you. > Hospital stay certificate As part of some supplementary health insurance schemes, a number of insurance providers offer a daily allowance for hospital stays and payment is received upon producing a hospital stay certificate. In order to obtain this, ask the hospital secretary before you leave or submit a request in writing to the Admissions Service who will then send the certificate to you. This certificate may also be used to make a hospital insurance claim. > Rest and recovery Social services are on hand to help at all stages when • returning home, supported by a coordinated healthcare centre; • being admitted to a rest or care home or rehabilitation centre. > Feedback form We would be very pleased to hear any comments, criticisms or suggestions you may have that might allow us to improve our services in future. To that end, we would ask you kindly to fill in the questionnaire found in the annex. Please return the completed form to the secretary on your ward or to the Admissions Service on the ground floor. 16 > The cost of your stay Your hospital bill will arrive at your home. It will include the costs that are to be paid by your insurance provider and those that are to be covered by you. The Billing Service is on hand to provide any information you require (T 02 555 43 08 - F 02 555 68 22 [email protected]) > Going home If you have any difficulties with transport, the hospital secretary will help to find the right solution (ambulance or taxi). Provide them with the details of your insurance provider and state any preferences you may have. A guide to your hospital stay - Hôpital Erasme Annexes 1. Charter of patient rights and responsibilities During a consultation, examination or admission to hospital, we are obliged to respect the law of 22/08/2002 regarding patient rights and to: 1. provide you with high quality healthcare without any distinction based on nationality, social class or personal wealth, and to respect human dignity and personal autonomy at all times; 2. allow you to choose your own practitioner including the right to change practitioner at any time; 3. supply you with clear information regarding your state of health and planned treatment, in language that is clear and easy to understand, in writing where necessary, and to a trusted person if the need arises. You have the right to refuse this information, unless this refusal would pose a danger to yourself or others. 4. request your informed consent for all forms of medical intervention using language which is clear and sufficiently detailed regarding the aim of treatment, its nature, degree of urgency and duration as well as any adverse effects it may have, side effects and inherent and relevant risks, how your health will be monitored, possible alternatives and financial repercussions. You have the right to change your mind, to specify the conditions under which treatment must be provided, continued or stopped. 5. guarantee the confidentiality of your medical file and allow you to consult it at any time and to obtain, as soon as possible, a copy upon request and in exchange for a fee. 6. guarantee the protection of your privacy, especially where information relating to your health is concerned, and to respect your personal privacy. Unless you agree otherwise, only persons whose presence is justified in providing the relevant services may be present for the provision of care, examinations and treatment. 7. offer you relief from any pain you are experiencing. We ask you to respect: 1. the privacy and well-being of other patients in particular by observing visiting hours; 2. all members of hospital staff and their work; 3. the equipment and materials provided to you. We are committed to doing everything possible to guarantee you a high quality stay. If, however, you have any comments, suggestions or complaints, please contact our ombudsman, Alain Heselwood (T 02 555 44 91 - F 02 555 43 76 M [email protected]). 18 The role of the ombudsman is defined in the law of 22/08/2002 as well as the Royal Decree of 08/07/2003. The primary aim of the ombudsman is to guarantee that your rights are respected when making a complaint in accordance with patient rights enshrined in law. The ombudsman shall not intervene in matters relating to accommodation (heating, meal quality, etc.) or in personal disputes (altercations, relationship conflicts), lost or stolen objects, etc. It is free and confidential to call the ombudsman. The ombudsman is bound by professional secrecy. 2. Quality and security of care The Hôpital Erasme takes care to ensure that the care provided is of the highest possible quality and that it is delivered in optimal security conditions. To that end, the hospital actively takes part in a number of initiatives supported by the federal and community public authorities. Reducing hospital-acquired infections, the removal of undesirable side effects when taking medicines and reducing the risk of patient identification errors are priority objectives. You have an important role to play in promoting high quality care. As part of this, please inform the team responsible for your treatment of any change in your circumstances. 3. Participation in clinical research Prospective research protocols include subjects (either in good health or suffering from a particular medical problem) who agree to take part. All research projects must first have been accepted by the Ethics Committee after being submitted by the doctor or other healthcare professional responsible. All clinical research participants must have received prior information on the aims of the research, possible risks and benefit and told of their rights. They must provide written consent by signing a document which sets out their rights and commitments as well as those of the researcher. Participation is on a free and voluntary basis. Any pressuring of the patient to take part will not be tolerated and the patient may refuse to take part even after having signed the consent form without the fear of receiving inferior quality treatment from the doctor of medical staff thereafter. Protection of privacy Any hospital visit involves collecting, registering, storing and transferring personal data. These may be administrative or may have to do with your health (diagnostics, treatment and healthcare, nurses or paramedics) and are treated in accordance with the law. The Medical Director is responsible for how data is handled and has made the relevant declarations before the Commission for the Protection of Privacy. Administrative data is essential to the management of your file, communicating with the relevant authorities and bodies (Public Health Ministry, insurance providers, INAMI, etc.) and for invoicing for services provided. You may, at no cost, request access to any of your personal details, provide supplementary information and make corrections by consulting the Admissions Service (T 02 555 39 30). Unless refusal has been provided in writing, data regarding your health may be used in conjunction with scientific research projects. You retain the right to oppose the use of your data in this way. Data relating to sampling are centralised in a national database: the Belgian Virtual Tumourbank (www.virtualtumourbank.be ). Don’t hesitate to send any suggestions or ideas for improvement to us by email: [email protected] 22 Fonds Erasme POUR LA RECHERCHE MÉDICALE The Fonds Erasme pour la Recherche Médicale was set up to provide financial support for medical research at the Hôpital Erasme. Funding comes exclusively from donations made by individuals or companies and in rare instances public authorities. It supports medical research thanks to bursaries and research mandates granted to young doctors and thanks to research agreements signed with groups of clinical researchers from the hospital. Each year, the Fonds Erasme spends an average of 1.3 million € to support medical research at the Hôpital Erasme. The aim of the bursaries and research mandates granted to young doctors who are training in specialist areas (or just after completing their training) is to allow them to complement their training with scientific research. Research offers irreplaceable schooling in learning rigorous methodology and the development of the critical thinking that is a prerequisite for high quality modern medicine. Research agreements are signed between the Fonds Erasme and groups of clinical researchers at the hospital so that they can pursue innovative projects. The priority of the Fonds Erasme is to finance projects that attempt to transfer the results of fundamental research to clinical practice so that patients can benefit from the knowledge generated by research in the laboratory.
