What you need for a proper and safe fitting:
UA Braces mouthguard, pot for boiling water with 4 inches of water to
completely submerge the mouthguard, bowl of cold tap water, slotted
spoon, mirror and a clock or watch with second hand.
What you need for a proper and safe fitting:
UA Braces mouthguard, microwave oven (1100 watts or more),
microwave safe dish, wet paper towel, bowl of cold tap water,
mirror and a clock or watch with second hand.
1. Bring water to a boil.
CAUTION Boiling water is hot and can cause injury. Adult
supervision required.
2. Remove boiling water from the heat source and allow water to cool
for 30 seconds.
3. Carefully submerge the UA Braces mouthguard in the hot water
for 30 seconds.
4. Carefully remove mouthguard from the hot water with a slotted spoon
and place mouthguard in cold water to cool for 5 seconds. NOTE: Use
caution when handling the UA Braces mouthguard.
5. Gently glide your index finger inside the molar channel to ensure there is
ample space for the mouthguard to fit inside your mouth. Do not allow
the sides to fold over as the material will “glue together” making it
difficult to get an optimal fit.
CAUTION Before placing the mouthguard into your mouth, test its
temperature with the tip of your finger. The mouthguard should not
be hot, but at a warm and comfortable temperature.
6. Using a mirror, carefully place the mouthguard in your mouth, lining up the
“notch” in the center of the mouthguard with the center of your front teeth.
7. GENTLY bite down on the UA Braces mouthguard. NOTE: Do not
clench down excessively with your molars. DO NOT SUCK ON THE
MOUTHGUARD OR CLOSE YOUR LIPS TIGHT TOGETHER OR
PRESS ON THE MOUTHGUARD WITH YOUR FINGERS
DURING FORMING.
8. After 1 minute, remove the UA Braces mouthguard and place in
bowl of cold water for 5 minutes. You now have a “custom-like”
fitting mouthguard.
1. Wrap the UA Braces mouthguard in wet paper towel, and place on
microwave safe dish.
2. Place dish with the wrapped mouthguard in the microwave oven on high
setting for 45 seconds. Follow the microwave oven’s instructions for
safe operation.
3. Carefully remove the dish and mouthguard from the microwave oven.
CAUTION Paper towel and dish may be hot and can cause injury.
4. Carefully unwrap the mouthguard and place mouthguard in cold water
to cool for 5 seconds. NOTE: Use caution when handling the UA
Braces mouthguard.
5. Gently glide your index finger inside the molar channel to ensure there is
ample space for the mouthguard to fit inside your mouth. Do not allow

DENTAL LIMITED WARRANTY
CERTIFICATE OF LIMITED WARRANTY AND DISCLAIMER
WARRANTY: Bite Tech, Inc. (Bite Tech) warrants this mouthguard to be free from defects in material and workmanship which could cause damage or
injury to the sound, natural teeth of the purchaser herein when properly fitted and used for a period of one (1) year from the date of purchase.
LIMITATION OF LIABILITY: Bite Tech’s obligation under this warrant shall be limited to the actual costs incurred by the user for necessary dental
treatment by an appropriately licensed dental professional, up to a maximum liability of $500 per injured or replaced tooth and with a maximum
liability per incident of $32,000. Bite Tech’s obligation shall be reduced by any insurance coverage available to the user and applicable to the treatment
of the injury claimed. This limited warranty does not provide payment or indemnify for orthodontic treatments or preparation of a tooth to be used as an
anchor for a bridge.
The remedies set forth herein are exclusive and the total liability of Bite Tech with respect to use of this mouthguard, whether based
on contract, warranty, negligence, indemnification, strict liability or otherwise. In no event shall Bite Tech be liable for consequential,
incidental or special damages.
CONDITIONS: This limited warranty applies only to injuries to the purchaser’s teeth that are free from any preexisting natural or unnatural defect, including
prior dental work. This limited warranty only applies if the purchaser suffered a dental injury in a supervised athletic contest or coach-supervised training session.
Bite Tech shall have no responsibility
or liability hereunder, unless:
1. The mouthguard was fitted in accordance with the printed instructions accompanying the mouthguard; and
2. The mouthguard was not damaged or altered in any way, whether by chewing on the mouthguard or by other action to alter its configuration or
chemistry (other than by the proscribed fitting), except as occurs by normal use; and
3. A complete, written notice of the injury and damage, together with a copy of the receipt of purchase, is received by Bite Tech within thirty (30) days
after the occurrence of the injury; and
4. The user provides a written statement from an appropriately licensed dentist providing necessary treatment within thirty (30) days after the date of
injury that substantiates the injury, damage and treatment provided; and
5. The user provides a statement by a supervising coach or participating game official that the mouthguard was in proper use at the time of injury; and
6. The applicable mouthguard is returned to Bite Tech with the written notice of injury. The mouthguard shall become the property of Bite Tech upon
receipt of return.
NO MOUTHGUARD CAN PREVENT ALL INJURIES OR ALL DENTAL DAMAGE AND NO SUCH WARRANTY IS BEING MADE. THIS WARRANTY IS
MADE IN LIEU OF ALL OTHER WARRANTIES EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO THE IMPLIED WARRANTY FOR A PARTICULAR PURPOSE AND ANY IMPLIED WARRANTY ARISING OUT OF A COURSE OF A DEALING, CUSTOM, USAGE OR TRADE. EXCEPT AS IS

Matériel nécessaire pour un ajustement adéquat et sécuritaire :
le protège-dents pour appareil orthodontique UA, une casserole
remplie de 4 pouces d’eau bouillante pour y plonger complètement
le protège-dents, un bol d’eau froide, une cuillère à rainures et un
miroir, ainsi qu’une horloge ou une montre dotée d’une aiguille
pour les secondes.
Matériel nécessaire pour un ajustement adéquat et sécuritaire : le
protège-dents pour appareil orthodontique UA, un four à microondes (puissance de 1 100 watts ou supérieure), une assiette allant
au four à micro-ondes, un essuie-tout humide, un bol d’eau froide
et un miroir, ainsi qu’une horloge ou une montre dotée d’une aiguille
pour les secondes.
1. Faites bouillir de l’eau.
AVERTISSEMENT : l’eau bouillante est très chaude et peut causer
des blessures. La supervision d’un adulte est requise.
2. Enlevez l’eau bouillante de sa source de chaleur et laissez-la refroidir
pendant 30 secondes.
3. Immergez délicatement le protège-dents pour appareil orthodontique
d’UA dans l’eau chaude pendant 30 secondes.
4. Retirez délicatement le protège-dents de l’eau chaude à l’aide de la
cuillère à égoutter, et placez-le dans l’eau froide pendant 5 secondes.
REMARQUE : Manipulez le protège-dents avec soin.
5. Glissez délicatement votre index dans la cavité des molaires pour
vous assurer qu’il y a suffisamment d’espace pour placer les dents dans
le protège-dents. Évitez que les côtés se plient, car la matière peut se
« coller » sur elle-même et ainsi réduire les chances d’obtenir un
ajustement optimal.
AVERTISSEMENT : avant de mettre le protège-dents dans la
bouche, vérifiez sa température du bout du doigt. Le protège-doigt ne
doit pas être chaud, mais plutôt tiède et d’une température confortable.
6. À l’aide d’un miroir, mettez délicatement le protège-dents dans la bouche
en alignant la « coche » du centre du protège-dents avec le centre de
vos dents avants.
7. Mordez DÉLICATEMENT dans le protège-dents pour appareil
orthodontique d’UA. REMARQUE : Ne mordez pas trop fort avec
vos molaires. NE CRÉEZ AUCUNE FORCE D’ASPIRATION SUR
LE PROTÈGE-DENTS, NE FERMEZ PAS LES LÈVRES, ET
N’APPUYEZ PAS SUR LE PROTÈGE-DENTS AVEC VOTRE
DOIGT PENDANT L’AJUSTEMENT.
8. Après 1 minute, enlevez le protège-dents pour appareil orthodontique
d’UA et déposez-le dans le bol d’eau froide pendant 5 minutes. Vous
avez maintenant un protège-dents entièrement adapté à votre bouche.
1. Mouillez l’essuie-tout, enroulez-le autour du protège-dents pour
appareil orthodontique d’UA, et placez le tout dans une assiette allant
au micro-ondes.
2. Placez l’assiette contenant le protège-dents enroulé dans l’essuie-tout
mouillé au micro-ondes et faites-le chauffer à puissance élevée pendant
45 secondes. Suivez les instructions d’utilisation du micro-ondes pour
une utilisation en toute sécurité.
3. Enlevez délicatement l’assiette du micro-ondes.
AVERTISSEMENT : l’essuie-tout et l’assiette risquent d’être
suffisamment chauds pour causer une blessure.
4. Enlevez délicatement le protège-dents de l’essuie-tout et placez le
protège-dents dans l’eau froide pendant 5 secondes.
REMARQUE : Manipulez le protège-dents pour appareil orthodontique
d’UA avec soin.

GARANTIE : Bite Tech, Inc. (Bite Tech) garantit ce protège-dents contre tout défaut matériel ou de fabrication susceptible de causer des dommages ou des lésions aux dents saines et naturelles de
l’acheteur aux présentes lorsqu’il est ajusté et utilisé adéquatement pendant un (1) an à compter de la date d’achat.
LIMITATION DE RESPONSABILITÉ : L’obligation de Bite Tech en vertu de la présente garantie se limite aux coûts effectivement engagés par l’utilisateur pour le traitement dentaire nécessaire
effectué par un dentiste agréé, jusqu’à concurrence d’un montant maximal de 500 $ par dent endommagée ou remplacée et de 32 000 $ par incident.

3. un avis écrit et complet détaillant la lésion et les dommages ainsi qu’une copie de la preuve d’achat sont reçus par Bite Tech dans les trente (30) jours suivant la survenance de la blessure; et
4. l’utilisateur fournit, dans les trente (30) jours suivant la date de la blessure, une déclaration écrite par le dentiste agréé qui a dispensé le traitement requis comportant des preuves à l’appui de
la lésion, des dommages et du traitement prodigué; et
5. l’utilisateur fournit une déclaration d’un entraîneur qui était chargé de la supervision de l’activité sportive ou d’un officiel en service lors de la rencontre sportive stipulant que le protège-dents
était utilisé adéquatement au moment de la blessure; et
6. le protège-dents applicable est retourné à Bite Tech avec l’avis écrit détaillant la blessure. Le protège-dents deviendra la propriété de Bite Tech dès la réception du produit.
