The Zimmer One-Piece 3.0mm Implant must be splinted if two or more are used adjacent to each other. The Zimmer One-Piece 3.0mm Implant may be immediately restored with a temporary prosthesis that is not in functional occlusion. ZIMMER ONE-PIECE DRIVER Description The Zimmer One-Piece Driver is a stainless steel (SS 465) that is used to drive the implant into the osteotomy Indications: Used to thread the implant into the osteotomy using an accessory tool such as the GemLock® Retaining Square Ratchet (RSR). ZIMMER ONE-PIECE SURGICAL TRY-INS Description The Zimmer One-Piece Try-ins are similar in shape and design to the abutment and replicate the exact geometry of the implant’s prosthetic portion. It is placed in the osteotomy to verify position and angulation.The Zimmer One-Piece Try-in is composed of titanium alloy (Ti-6Al-4V). Indications: The Zimmer One-Piece Try-ins are placed in the osteotomy to verify position and angulation. CONTRAINDICATIONS Zimmer One-Piece 3.0mm implants are contraindicated for molar regions. Zimmer One-Piece Implants should not be placed if there is insufficient alveolar bone width and height to surround the implant (a minimum of 1 mm circumferentially and 2 mm apical). Implants placed in the maxilla should not perforate the sinus. Poor bone quality, poor patient oral hygiene, heavy smoking or use of chewing tobacco, and generalized diseases (diabetes, etc.) may contribute to lack of integration, loss of supporting bone structure, and subsequent implant failure. Severe bruxism, clenching, or overloading may cause failure of abutments and implants. Relative contraindications include patients with psychological, mental and manual dexterity impairments, and those whose psychological, esthetic and/or functional needs cannot be met. Exposure to radiation and chemotherapy may impact implant survival. Physician consultation is highly recommended prior to implant treatment. WARNING Surgical and restorative techniques required to place dental implants are highly specialized and complex procedures. Practitioners should attend courses of study to familiarize themselves with implantology techniques. Improper technique can cause bone loss, patient injury, pain and implant failure. Zimmer Dental implant systems are intended to be used only with Zimmer Dental specially designed bone drills and prosthetics. Implants placed at unsuitable angles relative to existing dentition or multiple implants placed at convergent/divergent manner can result in complex restorations that may overload implants, potentially leading to implant failure (including fracture). A thorough diagnostic work-up and use of x-rays and surgical templates are recommended to help ensure proper angulation and avoidance of certain anatomical features such as sinus membranes, adjacent teeth and craniofacial nerves. Other relative warnings include steroid and anticoagulant treatment which may affect the surgical site, surrounding tissue, or patient’s healing function. Exposure to long-term use of bisphosphonate drugs especially with chemotherapy may impact implant survival. Careful patient selection including consultation with the patient’s physician is strongly recommended prior to implant treatment. Excessive mobility, bone loss, or infection may indicate the implant is failing. Any implant which appears to be failing should be treated or removed as soon as possible. If removal is necessary, curette any soft tissue from the implant site and allow site to heal as though it were an atraumatic extraction. Due to the metal conductivity, electrosurgery around the implants and intraoral abutment preparations without irrigation could result in tissue damage, patient injury and implant failure. Patients should consult with their physician and imaging technician prior to undergoing a Magnetic Resonance Imaging (MRI) procedure. Zimmer implants and abutments have not been evaluated for safety and compatibility in the Magnetic Resonance (MR) environment. Zimmer implants and abutments have not been tested for heating or migration in the MR environment. PRECAUTIONS Proper case planning is essential to the long-term success of both the prosthesis and the implant. Overload is one of the key contributors to implant failure. Ensure the implant size and abutment angulation are appropriate for the occlusal load. Highly angulated abutments should be avoided. BREAKAGE Fractures of the implant-abutment assembly can occur when applied loads exceed the normal functional design tolerances of the implant components. Potential overloading conditions may result from significant bone loss (e.g. >3mm), deficiencies in implant numbers, lengths and/or diameters to adequately support a restoration, excessive cantilever length, , abutment angles greater than 30 degrees, occlusal interferences causing excessive lateral forces, patient parafunction (e.g. bruxing clenching), loss or changes in dentition or functionality, , inadequate prosthesis fit, and physical trauma. Additional treatment may be necessary when any of the above conditions are present to reduce the possibility of breakage. CHANGES IN PERFORMANCE It is the responsibility of the clinician to instruct the patient on all appropriate contraindications, side effects, and precautions as well as the need to seek the services of a trained dental professional if there are any changes in the performance of the implant (e.g., looseness of the prosthesis, infection or exudate around the implant, pain, or any other unusual symptoms that the patient has not been told to expect). HYGIENE & MAINTENANCE Long-term implant health is directly related to the maintenance of oral hygiene. Potential implant candidates should establish an adequate oral hygiene regimen prior to implant therapy. Following implant placement, the clinician should instruct the patient on proper tools and techniques to ensure long-term maintenance of the implant(s). The patient should also be instructed to maintain routinely scheduled prophylaxis and evaluation appointments. TREATMENT PLANNING Adequate preoperative imaging and diagnostic evaluation are necessary to determine available bone anatomy in prospective implant sites. The location of important anatomical landmarks, such as the mandibular canal, maxillary sinuses, craniofacial nerves and adjacent teeth, should be established prior to use of Zimmer Dental implants. Care must be taken to evaluate the quality and quantity of the residual bone, especially after an implant failure and when implants will be immediately placed into extraction sites. Thorough clinical evaluation is imperative prior to all implant surgeries. ABUTMENT PREPARATION Minor abutment modifications may be performed intraorally with irrigation. To minimize heat generation, intermittent cutting using medium grit diamond burs under copious irrigation is recommended. GENERAL CONSIDERATIONS Control of biomechanical stresses is the key factor to long-term success of the prosthesis. Even after implant integration, imbalances in occlusal forces can lead to implant failure. Implant patients should be monitored post-operatively for , periimplant bone loss and tooth wear as signs of occlusal overloading. ADVERSE EFFECTS The following complications may occur relative to implant placement: pain, discomfort, dehiscence, delayed healing, paresthesia, hyperesthesia, edema, hemorrhage, hematoma, infection, inflammation, the need for additional surgery or removal, local and generalized allergic reaction, lack of integration, loss of bone, and loss of implant. Other adverse effects may also occur as a result of iatrogenic factors and host responses. STERILITY All implants have been gamma radiation sterilized and are for single use only. Do not resterilize implants. Zimmer dental prosthetic and ancillary components are sold sterile or non-sterile. Refer to the specific packaging for verification of sterility. Sterilize non-sterile product prior to use in patients. Some Zimmer Dental products that are sold sterile are for single use only, refer to specific packaging for single use only designation. SINGLE USE Reuse of a single use device that has come in contact with blood, bone, tissue, body fluids or other contaminants may lead to patient or user injury. Possible risks associated with reuse of a single use device include, but are not limited to, mechanical failure and transmission of infectious agents. SHELF LIFE The product expiration date is indicated by the hourglass symbol on the product label, followed by the year and month of expiration. Caution: Do not use sterile devices if the packaging providing the sterile barrier, including the outer cap, vial, or tray has been damaged or compromised in any manner (i.e. cracked, crushed, or torn). PRODUCT PACKAGING All implants have been cleaned, packaged in double vials within an environmentally controlled room, and sterilized for convenience and immediate use. The implants are suspended on a carrier for transfer to the prepared surgical site without risk of contact contamination. Both the implant and the inner vial packaging are sterile. The label on the outer vial packaging for each implant contains a lot number that should be recorded in the patient’s file to ensure complete traceability of the product. The patient chart label provided containing the lot number can also be placed in the patient file for traceability. Prosthetic and surgical components have been cleaned, packaged within an environmentally controlled room, and sterilized (if applicable, refer to product labeling) for convenience and immediate use. They are provided in a sealed plastic vial, Tyvek®** sealed blister, or sealed polybag. The label on the outer vial packaging contains a lot number that should be recorded in the patient’s file to ensure complete traceability of the product. The implant packaging system contains color coding on the cap and the patient chart label to indicate the implant platform. Refer to the Color Reference Chart for color coding. CLEANING/STERILIZATION INFORMATION Disinfection and sterilization procedures should conform to OSHA or local guidelines for blood borne pathogens. Clinically contaminated implants should not be cleaned and resterilized under any circumstances. CLEANING 7483 REV_1 02_14 ndd 1 STERILIZATION Individual parts should be placed in appropriate autoclave or dry heat pouch prior to sterilization. When sterilizing parts within a kit, parts should be placed in appropriate locations and kit should be wrapped in sterilization wrap. The following sterilization parameters (method, time and temperature) are required to achieve a 10-6 sterility assurance level (SAL). Local or national specifications should be followed where steam sterilization requirements are stricter or more conservative than those listed in the table. Exceeding these sterilization parameters may result in damage to plastic components. Verify the calibration of your unit to ensure recommended temperatures are reached. To ensure autoclave is performing effectively, periodic use of biologic indicators should be considered. Chemiclave sterilization is NOT recommended for any Zimmer Dental products. IMMEDIATE LOADING: Immediate loading is indicated when there is good primary stability and an appropriate occlusal load. Immediate loading is defined as the placement of one or more implants in either a partially or fully edentulous arch and placing the restorative portion either in-function or out-of-function at the time of implant placement and final restoration is in-function within the first two weeks of implant placement. TECHNIQUE INFORMATION Loading To maximize the load-bearing capacity of implant restorations in high-stress areas, the following procedures are recommended: • Consider using multiple splinted implants to support restoration. Zimmer One-Piece 3.0mm Implant are contraindicated for molar regions and must be splinted if two or more are used adjacent to each other. • If Buccolingual dimension of the ridge allows it, consider using a wider diameter implant to support the restoration. • Place an adequate number of implants for optimum load distribution. INSERTION PROCEDURES STEP 1 – Record the Data for Each Implant Placed: Record the part number, log number and product description in the patient’s chart, or attach the patient chart label that is included in the packaging on the patient’s chart. STEP 2 – Prepare osteotomy Initiate the osteotomy using the pilot drill. Place the Surgical Try-in in the osteotomy to verify position and angulation. Complete the osteotomy using the appropriate drills and drilling sequence per the surgical technique manual. For dense bone the appropriate sized tap may be used to thread the osteotomy for the desired implant diameter. STEP 3 – Open the sterile Outer Vial: Twist and pull to unscrew the color-coded cap, breaking the sterile barrier. STEP 4 – Remove the Inner Vial: Empty out the contents of the sterile inner vial containing the implant onto a sterile field. STEP 5 – Select the Appropriate Instrumentation: • 3.0mm Straight Zimmer One-Piece Implants: Remove the implant driver [ZOPDRS] with the blue colored groove from the surgical tray. Connect the driver to either a Stainless Steel Ratchet [RSR] or a Stainless Steel Screwdriver Handle [SSHS], ensuring that the driver is properly retained within the instrument. • 3.0mm Angled Zimmer One-Piece Implants Remove the implant driver [ZOPDRA] with the blue colored groove that is labeled“3.0A”from the surgical tray. Connect the driver to either a Stainless Steel Ratchet [RSR] or a Stainless Steel Screwdriver Handle [SSHS], ensuring that the driver is properly retained within the instrument. STEP 6 – Remove the Implant from the Inner Vial: To remove 3.0mm Zimmer One-Piece Implants, align the vertical black line on the implant driver with the vertical line on the inner vial and then place the driver over the abutment portion of the implant, making sure to press down onto the implant with firm pressure. Carefully pull the implant out of the vial, ensuring that the implant is captured within the implant driver. STEP 7 – Initiate the Implant into the Osteotomy: Carry the implant via the proper delivery/driver tool [ZOPDRS, ZOPDRA] to the receptor site and place directly into the prepared osteotomy. STEP 8 – Complete Seating with the Appropriate Instruments: Rotate the implant into place with delivery/driver tool [ZOPDRS, ZOPDRA] and Stainless Steel Ratchet [RSR] or Stainless Steel Screwdriver Handle [SSHS]. Remove the insertion instruments. Record a one-stage, full arch impression. When placing Zimmer One-Piece Implants into dense bone, thread the implant with delivery/driver tool [ZOPDRS, ZOPDRA] into the osteotomy until slight resistance is encountered and the labial side of prosthetic section is positioned facially. To ensure the driver tool engages properly, apply a steady downward force to the implant as the driver screws it into the osteotomy. STEP 9 – Prepare the Implant for Healing: Utilize a soft-set or temporary cement to retain the Healing Cap or the Provisional Coping. ZIMMER ONE-PIECE IMPLANTATE Beschreibung Das Zimmer® One-Piece Implantat ist ein enossales Zahnimplantat, bei dem die beiden Komponenten, Implantat und Abutment, zu einem einteiligen Produkt, der Implantat-Abutment-Einheit, kombiniert wurden. Der Implantatteil ist ähnlich gestaltet wie das bestehende Zimmer Dental Tapered Screw-Vent® Implantat. Der Abutment-Teil hat ein konturiertes Austrittsprofil, das eine ästhetische Restauration unterstützt. Der Abutment-Teil des Implantats besitzt außerdem einen bereits maschinierten Rand, der in verschiedenen Größen angeboten wird, um die prothetische Versorgung zu vereinfachen. Das Zimmer One-Piece Implantat besteht aus einer Titanlegierung (Ti-6Al-4V) und hat eine mikrotexturierte (MTX) Oberfläche. Indikationen: Zimmer One-Piece 3,0 mm Implantate sind indiziert zur Unterstützung und Retention von festsitzenden Einzelzahn- und Brückenrestaurationen im Bereich der mittleren und seitlichen unteren sowie der seitlichen oberen Schneidezähne teilbezahnter Patienten. Das Zimmer One-Piece 3,0 mm Implantat muss verblockt werden, wenn zwei oder mehr Implantate nebeneinander verwendet werden. Das Zimmer One-Piece 3,0 mm Implantat kann sofort mit einem temporären Zahnersatz versorgt werden, der nicht in funktioneller Okklusion steht. ZIMMER ONE-PIECE SCHRAUBENDREHER Beschreibung Der Zimmer One-Piece Schraubendreher ist ein Edelstahlwerkzeug (SS 465), das zum Eindrehen des Implantats in die Osteotomie verwendet wird. Indikationen: Zum Eindrehen des Implantats in die Osteotomie mit einem Zusatzinstrument wie etwa der GemLock® Vierkant-Ratsche mit Haltefunktion (Retaining Square Ratchet, RSR). Ausgeprägtes Zähneknirschen und -pressen (Bruxismus) und Überbelastung können zum Abutment- und Implantmisserfolg führen. Zu den relativen Kontraindikationen zählen Patienten mit Einschränkungen der psychischen, geistigen und manuellen Funktionen sowie solche, deren psychische, ästhetische und/oder funktionelle Ansprüche nicht erfüllt werden können. Durch Strahlen- und Chemotherapie kann die Haltbarkeit von Implantaten beeinträchtigt werden. Vor einer Implantatbehandlung wird unbedingt die Konsultation eines Allgemeinarztes empfohlen. WARNHINWEIS Die für die Insertion zahnärztlicher Implantate erforderlichen chirurgischen und restaurativen Techniken sind hochspezialisierte und komplexe Verfahren. Zahnärzte sollten entsprechende Schulungskurse besuchen, um sich mit implantologischen Techniken vertraut zu machen. Eine falsche Technik kann zu Knochenverlust, Schädigung des Patienten, Schmerzen und Implantatmisserfolg führen. Zimmer Dental Implantatsysteme sollten nur mit speziell entwickelten Knochenbohrern und Prothetikkomponenten von Zimmer Dental verwendet werden. Implantate, die in ungeeigneter Winkelneigung zu vorhandenen Zähnen, oder mehrere Implantate, die divergierend bzw. konvergierend gesetzt werden, können komplizierte Restaurationen erforderlich machen, die zur Überlastung von Implantaten und ihrem potenziellen Misserfolg (einschließlich Page 1 07/03/14 7 2 8 3 9 4 10 5 11 6 12 Fraktur) führen. Eine gründliche diagnostische Vorarbeit und Verwendung von Röntgenaufnahmen sowie Bohrschablonen werden empfohlen, um eine korrekte Angulation und Schonung bestimmter anatomischer Strukturen, wie etwa Sinusmembranen, Nachbarzähne und kraniofaziale Nerven, zu gewährleisten. Als Sofortbelastung bezeichnet man die Platzierung eines oder mehrerer Implantate in einen unbezahnten oder teilbezahnten Kiefer und das Einsetzen der Prothetikkomponente entweder in oder außer Funktion zum Zeitpunkt der Implantation, wobei die endgültige Restauration innerhalb der ersten zwei Wochen nach Implantation in Funktion gesetzt wird. Zu weiteren Bedingungen, bei denen besondere Vorsicht geboten ist, gehören Behandlungen mit Steroiden und Antikoagulanzien, die den Operationssitus, das umgebende Gewebe oder die Heilungsabläufe des Patienten beeinträchtigen können. Eine Langzeittherapie mit Bisphosphonaten, insbesondere bei einer Chemotherapie, kann das Überleben des Implantats beeinträchtigen. Eine sorgfältige Patientenauswahl einschließlich Konsultation des zuständigen Hausarztes vor Beginn der Implantattherapie wird unbedingt empfohlen. Übermäßige Implantatbeweglichkeit, Knochenabbau oder Infektionen können den Implantatmisserfolg anzeigen. Jedes Implantat mit diesen Anzeichen sollte so schnell wie möglich behandelt oder entfernt werden. Sollte die Entfernung des Implantats notwendig sein, muss das Granulationsgewebe im Wundbereich sorgfältig kürettiert werden, so dass dieser Bereich wie eine atraumatische Extraktion heilen kann. Aufgrund der elektrischen Leitfähigkeit von Metall könnten eine elektrochirurgische Behandlung im Bereich der Implantate und eine intraorale Abutment-Präparation ohne Kühlung zu Gewebeschäden, Schädigung des Patienten und Implantatmisserfolg führen. Die Patienten sollten sich mit ihrem Arzt und dem Bildgebungsspezialisten beraten, bevor sie sich einer Magnetresonanztomographie (MRT) unterziehen. Zimmer Implantate und Abutments wurden nicht auf Sicherheit und Verträglichkeit in einer Magnetresonanz-Umgebung beurteilt. Zimmer Implantate und Abutments wurden nicht auf Erhitzung oder Migration in einer Magnetresonanz-Umgebung getestet. TECHNIKINFORMATIONEN Belastung Folgende Verfahren werden empfohlen, um implantatgetragenen Restaurationen in Bereichen mit hoher Belastung höchstmögliche Belastbarkeit zu verleihen: VORSICHTSMASSNAHMEN Eine korrekte Fallplanung ist entscheidend für den Langzeiterfolg sowohl des Zahnersatzes als auch des Implantats. Überlastung ist eine der wichtigsten Ursachen für einen Implantatmisserfolg. Stellen Sie sicher, dass die Implantatgröße und Abutment-Angulation für die okklusale Belastung geeignet sind. Stark abgewinkelte Abutments sollten vermieden werden. BRUCH Frakturen der Implantat-Abutment-Einheit können auftreten, wenn die applizierten Belastungen die normalen funktionellen Verformungstoleranzen der Implantatkomponenten übersteigen. Zu den möglichen Überlastungsursachen gehören ausgeprägter Knochenverlust (z. B. > 3 mm), unzureichende Anzahl, Längen und/oder Durchmesser der Implantate für eine angemessene Unterstützung der Restauration, zu große Hebellänge bei Freiendsituationen, Abutment-Winkel von mehr als 30 Grad, okklusale Interferenzen, die übermäßige laterale Kräfte verursachen, Parafunktionen des Patienten, wie Zähneknirschen und -pressen (Bruxismus), Zahnverlust oder -veränderung bzw. Funktionsverlust oder -veränderung, mangelhafte Passform des Zahnersatzes und körperliches Trauma. Bei Vorliegen einer der genannten Veränderungen kann eine zusätzliche Behandlung erforderlich sein, um das Frakturrisiko zu reduzieren. LEISTUNGSVERÄNDERUNGEN Der Zahnarzt ist dafür verantwortlich, den Patienten über alle möglichen Kontraindikationen, Nebenwirkungen und Vorsichtsmaßnahmen aufzuklären. Er muss den Patienten außerdem darauf hinweisen, einen fachkundigen Zahnarzt aufzusuchen, wenn Leistungsveränderungen des Implantats auftreten, wie z. B. Lockerung der Suprakonstruktion, Infektionen oder Absonderungen im Implantatbereich, Schmerzen oder sonstige ungewöhnliche Symptome, auf die der Patient nicht hingewiesen wurde. HYGIENE UND ERHALTUNGSTHERAPIE Die langfristige Implantatgesundheit hängt direkt mit der Aufrechterhaltung der Mundhygiene zusammen. Potenzielle Implantatkandidaten sollten über eine angemessene Mundhygiene verfügen, bevor die Implantatbehandlung beginnt. Nach der Implantation muss der Zahnarzt den Patienten über geeignete Instrumente und Techniken zur Pflege und Sicherung des Langzeiterhalts des Implantats bzw. der Implantate informieren. Der Patient sollte auch angewiesen werden, regelmäßige Kontrolltermine für Prophylaxe und Untersuchung wahrzunehmen. BEHANDLUNGSPLANUNG Eine angemessene präoperative Bildgebung und diagnostische Beurteilung sind erforderlich, um die verfügbare Knochenanatomie an den geplanten Implantationsstellen zu beurteilen. Vor der Insertion von Zimmer Zahnimplantaten müssen wichtige anatomische Bezugspunkte, wie etwa Unterkieferkanal, Kieferhöhlen, kraniofaziale Nerven und Nachbarzähne, lokalisiert werden. Die Qualität und Quantität des verbliebenen Knochens muss insbesondere nach einem Implantatmisserfolg und bei der Sofortimplantation in Extraktionsalveolen sorgfältig beurteilt werden. Vor jedem Implantateingriff ist unbedingt eine gründliche klinische Beurteilung durchzuführen. PRÄPARATION DES ABUTMENTS Kleinere Abutment-Modifikationen können intraoral mit Spülung durchgeführt werden. Um die Wärmeentwicklung zu minimieren, wird ein intermittierendes Beschleifen mit mittelgroben Diamantschleifern unter reichlicher Wasserkühlung empfohlen. ALLGEMEINE HINWEISE Die Kontrolle von biomechanischen Belastungen ist der zentrale Faktor für den langfristigen Erfolg des Zahnersatzes. Auch nach der Integration des Implantats können unausgewogene Okklusalkräfte zu einem Versagen des Implantats führen. Implantatpatienten sollten postoperativ auf periimplantären Knochenverlust und Abrasion von Zähnen als Symptome einer okklusalen Überlastung kontrolliert werden. UNERWÜNSCHTE WIRKUNGEN Die folgenden Komplikationen können im Zusammenhang mit einer Implantatinsertion auftreten: Schmerzen, Unwohlsein, Dehiszenz, verzögerte Heilung, Parästhesie, Hyperästhesie, Ödem, Blutung, Hämatom, Infektion, Entzündung, notwendige zusätzliche Operationen oder Entfernung, lokale und generalisierte allergische Reaktion, fehlende Integration, Knochenabbau und Verlust des Implantats. Andere unerwünschte Wirkungen können auch als Folge von iatrogenen Faktoren und Abstoßungsreaktionen auftreten. STERILITÄT Alle Implantate wurden durch Gammastrahlung sterilisiert und sind Einwegprodukte. Implantate nicht resterilisieren. Prothetische und Hilfskomponenten von Zimmer Dental werden steril oder unsteril geliefert. Anhand der Verpackung kann festgestellt werden, ob die Lieferung in sterilem Zustand erfolgte. Unsterile Produkte müssen vor Gebrauch am Patienten sterilisiert werden. Einige Zimmer Dental Produkte, die steril verkauft werden, sind nur zum Einmalgebrauch bestimmt und haben dann eine spezielle Verpackung. EINMALGEBRAUCH Die Wiederverwendung eines Einmalartikels, der in Kontakt mit Blut, Knochen, Gewebe, Körperflüssigkeiten oder anderen kontaminierenden Elementen gekommen ist, kann zu Schädigungen von Patient oder Anwender führen. Zu den möglichen Gefahren in Verbindung mit der Wiederverwendung eines Einmalartikels gehören u. a. mechanisches Versagen und Übertragung infektiösen Materials. LAGERFÄHIGKEIT Das Haltbarkeitsdatum (Jahr und Monat) wird neben dem Stundenglas auf dem Produktetikett angegeben. Vorsicht: Sterile Produkte nicht verwenden, wenn die Verpackung, die die sterile Barriere bildet, einschließlich äußerer Verschlusskappe, Fläschchen oder Tray, beschädigt oder auf irgendeine Weise beeinträchtigt (z. B. gesprungen, gequetscht oder zerrissen) ist. PRODUKTVERPACKUNG Alle Implantate wurden gereinigt, in einem temperatur- und feuchtigkeitskontrollierten Raum in doppelte Fläschchen verpackt und für die sofortige Verwendung sterilisiert. Die auf dem Etikett für die Patientenakte angegebene Chargennummer kann zur Rückverfolgbarkeit auch in die Patientendatei eingegeben werden. Die prothetischen und chirurgischen Komponenten wurden gereinigt, in einem temperatur- und feuchtigkeitskontrollierten Raum verpackt und für die bequeme und sofortige Verwendung sterilisiert (falls zutreffend, siehe Produktetikett). Sie werden in einem versiegelten Kunststofffläschchen, einer mit Tyvek®** versiegelten Blisterpackung oder einem versiegelten Kunststoffbeutel geliefert. Das Etikett auf der äußeren Fläschchenverpackung enthält eine Chargennummer, die in die Patientendatei eingetragen werden sollte, um die vollständige Nachverfolgbarkeit des Produkts zu gewährleisten. Das Implantat-Verpackungssystem enthält eine Farbkodierung auf der Kappe und dem Etikett für die Patientenakte, die die Implantatplattform anzeigt. INFORMATIONEN ZU REINIGUNG/STERILISATION Die Desinfektions- und Sterilisationsverfahren müssen den OSHA- oder örtlichen Richtlinien für hämatogene Erreger entsprechen. Alle Teile in ein Ultraschallreinigungsgerät mit enzymatischem Reinigungsmittel legen, das mit Leitungswasser gemäß Anweisungen des Herstellers verdünnt ist. 10 Minuten beschallen. Lokale oder nationale Spezifikationen sollten eingehalten werden, wenn die Anforderungen an eine Dampfsterilisation strenger oder konservativer als die in der Tabelle aufgeführten sind. The implant section is designed similarly to the existing Zimmer Dental Tapered Screw-Vent® implant. The abutment section has a contoured emergence profile to aid in esthetic restoration. The abutment portion of the implant features a pre-machined margin, offered in different sizes, to facilitate the restoration process. The Zimmer One-Piece implant is composed of titanium alloy (Ti-6Al-4V) and has a microtextured (MTX) surface. Kits, Trays, and Blocks - Remove all parts and insert from the tray. Clean parts per the above instructions. Thoroughly rinse the kits under running tap water to remove all visible soil. Use a soft bristle brush to clean the kits until all visible soil is removed. A syringe or pipe cleaner may be used to aid in the rinsing. Assure that all hard to reach areas are accessed. After the rinsing, prepare the enzymatic detergent following the manufacturer’s specifications. Fully immerse the kit in the prepared detergent and allow the kit to soak in the detergent for a minimum of one minute. Following the soak use a damp cloth and/ or a soft bristle brush to wipe and remove any excess debris/soil from each component. A syringe or a pipe cleaner may be used to aid in the cleaning. Rinse the kits with lukewarm tap water to eliminate all residual enzymes and detergent, thoroughly for a minimum of three minutes. Dry the components. Reassemble the contents of the kit and follow the guidelines for sterilization. Note: This procedure should be performed after an instrument used during a surgery comes into contact with the surgical tray or prosthetic tray. SCHRITT 7 – Implantat in die Osteotomie einbringen: Tragen Sie das Implantat mit dem passenden Einbring-/Schraubendreher-Werkzeug [ZOPDRS, ZOPDRA] zur Insertionsstelle und setzen Sie es direkt in die präparierte Osteotomie. Une fois les implants posés, le chirurgien doit informer le patient des instruments et techniques d’entretien à utiliser pour assurer leur durabilité. Le patient doit aussi être informé qu’il doit se soumettre à une visite périodique chez son dentiste pour des raisons de prophylaxie et d’évaluation. PLANIFICATION DU TRAITEMENT Une évaluation de diagnostic et un examen par imagerie adéquats préalables sont nécessaires pour déterminer l’anatomie osseuse disponible sur les sites d’implantation prévus. CONSIDÉRATIONS GÉNÉRALES La maîtrise des contraintes biomécaniques constitue le facteur clé pour une réussite à long terme de la prothèse. Même à l’issue d’une intégration réussie, l’implant n’est pas à l’abri d’un échec, notamment en raison d’un déséquilibre des forces occlusales. Le suivi postopératoire du patient doit comprendre une surveillance des signes de surcharge occlusale : perte osseuse péri-implantaire ou usure des dents. IMPLANTS ZIMMER ONE-PIECE Description Zimmer® One-Piece est un implant dentaire endo-osseux conçu avec l’implant et les composants du pilier combinés en un seul dispositif, désigné sous le nom d’ensemble implant-pilier. La partie implant est conçue de manière similaire à l’implant Zimmer Dental Tapered Screw-Vent® existant. La partie pilier présente un profil d’émergence prothétique pour faciliter la restauration esthétique et dispose d’une limite cervicale pré-usinée, disponible en différentes tailles, pour faciliter le processus de restauration. L’implant Zimmer One-Piece est composé d’un alliage de titane (Ti-6Al-4V) et présente une surface microtexturée (MTX). Indications : Les implants Zimmer One-Piece de 3,0 mm sont indiqués pour le soutien et la rétention de prothèses unitaires scellées et de prothèses partielles scellées dans les zones partiellement édentées des incisives latérales et centrales (mandibule) et latérales (maxillaire). L’implant Zimmer One-Piece de 3,0 mm doit être solidarisé si plusieurs implants sont utilisés de façon adjacente. L’implant Zimmer One-Piece de 3,0 mm peut être immédiatement restauré avec une prothèse provisoire sous réserve que celle-ci ne crée pas d’occlusion fonctionnelle. INSTRUMENT DE POSE ZIMMER ONE-PIECE Description L’instrument de pose Zimmer One-Piece est un instrument en acier inoxydable (SS 465) utilisé pour visser l’implant dans l’ostéotomie Indications : Utilisé pour fileter l’implant dans l’ostéotomie à l’aide d’un accessoire tel que la clé à cliquet carrée de blocage GemLock® (RSR). RÉPLIQUES CHIRURGICALES ZIMMER ONE-PIECE Description Les répliques Zimmer One-Piece présentent une forme et une conception similaires à celles du pilier et reproduisent la géométrie exacte de la partie prothétique de l’implant. Elles sont placées dans l’ostéotomie pour vérifier la position et l’angulation. La réplique Zimmer One-Piece Try-in est en alliage de titane (Ti-6Al-4V). Indications : Les répliques Zimmer One-Piece sont placées dans l’ostéotomie pour vérifier la position et l’angulation. CONTRE-INDICATIONS Les implants Zimmer One-Piece de 3,0 mm sont contre-indiqués pour les régions molaires. Les implants Zimmer One-Piece ne doivent pas être mis en place si la largeur et la hauteur de l’os alvéolaire sont insuffisantes pour envelopper l’implant (un minimum de 1 mm de circonférence et de 2 mm dans la partie apicale). Les implants placés dans le maxillaire ne doivent pas perforer les sinus. Une faible densité osseuse, une mauvaise hygiène buccale du patient, une forte consommation de tabac ou l’habitude de mâcher du tabac et des maladies généralisées (diabètes, etc.) peuvent compromettre l’ostéo-intégration, provoquer une perte du support osseux et donc, conduire à l’échec de l’implant. Un bruxisme grave, les crispations ou la surcharge peuvent provoquer l’échec des piliers et des implants. Parmi les contre-indications associées, mentionnons les patients souffrant de troubles psychologiques, de troubles mentaux et d’une dextérité manuelle diminuée, et ceux dont les exigences psychologiques, esthétiques et/ou fonctionnelles ne peuvent pas être satisfaites. Les rayonnements et la chimiothérapie peuvent avoir un impact sur la stabilité de l’implant. Il est fortement recommandé de consulter un médecin avant tout traitement par implants. AVERTISSEMENT Les techniques chirurgicales et de restauration utilisées pour la pose d’implants dentaires font appel à des procédures complexes et hautement spécialisées. Les praticiens doivent suivre des cours afin de se familiariser avec les techniques reconnues en implantologie orale. Des techniques inappropriées peuvent entraîner des pertes osseuses, des lésions au patient, des douleurs ainsi que l’échec de l’implant. Les systèmes d’implants Zimmer Dental ne peuvent être utilisés qu’avec les forets et composants prothétiques spéciaux Zimmer Dental. Les implants formant un angle important par rapport à la denture existante, ou les implants multiples convergents ou divergents, peuvent provoquer des surcharges de l’implant en cas de restaurations complexes, et éventuellement entraîner l’échec de l’implant (fractures comprises). Une recherche diagnostique complète, des radiographies et l’utilisation de gabarits chirurgicaux sont recommandées pour garantir une angulation correcte et éviter certaines régions anatomiques telles que les sinus, les dents adjacentes et les nerfs du crâne et de la face. Parmi les autres avertissements associés, mentionnons les traitements à base de stéroïdes et d’anticoagulants qui sont susceptibles d’affecter le site chirurgical, les tissus environnants ou la cicatrisation du patient. L’utilisation à long terme de médicaments à base de bisphosphonate, en particulier en chimiothérapie, peut menacer la stabilité des implants. Il est fortement recommandé, avant tout traitement par implants, d’étudier soigneusement le cas, notamment en consultant le médecin du patient. Une mobilité excessive, une perte osseuse ou une infection peuvent indiquer l’échec de l’implant. Tout implant qui provoque de tels symptômes doit être traité ou déposé dès que possible. Si la dépose s’avère nécessaire, il convient de cureter les tissus mous au niveau du site implantaire et de laisser cicatriser comme s’il s’agissait d’une extraction classique. En raison de la conductivité des métaux, l’électrochirurgie dans la zone des implants et la préparation intra-buccale sans irrigation des piliers peuvent endommager les tissus, provoquer des lésions au patient et l’échec des implants. Les patients devront être informés de consulter leur médecin et leur radiologue avant toute procédure d’imagerie par résonance magnétique (IRM). L’innocuité et la compatibilité des implants et piliers Zimmer n’ont pas été évaluées dans un environnement de résonance magnétique (RM). L’élévation de la température ou la migration des implants et piliers Zimmer n’ont pas été testées dans un environnement de RM. PRÉCAUTIONS Un plan de traitement soigneusement étudié constitue le facteur clé pour une réussite à long terme de la prothèse et de l’implant. La surcharge est l’un des principaux facteurs d’échec de l’implant. La taille de l’implant et l’angulation du pilier doivent être appropriées pour la charge occlusale. Il est conseillé d’éviter les piliers à forte angulation. FRACTURE Les fractures de l’ensemble implant-pilier sont généralement imputables à des charges supérieures à celles pour lesquelles les implants et les composants sont conçus. Les causes de surcharges potentielles sont notamment les suivantes : perte osseuse importante (par ex., supérieure à 3 mm), nombre, longueur et/ou diamètre d’implant inappropriés pour la restauration à supporter, longueur de porte-à-faux excessive, angles de pilier supérieurs à 30 degrés, interférences occlusales provoquant des forces latérales excessives, parafonctions du patient (par ex., bruxisme, crispations), perte ou modification de la denture ou de la fonction dentaire, adaptation incorrecte de la prothèse et traumatismes physiques. Un traitement supplémentaire peut être nécessaire dans ces conditions, pour réduire le risque de fracture. Gli impianti sono sospesi su un dispositivo per il trasporto, in modo da consentirne il trasferimento nel sito chirurgico preparato senza rischio di contaminazione da contatto. Sia l’impianto che la confezione a fiala interna sono completamente sterili. L’etichetta sulla confezione a fiala esterna di ciascun impianto indica un numero di lotto che deve essere registrato sulla cartella del paziente per assicurare la completa tracciabilità dell’impianto. L’etichetta per la carta del paziente fornita contenente il numero di lotto può anche essere posta nella cartella del paziente per la tracciabilità. Le componenti protesiche e chirurgiche sono state pulite, confezionate in una stanza controllata dal punto di vista ambientale, e sterilizzate (se applicabile, fare riferimento all’etichetta del prodotto) per comodità e uso immediato. Sono forniti in un flacone di plastica sigillato, in un blister sigillato Tyvek®** , o in una polybag sigillata. L’etichetta sulla confezione a fiala esterna di ciascun impianto indica un numero di lotto che deve essere registrato sulla cartella del paziente per assicurare la completa tracciabilità del prodotto. Il sistema di confezionamento dell’impianto contiene un codice colore sul tappo e un’etichetta carta del paziente per indicare la piattaforma dell’impianto. Fare riferimento alla Carta di riferimento per il codice dei colori. INFORMAZIONI SU DISINFEZIONE E STERILIZZAZIONE Le procedure di disinfezione e di sterilizzazione devono rispettare le norme OSHA o le normative localmente in vigore per la protezione da microrganismi patogeni trasmessi attraverso il sangue. Gli impianti contaminati clinicamente non devono mai essere puliti, né risterilizzati in nessuna circostanza. PULIZIA Attenersi alle linee guida riportate di seguito per la pulizia dei prodotti: Frese, Strumenti e Componenti - Smontare i componenti in più pezzi, se pertinente. Risciacquare con acqua fredda o tiepida per due minuti e mezzo. Per le frese riutilizzabili usare il filo per pulizia Zimmer per rimuovere tutti i detriti dal canale di irrigazione. Utilizzando un ago di calibro 25 far passare l’acqua nel lume della fresa per rimuovere i detriti residui. Mettere tutte le parti in una vasca a ultrasuoni con un detergente enzimatico diluito con acqua corrente, seguendo le direttive del produttore. Eseguire la sonicazione per 10 minuti. Risciacquare con acqua corrente per tre minuti. Kit, Vassoi e Blocchi - Rimuovere tutte le parti e l’inserto dal vassoio. Pulire i componenti seguendo le istruzioni elencate sopra. Risciacquare bene i kit con acqua corrente per rimuovere completamente lo sporco visibile. Utilizzare una spazzola a setole morbide per pulire i kit fino a quando ogni traccia di sporco visibile sarà stata rimossa. Durante il risciacquo può essere utilizzata una siringa o uno scovolino. Assicurarsi di avere raggiunto anche i punti più difficili. Dopo il risciacquo, preparare il detergente enzimatico seguendo le indicazioni del produttore. Immergere il kit nel detergente così preparato e lasciarlo immerso per almeno 1 minuto. Successivamente, utilizzare un panno umido e/o una spazzola morbida per strofinare e rimuovere ogni residuo di detriti /sporco da ogni componente. Durante la pulizia si può utilizzare una siringa o uno scovolino. Per eliminare gli enzimi residui e il detergente, risciacquare abbondantemente il kit con acqua tiepida, per almeno tre minuti. Asciugare i componenti del kit. Riassemblare il contenuto del kit e seguire le linee-guida per la sterilizzazione. Nota: seguire questa procedura ogni volta che uno strumento utilizzato in chirurgia entra in contatto con il vassoio chirurgico o con il vassoio protesico. Parti poste singolarmente nella sacca o nel kit Parti poste singolarmente nella sacca STERILIZZAZIONE Le singole parti dovrebbero essere poste in un’appropriata sacca per sterilizzazione in autoclave o a calore secco prima di procedere con la sterilizzazione. Quando si sterilizzano componenti all’interno di un kit, i componenti devono essere inseriti nelle posizioni appropriate e il kit deve essere rivestito con carta da imballaggio per sterilizzazione. Sono richiesti i seguenti parametri di sterilizzazione (metodo, tempo e temperatura) per raggiungere un livello assicurato di sterilizzazione di 10-6 (SAL). Seguire le specifiche locali o nazionali laddove i requisiti per la sterilizzazione a vapore siano più severi o più prudenti rispetto a quelli elencati nella tabella. Il superamento dei parametri di sterilizzazione indicati può danneggiare i componenti di plastica. Verificare la calibratura della propria unità per assicurarsi che vengano raggiunte le temperature raccomandate. Per assicurarsi che l’autoclave funzioni correttamente, devono essere utilizzati periodicamente degli indicatori biologici. La sterilizzazione con Chemiclave NON è raccomandata per i prodotti Zimmer Dental. CARICO IMMEDIATO: Il carico immediato è raccomandato in presenza di una buona stabilità primaria e di un carico occlusale appropriato. Il carico immediato è definito come la sistemazione di uno o più impianti in un arco parzialmente o completamente edentulo e sistemazione della porzione di restauro protesico sia in funzione o fuori funzione al momento della sistemazione dell’impianto e restaurazione protesica finale in funzione entro le prime due settimane dalla sistemazione dell’impianto. INFORMAZIONI TECNICHE Carico Per massimizzare la capacità di sopportazione di carico dell’impianto in aree di alto stress, si raccomandano le seguenti procedure: • Considerare l’uso di impianti multipli splintati per il restauro protesico. Gli impianti Zimmer One-Piece 3,0mm sono controindicati per le regioni molari e devono essere splintati se ne vengono usati due o più adiacenti tra loro. • Se la dimensione buccolinguale della cresta lo consente, considerare di utilizzare un impianto di diametro più largo per il supporto del restauro protesico. • Posizionare un adeguato numero di impianti per ottenere una distribuzione ottimale del carico. PROCEDURE DI INSERIMENTO FASE 1 – Annotare i dati relativi a ciascun impianto applicato: Annotare il numero della parte, il numero di catalogo e la descrizione del prodotto nella cartella del paziente, o attaccare l’etichetta carta del paziente inclusa nella confezione sulla cartella del paziente. FASE 2 – Preparare l’osteotomia Iniziare l’osteotomia usando la fresa pilota. Posizionare il modello chirurgico nell’osteotomia per verificare posizione e angolazione. Completare l’osteotomia usando le frese e la sequenza di fresatura appropriate secondo il manuale di tecnica chirurgica. Per l’osso denso il maschiatore della dimensione appropriata può essere usato per filettare l’osteotomia per il diametro desiderato dell’impianto. FASE 3 – Aprire la fiala esterna sterile: Girare e tirare per svitare il tappo codificato con colore, rompendo la barriera sterile. STEP 4 – Rimuovere la fiala interna: Svuotare il contenuto della fiala interna sterile che contiene l’impianto su un campo sterile. FASE 5 – Selezionare la strumentazione appropriata: • Impianti diritti da 3,0mm Zimmer One-Piece: Rimuovere il driver dell’impianto [ZOPDRS] con la scanalatura colorata di blu dal vassoio chirurgico. Connettere il driver a una chiave in acciaio inossidabile [RSR] o al manico del cacciavite in acciaio inossidabile [SSHS], assicurandosi che il driver sia adeguatamente trattenuto nello strumento. • Impianti angolati da 3,0mm Zimmer One-Piece Rimuovere il driver dell’impianto [ZOPDRA] con la scanalatura colorata di blu che è etichettata“3.0A” dal vassoio chirurgico. Connettere il driver a una chiave in acciaio inossidabile [RSR] o al manico del cacciavite in acciaio inossidabile [SSHS], assicurandosi che il driver sia adeguatamente trattenuto nello strumento. FASE 6 – Rimuovere l’impianto dalla fiala interna: Per rimuovere gli impianti da 3,0mm Zimmer One-Piece, allineare la linea nera verticale sul driver dell’impianto con la linea verticale sulla fiala interna e poi posizionare il driver sulla porzione di moncone dell’impianto, assicurandosi di premere sull’impianto con una pressione ferma. Tirare fuori dalla fiala con attenzione l’impianto assicurandosi che l’impianto sia catturato all’interno del driver dell’impianto. FASE 7 – Collocare l’impianto nell’osteotomia: Trasportare l’impianto con gli strumenti di posizionamento/driver appropriati [ZOPDRS, ZOPDRA] fino al sito recettore e posizionarlo nella osteotomia preparata. FASE 8 – Sistemazione completa con gli strumenti appropriati: Ruotare l’impianto in posizione con strumenti di posizionamento/driver [ZOPDRS, ZOPDRA] e chiave in acciaio inossidabile [RSR] o manico del cacciavite in acciaio inossidabile [SSHS]. Rimuovere gli strumenti di inserimento. Prendere un’impronta di tutto l’arco in una volta. Quando si posizionano impianti Zimmer One-Piece in ossa dense, avvitare l’impianto con strumenti di posizionamento/driver [ZOPDRS, ZOPDRA] nell’osteotomia fino a che si incontra resistenza e la faccia labiale della sezione protesica sia posizionata in posizione facciale. Per assicurarsi che lo strumento driver sia impegnato in maniera corretta, applicare una forza ferma verso il basso sull’impianto mentre il driver lo avvita nell’osteotomia. FASE 9 – Preparare l’impianto per la guarigione: Utilizzare un soft-set o un cemento temporaneo per tenere in posizione il cappuccio di guarigione o la coppetta provvisioria. impianto Zimmer One-Piece. INSTRUCCIONES DE USO DEL SISTEMA DE IMPLANTES ZIMMER® ONE-PIECE Antes de usar un producto Zimmer Dental, el cirujano o dentista responsable de la intervención debe estudiar cuidadosamente las indicaciones, contraindicaciones, recomendaciones, advertencias e instrucciones, así como toda la restante información específica del producto (descripción técnica del producto, descripción de la técnica quirúrgica y de restauración, hoja del catálogo, etc.) y seguir fielmente todas ellas. PRUEBAS QUIRÚRGICAS DE ZIMMER ONE-PIECE Descripción Las pruebas de Zimmer One-Piece tienen una forma y un diseño parecidos a los del pilar y simulan la geometría exacta de la parte protésica del implante. Se sitúan en la osteotomía para comprobar la posición y la angulación. La prueba Zimmer One-Piece está compuesta de aleación de titanio (Ti-6Al-4V). Indicaciones: Las pruebas Zimmer One-Piece se sitúan en la osteotomía para comprobar la posición y la angulación. CONTRAINDICACIONES Los implantes Zimmer One-Piece de 3,0 mm están contraindicados para las regiones molares. Los implantes Zimmer One-Piece no se deben colocar si la anchura y la altura de hueso alveolar no son suficientes para rodear el implante (un mínimo de 1 mm de circunferencia y 2 mm de ápice). Los implantes colocados en el maxilar no deben perforar el seno. La mala calidad del hueso, la insuficiente higiene oral del paciente, el abuso del tabaco o el consumo de tabaco de mascar, y las enfermedades generalizadas (diabetes, etc.) pueden contribuir a una falta de osteointegración, falta de estructura ósea de apoyo y el consiguiente fracaso del implante. Un bruxismo grave o una sobrecarga pueden provocar el fracaso de los pilares y los implantes. Entre otros pacientes contraindicados se encuentran los pacientes con deterioro psicológico, mental y de la habilidad manual, y aquellos que no pueden satisfacer sus necesidades psicológicas, estéticas o funcionales. La exposición a radiación y la quimioterapia pueden afectar a la supervivencia del implante. Es imprescindible consultar con el especialista antes del tratamiento con implante. ADVERTENCIA Las técnicas quirúrgicas y de restauración que se requieren para colocar implantes dentales son procedimientos altamente especializados y complejos. Los dentistas deben asistir a cursos prácticos para familiarizarse con las técnicas de implantología. Una técnica inadecuada puede causar la pérdida de hueso, lesiones al paciente, dolor y el fracaso del implante. Los sistemas de implantes Zimmer Dental están destinados para utilizarse solo con las fresas y prótesis especialmente diseñadas por Zimmer Dental. Los implantes colocados en ángulos inadecuados en relación a la dentición existente o los implantes múltiples colocados de manera convergente o divergente pueden dar como resultado restauraciones complejas que pueden suponer la sobrecarga de dichos implantes, provocando posiblemente el fracaso de los mismos (incluida la rotura). Se recomienda elaborar un diagnóstico concienzudo y utilizar radiografías y férulas quirúrgicas como ayuda para asegurar una angulación correcta, y evitar determinadas características anatómicas como membranas del seno, dientes adyacentes y nervios craneofaciales. Otras advertencias relativas son los tratamientos con esteroides y anticoagulantes, que pueden afectar a la zona quirúrgica, al tejido circundante o al proceso de cicatrización del paciente. Los tratamientos prolongados con bifosfonatos, especialmente con quimioterapia, pueden afectar a la supervivencia del implante. La cuidadosa selección de pacientes, incluida la consulta con el especialista que les trata, es un requisito imprescindible antes de iniciar un tratamiento con implantes. La movilidad excesiva, la pérdida de hueso o la infección pueden ser indicios del fracaso del implante. Cualquier implante con signos de fracaso debe ser tratado o extraído lo antes posible. PRECAUCIONES La planificación adecuada del caso es esencial para el éxito a largo plazo tanto de la prótesis como del implante. La sobrecarga es uno de los elementos clave que contribuyen al fracaso de un implante. Asegúrese de que el tamaño del implante y la angulación del pilar sean los adecuados para la carga oclusal. Deben evitarse los pilares muy angulados. HIGIENE Y MANTENIMIENTO La salud a largo plazo de los implantes depende directamente de la observación de una higiene oral adecuada. Los candidatos potenciales a implantes deben establecer un régimen de higiene oral adecuado antes del tratamiento con implantes. Después de la colocación del implante, el médico debe dar instrucciones al paciente sobre los instrumentos y técnicas adecuadas para asegurar el mantenimiento a largo plazo de los implantes. Se debe instar al paciente a observar rutinas programadas de profilaxis y acudir a consultas de evaluación. Es indispensable una evaluación clínica exhaustiva antes de proceder a la cirugía implantológica. PREPARACIÓN DE PILARES Se pueden realizar pequeñas modificaciones de los pilares intraoralmente con irrigación. Para reducir la cantidad de calor generado, se recomienda utilizar fresas de diamante de grano mediano con abundante irrigación aplicadas de manera intermitente. CONSIDERACIONES GENERALES El control de las fuerzas biomecánicas es el factor clave para el éxito prolongado de la prótesis. Incluso después de la integración del implante, los desequilibrios en la fuerza oclusal pueden ocasionar el fracaso del implante. Debe realizarse un estrecho seguimiento de los pacientes de implantes tras la operación para detectar pérdida de hueso periimplantario y desgaste de dientes como signos de sobrecarga oclusal. EFECTOS ADVERSOS Con relación a la colocación de implantes, pueden producirse las siguientes complicaciones: dolor, incomodidad, dehiscencia de la herida, retraso en la cicatrización, parestesia, hiperestesia, edema, hemorragia, hematoma, infección, inflamación, necesidad de cirugía adicional o extracción, reacción alérgica local y generalizada, falta de osteointegración, pérdida de hueso y pérdida del implante. También pueden producirse otros efectos adversos como resultado de factores iatrogénicos y respuestas del receptor. ESTERILIDAD Todos los implantes están esterilizados con radiación gamma y diseñados para un solo uso. No vuelva a esterilizar los implantes. Los componentes protésicos y auxiliares de Zimmer se venden esterilizados o sin esterilizar. Consulte el envase específico para verificar la esterilidad. UN SOLO USO La reutilización de un dispositivo de un solo uso que ha estado en contacto con sangre, hueso, tejido, líquidos corporales u otros contaminantes puede ocasionar lesiones al paciente o al usuario. Entre los posibles riesgos de la reutilización de un dispositivo de un solo uso se pueden citar, entre otros, el fracaso mecánico y la transmisión de agentes infecciosos. Los implantes clínicamente contaminados no deben limpiarse ni volver a esterilizarse bajo ninguna circunstancia. LIMPIEZA Siga las siguientes pautas para limpiar los productos: Fresas, instrumentos y componentes. Si procede, desmonte los componentes de varias piezas. Aclare con agua corriente tibia durante dos minutos y medio. Para las fresas reutilizables, emplee el alambre de limpieza de Zimmer para eliminar cualquier suciedad que pueda haber en el conducto de irrigación. Con una aguja del calibre 25, lave la luz de la fresa con agua para eliminar cualquier suciedad que pueda haber. INFORMACIÓN TÉCNICA CARGA Para conseguir la mejor capacidad de carga en las restauraciones de implantes en áreas sometidas a mayor tensión, se recomienda realizar el siguiente procedimiento: • Tenga en cuenta la posibilidad de utilizar varios implantes ferulizados para soportar la restauración. Los implantes Zimmer One-Piece de 3,0 mm están contraindicados para las regiones molares y se deben ferulizar si se utilizan dos o más implantes situados uno junto a otro. • Si el tamaño vestíbulo-lingual de la cresta lo permite, considere el uso de un implante de mayor diámetro para soportar la restauración. • Coloque una cantidad apropiada de implantes para conseguir una distribución óptima de la carga. PROCEDIMIENTOS DE INSERCIÓN PASO 1 – Documente los datos de cada implante colocado: Registre el número de pieza, el número de registro y la descripción del producto en el informe del paciente, o bien adjunte la etiqueta del informe del paciente incluida en el envase al informe del paciente. Conecte el destornillador a una carraca de acero inoxidable [RSR] o a un mango del destornillador de acero inoxidable [SSHS], asegurándose de que el destornillador quede correctamente retenido dentro del instrumento. • Implantes Zimmer One-Piece en ángulo de 3,0 mm: Retire el destornillador del implante [ZOPDRA] con la ranura de color azul que tiene la inscripción“3.0A” de la bandeja quirúrgica. FORHOLDSREGLER Det er vigtigt i forvejen at planlægge hele behandlingsforløbet for at kunne opnå et langvarigt, godt resultat for både protese og implantat. Overbelastning er en af hovedårsagerne til implantatsvigt. Det skal sikres, at implantatstørrelse og abutmentvinkel er hensigtsmæssig for den okklusale belastning. Stærkt vinklede abutments bør undgås. BRUD Frakturer på implant-abutmentsamlingen kan forekomme, når belastningerne overstiger implantatkomponenternes normale funktionstolerance. Potentielle overbelastningsforhold kan være et resultat af væsentligt knogletab (f.eks. >3 mm), utilstrækkelige implantatantal, -længder og/eller -diametre til passende støtte af en restaurering, for lang ekstension, abutmentvinkler på mere end 30 grader, okklusale interferenser, som forårsager for kraftige laterale kræfter, patientens parafunktion (f.eks. bruksisme under sammenbidning), tab eller ændringer i tandstilling eller funktion, dårlig protesetilpasning og fysisk traume. ÆNDRINGER I IMPLANTATETS FUNKTIONALITET Det er klinikerens ansvar at instruere patienten om alle relevante kontraindikationer, bivirkninger og forsigtighedsforanstaltninger samt informere om, at der omgående bør søges hjælp hos en tandlæge, hvis der skulle opstå ændringer i implantatets funktionalitet (fx at protesen løsner sig, der opstår infektion eller pus ved implantatet, smerte eller andre usædvanlige symptomer, som patienten ikke burde kunne forvente). HYGIEJNE OG VEDLIGEHOLDELSE Langvarig holdbarhed af implantatet er direkte relateret til opretholdelse af god oral hygiejne. Potentielle implantatbærere bør etablere en tilstrækkelig mundhygiejnerutine, før implantatet indopereres. Efter at implantatet er indopereret, bør klinikeren instruere patienten i anvendelsen af de rigtige redskaber og teknikker for at sikre, at implantaterne får så lang en levetid som muligt. Patienten skal også vejledes i at overholde planlagte profylakse- og evalueringsbesøg. PLANLÆGNING AF BEHANDLING Passende præoperativ billeddiagnostik og diagnostisk evaluering er nødvendig for at fastlægge knogleanatomien på potentielle implantatsteder. Restknoglens kvalitet og kvantitet skal evalueres nøje, især efter et implantatsvigt og ved øjeblikkelig indsættelse af implantater i ekstraktionsalveoler. Grundig klinisk vurdering er absolut påkrævet før al implantatkirurgi. ABUTMENTPRÆPARATION Mindre abutmentmodifikationer kan udføres intraoralt med skylning. For at mindske risikoen for varmedannelse anbefales intermitterende boring med brug af medium grove diamantbor under rigelig vandafkøling. Generelle overvejelser Kontrol af biomekanisk stress er den vigtigste faktor for langvarig proteseholdbarhed. Selv efter implantatets integration kan ubalancerede okklusale kræfter føre til implantatsvigt. Implantatpatienter skal monitoreres postoperativt for tegn på knogletab rundt om implantatet og tandslid som tegn på okklusal overbelastning. BIVIRKNINGER Følgende komplikationer kan opstå i forhold til implantatets placering: smerte, ubehag, sårruptur, forsinket heling, paræstesi, hyperæstesi, ødem, blødning, hæmatom, infektion, inflammation, behov for yderligere kirurgi eller fjernelse, lokal og generel allergisk reaktion, manglende integration, knogletab og tab af implantat. Der kan opstå andre bivirkninger som følge af iatrogene faktorer og værtsrespons. O STERILITET Alle implantater er blevet steriliseret med gammabestråling og er kun til engangsbrug. Implantaterne må ikke resteriliseres. Protese- og tilhørende komponenter fra Zimmer Dental sælges sterile og usterile. Der henvises til den specifikke emballage vedrørende verifikation af sterilitet. Steriliser usterile produkter inden anvendelse på patienter. Nogle Zimmer Dentals produkter, der sælges sterile, er kun til engangsbrug, se de specifikke emballager til engangsprodukterne. ENGANGSBRUG Genbrug af en komponent til engangsbrug, der har været i kontakt med blod, knogle, væv eller andre legemsvæsker, kan føre til skade på patient eller bruger. Mulige risici, der er forbundet med genbrug af en komponent til engangsbrug, omfatter, men er ikke begrænset til mekanisk fejl og smitteoverførsel. HOLDBARHED Produktets udløbsdato er angivet ved timeglassymbolet på produktmærkaten efterfulgt af udløbsår og -måned. Forsigtig: Anvend ikke sterilt udstyr, hvis emballagen, herunder yderhætte, hætteglas og bakke, på nogen måde er blevet beskadiget eller kompromitteret på nogen måde (dvs. er revnet, knust eller revet over). PRODUKTEMBALLAGE Alle implantater er rengjort og emballeret i dobbelte ampuller i et miljøkontrolleret lokale og steriliseret mhp. nem og umiddelbar brug. Implantaterne er ophængt på en transportkomponent, så de kan overføres til det forberedte, kirurgiske felt uden kontaminationsrisiko ved kontakt. Både implantatet og den indvendige emballageampul er sterile. Etiketten på den udvendige emballageampul med hvert implantat er forsynet med et lotnummer, der skal noteres i patientens journal for at sikre produktets sporbarhed. Det medfølgende tanddiagrammærkat, der indeholder partinummeret, kan også sættes ind i patientjournaler for sporbarhed. Protese- og kirurgiske komponenter er rengjorte og emballerede i et miljøkontrolleret lokale og steriliseret (hvis relevant, se produktmærkatet) mhp. nem og umiddelbar brug. De leveres i et forseglet plastikhætteglas, Tyvek®** forseglede blisterpakninger eller forseglede plastposer. Mærkatet på hætteglassets ydre emballage er forsynet med et partinummer, som skal noteres i patientens journal for at sikre produktets sporbarhed. Implantatets emballeringssystem har en farvekodet hætte og tanddiagrammærkat til at angive implantatplatformen. Se farveskemaet for farvekodning. INFORMATION OM RENGØRING/STERILISERING Desinficerings- og steriliseringsprocedurer bør være i overensstemmelse med OSHA eller lokale retningslinjer for blodbårne patogener. Klinisk kontaminerede implantater må ikke renses og resteriliseres under nogen omstændigheder. RENGØRING Anvend følgende retningslinjer for rengøring af produkterne: Bor, instrumenter og komponenter - Skil de komponenter ad, der består af flere dele. Skyl med koldt/lunkent vand i to et halvt minut. Brug Zimmer rengøringswire til at fjerne evt. debris fra irrigationskanalen på genanvendelige bor. Brug en 25-gauge kanyle og skyl borets lumen med vand for at fjerne eventuelt resterende snavs. Læg alle dele i en ultralydsrenser med et enzymatisk rengøringsmiddel fortyndet med postevand ifølge producentens vejledning. Brug ultralyd i 10 minutter. Skyl med postevand i tre minutter. Kit, bakker og blokke - Tag alle dele og indlæg ud af bakken. Rengør delene ifølge instrukserne ovenfor. Skyl kittene grundigt under løbende vand for at fjerne alt synligt snavs. Anvend en blød børste til at rengøre sættene med, indtil al synligt snavs er fjernet. Der kan anvendes en sprøjte eller piberenser som hjælp ved skylningen. Sørg for, at alle svært tilgængelige områder rengøres. Efter skylningen klargøres det enzymatiske rensemiddel ifølge fabrikantens specifikationer. Læg kittet helt ned i det klargjorte rensemiddel, og lad det ligge i blød i rensemidlet i minimum et minut. Efter iblødlægningen anvendes en fugtig klud og/eller en blød børste til aftørring og til at fjerne alt overskydende debris/snavs fra hver komponent. Der kan anvendes en sprøjte eller piberenser som hjælp til rengøringen. Skyl kittene grundigt med lunkent vand fra hanen for at fjerne alle rester af enzym og rensemiddel i minimum tre minutter. Tør alle delene. Saml kittets indhold igen, og følg retningslinjerne for sterilisering. Bemærk: Denne procedure bør udføres, når et instrument brugt under et kirurgisk indgreb kommer i kontakt med operations- eller protesebakken. STERILISERING De enkelte dele skal anbringes i en passende autoklave eller en pose til tør varme inden sterilisering. Hvis delene steriliseres sammen i et kit, skal delene lægges på deres korrekte pladser i kittet, og kittet skal pakkes ind i indpakningsmateriale til sterilisering. Følgende steriliseringsparametre (metode, tid og temperatur) er nødvendige for at opnå et 10-6 sterilitetssikringsniveau (SAL). Lokale eller nationale specifikationer skal følges, hvor kravene til dampsterilisering er strengere eller mere konservative end de krav, der er angivet i skemaet. Hvis disse steriliseringsparametre overskrides, kan det resultere i beskadigelse af plastikkomponenter. Bekræft kalibreringen af apparatet for at sikre, at de anbefalede temperaturer ikke overstiges. Det bør overvejes at anvende biologiske indikatorer periodisk for at sikre, at autoklavens ydelse er effektiv. Kemisk sterilisering anbefales IKKE til nogen af produkterne fra Zimmer Dental. Zimmer One-Piece implantater bør ikke placeres, hvis der er utilstrækkelig alveolær knoglebredde og -højde til at støtte implantatet (der bør være mindst 1 mm i omkreds og 2 mm apikalt). Implantater i overkæben må ikke perforere sinus. Dårlig knoglekvalitet, dårlig mundhygiejne, stort tobaksforbrug eller systemisk sygdom hos patienten, (fx diabetes) kan medvirke til, at implantatet ikke kan blive integreret, tab af understøttende knogle og deraf følgende implantatsvigt. Alvorlig bruksisme, sammenbidning og overbelastning kan forårsage abutment- og implantatsvigt. Relative kontraindikationer omfatter patienter med psykologisk, mental og finmotorisk nedsat funktion, og de, hvis psykologiske, æstetiske og/eller funktionelle behov ikke kan blive opfyldt. Eksponering for stråler og kemoterapi kan påvirke implantatets levetid. Det anbefales at konsultere en læge inden implantationen. ADVARSEL De kirurgiske og restorative teknikker, der anvendes til at anbringe dentale implantater, er meget specialiserede og komplekse procedurer. Tandlæger bør følge undervisningskurser for at gøre sig bekendt med implantationsteknikkerne. Ukorrekt teknik kan medføre knogletab, patientskade, smerte og implantatsvigt.
