NOTE: This form is to be submitted with your 6-month, 2-year, and finally, your 3-year Log Book
submissions. Your Log Book report will not be reviewed without it. Four years of applicable
engineering or geoscience work experience is required to gain professional status.
NOTE: Vous devez remettre ce rapport avec vos journaux après 6 mois, 2 années et 3 années

contained in your Log Book Report. If the supervisor is not registered with APEGNB, he/she must
affix their stamp to this document. Your Progress Report Form and Log Book Report will not be
reviewed without the signature.
NOTE: N’oubliez pas de faire signer ce rapport par votre superviseur en ingénierie ou en
