Lungart external patient PARAFFIN BLOCK COLLECTION PROCEDURE Lung ART : Pharmacogenetics study Evaluation of the expected benefit and the risk of toxicity related to radiotherapy after surgery Stage III clinical trial comparing conformal thoracic radiotherapy after surgery to no radiotherapy after complete surgery in patients with non-small cell lung cancer with N2 mediastinal involvement. This study is an integral part of the Lung ART protocol: it should always be submitted to all patients and requires their informed consent (cf patient information leaflet and consent form). Dispatch enveloppe : For a new randomised patient, a dispatch envelope is sent by Denise Avenell to the pathologist (see histological report). It contains: A letter to the pathologist An envelope with recipient address A tissue sample (paraffin block) identification form A copy of the letter to the pathologist is sent to Mme Marty, Biology and Pathology Department, Institut Gustave-Roussy, Tel 01 42 11 44 04 or 01 42 11 44 67, E_mail : [email protected] Tissue Sample Collection Procedure : 1- Select a representative tumor sample paraffin block from the surgical resection specimen. 2- Fill in the tissue sample identification form. 3- Put the paraffin block and the tissue sample identification form in the envelope with the recipient address. For the sending: 4- Send the envelope within 15 days of receipt of the letter asking for the tissue sample. 5- Use regular mail with appropriate stamps. 6- A notification of receipt will be sent to the pathologist by Denise Avenell. NB : When shipping different patient tissue samples, you can send them together in the same envelope. The sender is responsible if he or she does not follow these instructions.
