METHODE Le PIFA® Heparin / Platelet Factor 4 Rapid Assay est un dispositif de diagnostic in vitro pour la détection des anticorps dirigés contre le Facteur Plaquettaire 4 (PF4). Ces anticorps sont retrouvés chez certains patients qui suivent un traitement à l’héparine. 2. RESUME Le risque d’une thrombopénie induite par l’héparine (TIH) est accru de façon significative chez les patients récemment traités à l’héparine. La TIH est souvent causée par des anticorps qui activent les plaquettes en reconnaissant le complexe Héparine / PF4. En conséquence, la détection rapide de ces anticorps est devenue une norme en hématologie et en cardiologie. La majorité des tests de laboratoire pour la TIH sont classifiés par la CLIA (Clinical Laboratory Improvement Amendments) comme étant d’une haute complexité, la réalisation du dosage prenant parfois plusieurs heures et nécessitant souvent une confirmation de la TIH, avec ou sans thrombose, après l’apparition des symptômes chez le patient. Il y a donc un réel besoin de disposer d’un test simple et rapide qui aiderait les cliniciens à identifier et à traiter les patients à risque de TIH ou de thrombose. 3. EXPLICATION La TIH est causée par des anticorps héparine-dépendants dirigés contre le complexe Héparine / PF4, et de 1 à 5% des patients traités à l’héparine développent ce type d’anticorps(1). Ces anticorps apparaissent initialement quand les patients sont sous thérapie à l’héparine depuis au moins 5 jours. Une réponse immunitaire peut être observée plus tôt (1-2 jours) si le patient a été exposé au préalable à l’héparine. Les symptômes majeurs de la TIH sont une chute brutale des plaquettes et l’apparition de thromboses. Des réactions cutanées peuvent aussi apparaître, allant d’une simple réaction allergique à des lésions nécrotiques. Il existe deux catégories de méthodes utilisées par les laboratoires pour identifier les anticorps TIH : Méthodes fonctionnelles / activation : test de libération de sérotonine radio marquée 14 C (SRA) et le test d’agrégation plaquettaire (TAP). Méthodes immunologiques / antigène : tests immuno-enzymatiques (ELISA) et tests d’immuno-filtration de particules (PIFA®). Les méthodes SRA, TAP et ELISA sont classifiés par la CLIA comme étant d’une haute complexité, nécessitant plusieurs heures, et étant utilisées comme confirmation, après apparition des premiers symptômes de la TIH chez le patient(2). Le PIFA® Heparin / PF4 Rapid Assay est le seul essai manuel qui peut être réalisé facilement en quelques minutes, il est donc classifié par la CLIA comme étant d’une complexité modérée. 4. PRINCIPE DU TEST ® Le PIFA Heparin / PF4 Rapid Assay est basé sur le principe de l’immuno-filtration de particules (PIFA®). Des microparticules colorées, greffées avec du Facteur Plaquettaire 4 purifié à partir de plasmas riches en plaquettes, fournissent un signal visuel pour le résultat du test (coloration). La capacité des particules complexées ou non à franchir la membrane filtrante est proportionnelle à la réactivité ou à la non-réactivité de l’échantillon testé. Le PIFA® Heparin / PF4 Rapid Assay consiste en un « MINI-REACTEUR » qui contient une membrane de filtration, une fenêtre « REACTIF », une fenêtre « TEST », une fenêtre « CONTROL » et un bouton poussoir pour déclencher la réaction en libérant le réactif, nommé « TOUR », qui contient le réactif à base de microparticules. Le « MINI-REACTEUR » contient une chambre de réaction qui va permettre au réactif de réagir avec le sérum frais du patient. Le réactif contenu dans la « TOUR » est libéré dans la chambre de réaction avant de mettre l’échantillon par pression sur la « TOUR ». Le réactif contient des microparticules greffées avec du Facteur Plaquettaire 4 purifié, ainsi que des agents qui favorisent l’agrégation rapide ou la formation de complexes en présence des anticorps spécifiques anti-PF4 présents dans l’échantillon. Une fois que le réactif a réagi avec l’échantillon dans la chambre de réaction, le mélange réactionnel s’accumule automatiquement au-dessus de la membrane filtrante. Les microparticules non complexées sont séparées des complexes formés en traversant cette membrane. Par conséquent, un échantillon positif va déclencher la formation de complexes avec les microparticules qui ne diffuseront pas au niveau de la membrane. Le réactif constitué de microparticules de couleur bleue ne pourront donc pas migrer vers la fenêtre « TEST ». Inversement, un échantillon non-réactif ne formant pas de complexes 5. MATERIEL FOURNI Un coffret contenant 6 dispositifs unitaires « MINI-REACTEUR » et un mode d’emploi. 6. CONDITIONS DE CONSERVATION Les « MINI-REACTEUR » doivent être conservés à +2/+8°C. 11. PERFORMANCES DU TEST Akers Biosciences Inc. a mené des séries d’évaluations pour déterminer la performance de PIFA® Heparin / PF4 Rapid Assay pour la détection des anticorps anti-Héparine / CXCL4. Les études comparatives pour déterminer la performance de PIFA® Heparin / PF4 Rapid Assay ont été menées par des laboratoires d’analyses médicales externes avec des échantillons de patients collectés sur le terrain. La méthode standard des laboratoires concernés était un test ELISA commercial. SPECIFICITE / SENSIBILITE ELISA Positif PIFA® Négatif Positif 21 3 Négatif 2 153 Spécificité : 98.1% Sensibilité : 91.3% Accord global : 97.2% Stabilité à température ambiante : 8 heures, avant stockage au réfrigérateur pour une utilisation ultérieure. FIDELITE - Ne pas congeler les tests : Si les dispositifs sont congelés à réception, les résultats peuvent être erronés. - Ne pas utiliser les tests au-delà de la date d’expiration. La fidélité de PIFA® Heparin / PF4 Rapid Assay pour la détection des anticorps antiHéparine / PF44 a été démontrée en analysant 10 aliquotes de 5 échantillons pour la fidélité inter-jour et 10 aliquotes pour la fidélité intra-jour. La fidélité du test a été de 100 % dans les deux cas. 8. PRECAUTIONS - Ne pas exposer PIFA® « MINI-REACTEUR » à une température supérieure à 40°C ou inférieure à 0°C. - Ramener chaque PIFA® « MINI-REACTEUR » à une température ambiante (18-27°C), dans son sachet d’origine, pendant 30 minutes minimum, avant utilisation. Le PIFA® Heparin / PF4 Rapid Assay doit être réalisé EXCLUSIVEMENT avec du sérum frais. Les échantillons prélevés sur tube contenant un anticoagulant ne sont pas adaptés pour ce test et ne doivent donc pas être utilisés. - Les échantillons de sérum frais doivent être collectés dans de bonnes conditions d’asepsie et manipulés selon les bonnes pratiques de laboratoire. - Les échantillons de sérum congelés, décongelés, hémolysés, ictériques ou lipémiques (en excès), contaminés par des bactéries, ou de contrôles d’autres coffrets, ne doivent pas être utilisés. Ils peuvent induire des résultats erronés. - Un temps d’incubation inadéquate, un mélange incomplet ou le non-respect des procédures du test peuvent produire des résultats erronés. - Voir aussi le chapitre 10. Limites du test. 9. CONTROLE DE QUALITE Les dispositifs PIFA® Heparin / PF4 Rapid Assay sont à usage unique. Akers Biosciences Inc. recommande aux laboratoires de tester un échantillon représentatif par lot en tant que contrôle qualité. Il est également recommandé d’utiliser un contrôle qualité positif et négatif, dans les cas suivants : - Première utilisation d’un nouveau lot de PIFA® Heparin / PF4 Rapid Assay - Après chaque série de 100 tests avec le même lot de dispositifs. Le PIFA® Heparin / PF4 Rapid Assay dispose d’un contrôle interne. Une coloration rouge de la fenêtre « CONTROL » indique que le test a fonctionné. S’il n’y a pas de coloration rouge dans la fenêtre « CONTROL » dans un délai de 10 minutes maximum, après avoir mis en œuvre le test, le résultat obtenu est considéré comme invalide. Les contrôles doivent être testés selon les usages du laboratoire de la même manière que les spécimens. Utilisez des échantillons contrôles confirmés. Pour plus d’information sur ces produits, veuillez contacter votre distributeur local. 10. LIMITES DU TEST Le PIFA® Heparin / PF4 Rapid Assay est un test de dépistage. Il doit être utilisé pour une détermination qualitative des anticorps dirigés contre le complexe Héparine / PF4. Dans certains cas, il peut y avoir des anticorps qui réagissent avec le complexe Héparine / PF4 mais demeurent insensibles à ce test. Le résultat ne doit pas s’appuyer uniquement sur ce test. 12. Retirer le PIFA® Heparin / PF4 Rapid Assay du réfrigérateur et le laisser à température ambiante (18-27°C / 64-81°F) dans son emballage d’origine pendant 30 minutes au minimum. S’assurer que le dispositif n’est pas froid au toucher. Fenêtre « REACTIF » Un PIFA® « MINI-REACTEUR » sorti de son emballage doit être utilisé dans un délai de 60 minutes maximum. Fenêtre « TEST » « CHAMBRE DE REACTION » 4. 4 2. Vérifier la date de péremption du dispositif. 3. Etiqueter le dispositif avec les identifiants du patient et le placer sur une surface plane. Fenêtre « CONTROL » 5. 1. REALISATION DU TEST Appuyer sur la « TOUR » complétement pour libérer les réactifs dans la chambre de réaction. 5. 1 Incliner le « MINI-REACTEUR » à 45° de façon à surélever la « TOUR ». Tapoter la bande des résultats avec le doigt jusqu’à ce qu’une couleur bleue apparaisse dans la fenêtre « REACTIF ». 5 PIFA® Heparin / PF4 « MINI-REACTEUR » 1. Après ce temps d’une minute, remonter la « TOUR » jusqu’en position d’arrêt (1er cran). PRELEVEMENT DE L’ECHANTILLON Le PIFA® Heparin / PF4 Rapid Assay doit être réalisé EXCLUSIVEMENT avec du sérum frais. N’utilisez pas des échantillons congelés ou décongelés. Selon le guide de bonnes pratiques H18-A3 de la CLSI « Procedures for the handling and processing of blood specimens », le sérum doit être séparé des cellules sanguines dans les 2 heures qui suivent la sédimentation(3). 2. Les échantillons de sang total doivent être collectés dans des tubes de type Vacutainer® à bouchon rouge. Des tubes unis, en verre ou en plastique, avec des activateurs de la coagulation doivent être utilisés pour obtenir des échantillons de sérums des patients. L’utilisation d’autres tubes, y compris des tubes séparateurs de sérum (SST), n’est pas recommandée. 2. Prélever 30 µL d’échantillon de sérum frais avec une micropipette. Introduisez le cône de la micropipette dans le « PUITS ECHANTILLON » de la « TOUR » jusqu’à ce qu’il touche le fond du puits. Remonter légèrement et déposer délicatement l’échantillon. 6. 2 PREPARATION DE L’ECHANTILLON IMPORTANT : Le sérum doit être séparé du culot sanguin dans les 60 minutes qui suivent la centrifugation. Au-delà de ce temps, le résultat du test peut être invalide. 3. CONSERVATION DES ECHANTILLONS Poser le dispositif sur une surface plane. Quand une couleur rouge apparaît dans la fenêtre « CONTROL », interpréter le résultat dans la fenêtre « TEST ». Un bon éclairage est nécessaire pour lire correctement le résultat. Le résultat du test est stable pendant une heure. L’apparition de couleur rouge dans la fenêtre « CONTROL » est dépendante du flux de liquide, un délai de 1 à 10 minutes est nécessaire pour lire le résultat. Au-delà de 10 minutes, le test est considéré comme invalide. 1. Activation de la coagulation : les échantillons de sang total doivent être coagulés avant d’être centrifugés. - Tubes en plastique à bouchon rouge avec activateur de coagulation : Consulter la notice du fabricant pour connaître le temps nécessaire à la coagulation (30 - 60 minutes). - Tubes en verre à bouchon rouge : Laisser l’échantillon coaguler pendant 30 minutes environ. 2. Centrifugation : L’échantillon doit être centrifugé immédiatement après la coagulation. Consultez la notice du fabricant des tubes de collecte pour connaître le temps et la vitesse de centrifugation. Apparition de la couleur bleue dans la fenêtre « REACTIF » 6. 3. INTERPRETATION DES RESULTATS Secouer le dispositif vigoureusement dans le plan horizontal pendant 5 secondes (12-15 fois). Dans tous les cas, ramener les échantillons à température ambiante avant de les tester. Résultat négatif / échantillon non-réactif : L’intensité de la couleur bleue dans la fenêtre « TEST » varie entre le bleu pâle à un bleu foncé, en fonction de l’échantillon. 7. ELIMINATION DES DECHETS Eliminer le dispositif usagé dans des conteneurs adaptés conformément aux procédures d’élimination des déchets médicaux en vigueur au laboratoire et à la réglementation locale. 4. The PIFA® Heparin / Platelet Factor 4 Rapid Assay is a qualitative in vitro diagnostic device designed for the detection of antibodies to Platelet Factor 4 (PF4) sensitized microspheres. These antibodies are found in some patients undergoing heparin therapy. 2. SUMMARY The risk of heparin induced thrombocytopenia (HIT) is greatly increased in patients with recent exposure to heparin. HIT is often caused by platelet activating antibodies that recognize complexes of Heparin / PF4. As a result, antibody detection is rapidly becoming a standard of care in hematology and cardiology. The majority of laboratory tests for HIT are classified by CLIA as high complexity, take many hours to perform, and often provide confirmation of HIT or HIT and thrombosis (HITT) after the symptoms are seen in a patient. As a result, there is a need for an easily performed, rapid test that helps clinicians identify and treat patients at risk for HIT or thrombosis. 3. There are two categories of laboratory methods used to identify HIT antibodies: Functional / Activation : Aggregation Test (PAT). 14 C Serotonin Release Assay (SRA) and the Platelet Assays Antigen Assays / Immunoassays : the Enzyme-Linked Immunoassay (ELISA) and the Particle ImmunoFiltration Assay (PIFA®). SRAs, PATs and ELISAs are CLIA-classified high complexity, take many hours to perform, and are used primarily as a confirmation of HIT after the symptoms are seen in a patient(2). The PIFA® Heparin / PF4 Rapid Assay is the only manual assay that can easily be performed in minutes and is CLIA-classified as moderate complexity. PRINCIPE DU TEST The PIFA® Heparin / PF4 Rapid Assay consists of a « MINIREACTOR » device that contains a membrane filtration system, a « REAGENT » Window, « TEST » Result Window, « CONTROL » Window, and a push button reagent dispensing system, referred to as the « TOWER », that contains microparticle-based reaction reagents. The « MINIREACTOR » contains a reaction chamber that allows the reagents to react with the patient’s fresh serum sample. The reagent contained in the « TOWER » is added to the reaction chamber followed by the sample. The reagent contains microparticles coated with purified PF4 protein, as well as additional enhancing agents designed to promote rapid aggregation or matrix formation of the particles in the presence of specific antibodies in the test sample. Once the reagent has reacted with the sample in the reaction chamber, the reaction mixture automatically collects over the membrane filtration system. This system acts to filter matrixed particles, while allowing non-matrixed particles to pass through. Thus, a reactive sample will form a matrix of particles that will be trapped within the membrane. Since the dyed particles are trapped within this membrane filter, no particles and hence, no BLUE color, are able to migrate into the « TEST » result window. Conversely, a nonreactive sample will not form a matrix of particles. These non-matrixed particles will pass through the membrane filter into the wicking layers, and BLUE color will migrate into the « TEST » result window. MATERIALS PROVIDED Kit containing: 6 « MINIREACTOR » Devices 1 PIFA® Heparin / PF4 Package Insert PRECAUTIONS - Do not expose PIFA® « MINIREACTOR » to temperatures greater than 40°C (104°F) or below 0°C (32°F). - Allow each PIFA® « MINIREACTOR » to warm to an ambiant temperature (18-27°C), in the individual foil sealed pouch, for a minimum of 30 minutes prior to performing the test. The PIFA® Heparin / PF4 Rapid Assay must be performed using FRESH patient serum ONLY. Anticoagulated samples are not suitable for test with this assay and must not be used. - All fresh serum specimens should be collected using aseptic technique and should be handled in accordance with good laboratory practices - Frozen, thawed, hemolyzed, icteric, lipemic (of an excessive nature), bacterial contaminated specimens, or controls from other test kits should not be used, and can produce erroneous results. - Inadequate incubation time, incomplete mixing, or improperly performed test procedures can produce erroneous results - See also 10. Limitations QUALITY CONTROL The PIFA® Heparin / PF4 Rapid Assay is a single-use / unit use test system. Akers Biosciences Inc. recommends that the laboratory analyzes a sample representative of the lot for Quality Control. It is also recommended that a positive and negative control be utilized in the following circumstances: ® The PIFA® Heparin / PF4 Rapid Assay is based upon principles of the Particle ImmunoFiltration Assay (PIFA®). Dyed microparticles coated with purified Platelet Factor 4 (PF4) protein derived from platelet-rich plasma provide the visual signal for the results of the assay. The ability of matrixed or non-matrixed particles to move through a filter medium is the measure of the reactivity / nonreactivity of the test sample. 5. Please note: - Do not freeze tests. - If the test is frozen upon receipt, results will be invalid. - Do not use any tests beyond their expiration date. 9. SPECIFICITY / SENSIBILITY ELISA Positive Devices that have been removed from refrigeration and stored at an ambient temperature for a maximum of 8 hours may be returned to refrigeration for future use. 8. originating from field sources. The standard laboratory method was a commercially available ELISA technique. STORAGE CONDITIONS The PIFA® « MINIREACTOR » must be stored refrigerated at 2-8°C (36-46°F). EXPLANATION HIT is caused by heparin-dependent antibodies formed to the heparin / platelet factor 4 complex, and 1-5% of adults exposed to heparin develop these antibodies(1). These antibodies are initially formed when a patient has been on heparin therapy for five or more days. An immune response to a heparin dose may be observed sooner (1-2 days) if the patient has had previous exposure to heparin. The hallmark symptoms of HIT are a drastic fall in platelet count and thrombosis. Other symptoms may include cutaneous reactions, from a simple allergic reaction to lesions to necrosis. 4. INTENTED USE CRYOPEP MATERIALS REQUIRED BUT NOT PROVIDED - during the initial use of a new lot of PIFA Heparin / PF4 Rapid Assay devices - after running 100 tests of the same lot of devices The PIFA® Heparin / PF4 Rapid Assay provides an internal device control with each test run. The appearance of RED in the « CONTROL » window indicates that the device has functioned as designed. If RED does not develop in the « CONTROL » window within a maximum of 10 minutes after performing the test procedure, the test result is considered invalid. Controls should be assayed as required by your laboratory’s standard quality control procedures using the same procedure as the specimens. Use only confirmed Heparin / PF4 antibody positive and negative samples as controls. LIMITATIONS The PIFA® Heparin / PF4 Rapid Assay should be used for the qualitative detection of any antibody directed against the PF4 complex, and should be used as a screening test. There may be some antibodies reactive to the Heparin / PF4 complex that are non-reactive with this test. Test results should not be relied upon solely to identify an antibody to the PF4 complex. PIFA® Negative Positive 21 3 Negative 2 153 Specificity : Sensibility : 98.1% 91.3% Overall agreement : 97.2% REPRODUCTIBILITY The reproducibility of the PIFA® Heparin / PF4 Rapid Assay in detecting Heparin / PF4 antibodies was demonstrated by testing 10 aliquots of 5 specimens for inter-day evaluation and 10 aliquots for intra-day evaluation. Reproducibility of the PIFA® Heparin / PF4 Rapid Assay was determined to be 100% in both studies. 12. REFERENCES 1. Visually inspect device to confirm date of use is prior to expiration date. « CONTROL » WINDOW 3. Label the device with the patient’s identification number and place on a level surface. 5. SPECIMEN COLLECTION FRESH serum must be used with the PIFA® Heparin / PF4 Rapid Assay. Do not use frozen and / or thawed specimens. In accordance with CLSI Guideline H18-A3, “Procedures for the Handling and Processing of Blood Specimens”, serum should be separated from contact with cells within a maximum of 2 hours of the draw(3). Whole blood specimens must be collected in red top tubes ONLY. Plain, glass red top tubes OR plastic, red top tubes, with clot activator must be used to obtain patient specimens; no other collection tubes are recommended for use, including gel serum separation tubes (SST). 2. 2. IMPORTANT : Patient specimen should not stand on the clot for longer than 60 minutes after centrifugation or the PIFA® test result may be invalid. Using a pipettor, obtain a 30 µL sample of the patient’s FRESH serum. Insert the pipettor tip into the « SAMPLE WELL » in the « TOWER ». When the pipettor tip makes contact with the bottom of the well, pull up slightly and dispense the sample. 6. Lay unit on a level surface. When a RED color appears in the « CONTROL » window, interpret the result in the « TEST » window in well-lit conditions. The test result is stable for one (1) hour. Since flow rate is sample dependent, the time interval for a RED color to develop in the « CONTROL » window varies, ranging from 1 to 10 minutes. SPECIMEN PREPARATION 1. Clot activation : Patient specimens must be allowed to clot before centrifugation - Plastic, Red Top Tubes with Clot Activator : Consult collection tube manufacturer’s instructions for mixing protocols and recommended clotting times (clotting times range from thirty (30) to sixty (60) minutes). - Plain, glass red top tubes : Allow specimen to clot for approximately 30 minutes. 2. Centrifugation : The specimen should be centrifuged at the end of the clotting period in strict accordance with the collection tube manufacturer’s instructions for speed and duration of centrifugation. 3. Blue color appears in « RÉAGENT » window 2 If RED fails to appear in the « CONTROL » window, beyond the 10 minutes mark, the test result is considered invalid. 6. INTERPRETATION OF RESULTS The following PIFA® Interpretation Guide is provided to assist in the determination of the results : 3. Note : Any refrigerated serum should reach an ambient temperature prior to performing the test. 3 Reactive / Positive Result : The « TEST » window may appear to be a pale grey or « wet white » color as the wicking material will have been moistened by the reagent / sample mixture. ** Non-Reactive / Negative result : The intensity of the blue color in the « TEST » window may range from a pale blue to a vibrant blue as the color is sample dependent. 7.
