Tło wklejki tworzą kolorowe linie giloszowe wykonane techniką offsetową
z zastosowaniem druku irysowego. Linie te w środkowej części wklejki układają się w rozetę.
Guilloche lines. Background of the inset is formed by offset-printed, colourful guilloche
lines utilising the iris print technique. In the central section of the inset the lines make
a rosette.
Filets azurés. Le fond de l’insert est composé de filets azurés de plusieurs couleurs, appliqués
avec la technique d’offset, avec l’impression Iris. Dans la partie centrale du formulaire, ces lignes
composent une rosace.
Mikrodruk
Dwukolorowa linia mikrodruku o treści„RZECZPOSPOLITA POLSKA” znajduje się
pomiędzy numerem seryjnym wklejki i polem kodu OCR.
Microprint. The dichromatic line of „RZECZPOSPOLITA POLSKA” microprint is located
between the serial number of the inset and the OCR code field.
Micro-impression. Une ligne de micro-impression en deux couleurs composée de mots
: «RZECZPOSPOLITA POLSKA» se trouve entre le numéro de série du l’insert et le champ
du code OCR.

Numeracja wklejki została wykonana techniką typograficzną.
Numbering. Numbering of the inset was performed with the letterpress technique.
Numérotation. Le numéro de l’insert est appliqué avec une technique typographique.
Elementy widoczne w promieniach UV
Na wklejce, w jej centralnej części, znajdują się elementy graficzne wydrukowane
farbą aktywną w świetle UV. Elementy te, niewidoczne w świetle dziennym,
świecą w dwóch kolorach pod wpływem promieniowania ultrafioletowego.
Elements visible in UV. In the central part of the inset there are graphical elements printed
with UV-active ink. Invisible in daylight, these elements glow in two colours when exposed
to ultraviolet rays.
Eléments visibles aux rayons UV. Sur l’insert, dans sa partie centrale, il y a des éléments
graphiques imprimés avec une peinture active sous lumière UV. Ces éléments, invisibles
à la lumière du jour, luisent en deux couleurs sous l’action du rayonnement ultraviolet.

różnią się między sobą wyłącznie informacją
określającą status posiadacza wpisu:

Siłom Zbrojnym Rzeczypospolitej Polskiej,
3. członek rodziny członka personelu cywilnego
towarzyszącego Siłom Zbrojnym Rzeczypospolitej Polskiej.
Informacja ta zamieszczona jest bezpośrednio
pod nazwiskiem i imieniem posiadacza wpisu.
Entry specimens
The ordinance defines three specimens of entries that
differ only with regard to the information defining the
status of the holder:
1. family member of a soldier,
2. civilian personnel member accompanying the armed
forces of the Republic of Poland,
3. family member of a civilian personnel member
accompanying the armed forces of the Republic of
Poland.
This information is located directly below the name and
surname of the holder.

Cet arrêté distingue trois spécimens d’inscription, différenciés
uniquement par l’information sur le statut du titulaire:
1. membre de famille d’un soldat,
2. membre du personnel civil accompagnant les Forces
Armées de la République de Pologne,
3. membre de famille d’un membre du personnel civil
accompagnant les Forces Armées de la République de

Bezpośrednio nad numerem wklejki określony jest
termin ważności wpisu. Ponadto wpis może stracić
ważność w wyniku jego uchylenia. W takim przypadku odciśnięta będzie pieczęć z napisem „UCHYLONO” oraz zniszczony (zdrapany po przekątnej)
optycznie zmienny element tzw. alphagram i napis
„WPIS/ENTRY/INSCRIPTION NATO – SOFA”
Validity of entries
The entry’s due date is located directly above the inlet
number. In addition, an entry may lose validity as a result
of cancellation. In such case the entry will bear the stamp
with the word „UCHYLONO” (cancelled) and the optically
variable element (the so-called alphagram) and inscription
WPIS/ENTRY/INSCRIPTION NATO – SOFA” will be destroyed
(diagonally scratched).
Validité des inscriptions
Directement au-dessus du numéro de l’insert, il y a la date
de la validité de l’inscription. L’inscription peut devenir
nulle aussi par la voie de son abrogation. Dans ce dernier
cas, sur l’insert il y a un cachet avec mention «UCHYLONO»
(ABROGE), l’élément optiquement variable (alphagramme)
et l’inscription «WPIS/ENTRY/INSCRIPTION NATO – SOFA»
sont alors détruits (grattés en diagonale).

Realizacja ratyfikowanej przez Rzeczpospolitą Polską umowy między państwami – stronami traktatu północnoatlantyckiego – dotyczącej statusu ich Sił Zbrojnych (NATO – SOFA) nakłada na Siły Zbrojne Rzeczypospolitej Polskiej obowiązek uregulowania problematyki wpisów w paszportach potwierdzających status
członków personelu cywilnego Sił Zbrojnych, członków ich rodzin oraz członków rodzin żołnierzy wyjeżdżających w celach służbowych za granicę (Dz. U. z 2000 r. Nr 21, poz. 257 oraz z 2008 r. Nr 170, poz. 1052).
Przepis art. III ust. 3 tej umowy stanowi, iż członkowie Sił Zbrojnych nie podlegają przepisom paszportowym i wizowym ani kontroli organów imigracyjnych przy wyjeździe lub wjeździe na terytorium państwa
przyjmującego. Ponadto członkowie rodzin żołnierzy, członkowie personelu cywilnego i członkowie ich
rodzin powinni posiadać stosowne wpisy w swoich paszportach potwierdzające ich status, które nadają
im w powyższym zakresie takie same uprawnienia jak członkom Sił Zbrojnych. W związku z powyższym
w ustawie o dokumentach paszportowych zamieszczony został przepis stanowiący o wpisach w paszportach potwierdzających status osób wymienionych w art. III ust. 3 umowy NATO – SOFA.
Na podstawie unormowań ustawowych wzory wpisów do paszportu oraz tryb postępowania w sprawie
dokonywania wpisu zostały określone w rozporządzeniu Ministra Obrony Narodowej z 10 lipca 2009 r.
w sprawie wpisów w paszportach potwierdzających status członka personelu cywilnego Sił Zbrojnych
Rzeczypospolitej Polskiej, członków rodzin tego personelu lub członków rodzin żołnierzy wyjeżdżających w celach służbowych za granicę (Dz.U. Nr 119, poz. 995).
W niniejszym opracowaniu prezentujemy Państwu wpisy w paszportach osób uprawnionych, dokonywane w formie wklejki do paszportu, zabezpieczonej zgodnie z najlepszymi światowymi standardami
i w pełni odpowiadające współczesnym warunkom identyfikacji osób.
Mamy nadzieję, że przedstawione informacje będą interesujące i przydatne zarówno dla osób uprawnionych do otrzymania wpisów do paszportów, jak również dla tych, do których obowiązków należeć będzie
ich dokonywanie i sprawdzanie.
Charakterystyka ogólna
Alphagram
Wklejka do paszportu potwierdzająca status członka personelu cywilnego Sił Zbrojnych Rzeczypospolitej Polskiej, członków rodzin tego personelu lub członków rodzin żołnierzy wyjeżdżających w celach
służbowych za granicę jest wpisem do dokumentu podróży dokonywanym przez Ministra Obrony Narodowej zgodnie z umową między państwami – stronami traktatu północnoatlantyckiego – NATO – SOFA.
Wpisu dokonuje się poprzez wklejenie blankietu na jedną ze stron wizowych paszportu. Jakość wykonania
i poziom zabezpieczeń wklejki skutecznie uniemożliwiają sfałszowanie wpisu, a jednocześnie pozwalają
zweryfikować jego autentyczność. W niniejszym folderze przedstawione zostały poglądowo najbardziej
charakterystyczne z użytych zabezpieczeń.
W lewym górnym rogu wklejki umieszczony jest tzw. alphagram – element
zmienny optycznie zmieniający barwę i połysk w zależności od kąta, pod jakim
na niego patrzymy (rodzaj hologramu).
Głównym elementem alphagramu jest orzeł – godło państwa polskiego.
Godło jest umieszczone na tle poziomych linii mikrotekstu o treści: „RZECZPOSPOLITA POLSKA”, przecinanych symetrycznie graficznymi liniami pionowymi. Wokół górnej części godła, na linii okręgu, został umieszczony napis
„Rzeczpospolita Polska”; natomiast wokół dolnej części godła widnieją litery „RP”
zmieniające kolor w zależności od kąta padania światła. Uzupełnieniem tła alphagramu są napisy „RP” rozmieszczone wzdłuż boków hologramu.
Alphagram. The top-left corner of the inset features the so-called alphagram – an optically variable element, that changes
colour and gloss depending on the viewing angle (a type of hologram).
The main element of the alphagram is an eagle – the national emblem of Poland. The emblem is set against the background
of horizontal microtext lines that read „RZECZPOSPOLITA POLSKA” (Republic of Poland), symmetrically crossed by vertical
graphical lines. Around the upper part of the emblem there is an arching inscription „Rzeczpospolita Polska”; while
around the lower part of the emblem the letters “RP” can be seen which change colour depending on the viewing angle.
The alphagram’s background is complemented by “RP” inscriptions located along the side of the hologram.
Alphagramme. A l’angle gauche supérieur de l’insert il y a un alphagramme – un élément optiquement variable dont
la couleur et la luisance changent selon l’angle de vue (une sorte d’hologramme).
L’élément principal de l’alphagramme est une représentation de l’aigle qui est l’emblème national polonais. Cet emblème est
situé sur un fond créé par des lignes horizontales du micro-texte : «RZECZPOSPOLITA POLSKA» (REPUBLIQUE DE POLOGNE),
symétriquement coupées par des lignes graphiques verticales. Autour de la partie supérieure de l’emblème, sur la ligne
du cercle, il y a l’inscription «Rzeczpospolita Polska» (République de Pologne), tandis qu’autour de la partie inférieure
de l’emblème, il y a les lettres «RP» dont la couleur change selon l’angle d’incidence de la lumière. Le fond de l’alphagramme
est complété par les mots «RP» repartis au long du coté de l’hologramme.

The execution of the Agreement ratified by the Republic
of Poland, binding the States – Parties to the North Atlantic
Treaty – and concerning the status of the Armed Forces thereof
(NATO – SOFA) imposes on the Armed Forces of the Republic
of Poland the obligation to regulate the issue of passport entries
confirming the status of civilian personnel of the Armed Forces,
their family members and family members of soldiers going
abroad on official purposes.
la mise en place de la Convention entre les Etats Parties au Traité
de l’Atlantique Nord sur le statut de leurs forces (NATO – SOFA),
ratifiée par la République de Pologne, impose aux forces
de la République de Pologne l’obligation de régler la question
de mentions spécifiques apposées dans les passeports des
éléments civils des forces, des personnes à leur charge et des
personnes à charge des militaires de carrière en déplacement
professionnel à l’étranger, faisant état de leur statut.
Elementy wykonane drukiem stalorytniczym
The provision of Art. III sec. 3 of the Agreement states that
the members of the Armed Forces are not subject to passport
and visa regulations, or control by the immigrations authorities
at the entry to or departure from the Host State. Furthermore,
the family members of soldiers, civilian personnel and their
family members should have appropriate entries in their
passports, confirming their status, which grant to them similar
privileges as to the members of the Armed Forces.
La disposition de l’art. III point 3 de la Convention stipule que
les membres des forces sont dispensés des formalités de passeport
et de visa, ainsi que de l’inspection par les services d’immigration
à l’entrée et à la sortie du territoire d’un Etat de séjour.
Les personnes à charge des militaires, les éléments civils et les
personnes à leur charge doivent être porteurs des passeports
faisant état de ladite qualité, leur affectant les droits égaux à ceux
des membres des forces.
W prawym dolnym rogu wklejki znajduje się jedno z ważniejszych zabezpieczeń dokumentu – element wykonany techniką druku stalorytniczego. Cechą
wyróżniającą staloryt jest faktura druku – nie jest on płaski, lecz wyraźnie
wyczuwalny dotykiem.
Zabezpieczenie ma formę prostokąta o wymiarach 25x8 mm, podzielonego
na trzy pola o szerokości w podstawie: 7, 10 i 8 mm. W pierwszym polu widnieje kontur wędrowca, drugie zawiera graficzne elementy ozdobne, trzecie
pole – obraz samolotu. W obszarze środkowego pola znajduje się efekt kątowy
w postaci liter „RP”, widoczny podczas obserwacji w świetle padającym pod kątem
45 stopni. W górnej części zabezpieczenia stalorytniczego znajduje się mikrodruk
– tekst „RZECZPOSPOLITA POLSKA” w trzech językach, naprzemiennie, kolejno po polsku,
angielsku i francusku.
In consideration of the foregoing, the Act on passport documents
was amended with the provision concerning the passport entries
confirming the status of persons mentioned in Art. III sec. 3
of the NATO – SOFA Agreement.
A cet égard, une disposition a été ajoutée à la loi sur les passeports
relative aux mentions à être apposées obligatoirement dans
les passeports et faisant état de la qualité des personnes énumérées
à l’Art. III point 3 de la Convention NATO – SOFA.
Pursuant to statutory regulations the passport entry specimens
and the procedure of making the entry have been set forth in
the Ordinance of the Minister of National Defence of 10 July
2009 on the passport entries confirming the status of members
of civilian personnel of the Armed Forces of the Republic of
Poland, family members of such personnel or soldiers going
abroad for official purposes (Polish Journal of Laws Dz. U. No.
119, item 995).
En vertu des dispositions législatives, l’ordonnance du ministre
de la défense du 10 juillet 2009 relative aux mentions spécifiques
apposées dans les passeports et faisant état de la qualité des éléments
civils des forces de la République de Pologne, des personnes à leur

General features
The passport inset confirming status of a civilian member in the armed forces of the Republic of Poland, family members
of such a civilian or family members of soldiers going abroad on official business is an entry in the travel document made
by the Minister of Defence in accordance with the agreement with signatory countries of NATO – SOFA. The entry is made
by insetting the form on one of the visa pages in the passport. The quality of workmanship and safety features of the inset
effectively prevent forgeries of the entry, while simultaneously enabling verification of its authenticity. This folder presents
the most characteristic safety features.
Caractéristiques générales
L’insert collé au passeport qui confirme le statut d’un membre du personnel civil des Forces Armées de la République de Pologne,
des membres de famille de ce personnel ou des membres de famille des soldats délégués à l’étranger est une inscription au
document de voyage effectuée par le Ministre de la Défense du pays, conformément aux accords signés entre les pays –
parties de l’Organisation du Traité de l’Atlantique Nord – NATO – SOFA. L’inscription est réalisée par le collage d’un formulaire
sur une des pages du passeport prévues pour les visas. La qualité de la réalisation et le niveau des sécurisations de l’insert
empêchent efficacement une falsification de l’inscription et en même temps elles permettent d’identifier son authenticité.
Le présent prospectus présente une description des protections les plus importantes.
Steel engraving elements
The bottom-right corner of the inset features one of the most important safety features securing the document – the element
made with the steel engraving printing technique. The distinguishing feature of steel engraving is the surface quality of print
– it is not flat but clearly palpable.
This safety feature has the form of a 25x8 mm rectangle divided into three fields that are respectively 7, 10 and 8 mm wide
at the base. The first field features the silhouette of a traveller, the second contains decorative elements, and the third
– a picture of an airplane. The central field features an angle effect in the form of „RP” letters visible when watched at the
angle of 45 degrees. The upper part of the steel engraving element features the microprint „RZECZPOSPOLITA POLSKA”
alternating in Polish, English and French.

A l’angle droit supérieur de l’insert il y a l’une des protections les plus importantes: un élément imprimé avec la méthode
d’impression en creux. Cette technique de gravure est très caractéristique car la facture de l’impression n’est pas plate et elle
est clairement perceptible au toucher.
Cette protection prend la forme d’un rectangle de 25x8 mm divisé en trois champs, dont les bases ont respectivement 7, 10
et 8 mm de largeur. Le premier champ présente un globe-trotter, le second contient des motifs ornementaux et le troisième
présente un avion. Dans la zone du champ central, il y a un élément angulaire sous forme des lettres «RP» visible lorsque
l’angle d’incidence de la lumière est de 45°. Dans la partie supérieure de cette protection gravée, il y a une micro-impression :
«RZECZPOSPOLITA POLSKA» (REPUBLIQUE DE POLOGNE) en trois langues, en alternance : polonaise, anglaise et française.
