MAKING A REFERRAL TO First Link® “Working together to link individuals and families affected by Alzheimer’s disease or a related dementia to a community of learning, services and support.” Who to refer to First Link®? Individuals with a diagnosis or Alzheimer’s disease or a related dementia Caregivers Family members Concerns with or diagnosis of MCI (Mild Cognitive Impairment) or other memory problems When to refer? As soon as there is a concern or when a diagnosis is made At any other time in the dementia journey Why refer? It is often difficult for individuals and families to ask for help Information and support will help patients and families to cope with more confidence Planning for the future is critical (re: Power of Attorneys) There are community and health care services that families need to know about With knowledge and skills people are able to maintain quality of life while living with dementia Who can make a referral? Physicians Health Care professionals Home Care Hospital/Community organizations Self-referral How to refer? Ask for consent from the individual or family member to forward their information to the Alzheimer Society Complete the referral form and fax to 705-495-0329 WHAT HAPPENS WHEN First Link® RECEIVES THE REFERRAL? Individuals or family members will receive: A phone call from the Alzheimer Society of Sudbury-Manitoulin North Bay & District within 5 business days unless otherwise specified or if a waitlist occurs. An intake form will be completed to assess their needs and what services we can provide Follow-up by our Family Support Counsellor, Education Coordinator, or an information package will be sent First Link® will send a confirmation to a physician or family health team referral when contact has been made with the individual (PWD) or family member.
