Nous vous remettons en annexe le formulaire de sortie pour la caisse de pensions PUBLICA (2ème pilier). Vous voudrez
bien le retourner, avant votre départ de l’EPFL, dûment complété, daté et signé à l’adresse suivante : Caisse Fédérale de
pensions PUBLICA, Eigerstrasse 57, Case postale, 3000 Berne 23.
Un remboursement en espèces de la prestation de sortie est possible en cas de départ définitif de la Suisse. Pour
toute destination hors de Suisse et du Lichtenstein, les documents suivants doivent être annexés :
- attestation de départ délivrée par le Contrôle des habitants de votre commune de domicile
- déclaration de consentement écrite du conjoint avec signature authentifiée par un notaire ou par Publica à
Berne
Pour un départ à destination de l’un des Etats membres de la Communauté européenne et de l’AELE (Islande et
Norvège), en plus des documents précités, le formulaire d’ouverture d’un compte de libre passage auprès d’une
banque ou d’une assurance doit également être annexé.

AVS/AI

toutes les informations utiles concernant l’AVS. Le formulaire de demande de remboursement des cotisations pourra être

Si vous vous installez dans un pays avec lequel la Suisse a conclu une convention de sécurité sociale (par exemple un
pays de la CE https://www.ahv-iv.ch/fr/M%C3%A9mentos-Formulaires/Formulaires/Prestations-de-lAVS, pensez à
demander vos prestations à l’AVS en cas de retraite à 64 ans (femmes) respectivement 65 ans (hommes), d’invalidité ou
de décès.

Votre abonnement CFF est à restituer à votre supérieur hiérarchique le dernier jour de votre présence dans l’Ecole. Pour

Assurance-accidents
Pendant votre période d’activité à l’EPFL, vous étiez assuré(e) contre les accidents par la SUVA (Assurance-accidents)
en application de la LAA (Loi sur l’Assurance-Accidents). En raison de votre départ, nous vous informons que la
couverture accident selon la LAA cesse de produire ses effets trente jours calendrier après la résiliation des rapports de
service. Si vous n’avez pas de nouvel employeur pour au moins 8 heures par semaine, il vous appartient de vous assurer
personnellement auprès d’une caisse maladie en Suisse pour une couverture accidents non-professionnels. La SUVA
propose une prolongation de 6 mois consécutifs au maximum de votre couverture d’assurance accidents nonprofessionnels pour la somme de CHF 45.--/mois. Le premier mois étant assuré d'office, il est exempt de prime. La
conclusion de cette «Assurance par convention» doit intervenir avant le trentième jour qui suit votre départ. Vous
pouvez passer directement par le site internet www.suva.ch/assurance-convention pour effectuer la transaction. Par ce
biais, un certificat d’assurance international peut être délivré immédiatement. Pour tout complément d’information

Please find enclosed the form that needs to be completed in order to terminate your membership in the PUBLICA pension
scheme. Please complete and return the form, duly dated and signed overleaf before leaving EPFL, to the following
address: Swiss Federal Pension Fund PUBLICA, Eigerstrasse 57, Case postale, CH-3000 Berne 23.
A refund in cash of the termination benefit can be requested by reason of final departure from the Switzerland.
For any destination outside Switzerland and Lichtenstein the following documents are required :
- departure certificate issued by the control of the inhabitants of your commune of residence
- spouse declaration of consent written with signature authenticated by a notary or by Publica in Bern
For a departure for one of the Member States of the European Community and the EFTA States (Iceland and
Norway), in addition to the above-mentioned documents, the form for opening an vesting account from a Vesting
Fundation or insurance must also be attached.

Old-age and disability pension scheme (AVS/AI)
Please visit following website https://www.ahv-iv.ch/en/Leaflets-forms/Leaflets/International in order to obtain all the
relevant information concerning the AVS. The refund of contributions application form can be obtained on the site
following website https://www.ahv-iv.ch/en/Leaflets-forms/Forms/General-administration-forms .
If you reside in a State with which Switzerland has signed a convention on social security (for example the EU)
https://www.ahv-iv.ch/en/Leaflets-forms/Forms/Benefits-of-AHV please remember to ask for your "AVS" benefits at the
age 64 for females and 65 for males in case of disability or death.

Half-fare rail travel card
Owing to your departure and as stated on delivery of your travel card on your arrival, you are requested to hand in your
travel card to your supervisor on your last day at the Institute. Should you require any further information, please contact

Accident insurance
During your employment at the EPFL, you were insured against accidents by SUVA (Swiss National Accident Insurance
Fund) in accordance with the Law on Accident Insurance (LAA). Please note that, owing to your departure, accident
insurance under the LAA will cease to be effective thirty calendar days after termination of your employment contract. If
you do not have a new employer for at least eight hours a week, it is your responsibility to take out personal accident
insurance cover with a health insurance fund. SUVA offers an extension of accident insurance cover for a maximum of six
consecutive months at a cost of CHF 45 per month. The first month is automatically covered so no premium is due. This
"insurance by agreement" (Assurance par convention) must be concluded within 30 days after your departure. The
