The fact that patient age was associated with increased LOS was not surprising. Interestingly, discharge to a long-term facility was not a significant factor if the patient came from such an institution, that is, I suppose, if they entered the hospital with a built-in discharge plan. Patients who entered the hospital from the community but were discharged to long-term facilities had a 75% increase in LOS, a phenomenon with which I suspect most Canadian physicians are familiar. Women had an LOS that was 10% longer than that of men, which is puzzling after correction for severity. Men were admitted more frequently for pneumonia than women, and although mortality did not differ in the study by McGregor et al, others have found higher mortality rates in men (4). McGregor et al speculate that women have more disabilities than men; disabilities that are not considered in corrections for pneumonia severity and comorbidities noted in discharge documentation. Finally, patients admitted under family practitioners had a 42% longer LOS than those admitted under specialists. This makes sense if the specialists were largely full-time hospital people or if specialist admission gave access to care by house staff, simply because the responsible physicians are around all the time. It is these kinds of data that probably have given rise to the ‘hospitalist’ phenomenon. In any event, it is likely that LOS for patients with pneumonia will continue to decline, at least in urban teaching hospitals like VGH. C’est probablement ce type de données qui a suscité le phénomène des « hospitalistes ».
