Victoria Transition Group ---Summary Meeting Notes November 5, 2009 Arlene Zuckernick Next Meeting: Thursday, January, 7,2009, 7-9 pm Community Living Victoria (Lunch Room), Cedar Hill Cross Road RSVP by December 18,2009 [email protected] Families and selfadvocates who were unable to attend the first meeting are encouraged to join us. Agenda to follow at later date. The following is a summary of some of the discussion and decisions and thoughts of the group at the November 5 meeting. It should not be considered as minutes of the meeting. Goals for Group Originally proposed goals We are committed to: 5 Exploring best practices and innovation for services (employment, true community inclusion, self-direction, individualized funding, education and housing options) for young adults - 16 years to 25ish in the Greater Victoria area. 5 Putting some energy into making best practices happen where they don’t exist in our region 5 Working on communicating our collective viewpoint to decision-makers 5 Sharing ideas, and resources of common interest There was agreement with these goals. Additional one to be added: 5 Identify key gaps in system and services and work to fill Ideas Introduced for further discussion and exploration (Note: Not in any specific order) Issues of Family Governance and portability of funding – possibilities for pilots, models and innovation funds; Creative use of funding opportunities Family Governance— --exploring pooling resources, --access innovation funds --meaningful activities during the day time Work towards a seamless transition - not having to reapply or apply for supports just because 19 years or when school finished—the needs don’t change overnight - extend At Home Program exemption to complete Income Assistance Application Simplify the paper process Consider Room-mate registry Inclusive peer group—make connections (eg VOCYL (Victoria Opportunities for Community Youth Leadership) Look at Employment, entrepreneurship, creative employment opportunities Tap into families’ strengths and experiment Transform transition to an exciting, not terrifying experience. Tell our stories and sharing with community at large Health care support for adults and when we are no longer here Setting the foundation to ensure longevity Wellness approaches Quality and accountability issues need to be looked at Start Transition earlier (ages 12-13), look at models that work and don’t reinvent the wheel Vocational training during school years Data collection and tracking Guaranteed funding at end of high school Using resources better—building community capacity What are the gaps and problems Complexity of the system Lack of coordinated planning Day wasting activities instead of useful Social isolation Lack of opportunities to learn from those who have gone ahead—need to learn from the experts What are parent’s rights, entitlements – legal and political— Child as minor and then as adult. Lack of effective case management in transition—support the broker model Lack of support for working parents after daytime Ideas for next meetings Discussion re “what does best practices mean”? Engage in a reflective discussion working towards a common understanding. “Primer” on how to proceed with Main Transition Planning 16-19 year olds Leave some time for socializing with each other at end of evening What do you hope for yourself or your loved one in 5 years? • • • • • • • • • • • • • • • • • • • • • • • • • happy, daily structure, friends, a living arrangement? safe, collaborative, shared housing … somehow “Planned Transition” possibility – with CLBC support, ie. money and resources to make it happen Acceptance in the community Independence and employment Some kind of structure to day/leisure Safe adult living arrangements Employment, Independent living Rewarding employment, Independent living arrangements, drivers license Independence, Peaceful & joyful life for her Community involvement and support/volunteer, Lively day program Independence, not group home, university, a job, travel Work, friends, safety, comfort, love “That she will be talking with her peers about what she wants to be when she grows up” In 5 years we will be happy with the activities and community our then adult son is involved with Able to help families in SD62 go through transition with confidence and dignity A meaningful life with work, friends, safety Placed with a family or caregiver – have a job and someone who really cares to be living independently (away from mom and dad but close), job too For my son to have a group of peers he’s chosen Supported job and/or fulfilling volunteer work Social time/peer interaction For my son to be part of the community To go to Disneyland on a holiday, both my sons to have more friends We would like our son to settled in a loving group home.
