Abnormal BDNF signalling can influence neuronal differentiation and synaptic function leading to altered brain development and functioning. Therefore, BDNF may be a marker of abnormal neurodevelopment and neurotransmission in psychiatric diseases, including depression and schizophrenia. In schizophrenia, both decreased and increased serum BDNF levels are reported in patients suffering from chronic schizophrenia relative to healthy controls. The significant heterogeneity across studies remains unexplained. Many parameters may contribute to this heterogeneity including differences in the illness duration, age, gender and medication status of the participants and the clinical subtype of schizophrenia. To clarify these issues we aim to study more homogenous samples of patients. Because BDNF levels usually measured correspond to 3 isoforms of BDNF co-expressed (ProBDNF, mature BDNF and truncated BDNF), each exhibiting a specific physiological function, the BDNF status of patients will be precise according to BDNF isoform levels. The present study will be focused on patients with schizophrenia with positive and negative symptoms refractory to drug treatments. Correlations between serum BDNF levels and psychopathological variables, such as the PANSS Positive and Negative subscale scores, will be searched. As controls, a group of healthy subjects adjusted to patients regarding gender and age will be added. For patient recruitment and their assessment, the present project relies on a multicentric PHRC (Hospital Program for Clinical Research) developed by the team in the Psychiatric Hospital Le Vinatier. Blood will be taken by a nurse. Serum BDNF (total and isoforms), will be measured by enzyme-linked immunosorbent assay (ELISA) in collaboration with Pascale Giraudon (Fluid team, member of Centre de Recherche en Neurosciences de Lyon). Des altérations des voies de signalisation du BDNF peuvent conduire à des altérations du développement et du fonctionnement cérébral telles qu’elles sont observées dans les pathologies psychiatriques comme la dépression et la schizophrénie. A l’heure actuelle, de nombreuses études ont rapportés des altérations des taux périphériques de BDNF chez les patients atteints de schizophrénie par comparaison à des sujets sains mais les résultats sont très hétérogènes. De nombreux auteurs suggèrent que l’hétérogénéité des résultats pourrait être liée à l’hétérogénéité clinique des patients inclus dans les études, en particulier en termes de type de symptômes prédominants. Pour clarifier ce problème nous chercherons le statut du BDNF chez des patients stratifiés sur leurs caractéristiques cliniques. D’autre part, jusqu’à présent les études se sont exclusivement intéressées au taux de BDNF total. Or tel qu’il est mesuré actuellement, le BDNF total correspond à un ensemble de 3 isoformes coexprimées (ProBDNF, BDNF mature et BDNF tronqué), chaque forme étant impliquée dans une fonction physiologique spécifique. Dans ce projet nous chercherons à préciser le statut BDNF des patients en fonction du niveau des isoformes du BDNF. Dans ce projet seront inclus des patients atteints de schizophrénie qui présentent des symptômes résistants aux traitements pharmacologiques avec une prédominance de symptômes positifs ou négatifs. Des corrélations entre les niveaux sériques de BDNF et des données psychopathologiques évaluées par des échelles spécifiques (ex : Positive and Negative subscale scores-PANSS) seront recherchées. Un groupe de sujets sains appariés en genre et en âge servira de groupe contrôle. Un PHRC multicentrique porté par l’équipe de recherche au sein de l’hôpital psychiatrique du Vinatier permettra le recrutement des patients. Les prélèvements sanguins seront réalisés par une infirmière de l’équipe.
