Here are step-by-step instructions (for most items) for completing the Long-Stay Visa Application for France. Attach a
passport size photo to the application form and bring application with you to your visa interview.
#4 – Date of Birth: Remember to use Euro date/month/year format.
#11 – National Identity Number – leave blank
#12 – Type of Passport – Check “Ordinary passport” (unless one of other options applies to you)
#13 – Complete this section with your passport information. List all dates: day/month/year.
#21 – Current Occupation = “Student”
#22 – Employer = List your current college or university information (i.e. your campus study/education abroad
office).
#23 – Purpose of Visa = “Studies”

Studies for the KIIS Caen, France Spring 2014 Semester program covers my full tuition at the Université de Caen,
room and board, group excursions, SMENO coverage (sécurité sociale étudiante), comprehensive medical insurance,
and security evacuation protection. In addition, I have set aside $_______ (enter amount) for personal spending
money while in France.”
