La lésion se forme habituellement dans la partie antérieure du maxillaire supérieur. Sa découverte est souvent fortuite lors d’un examen radiologique de routine qui permet de mettre en évidence une radioclarté bien limitée, développée le plus souvent aux dépens d’une dent incluse. Il est cependant difficile, voire impossible, de différencier la TOA du kyste dentigère à l’issue de l’examen radio-clinique initial, ce qui justifie à cet égard l’examen anatomopathologique systématique et minutieux de toutes les lésions kystiques. Le traitement par énucléation simple paraît être la thérapeutique de choix, compte tenu de la constante bénignité de ces lésions. Ce travail expose une observation et passe en revue les données épidémiologiques, cliniques, diagnostiques, thérapeutiques et pronostiques de ce type de tumeur. denomatoid odontogenic tumor is a benign epithelial tumor, observed most frequently in female patients aged between 10 and 20 year-old. Occurring usually in the anterior part of the maxilla, its discovery is often fortuitous during routine radiological examinations, which show a well defined clear zone, mostly developed in the detriment of an impacted tooth. However, it’s difficult or almost impossible to differentiate this tumor from a dentigerous cyst on radiology basis. To this regard, systemic and meticulous anatomopathological examinations are necessary for all cystic lesions. Simple enucleation appears to be the treatment of choice, in view of the constantly reported benign nature of these tumors. In this article, a case report is presented. Epidemiological, clinical, diagnostic, treatment and prognosis characteristics of this tumor will be reviewed. En 1948, STAFNE fut le premier à l’individualiser comme une entité histologique distincte parmi les tumeurs odontogènes (Dilu et coll., 1999). Finalement, c’est PHILIPSEN et BIRN, en 1969, qui retiennent la terminologie de tumeur odontogénique adénoïdectomie (Philepsen et coll., 1991). Elle est aussi connue sous la dénomination d’adéno-améloblastome du fait de la présence des aspects glanduliformes ou des formations canaliculaires avec une basale nette. Elle fait partie du groupe des tumeurs odontogéniques épithéliales et conjonctives, avec ou sans formation du tissu dentaire dur selon la classification de l’OMS en 1992 (Dilu et coll., 1999 ; Lahlou et coll., 2005). L T Observation Case report H.M., âgé de 18 ans, nous est adressé en consultation pour une tuméfaction génienne basse gauche évoluant depuis 2 ans sans trouble fonctionnel notoire. H.M., 18-year-old, was referred to us for a consultation of a tumefaction on lower left of the chin evolving for 2 years without any known dysfunction. Le patient ne présente aucune particularité sur le plan général. The patient presents no particularity on the systemic scheme. L’interrogatoire ne rapporte aucun antécédent d’extraction, ni d’épisode infectieux antérieur. The interrogation reports no previous history of extraction or previous infectious episode. L’examen exobuccal montre l’existence d’une tuméfaction de la région génienne basse gauche ; les téguments de la joue sont normaux et bien mobiles. An extraoral examination shows an existence of a tumefaction on the lower left genial region while the integuments of the cheek are normal and mobile. Par ailleurs, il n’existe aucun trouble de la sensibilité labio-mentonnière et aucune adénopathie. Moreover, no sensibility dysfunction of labiogenien tissue strap nor adenopathy are detected. L’examen endobuccal met en évidence une tuméfaction intéressant les deux corticales, s’étendant de la 31 jusqu’à la face mésiale de la 36 (Fig. 1). An intraoral examination shows a tumefaction involving both sides of cortical bone, extending from the tooth 31 until the mesial surface of the tooth 36 (Fig. 1). The space supposed to be occupied by this latter tooth is compensated by the tipping of the adjacent teeth. The teeth in the concerned area are vital. Besides, the rest of the intraoral examinations shows : he adenomatoid odontogenic tumor was described for the first time in 1907 by DREIBLADT as a " pseudo-adenoma adamantinoma ". In 1948, STAFNE was the first to characterize it as a distinct histological entity from the other odontogenic tumors (Dilu et al., 1999). Finally, PHILIPSEN and BIRN, in 1969, have termed it as adenomatoid odontogenic tumor (Philepsen et al., 1991). It is also known under the name of adeno-ameloblastoma because of the presence of glanduliform appearance or canalicular formations with a clear base. According to the WHO’s classification in 1992, it is categorized in the group of epithelial and connective tissue odontogenic tumors, with or without hard dental tissue formation (Dilu et al., 1999 ; Lahlou et al., 2005). 18 Revue d’Odonto-Stomatologie/février 2006 1 Fig. 1 : Vue endobuccale : tuméfaction s’étendant de la 33 à la 36. L’absence de la 34 sur l’arcade, la présence d’une tuméfaction d’évolution lente et indolore et l’absence de signes neurologiques, nous ont fait évoquer le diagnostic d’une lésion kystique ou tumorale bénigne. The absence of the tooth 34 on the arch, the presence of a tumefaction of slow and painless evolution and the absence of neurological signs, evoked the diagnosis of a cystic or benign tumor lesion. Un bilan radiographique est alors réalisé comportant une radiographie rétroalvéolaire, un orthopantomogramme et un mordu occlusal. Radiographic examinations including retroalveolar radiography, orthopantogram and occlusal radiography were performed. Les radiographies rétroalvéolaire et panoramique (Fig. 2 et 3) montrent : ■ une vaste géode radioclaire uniloculaire de forme ovalaire, à contours nets, s’étendant transversalement de la 33 jusqu’à la racine mésiale de la 36 et verticalement vers la crête alvéolaire néanmoins respectée ; le nerf alvéolaire inférieur est refoulé ; ■ cette radioclarté englobe les 2/3 de la prémolaire incluse biradiculée, refoulée, dont l’apex se situe à proximité du bord basilaire ; on note également un élargissement desmodontal du 1/3 apical ; ■ de fines inclusions radioopaques dispersées à l’intérieur de l’image radioclaire ; ■ le trou mentonnier apparaît en mésial de l’image radioclaire et du 1 /3 apical de la dent incluse. Retroalveolar and panoramic radiographies (Fig. 2 and 3) show : ■ a vast unilocular radiolucent body of oval shape, with clear contours, extending transversely from the tooth 33 until the mesial root of the tooth 36 ; however, vertically towards the alveolar ridge, the tissue remains intact ; the inferior alveolar nerve is repressed; ■ this radiolucency encloses 2/3 of the repressed impacted two-root premolar with the apex situated near the lower border; a widening of periodontal ligament of the apical 1/3 is also observed; ■ some small radiopaque inclusions scatter inside the radiolucent image; ■ the genial foramen is seen on the mesial side of the radiolucent image and on the apical l/3 of the impacted tooth. L’examen radiographique des autres dents révèle par ailleurs : ■ la présence d’une lyse osseuse en rapport avec la 26, 36, et la 46 ; ■ des images radioclaires en rapport avec la 46 ; ■ les dents de sagesse immatures en voie d’éruption. Moreover, radiographic examinations of the other teeth reveal : ■ the presence of an osseous lysis in relation to the teeth 26, 36, and 46 ; ■ radiolucency in relation to the tooth 46 ; ■ immature wisdom teeth in process of an eruption. A travers ces renseignements cliniques et radiologiques, divers diagnostics sont évoqués notamment : ■ le kyste dentigère, en raison de la présence de l’image radioclaire englobant la couronne de la dent ; ■ le kyste odontogène calcifié, du fait de la présence de calcifications au centre de la radioclarté ; ■ l’améloblastome, vu l’image monogéodique en rapport avec une dent incluse ; ■ la tumeur odontogène adénomatoïde, en raison d’une image radioclaire englobant une dent incluse, associée à des calcifications ; ■ et enfin, la tumeur épithéliale calcifiante ou tumeur de Pindborg qui présente les mêmes caractéristiques radiologiques. According to this clinical and radiological information, different diagnosis can be evoked notably: ■ dentigerous cyst, due to the presence of the radiolucency containing the tooth crown ; ■ calcifying odontogenic cyst, because of the presence of calcifications in the center of the radiolucency ; ■ ameloblastoma, due to a unicystic image in relation with an impacted tooth ; ■ Adenomatoid odontogenic tumor, because of a radiolucency containing an impacted tooth in association with calcifications ; ■ Finally, calcified epithelial tumor or Pindborg’s tumor which presents the same radiological characteristics. La trépanation de l’os en regard de la tuméfaction est effectuée à la pince gouge, ce qui extériorise une lésion à paroi épaisse (Fig. 6). L’agrandissement de l’orifice d’accès se poursuit à la pince gouge (Fig. 7), jusqu'à permettre l’énucléation complète de la lésion (Fig. 8) englobant la dent incluse présentant deux racines dont la bifurcation est basse, au niveau du 1/3 apical (Fig. 9). Après régularisation des bords et toilette de la cavité (Fig. 10), les sutures sont réalisées par points séparés (Fig. 11). ■ ■ retracted by a retractor; the exposed cortical vestibular bone appears very fine and disrupted on the superior part of the tumefaction (Fig. 5). Enlargement of the access’s opening is continued with the gouge forcep (Fig. 7), until a complete enucleation of the lesion (Fig. 8) involving the impacted tooth presenting 2 roots with the low bifurcation situated on the apical 1/3 of the root (Fig. 9). After trimming the bone edges and cleansing of the exposed cavity (Fig. 10), separate point sutures are done (Fig. 11). Upon this examination, the lesion is concluded to be an adenomatoid odontogenic tumor. L’âge de découverte de cette tumeur est extrêmement variable avec un pic de fréquence au cours de la deuxième décennie (entre 10 et 20 ans) (Dilu et coll., 1999 ; Precious et coll., 1984) ; ce qui correspond à l’âge de notre patient. The tumors affect women more than men. The female/male ratio is 2:1 according to Stroncek et al., 3:1 according to Toida et al., reported by Pecheur et al. (Dilu et al., 1999 ; Pecheur et al., 1992). En ce qui concerne la localisation tumorale, il est noté un nombre plus important de localisations maxillaires que mandibulaires (Dilu et coll., 1999 ; Lesclous et coll., 1989). Regarding to the tumor localization, it is found with a more significant number in maxilla than mandible (Dilu et al., 1999 ; Lesclous et al., 1989). Au niveau du maxillaire, elle intéresse avec prédilection la région antérieure, notamment la région canine. A la mandibule, la localisation préférentielle se situerait dans la région symphysaire et parasymphysaire (Olgac et coll., 2003 ; Pecheur et coll., 1992 ; Precious et coll., 1984). In maxilla, it preferentially involves the anterior region, notably the canine region. En cas de symptomatologie, la lésion se manifeste par une tuméfaction osseuse non douloureuse – ce qui a été décelé chez notre patient – le plus souvent ferme, plus rarement associée à des zones rénitentes, augmentant lentement de volume et pouvant parfois amener à des déplacements dentaires, des perturbations fonctionnelles ou des déformations extérieures visibles (Curran et coll., 1997 ; Lesclous et coll., 1989 ; Pecheur et coll., 1992). Sur le plan radiologique, l’aspect de la lésion est celui d’une image radioclaire bien circonscrite développée aux dépens d’une dent incluse et évoquant au premier abord un kyste dentigère coronodentaire (Cantaloube et coll., 1987 ; Precious et coll., 1984). Les tumeurs plus évoluées peuvent contenir des foyers radioopaques discrets ou marqués, constituant un critère utile de diagnostic (Marx et Stern, 2003 ; Olgac et coll., 2003). Les dents voisines peuvent être déplacées si la lésion est étendue mais les racines ne sont normalement pas résorbées, ce qui est retrouvé dans notre cas (Olgac et coll., 2003 ; Precious et coll., 1991). Inégalement réparti au sein de la radioclarté, un matériau radioopaque minéralisé réalise une fine ponctuation ou se groupe en amas plus volumineux (Bouyssi et coll., 1997 ; Dilu et coll., 1999). Cette image prête à confusion avec la TOA, comme dans notre observation. Le diagnostic est surtout histologique puisque dans le KOC, on retrouve un contingent de cellules épithéliales momifiées dites cellules fantômes qui caractérisent cette lésion (Bouyssié et coll., 1997 ; Marx et Stern, 2003). The AOT, presenting calcifications at a more developing stage, resembles a calcified odontogenic cyst (COC), who appears as an osteolytic image, sometimes of big dimension, more frequently polylocular than unilocular, usually with well defined outlines. Unevenly distributed within the radiolucency, mineralized radioopaque materials are observed as a fine punctuation or grouped in a more voluminous mass (Bouyssi et al., 1997 ; Dilu et al., 1999). This image can thus lead to a confusion with the AOT, as observed in our observation. The diagnosis will especially be based on histological findings since in case of the COC, a gathering of mummified epithelial cells called ghosts cells will be identified presenting a characteristic of this lesion (Bouyssié et al., 1997 ; Marx and Stern, 2003). A ce stade également, cette tumeur doit être différenciée de la tumeur épithéliale odontogène calcifiée, ou tumeur de Pindborg, qui se présente radiologiquement comme une image radioclaire uni ou multiloculaire sans liseré de condensation périphérique, parsemée d’images radioopaques de tailles variables et associée dans 60 % des cas à une dent incluse ou un odontome (Hafian et coll., 2004 ; Marx et Stern, 2003). A la coupe de la pièce, il est fréquent de noter un liquide sérohématique et dans la lumière du kyste, des végétations très importantes donnant un aspect grumeleux (Cantaloube et coll., 1987 ; Philepsen et coll., 1991). Elles sont traversées par des structures canaliculaires bordées d’un épithélium cylindrique avec des rangées de noyaux situés du côté opposé de la lumière. A l’intérieur de celle-ci, existe un matériel éosinophile amorphe au contact du pôle apical des cellules. Des plages de calcifications sont présentes par endroit. Cette technique apparaît tout à fait suffisante pour éradiquer ce type de lésion et à ce jour, aucune récidive après un tel traitement n’a été signalée dans la littérature (Curran et coll., 1997 ; Marx et Stern, 2003 ; Precious et coll., 1984). From the treatment plan’s point of view, these tumors of benign nature are managed by a simple surgical enucleation. This technique seems quite sufficient to eradicate this type of lesion and until now, no recurrence following the treatment was reported in the literature (Curran et al., 1997 ; Marx and Stern, 2003 ; Precious et al., 1984). Conclusion La TOA est une lésion constamment bénigne d’évolution lente. La sémiologie clinique et l’aspect radiologique non caractéristiques sont la source d’une difficulté d’orientation diagnostique préopératoire. Le diagnostic de certitude repose uniquement sur l’examen histologique. The AOT is commonly a benign slow-growing lesion. The clinical sign and symptom and non-characteristic radiological aspect are the source of a difficulty for preoperative diagnostic orientation.
