REPORTING AN EMPLOYMENT INJURY REPORTING AN EMPLOYMENT INJURY WITHOUT ABSENCE As soon as a worker suffers an employment injury, he/she shall immediately declare it to his/her supervisor or to an Employer’s representative. This must be done prior to leaving the premises if possible or as soon as possible (Section 265 Act Respecting Industrial Accidents and Occupational Diseases). CSST The worker who has suffered an employment injury or contracted an occupational disease shall complete the school board’s Accident Report form and submit it to his/her supervisor. The immediate supervisor shall complete the section “For Administrative Use Only” and forward the form to the Human Resources Department to the attention of Josée Desmarais (by fax and original copy by internal mail). REPORTING AN EMPLOYMENT INJURY WITH ABSENCE In the case of an employment injury which results in an absence from work (as prescribed by a physician on a CSST medical certificate beyond the day of the accident), the following procedure applies: - the procedure “Reporting an Employment Injury without Absence” (see above) shall be followed; - the worker is responsible for providing the Human Resources Department with a copy of the medical certificate issued by the attending physician (Section 267 of the Act), as well as the accident report form provided by the school board. Human Resources must be informed of every medical appointment and a copy of every CSST medical report is to be sent to Human Resources; PROCEDURE TO FOLLOW WHEN AN EMPLOYMENT INJURY OCCURS - should the worker be absent from work for more than ten (10) working days or required to undergo therapy (ex.: physiotherapy), he/she shall complete the CSST “Worker’s Claim” form, send the original to CSST and a copy to Human Resources; - the employee must inform Human Resources when he/she has a medical appointment and send a copy of the CSST medical attestation (Employer’s copy) after each visit. Medical Assistance The Employer or his representative shall respond immediately by administering first aid to the victim of an employment injury and by providing transportation to a health care institution if necessary. The cost of the worker’s transportation shall be assumed by the Employer. The worker who must take time off from work for a medical consultation, physiotherapy or any other type of therapy shall give prior notice to his/her immediate supervisor. The supervisor will forward an absence form to the school board and will indicate CSST on it. Investigation and Analysis of the Employment Injury An employee who is a victim of an employment injury shall provide all necessary information to his/her immediate supervisor and to the administrators responsible for carrying out the investigation of the incident. Telephone: 450 621-5600 extension 1315 Fax: 450 965-9736 DÉCLARATION D’UN ACCIDENT DU TRAVAIL DÉCLARATION CSST D’UN ACCIDENT DU TRAVAIL SANS PERTE DE TEMPS L’employé victime d’un accident du travail doit en aviser son supérieur immédiat ou, à défaut, un représentant de l’Employeur avant de quitter l’établissement lorsqu’il en est capable ou sinon dès que possible (Article 265 de la Loi sur les accidents du travail et les maladies professionnelles (LATMP)). L’employé victime d’un accident du travail ou d’une maladie professionnelle doit remplir le rapport d’accident de la commission scolaire, le signer et le remettre à son supérieur immédiat. Le supérieur immédiat remplit la partie du formulaire « Réservé à l’administration » et l’achemine au Service des ressources humaines à l’attention de Josée Desmarais (par télécopieur et la copie originale par courrier interne). DÉCLARATION D’UN ACCIDENT DU TRAVAIL AVEC PERTE DE TEMPS Dans le cas d’un accident du travail avec perte de temps, c’està-dire un accident qui entraîne un arrêt de travail prescrit par un médecin sur une attestation médicale CSST, et ce, audelà du jour de l’accident, voici la procédure à suivre : - suivre procédure «Déclaration de l’accident du travail, sans perte de temps », tel que décrit ci-dessus; - l’employé à la responsabilité de remettre une copie de l’attestation médicale (Article 267 de la loi) émise par son médecin traitant au Service des ressources humaines, de même que le rapport d’accident de la commission scolaire; PROCÉDURE POUR DÉCLARER UN ACCIDENT DU TRAVAIL Dans le présent document, le masculin est utilisé sans aucune discrimination et dans le seul but d’alléger le texte. si l’employé s’absente plus de 10 jours ouvrables ou doit subir des traitements (ex: physiothérapie), il doit aussi remplir le formulaire « La réclamation du travailleur » (formulaire CSST), envoyer l’original à la CSST et une copie au Service des ressources humaines; - l’employé doit aviser le Service des ressources humaines lorsqu’il a un rendez-vous médical et transmettre la copie Employeur de l’attestation médicale CSST de chaque visite. Assistance médicale L’employeur ou son représentant doit immédiatement donner les premiers secours à l’employé victime d’une lésion professionnelle et, s’il y a lieu, le faire transporter dans un établissement de santé. Les frais de transport sont assumés par l’employeur. En tout temps, si l’employé s’absente de son travail pour une consultation médicale ou de la physiothérapie ou tout autre traitement relié à son accident, il doit en aviser au préalable son supérieur immédiat. Le supérieur immédiat enverra une déclaration d’absence à la commission scolaire en indiquant sur le formulaire « CSST ». Enquête et analyse de l’accident du travail L’employé victime d’un accident du travail doit fournir les renseignements nécessaires au supérieur immédiat et aux gestionnaires afin de réaliser l’enquête et l’analyse de l’accident. Ceci vise principalement à identifier les causes de cet événement et d’en prévenir la répétition.
