Dr. Aisha Cortoos – Brugmann 6 février 2012 Opbouw van slaap 1. 2-processen model voor slaapregulatie : proces ‘S’ Figure 1. Schematic representation of process S adapted from Achermann & Borbély (2003): time course (X-axis) of process S-regulated sleep propensity (Y-axis). Dr. Aisha Cortoos – Brugmann 6 février 2012 Opbouw van slaap Manipulaties van Proces S Dr. Aisha Cortoos – Brugmann 6 février 2012 Sleep or wake problem? Good sleepers who sleep less ≠ Insomnia patients We know that sleep shortage results in an increase of proces S (sleep pressure), which in turn results in ‘catching up on sleep’. Then why aren’t insomnia patients falling asleep quicker if they are exposed to sleep shortage? Saper, Scammel & Jun, Nature, 2005 Dr. Aisha Cortoos – Brugmann 6 février 2012 Insomnia: disruption of sleep or wake drive? Wake drive alert + - + - Sleep drive - + sleep (De Valck &Cluydts, 2003) Dr. Aisha Cortoos – Brugmann 6 février 2012 1. Insomnia a. Introduction b. Symptomatology c. Prevalence 2. 3. 4. 5. Etiology Sleep physiology Treatment: Cognitive behavioral therapy for insomnia: CBT-I Conclusion Dr. Aisha Cortoos – Brugmann 6 février 2012 Treatment: Cognitive behavioral therapy for insomnia (CBTI) Structured program: 6 – 8 sessions of 1,5 – 2 hours 1. a. behavioral component z Excessive time in bed, staying in bed during the weekends, naps z Sleep incompatible behavior, wakefulness associated with bedroom, en bed time b. Cognitive component z Dysfunctional cognitions: unrealistic expectations, wrong attributions c. Psycho-education z Basic sleep hygiene Dr. Aisha Cortoos – Brugmann 6 février 2012 Treatment: Cognitive behavioral therapy for insomnia (CBT-I) a. Stimulus controle therapy reassociation of bed and bedroom with relaxation and fast sleep onset Avoid sleep incompatible activities and keep a consistent sleep-wake b. Sleep restriction Time in bed (TIB) is restricted to the reported subjective total sleep time (TST). ex.: TIB is 9 hours, but TST only 6h: then TIB = TST: from now on the time in bed is restricted to 6 hours (+ 30 min. SOL?) c. Relaxation deactivation of the arousal/wake system Progressive relaxation Biofeedback autogenic training Mindfulness Dr. Aisha Cortoos – Brugmann 6 février 2012 Treatment: Cognitive behavioral therapy for insomnia (CBT-I) 4. Cognitive therapy Change wrong or dysfunctional cognitions and attitudes about sleep (use DBAS) disrupt vicious cycle in thinking pattern ex.: unrealistic expactations : “never wake up, always sleep 8 hours,…” misconception about causes: “my insomnia is completely due to a chemical inbalance” enlargement of the consequences: “I can’t do anything after a bad night of sleep” … Dr. Aisha Cortoos – Brugmann 6 février 2012 Treatment: Cognitive behavioral therapy for insomnia (CBT-I) 5. Paradoxical intention persuade the patient to perform his/her most feared behavior, namely staying awake By not trying to fall asleep, performance anxiety drops/dissapears and sleep comes easier. 6. Sessie 7: Terugval preventie Eigen behandelplan opstellen Dr. Aisha Cortoos – Brugmann 6 février 2012 6. Behandeling: CBT effecten Effects of CBT-I 1. Effecten Dr. Aisha Cortoos – Brugmann 6 février 2012 Example 1. 2. 3. 4. Insomnia is characterised by a disruption in both nighttime as well as daytime functioning. Literature: ’24-hour arousal disorder’ Both sleep and wake system play a role 2. Standard treatment = Cognitive behavioral therapy for insomnia Despite it’s name, this is NOT a therapy, but rather a training! Focus on both the sleep system (sleep restriction) and wake system (stimulus control and cognitive training) Dr. Aisha Cortoos – Brugmann 6 février 2012 Conclusie 3.
