CareFusion approval is required before an RGA is issued. Product must have been purchased from CareFusion. Information required before an RGA is issued: • Account Number • Material/Model Number (s) • Quantity Being Returned • Lot # (Disposables, consumables) or Serial # (Equipment) • Reason for Return IV Set /consumable products: IV Set and consumable product returns that do not meet the following criteria will be returned to customer with no credits issued: All IV Sets and consumable products can be returned to CareFusion for 100% credit within 60 days of invoice date. Customer will pay and arrange transportation to CareFusion. After 60 days CareFusion will offer 90% credit for authorized product returns due to customer error and 100% credit for product returns due to a CareFusion shipping error with CareFusion paying and arranging return transportation. Returns of CareFusion IV Set/consumable products must occur within one year of sale date to receive credit IV Sets must be returned in unopened sealed cases or original packaging IV Set products that are converted to upgraded technology will be accepted back for return with 100% credit No returns will be authorized on customized, made-to-order (MTO) and special set products or due to contract cancellation or expiration Customer responsible for all transportation costs, except in the case of a CareFusion shipping error Product must be currently offered for sale by CareFusion and within 1 year of expiration © 2013 CareFusion Corporation or one of its subsidiaries. Product must be returned within 15 days of the issuance of the RGA Returned products will be credited only if customer account is current Spare Part/Accessory products: Spare Part and Accessory product returns that do not meet the following criteria will be returned to customer with no credits issued: All Spare Part and Accessory products can be returned to CareFusion for 100% credit within 60 days of invoice and customer will pay and arrange return transportation to CareFusion. After 60 days CareFusion will offer 85% credit for authorized product returns due to customer error and 100% credit for product returns due to a CareFusion shipping error with CareFusion paying and arranging return transportation. Returns of CareFusion Spare Parts/Accessory products must occur within one year of sale date to receive credit Spare Parts/Accessories must be returned in unopened, sealed and original packaging Customer responsible for all transportation costs, except in the case of a CareFusion shipping error Product must be currently offered for sale by CareFusion Product must be returned within 15 days of the issuance of the RGA Returned products will be credited only if customer account is current Equipment products: Customers must obtain an RGA number when returning Equipment for repair, whether or not Equipment is under a CareFusion warranty. CareFusion will offer 85% credit for authorized product returns and customer will pay and arrange return transportation to CareFusion. Equipment product returns that do not meet the following criteria will be returned to customer with no credits issued: Equipment returns must be sealed and in original packaging (cartons) Equipment must be received by CareFusion within 160 days of original shipment date No shipping damage to exterior boxes No customer labels or use No rework of material after original shipment date All anti-tamper seals intact Latest released software version Customer responsible for all transportation costs, except in the case of a CareFusion shipping error If Equipment return relates to software license termination, then software CDs and documentation must be returned with the Equipment Returned products will be credited only if customer account is current IV Pole products: IV Pole products – 1090, 907-0115, 915-0171 & 925-0176 - (excluding fully assembled IV Poles) can be returned to CareFusion for 85% credit for authorized product returns within 60 days of invoice date. Customer will pay and arrange transportation to CareFusion. CareFusion will provide 100% credit for product returns due to a CareFusion shipping error with CareFusion paying and arranging return transportation. L’approbation de CareFusion est requise avant qu’un ARM soit délivré. Le produit doit avoir été acheté auprès de CareFusion. Numéro de compte Numéro(s) de modèle/matériel Quantité retournée Numéro de lot (Jetables, consommables) ou Numéro de série (Équipement) Raison du retour Pièces et accessoires de rechanges Les retours de pièces de rechange et accessoires qui ne sont pas conformes aux critères suivants seront retournés au client sans qu’aucun crédit ne soit émis: Toutes pièces de rechange et accessoires peuvent être retournés à CareFusion pour crédit complet dans les soixante (60) jours suivant la date de facturation. Le client devra s’acquitter des frais de retour en plus d’organiser le transport de la marchandise vers CareFusion. Après les soixante (60) jours, CareFusion offrira un crédit de 85% pour les retours de produits autorisés causés par une erreur du client et un crédit de 100% pour un retour de produit causé par une erreur de CareFusion lors de l’expédition. CareFusion s’acquittera alors de la note pour les frais et organisera le transport. Pour tout crédit, les pièces de rechange et accessoires des produits de CareFusion doivent être retournés dans l’année suivant la date de la vente. Les pièces de rechange et accessoires doivent être retournés dans leurs emballages d’origine, encore fermé et scellé. Le client doit s’acquitter de tous les frais de transport, sauf dans les cas où CareFusion est à l’origine d’une erreur lors de l’expédition. Le produit doit être actuellement offert en vente par CareFusion. Le produit doit être retourné dans les quinze (15) jours suivant l’émission du ARM. Les produits retournés seront crédités seulement si le compte du client est à jour. Équipement: Les clients doivent obtenir un numéro ARM lors du retour d’équipement pour une réparation, que l’équipement soit ou non sous une garantie émise par CareFusion. Un crédit de 85% sera offert par Carefusion pour le retour de produits autorisés. Le client devra s’acquitter des frais de transport et organiser le retour vers CareFusion. Les retours d’équipements qui ne sont pas conformes aux critères suivants seront retournés au client sans qu’aucun crédit ne soit émis: Les équipements doivent être retournés dans leurs emballages d’origine, encore fermé et scellé. Les équipements doivent être reçus par CareFusion dans les (cent soixante) 160 jours suivant la date d’expédition originale. Les boîtes d’emballage doit être exempt de dommages extérieurs. Équipement non utilisées et sans identification du client. Aucune réparation sur l’équipement après la date d’expédition originale Tous les sceaux anti-sabotage sont intacts Contient la version de logiciels la plus récente Le client doit s’acquitter de tous les frais de transport, sauf dans les cas où CareFusion est à l’origine d’une erreur lors de l’expédition. Si la résiliation de la licence du logiciel est la raison du retour, tous les CD et la documentation concernant le logiciel doivent être inclus avec l’équipement lors du retour. Produits IV Pole: Les produits IV Pole – 1090, 907-0115, 915-0171 & 925-0176 - (excluant les produits IV Pole complètement assemblés) autorisé pour retour, peuvent être retournés à CareFusion pour un crédit de 85% dans les soixante (60) jours suivant la date de facturation. Le client devra s’acquitter des frais de retour en plus d’organiser le transport de la marchandise vers CareFusion. CareFusion procurera un crédit de 100% pour un retour de produit causé par une erreur de CareFusion lors de l’expédition. CareFusion s’acquittera alors de la note pour les frais et organisera le transport. Le retour des produits IV Pole qui ne sont pas conforme aux critères suivants seront retournés au client sans qu’aucun crédit ne soit émis: Les retours de CareFusion 1090, 907-0115, 915-0171 & 925-0176 produits IV Pole doivent se produire dans les soixante (60) jours suivant la date de facturation pour recevoir le crédit. IV Poles doivent être retournés dans leur emballage d’origine, encore fermé et scellé. Aucun retour autorisé sur « custom IV Poles », IV Poles non listés ci-dessus et IV Poles entièrement assemblés – tous codes et modèles confondus. Le client doit s’acquitter de tous les frais de transport, sauf dans les cas où CareFusion est à l’origine d’une erreur lors de l’expédition. Le produit doit être actuellement offert en vente par CareFusion. Le produit doit être retourné dans les quinze (15) jours suivant l’émission du ARM.
