Agreement between Atlantic Canada Kyokushin

Agreement between Atlantic Canada Kyokushin-kan Karate (also known as ACKK), Centres Ray Cormier, and
their affiliated schools, and the fighter or their legal guardian.
It is understood that ACKK and any and all centres will not be held responsible in any way for any accidents, losses, and
injuries of any nature, related to Kyokushin-kan ONE MATCH held March 31st 2012. The student, or their legal guardian,
if minor, and their heirs, also release all agents, instructors ¸members and representatives of the schools from any and all
types of lawsuits and/or claims for injuries, accidents or losses of any nature incurred during or resulting from training,
transportation to or this event. ACKK, Centres Ray Cormier and its affiliated schools also reserve the right to use the
student’s picture, video or likeness taken during this event, in its promotional activities without any form of compensation,
monetary or otherwise, unless given a written notice by the student or legal guardian before such promotions are made.
The student and/or legal guardian also releases any and all owners, or their representatives, of the halls where event is
taking place. Participation in any and all activities is at the student’s own risk. The student and/or legal guardian
realize and accept the risks associated with this type of sport, which is full contact karate. They also accept that
we offer first aid only and all costs associated to any injury in any form, during or resulting from this event is
their responsibility.
The student and/or their legal guardian if minor, have read and understood this agreement of non-responsibility, and
accept it in its entirety as a condition of being accepted and allowed to participate in the Kyokushin-kan ONE MATCH
March 31st 2012. The participant and or legal guardian also declare having read the rules of the event and accept them
as is, and declares that he/she is in good physical health and is physically fit enough to participate in said event.
Accord de non-responsabilité entre Atlantic Canada Kyokushin-kan Karate [aussi connu comme ACKK] , Centres
Ray Cormier, leur écoles affiliées ET combattant et/ou gardien/gardienne légal(e) si mineur.
Il est entendu qu’ACKK et tous les centres ne seront en aucune façon liés ou tenus responsables d’aucune blessure,
perte ou accident de toute nature qui soit. L’étudiant ou étudiante, ou gardien/gardienne légal si mineur , ne tiendra
aucunement responsable aucun représentant des écoles instructeurs, instructrices, ou n’importe quel membre des écoles
pour aucune blessure, pertes ou accident relié au Kyokushin-kan ONE MATCH du 31 mars 2012, ACKK et les centres
se réservent le droit d’utiliser la photo, vidéo ou ressemblance des étudiants prises durant les activités, pour toute forme
de promotion sans compensation monétaire ou autre, a moins de recevoir par écrit l’interdiction de l’étudiant avant que la
promotion soit faite.
ACKK, les écoles, leur membres, ainsi que les propriétaires ,ou leur représentants ,des salles ou l’activité aura lieu ne
seront tenus responsables d’aucune façon qui soit pour aucune blessure, perte ou accident et ne seront pas poursuivis en
aucune façon pour réclamations d’aucune nature qui soit par l’étudiant, étudiante, ou gardiens/gardiennes légaux ou
leurs héritiers. Participation a toute activité est la responsabilité de l’étudiant/étudiante ou gardien/gardienne légal
et est a leur propres risques. Ils/elles réalisent les risques reliés a ce type de sport, qui est le karate full contact.
L`étudiant, étudiante ou gardien/gardienne légale a lu et comprit en son entièreté cet accord de non-responsabilité, et
l’accepte comme telle, comme condition que l’étudiant/étudiante soit accepté comme combattant dans le Kyokushin-kan
ONE MATCH , le 31 mars 2012. Il/elle déclare aussi être en bonne santé physique et capable de participé comme
combattant , et a lu et accepte comme tel les règlements de l’événement.
